Inner ear decompression sickness following altitude chamber operation.

Decompression sickness (DCS) is a known hazard of altitude chamber operation. The musculoskeletal, dermal, neurological and pulmonary manifestations of DCS are well recognized, but inner ear injury has not been reported. We present the unusual case of a medical corpsman suffering from vestibular DCS after an altitude chamber exposure to 25,000 ft. The patient had a good clinical response to hyperbaric treatment, but there was laboratory evidence of mild residual vestibular damage with full compensation. This case suggests that aviation medical personnel should be more aware of the possible occ

Prevalence of decompression sickness among U-2 pilots.

BACKGROUND: Though it is rarely reported, decompression sickness (DCS) is an expected risk for U-2 aviators. The potential for chronic sequelae of untreated DCS in this population has never been addressed. METHODS: After conducting a preliminary survey at an active-duty U-2 squadron, a cohort of 416 U-2 pilots (active-duty and retired) were mailed two sequential anonymous surveys to assess demographic data, career prevalence of DCS symptoms, and overall health status with an emphasis on chronic musculoskeletal problems. RESULTS: The response rate for each mail-in survey was over 60%. During

Decompression sickness during construction of the Dartford Tunnel.

A clinical, radiological and statistical survey has been made of decompression sickness during the construction of the Dartford Tunnel. Over a period of two years, 1,200 men were employed on eight-hour shifts at pressures up to 28 pounds per square inch (p.s.i.). There were 689 cases of decompression sickness out of 122,000 compressions, an incidence of 0·56%. The majority of cases (94·9%) were simple “bends”. The remainder (5·1%) exhibited signs and symptoms other than pain and were more serious. All cases were successfully treated and no fatality or permanent disability occurred. In two seri

Saturation Diving Decompression Treatment for Severe Type II Decompression Sickness Following Emergency Ascent.

INTRODUCTION: Decompression sickness (DCS) is a critical concern in diving operations. Severe type II DCS, characterized by neurological involvement, is particularly disabling and life - threatening. Traditional decompression treatments often show limitations in severe cases. This case report highlights the effectiveness of saturation diving decompression in the treatment of such severe DCS. CASE REPORT: A 33-year-old male diver experienced severe type II decompression sickness (DCS) following an emergency ascent from a depth of 56 meters of seawater (msw) due to an underwater explosion. The

Circulating Bubbles in Decompression Sickness: Pathophysiology and Experimental Models.

Decompression sickness (DCS) is a severe condition caused by rapid reductions in ambient pressure during diving, altitude exposure, or aerospace operations, with circulating gas bubbles serving as a central initiating event. Stable nanobubbles on active hydrophobic spots along the vascular lumen can grow under decompression-induced supersaturation, detach, and enter the circulation. Their subsequent behavior is shaped by blood flow, vessel geometry, and coagulation and may lead to thrombosis and microvascular stasis. DCS pathogenesis involves three linked interfaces: the blood-bubble interface

[Progress in diagnosis and treatment of occupational decompression sickness].

Occupational decompression sickness is one of the occupational diseases caused by high pressure work. The pathogenesis and pathological changes of decompression sickness are related to the formation of bubbles inside and outside blood vessels and in tissues. The diagnosis of occupational decompression sickness includes acute decompression sickness and decompression osteonecrosis. Corresponding treatment measures should be taken according to the symptoms, signs and severity of decompression disease. In this paper, the incidence, diagnosis and treatment of decompression sickness were reviewed to

Clinical Significance of Mottling Rashes in Diving Decompression Sickness.

INTRODUCTION: Decompression sickness (DCS) is a medical condition caused by outgassing of dissolved nitrogen following rapid ascent by divers and aviators. Cutaneous DCS, historically termed cutis marmorata (CM), presents as a predominantly truncal reticular violaceous-to-dusky eruption. The prevailing theories for its pathogenesis include: localized cutaneous outgassing, paradoxical embolism across a right-to-left shunt (RLS), and brainstem emboli disrupting autonomic control of cutaneous microcirculation.METHODS: We conducted a systematic review of reports of cutaneous DCS to investigate rel

Severe acute kidney injury caused by decompression sickness syndrome.

Decompression sickness (DCS) occurs when divers are exposed to reduced barometric pressure during their ascent from depth. We report a case of DCS causing severe acute kidney injury (AKI) after an uneventful dive in which all decompression stops were made as instructed by a dive computer. A 26-year-old man presented with abdominal and bilateral flank pain ~ 24 hours after scuba diving to a depth of 23 m. Vitals and physical exam were unremarkable. Lab results revealed elevated serum creatinine at 2.3 mg/dL from a normal baseline and elevated blood urea nitrogen at 23 mg/dL. The patient was non

Inner-ear decompression sickness: 'hubble-bubble' without brain trouble?

Inner-ear decompression sickness (DCS) is an incompletely understood and increasingly recognized condition in compressed-air divers. Previous reports show a high association of inner-ear DCS with persistent foramen ovale (PFO), suggesting that a moderate-to-severe right-to-left shunt might represent a major predisposing factor, and more properly defining it as an event from arterial gas embolism (AGE). However, other conditions characterized by bubbles entering the arterial circulation, such as open-chamber cardiac surgery, do not produce inner-ear involvement, while sometimes damaging the brai

The initial signs and symptoms of altitude decompression sickness.

BACKGROUND: With the potential for higher aircraft and cabin altitudes, the way in which altitude decompression sickness (DCS) presents continues to be of interest. The majority of previous papers on the symptomatology of DCS are retrospective reviews of patients treated hours or days post-exposure. The initial presentation while still at altitude is the form of DCS that aircrew must be able to recognize in order to respond correctly. This paper reports the initial manifestations of DCS that occurred during a series of prospective hypobaric chamber studies. These studies had been specifically

Cardiac decompression sickness: report and discussion of a case.

A case of first degree atrio-ventricular block, probably representing cardiac involvement by decompression sickness, is presented. The conduction defect resolved spontaneously 36 h after the initiating decompression insult, and was not accompanied by any other cardiovascular changes. The contribution of a recompression treatment, which alleviated accompanying Type I decompression sickness (DCS) symptoms, to the resolution of cardiac DCS is not certain. Cardiac symptoms of DCS do not receive enough consideration. It is suggested that an electrocardiogram should, whenever possible, form part of

[Blood coagulation processes in decompression sickness and hyperbaric therapy].

The hyperaggregability of platelets is remarkably important in the pathogenesis of decompression sickness. The basis of this phenomenon might consist of an excessive production of metabolites of arachidonic acid (C 20:4) whose action favours aggregation (prostaglandin endoperoxides PGG2 and PGH2 and Tromboxane A2) in respect of the synthesis of its derivatives exerting an antithrombotic action (prostacyclin I2). The antiaggregating therapy usually associated to the hyperbaric treatment involves administration of acetylsalicylic acid in low doses (3.5-5 mg/kg every three days), associated if ne

Tissue-specific gradient factors to address undeserved decompression sickness.

BACKGROUND: "Undeserved hit" is a term used to define a case of decompression sickness (DCS) when the diver has adhered to the limits set by a dive computer. In a recent study, Divers Alert Network (DAN) Europe has analyzed over 39,000 dives with 295 undeserved hits. These 295 dives, and an additional 22 that were borderline or above accepted safe limits of decompression, were processed to determine the inert gas load at the end of the dive, expressed in terms of gradient factors (GF) in a ZH-L16C framework. The data show that DCS was observed with GF values as low as 70, but mostly involved t

Effect of angiotensin II on irradiation exacerbated decompression sickness.

In some complicated situations, decompression sickness (DCS) combined with other injuries, such as irradiation, will seriously endanger life safety. However, it is still unclear whether irradiation will increase the incidence of DCS. This study was designed to investigate the damage effects of irradiation on decompression injury and the underlying mechanism. Sprague-Dawley rats were exposed to irradiation followed by hyperbaric decompressing and the mortality and decompression symptoms were observed. Lung tissue and bronchoalveolar lavage fluid were collected to detect the lung lesion, inflamm

Patent Foramen Ovale Closure and Decompression Sickness Among Divers.

BACKGROUND: Decompression sickness is a diving-related disease that results in various clinical manifestations, ranging from joint pain to severe pulmonary and CNS affection. Complications of this disease may sometimes persist even after treatment with hyperbaric oxygen therapy. In addition, it may hamper the quality of life by forcing divers to restrict their recreational practice. The presence of a patent foramen ovale (PFO) increases the risk of decompression sickness by facilitating air embolization. Therefore, PFO closure may play a role in reducing such complications. However, PFO closur

A case of Löfgren's syndrome confused with decompression sickness.

A broad differential diagnosis is important to provide appropriate care. This may be challenging for conditions like decompression sickness (DCS) which can be easily confused with other conditions. In suspected DCS, treatment may be an important part of the diagnosis. An improvement in symptoms after hyperbaric oxygen treatment (HBOT) is consistent with a DCS event. However, HBOT may also impact symptoms in other conditions, including Löfgren's syndrome (LS). LS, a poorly understood, clinically distinct phenotype of sarcoidosis, is a complex, multi-system granulomatous inflammatory condition.

Health care worker decompression sickness: incidence, risk and mitigation.

Inadvertent exposure to radiation, chemical agents and biological factors are well recognized hazards associated with the health care delivery system. Less well appreciated yet no less harmful is risk of decompression sickness in those who accompany patients as inside attendants (IAs) during provision of hyperbaric oxygen therapy. Unlike the above hazards where avoidance is practiced, IA exposure to decompression sickness risk is unavoidable. While overall incidence is low, when calculated as number of cases over number of exposures or potential for a case during any given exposure, employee c

Inner-ear decompression sickness in nine trimix recreational divers.

INTRODUCTION: Recreational technical diving, including the use of helium-based mixes (trimix) and the experimentation of new decompression algorithms, has become increasingly popular. Inner-ear decompression sickness (DCS) can occur as an isolated clinical entity or as part of a multi-organ presentation in this population. Physiological characteristics of the inner ear make it selectively vulnerable to DCS. The inner ear has a slower gas washout than the brain thus potentially making it more vulnerable to deleterious effects of any bubbles that cross a persistent foramen ovale (PFO) and enter
