A gummy smile poses a restorative challenge for dentists attempting to achieve ideal esthetics. Many have advocated the use of a gingivectomy or laser-assisted gingival contouring procedure. However, this simplistic approach can potentially create a mucogingival defect or a biological width violation. To avoid these periodontal-restorative complications, it is important during treatment planning to assess the anatomical relationship that resulted in the gummy smile and choose the appropriate surgical treatment to eliminate this condition.

OBJECTIVE: The aim of the present study was to evaluate the effect of periodontal dressing on post-operative pain and swelling after surgical crown lengthening.
MATERIALS AND METHODS: A blind, randomized, clinical trial was carried out with 36 patients. Following surgical crown lengthening, the individuals were randomly allocated to the periodontal dressing group (PDG) and control group (CG, non-placement of periodontal dressing). Pain and discomfort were analyzed using a visual analog scale (VAS), verbal scale (VS) and the number of analgesics consumed in 7 days post-operatively. Post-operat

Orthodontic extrusion (OE), which is performed in many different clinical situations to move a tooth or its periodontal tissues coronally, is often associated with supracrestal fiberotomy and root planing (OEFRP) or followed by surgical crown lengthening. The OEFRP procedure must be carried out every 2 weeks during the entire extrusive orthodontic phase, and precise control of the technique itself can be quite difficult, especially when this approach is to be performed on a limited portion of the root perimeter in teeth affected by angular defects. The aim of this study was to show a new nonsu

This is the first article in a two-part series which aims to provide an overview of the different techniques used to increase clinical crown height. In the first paper, the focus will be on the management of patients who present with gingival tissue excess. The different aetiologies are discussed and illustrated with clinical cases, following which a range of procedures that may be employed in the management of these patients are presented. With an increasingly ageing population, more patients are taking regular medications prescribed from their general medical practitioner, and so having a wo

The presence of subgingival pathology, such as a deep secondary caries lesion, a fracture, an already existing restoration that needs to be replaced or the presence of a resorption defect, can constitute a difficult starting situation for the preservation of a tooth. How to deal with such a situation has been a topic of discussion for years. Surgical crown lengthening is one of the first and classic treatment options that is often chosen in such cases. Crown lengthening is a periodontal surgical technique to obtain extra supra crestal clinical crown length. The question is, what is the long-te

BACKGROUND: In patients with gingival exposure on smiling due to altered passive eruption, esthetic crown lengthening is often indicated. Meticulous planning and surgical precision are key for successful outcomes. Surgical guides are helpful tools that are seldomly reported on in the literature related to esthetic crown lengthening procedures.
TYPES OF STUDIES REVIEWED: The authors searched the literature for articles that described the planning, tools, and execution related to esthetic crown lengthening procedures.
RESULTS: Several techniques have been reported to guide the esthetic crown l

Cases in which a single tooth is in need of crown lengthening while remaining esthetically pleasing represent a unique challenge. A case is presented here in which crown lengthening in a challenging esthetic situation on a canine was achieved through forced eruption with a removable orthodontic appliance. The case was considered a success by both the clinician and the patient. Fabrication of the acrylic stent is a simple procedure that can be completed chairside, and the patient's acceptance of the appliance as well as the overall cost and time invested made this a very convenient alternative.

Although a number of techniques have been proposed for clinical crown lengthening procedures, all have some limitation in terms of function and esthetics. This report presents the clinical and radiographic results of a surgical extrusion technique for clinical crown lengthening. Atraumatic surgical extrusion using a specially designed instrument (Periotome) was performed in three cases in which it was expected that extensive resective osseous surgery would have to be used for crown lengthening. Full-thickness mucoperiosteal flaps were raised both labially and palatally. The tooth was carefully

OBJECTIVE: The appearance of the gingival tissue around the teeth is essential in the aesthetics of the anterior maxillary region of the oral cavity. Abnormalities in symmetry and contour can significantly compromise the appearance of the natural or restored dentition, so the outcome is highly relevant to treatment choice. This study evaluated the efficacy of crown lengthening for restoration of maxillary anterior tooth defects.
METHODS: A total of 80 patients (87 affected teeth) diagnosed with maxillary anterior tooth loss in Ganzhou People's Hospital between May 2020 and August 2021 were re

AIM: The objective of the present case series was to propose a modified technique for esthetic crown lengthening surgery (ECLS) and a new method for categorizing esthetic parameters in terms of the gingival tissue as well as to assess treatment outcomes after 6 months.
MATERIALS AND METHODS: Patients seeking ECLS were enrolled according to inclusion and exclusion criteria that are decribed in the materials and methods criteria. Digital photographs were obtained at baseline and at 6 months postsurgery. Soft tissue cone beam computed tomography (ST-CBCT) was performed at baseline, and the dimen

Crown lengthening by forced eruption is usually followed by conventional surgical procedures. The aim of the present study was to evaluate clinical use of Nd:YAG laser fiberotomy in crown lengthening by forced eruption as an alternative to surgery. Patients who needed crown lengthening were enrolled in this prospective case series and underwent forced eruption. Fiberotomy by Nd:YAG laser was performed 2 days after initiation of forced eruption and every 2 weeks afterward to reach optimum clinical crown length. Measurements included sulcus depth (SD), distance between the gingival margin and th

A new approach to clinical crown lengthening has been developed and described. The technique combines controlled eruptive tooth movement and incision of the supracrestal gingival attachment. The procedure was performed in patients with severe destruction of a tooth crown and in whom clinical crown lengthening procedures were essential before the teeth could be properly restored. Controlled eruptive forces were activated by simple orthodontic appliances. During the active phase of forced eruption, repeated intrasulcular incisions through the junctional epithelium and the supracrestal connective

BACKGROUND: The purpose of this study was to conduct survival analysis of teeth following clinical crown lengthening procedures (CLPs) and crown insertions via a retrospective cohort study.
METHODS: Patient- and tooth-related data were collected from 268 participants who received CLPs from 2009 to 2015. The Kaplan-Meier curve and the log-rank tests were used to estimate the probability of survival and compare the survival probabilities among different variables. A Cox multivariate proportional hazard regression model was used to investigate the collective effects of root canal treatment (RCT)

OBJECTIVE: To compare the clinical periodontal parameters of teeth restored with a single ceramic crown, with and without crown lengthening procedure.
METHODS: This prospective, longitudinal, controlled, and single-blinded clinical trial involved 22 patients with a total of forty-one teeth with ceramic crowns. The teeth were divided into two groups: test (n = 21), comprising teeth rehabilitated post crown-lengthening surgery, and control (n = 20), comprising teeth rehabilitated without crown-lengthening surgery. Plaque index (PI), gingival index (GI), probing depth (PD), bleeding on probing (

Background: Altered passive eruption (APE) is one of the etiological factors associated with excessive gingival display and is commonly treated with esthetic crown lengthening (ECL). However, existing classification systems provide limited guidance for selecting appropriate treatment approaches. Objectives: The aim of this study was to evaluate (1) the expected outcome of ECL in eliminating unattractive excessive gingival display (4 mm) based on digital smile assessment and (2) the distribution of teeth and patients according to the modified APE classification. Methods: Forty-two Thai patients

In cases where malocclusion is associated with intrinsic discoloration and/or discrepancies in tooth size and shape, such as peg-shaped laterals, orthodontics alone may not improve the aesthetics. In these situations, veneers may be considered as an adjunct to orthodontic treatment to improve the overall aesthetics. The aim of this study is to report a clinical case where an uneven occlusal plane was corrected, and the positioning of gingival zeniths, color, shape, and size of the dental elements involved were improved by means of gingivectomy and rehabilitation with 10 ceramic laminate veneer
