Management of infected radicular cyst by surgical decompression.

Treatment of maxillary incisor with an associated cystic lesion by conventional endodontic therapy combined with decompression is reported. Although small cystic lesions frequently heal simply with endodontic therapy, larger lesions may need additional treatment. In some cases, when surgical enucleation is elected, there is a chance for inadvertent injury to adjacent teeth or structures even though periapical surgery has a role in endodontics. Therefore a more conservative approach of decompression and a workable protocol for this was attempted with success and is presented here.

Surgical decompression for lumbar stenosis in pediatric achondroplasia.

BACKGROUND: Spinal stenosis is a common complication of achondroplasia. To our knowledge, no study has evaluated a greater than 2-year outcome after surgical intervention for spinal stenosis in such children or compared decompression with and without instrumentation in relation to revision surgery. Our purpose was to assess the efficacy of lumbar decompression and instrumentation for symptomatic stenosis in children with achondroplasia. METHODS: We retrospectively reviewed our institution's database to identify children (< or =18 y old) with achondroplasia undergoing initial spinal decompress

Results of surgical decompression in chronic tuberculous paraplegia.

The results of surgical decompression in 117 patients with chronic tuberculous paraplegia, defined as paraplegia persisting for more than 3 months, were reviewed. After surgery 90 patients were able to walk. Significant findings at surgery were marked extradural fibrosis with very little caseation and pus. The study showed that paraplegia of greater than 6 months' duration was associated with a poor result. There were no cases of reactivation of the disease in the follow-up period ranging from 1 year to 4 years.

Surgical decompression in spinal Paget's disease: illustrative case.

BACKGROUND: Paget's disease of bone (PDB) is a common bone metabolic pathology in older adults, characterized by mixed osteolytic, osteoblastic, and quiescent periods. Surgical guidelines for PDB involving the spine are not well-defined and are reserved for cases refractory to medical treatments, typically bisphosphonates like zoledronic acid. This case study describes a 52-year-old male with PDB who presented with rapidly progressing myelopathy symptoms refractory to standard medical treatment, warranting surgical decompression. OBSERVATIONS: Surgical decompression and fusion, involving lami

Microvascular decompression: salient surgical principles and technical nuances.

We developed a multiplex PCR (mPCR) assay to simultaneously detect Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma genitalium, Ureaplasma urealyticum, Corynebacterium spp. and seudomona aeruginosa. This method employs a single tube and multiple specific primers which yield 200, 281, 346, 423, 542, and 1,427 bp PCR products, respectively. All the PCR products were easily detected by agarose gel electrophoresis and were sequenced to confirm the specificity of the reactions. To test this method, DNA extracted from urine samples was collected from 96 sexually transmitted disease or prosta

The surgical outcome of decompression alone versus decompression with limited fusion for degenerative lumbar scoliosis.

OBJECTIVE The aim of this study was to assess the usefulness of radiological parameters for surgical decision-making in patients with degenerative lumbar scoliosis (DLS) by comparing the clinical and radiological results after decompression or decompression and fusion surgery. METHODS The authors prospectively planned surgical treatment for 298 patients with degenerative lumbar disease between September 2005 and March 2013. The surgical method used at their institution to address intervertebral instability is precisely defined based on radiological parameters. Among 64 patients with a Cobb ang

[Osteoplastic decompressive craniotomy--indication and surgical technique].

Decompressive craniotomy is usually carried out using decompressive craniectomy (osteoclastic decompressive craniotomy) when the bone flap is removed. In situations when the level of expansion does not call for decomopressive craniectomy, we do not remove the bone flap and we perform osteoplastic decompressive craniotomy. The indication is based on assessment and cross correlation of the following parameters: intracranial pressure, midline shift and the number of pathologies on CT, actual influence of antiedematous therapy, expected cerebral oedema progression and especially according to the si

Surgical options in ICH including decompressive craniectomy.

Intracerebral haemorrhage (ICH) accounts for 15 to 20% of strokes. The condition carries a higher morbidity and mortality than occlusive stroke. Despite considerable research effort, no therapeutic modality either medical or surgical has emerged with clear evidence of benefit other than in rare aneurysmal cases. Intracerebral haemorrhages can be divided into those that arise from pre-existing macroscopic vascular lesions - so called "ictohaemorrhagic lesions", and those that do not; the latter being the commoner. Most of the research that has been done on the benefits of surgery has been in th

Surgical shunts and TIPS for variceal decompression in the 1990s.

BACKGROUND: In the 1990s, liver transplantations and transjugular intrahepatic portosystemic shunts (TIPS) have become the most common methods to decompress portal hypertension. This center has continued to use surgical shunts for variceal bleeding in good-risk patients who continue to bleed through endoscopic and pharmacologic treatment. This article reports this center's experience with surgical shunts and TIPS shunts from 1992 through 1999. METHODS: Sixty-three patients (Child A, 43 patients; Child B, 20 patients) received surgical shunts: distal splenorenal, 54 patients; splenocaval, 4 pa

Microvascular decompression in the surgical management of trigeminal neuralgia.

The pathophysiological mechanisms underlying trigeminal neuralgia are not clearly understood and several therapeutic modalities have been advocated. Microvascular decompression (MVD) is a widely used surgical approach for the decompression of the affected root entry zone (REZ) of the fifth cranial nerve. In this paper, we present our experience based on 32 patients with trigeminal neuralgia who were treated with MVD, and discuss the role of this procedure in the surgical management of trigeminal neuralgia. All patients had typical TN pain and had had previous medical management which was unsuc

Surgical treatment of Graves' ophthalmopathy by decompressive orbitotomy.

Malignant exophthalmos is often not resolvable using only medical or radiation therapy. The pressure on the optic nerve which is generally associated with it evolves rapidly in many cases towards optic atrophy and subsequently loss of vision. Only surgical treatment offers any guarantee of success and of the various techniques proposed, subfrontal orbital decompression seems clearly the more effective, in that it makes for an optimal alleviation of pressure in the optic pathways. The Authors describe their technique, a variation on the original approach, and report the results achieved with it

Role of surgical decompression in ulnar neuritis of leprosy.

The present study was undertaken to evaluate the effect of neurolysis in relieving the pain of acute painful neuritis and assess its effect on the sensory motor deficit. Thirty eight ulnar nerves were operated and followed up for 8 to 125 weeks. Following surgery relief from pain was gratifying. However the pain recurred in cases of lepromatous leprosy in subsequent episodes of ENL reaction. Sensory recovery was appreciable and occurred within 8 weeks. Four cases had 75% recovery; five had 50 to 75% recovery and 11 cases had up to 50% recovery. Motor recovery took a longer time to appear. Prox

Decompressive Craniectomy: Surgical Techniques and Complication Avoidance.

Decompressive craniectomy (DC) is a procedure commonly employed to relieve medically-refractory intracranial hypertension by removing a portion of the skull, thereby allowing for cerebral expansion and reducing the risk of internal herniation and secondary brain injury. This intervention is indicated in patients with severe traumatic brain injury, malignant middle cerebral artery infarction, or other pathologies resulting in elevated intracranial pressure. Although the procedure can be life-saving, it carries risks including hemorrhage, infection, and postoperative hydrocephalus. This review p

Functional Outcomes after Surgical Decompression for Tarsal Tunnel Syndrome.

OBJECTIVES: To evaluate the functional outcomes of surgical decompression for tarsal tunnel syndrome (TTS) and to explore the clinical relevance of diagnostic tests such as electromyography (EMG), magnetic resonance imaging (MRI), and ultrasound. METHODS: We performed a retrospective single-center study of 15 patients with clinically diagnosed TTS who underwent open decompression (2015-2022). All had failed conservative management and completed ≥ 12 months of follow-up. Preoperative evaluation combined clinical exams with at least one confirmatory test (EMG, MRI, or ultrasound). Outcomes incl

Surgical technique management of microvascular decompression for trigeminal neuralgia.

Trigeminal neuralgia is a severe neuropathic disorder, affecting the distribution area of the trigeminal nerve and often impairs the quality of life of patients. More and more scholars agree that one of the pathogenesis of trigeminal neuralgia is due to the demyelinating lesion caused by vascular compression or arachnoid bundle wrapping on the root exit zone of trigeminal nerve. In this regard, the most effective method is microvascular decompression, which can relieve the compression of the offending vessels and the thickened arachnoid on the trigeminal nerve. However, it still has some disad

Surgical decompression for malignant cerebral oedema after ischaemic stroke.

BACKGROUND: Large territory middle cerebral artery (MCA) ischaemic strokes account for around 10% of all ischaemic strokes and have a particularly devastating prognosis when associated with malignant oedema. Progressive cerebral oedema starts developing in the first 24 to 48 hours of stroke ictus with an associated rise in intracranial pressure. The rise in intracranial pressure may eventually overwhelm compensatory mechanisms leading to a cascading secondary damage to surrounding unaffected parenchyma. This downward spiral can rapidly progress to death or severe neurological disability. Early

Imaging appearance following surgical decompression of the ulnar nerve.

Ulnar neuropathy at the elbow is the second most common entrapment neuropathy of the upper extremity. Yet, there is a paucity of literature focusing on the imaging appearance following surgical decompression of the ulnar nerve at the elbow. Diagnostic imaging studies obtained after surgical decompression at The University of Michigan were reviewed and imaging findings were documented. We aim to describe the various techniques of ulnar nerve decompression and corresponding post-operative appearance on imaging. Potential complications following decompression will also be described with imaging a

Surgical Freedom Evaluation During Optic Nerve Decompression: Laboratory Investigation.

BACKGROUND AND OBJECTIVE: Various surgical routes have been used to decompress the intracanalicular optic nerve. Historically, a transcranial corridor was used, but more recently, ventral approaches (endonasal and/or transorbital) have been proposed, individually or in combination. The present study aims to detail and quantify the amount of bony optic canal removal that may be achieved via transcranial, transorbital, and endonasal pathways. In addition, the surgical freedom of each approach was analyzed. METHODS: In 10 cadaveric specimens (20 canals), optic canals were decompressed via pterio
