Finding patients using similarity measures in a rare diseases-oriented clinical data warehouse: Dr. Warehouse and the needle in the needle stack.

OBJECTIVE: In the context of rare diseases, it may be helpful to detect patients with similar medical histories, diagnoses and outcomes from a large number of cases with automated methods. To reduce the time to find new cases, we developed a method to find similar patients given an index case leveraging data from the electronic health records. MATERIALS AND METHODS: We used the clinical data warehouse of a children academic hospital in Paris, France (Necker-Enfants Malades), containing about 400,000 patients. Our model was based on a vector space model (VSM) to compute the similarity distance

Design and evaluation of an automated pediatric acute lymphoblastic leukemia registry from clinical data warehouses.

BACKGROUND: Real-world data (RWD)–based pediatric acute lymphoblastic leukemia (ALL) registries often face scalability and reliability limits owing to manual data entry and inter-institutional heterogeneity. We evaluated the feasibility of building an automated, high-quality registry using electronic medical records (EMR) and clinical data warehouses (CDWs) to quantitatively assess the Pediatric ALL Automated Registry (PeARL) for (1) automatic extractability (ADE) (2), data quality, and (3) multicenter applicability. METHODS: In this study, we used CDWs to identify patients aged < 18 years wit

Design and Implementation of a Scalable Clinical Data Warehouse for Resource-Constrained Healthcare Systems.

Centralized electronic health record (EHR) repositories are critical for advancing disease surveillance, public health research, and evidence-based policymaking. However, developing countries face persistent challenges in achieving this due to fragmented healthcare data sources, inconsistent recordkeeping practices, and the absence of standardized patient identifiers. These issues hinder reliable record linkage, compromise data interoperability, and limit scalability-obstacles exacerbated by infrastructural constraints and privacy concerns. To address these barriers, this study proposes a scal

Risk of physical injuries in children and teens with ophthalmic diagnoses in the OptumLabs Data Warehouse.

BACKGROUND: To evaluate how eye diseases affect the risk of injuries (fractures, musculoskeletal, head and eye injuries) in children and teens. METHODS: Claims from the OptumLabs Data Warehouse, a longitudinal deidentified commercial insurance claims database, were analyzed. Eligible subjects were aged <19 years at the time of their eye disease diagnosis, enrolled in the health plan between 2007 and 2018, and had >1 visit for >1 significant eye disease (strabismus, amblyopia, nystagmus, structural diseases), based on ICD9/10 codes. Controls were children in the same database who had no eye di

Evaluating patient data quality in South Africa's National Health Laboratory Service Data Warehouse, 2017-2020: implications for monitoring child health programmes.

BACKGROUND: South Africa's National Health Laboratory Service (NHLS), the only clinical laboratory service in the country's public health sector, is an important resource for monitoring public health programmes. OBJECTIVES: We describe NHLS data quality, particularly patient demographics among infants, and the effect this has on linking multiple test results to a single patient. METHODS: Retrospective descriptive analysis of NHLS data from 1st January 2017-1st September 2020 was performed. A validated probabilistic record-linking algorithm linked multiple results to individual patients in li

Outwitting an Old Neglected Nemesis: A Review on Leveraging Integrated Data-Driven Approaches to Aid in Unraveling of Leishmanicides of Therapeutic Potential.

The global prevalence of leishmaniasis has increased with skyrocketed mortality in the past decade. The causative agent of leishmaniasis is Leishmania species, which infects populations in almost all the continents. Prevailing treatment regimens are consistently inefficient with reported side effects, toxicity and drug resistance. This review complements existing ones by discussing the current state of treatment options, therapeutic bottlenecks including chemoresistance and toxicity, as well as drug targets. It further highlights innovative applications of nanotherapeutics-based formulations,

Development and Validation of a Multi-Modal Algorithm for Chronic Kidney Disease Detection in a Hospital Clinical Data Warehouse.

Chronic kidney disease (CKD) is often underdetected in hospitalized older adults due to incomplete coding and laboratory data. Using eHOP, the clinical data warehouse of Rennes University Hospital, we developed and validated a rule-based interpretable multimodal algorithm combining laboratory and textual information for CKD phenotyping. Among 15,112 hospital visits (7,556 with CKD and 7,556 without), the algorithm achieved an accuracy of 92.1 % in agreement with ICD-10, recall 96.5 %, and precision 86.6 %. Validation on a clinician-reviewed subset confirmed the reliability of classifications.

Replicating medication trend studies using ad hoc information extraction in a clinical data warehouse.

BACKGROUND: Medication trend studies show the changes of medication over the years and may be replicated using a clinical Data Warehouse (CDW). Even nowadays, a lot of the patient information, like medication data, in the EHR is stored in the format of free text. As the conventional approach of information extraction (IE) demands a high developmental effort, we used ad hoc IE instead. This technique queries information and extracts it on the fly from texts contained in the CDW. METHODS: We present a generalizable approach of ad hoc IE for pharmacotherapy (medications and their daily dosage) p

A Dimensional Bus model for integrating clinical and research data.

OBJECTIVES: Many clinical research data integration platforms rely on the Entity-Attribute-Value model because of its flexibility, even though it presents problems in query formulation and execution time. The authors sought more balance in these traits. MATERIALS AND METHODS: Borrowing concepts from Entity-Attribute-Value and from enterprise data warehousing, the authors designed an alternative called the Dimensional Bus model and used it to integrate electronic medical record, sponsored study, and biorepository data. Each type of observational collection has its own table, and the structure

Feasibility of automated surveillance of implantable devices in orthopaedics via clinical data warehouse: the Studio study.

BACKGROUND: Total hip, knee and shoulder arthroplasties (THKSA) are increasing due to expanding demands in ageing population. Material surveillance is important to prevent severe complications involving implantable medical devices (IMD) by taking appropriate preventive measures. Automating the analysis of patient and IMD features could benefit physicians and public health policies, allowing early issue detection and decision support. The study aimed to demonstrate the feasibility of automated cohorting of patients with a first arthroplasty in two hospital data warehouses (HDW) in France. METH

[HOPIPRAC: Pharmacovigilance interface for detecting cases of adverse events in a hospital data warehouse].

INTRODUCTION: Spontaneous reporting is the reference method for collecting data on adverse drug reactions (ADRs). However, it remains insufficient. Text search in the electronic medical record has opened up a new way of collecting ADRs. However, these methods have certain limitations. Data warehouses constitute a rich digital environment bringing together all the informations in an electronic medical record. The Clinical Data Centre (CDC) at Brest University Hospital has developed a pharmacovigilance tool, the HOPIPRAC interface, which provides autonomy in querying the entire hospital database

Incidence estimates for lichen planopilaris and frontal fibrosing alopecia in a New York City health care system.

Lichen planopilaris (LPP) and frontal fibrosing alopecia (FFA) are scarring alopecias that cause significant distress and psychological morbidity. Limited studies have been performed examining the epidemiology of FFA and LPP. We performed a retrospective case cohort analysis by querying for patients with the ICD 10 code L66.1 (LPP, FFA) between 2015 and 2018 using the Clinical Data Warehouse (CDW) at NewYork-Presbyterian Hospital and Columbia Doctors. We calculated the one-year incidence of LPP/FFA between January 1, 2018 to December 31, 2018 by identifying all patients without a previously re
