Complete Blood Count in Delayed Graft Function: A Narrative Review.

Delayed graft function (DGF) is a common and critical complication following kidney transplantation, marked by acute kidney injury necessitating dialysis within the first postoperative week. Early detection of patients at risk is crucial for optimizing perioperative management and enhancing graft outcomes. While novel biomarkers have been suggested, their clinical application remains limited. This review explores the potential of complete blood count (CBC) parameters and derived indices as cost-effective, accessible alternatives. This narrative review synthesizes findings from studies examinin

Underappreciated Effects of Delayed Graft Function in Pediatric Kidney Transplantation.

PURPOSE: In kidney transplantation (KT), delayed graft function (DGF) is associated with worse outcomes. However, it is unclear what effect DGF plays in long-term survival compared to the impact of the various transplant, donor, and recipient risk factors associated with DGF. This study aims to determine the effect of DGF alone on long-term survival in pediatric deceased donor kidney transplant recipients (DDKTRs). METHODS: Using a national database, 1728 pediatric patients who received a kidney transplant between 1994 and 2009 (n = 978) and 2010 and 2022 (n = 750) were identified. Cases and

Mitochondrial membrane potential and delayed graft function following kidney transplantation.

Delayed graft function (DGF) complicates 20%-40% of deceased-donor kidney transplants and is associated with increased length of stay and subsequent allograft failure. Accurate prediction of DGF risk for a particular allograft could influence organ allocation, patient counseling, and postoperative planning. Mitochondrial dysfunction, a reported surrogate of tissue health in ischemia-perfusion injury, might also be a surrogate for tissue health after organ transplantation. To understand the potential of mitochondrial membrane potential (MMP) in clinical decision-making, we analyzed whether lowe

Delayed Graft Function Phenotypes and 12-Month Kidney Transplant Outcomes.

BACKGROUND: Ischemia-reperfusion injury (IRI) leading to delayed graft function (DGF), defined by the United Network for Organ Sharing as dialysis in the first week (UNOS-DGF), associates with poor kidney transplant outcomes. Controversies remain, however, about dialysis initiation thresholds and the utility for other criteria to denote less severe IRI, or slow graft function (SGF). METHODS: Multicenter, prospective study of deceased-donor kidney recipients to compare UNOS-DGF to a definition that combines impaired creatinine reduction in the first 48 hours or greater than 1 dialysis session

Highly sensitized patients with delayed graft function: a management protocol.

The postoperative management of the highly sensitized renal transplant recipient with delayed graft function on cyclosporine is complicated. Allografts with delayed graft function are reported to have a 20 to 30% poorer 1-year survival than allografts without delayed graft function. Several factors may be implicated in this poorer 1-year survival. A decrease in or cessation of cyclosporine dosage frequently is used in an attempt to minimize nephrotoxicity. Such under-immunosuppression can result in irreversible rejection. Occasionally, a pessimistic view of the prognosis for the transplant may

Effect of prolonged delayed graft function on long-term graft outcome in cadaveric kidney transplantation.

One-hundred-and-twenty-six cadaveric renal transplantations were analyzed for the effect of post-transplant renal function on graft outcome. Thirty-one grafts functioned immediately after transplantation (Group 1), 54 grafts failed to function temporarily up to 8 posttransplant days as evidenced by urine output < 400 ml/day or hemodialysis support (Group 2), 33 grafts showed temporary nonfunction > 8 days (Group 3) and 8 grafts never functioned (group 4). Five-year graft failure rates were 6.5% (2/31), 13.0% (7/54), 42.4% (14/33) and 100% (8/8) for Groups 1, 2, 3 and 4, respectively. Actuarial

Preoperative Nutritional Indices Predicting Delayed Graft Function in Pediatric Kidney Transplantation.

OBJECTIVES: The Prognostic Nutritional Index, which combines serum albumin and lymphocyte count, captures protein reserves and immune competence in a single, easily obtained metric. Whether this index can predict delayed graft function among pediatric kidney transplant recipients has not been thoroughly evaluated. Here, we examined whether use of this index pretransplant can predict delayed graft function in this population. MATERIALS AND METHODS: We reviewed children (aged 1 -18 years )who received kidney transplants at Başkent University Hospital from January 2013 to December 2024. Patients

Effects of Delayed Graft Function on Transplant Outcomes: A Meta-analysis.

Delayed graft function (DGF) is a frequent complication of kidney transplantation, but its impact on long- and short-term transplant outcomes is unclear. We conducted a systematic literature search for studies published from 2007 to 2020 investigating the association between DGF and posttransplant outcomes. Forest plots stratified between center studies and registry studies were created with pooled odds ratios. Posttransplant outcomes including graft failure, acute rejection, patient mortality, and kidney function were analyzed. Of the 3422 articles reviewed, 38 papers were included in this me

Evaluation of severity of delayed graft function in kidney transplant recipients.

BACKGROUND: The most common definition of delayed graft function (DGF) relies on dialysis during the first week post-transplant and does not consider DGF severity. The impact of DGF severity on long-term graft outcome remains controversial. METHODS: We analysed 627 deceased-donor kidney transplant recipients (KTRs) transplanted in 2005-2015 at our centre for DGF severity, associated risk factors and long-term consequences of DGF. RESULTS: We found 349 (55.7%) KTRs with DGF, which were classified into four groups according to DGF duration (0-1, 2-7, 8-14, >14 days) and were compared with KTR

Renal PGC1α May Be Associated with Recovery after Delayed Graft Function.

BACKGROUND: Delayed renal graft function (DGF) contributes to the determination of length of hospitalization, risk of acute rejection, and graft loss. Existing tools aid the diagnosis of specific DGF etiologies such as antibody-mediated rejection, but markers of recovery have been elusive. The peroxisome proliferator gamma co-activator-1-alpha (PGC1α) is highly expressed in the renal tubule, regulates mitochondrial biogenesis, and promotes recovery from experimental acute kidney injury. OBJECTIVES: We aimed to determine the association between renal allograft PGC1α expression and recovery fro

Delayed graft function in living-donor renal transplantation: 10-year experience.

BACKGROUND: Delayed graft function (DGF), a dialysis requirement within a week after transplantation, can occur in deceased-donor renal transplantation. DGF is rare, in living-donor renal transplantation (LDRT) and its incidence and risk factors have not been established. METHODS: We investigated the incidence and clinical characteristics of DGF in LDRT over 10 years. We compared HLA mismatches, panel reactive antibody status, frequency of nonrelated donors, donor age, sex match, recipient-donor body weight ratio, total ischemia time, and transplanted kidney weight between DGF and non-DGF pat

Inhibition of USAG-1 improved delayed graft function in renal transplantation.

Delayed graft function (DGF) is a critical complication following kidney transplantation. This study aimed to identify novel biomarkers and therapeutic targets for DGF. Transcriptomic data from the Gene Expression Omnibus (GEO) database were analyzed to screen for DGF-associated core genes. Serum levels of uterine sensitization-associated gene-1 (USAG-1) and kidney injury molecule-1 (KIM-1) were measured one day post-transplantation, and their predictive values for DGF were compared. Mouse models of DGF were established by inducing graded warm ischemia. USAG-1 was identified as a core gene sig

Blessing and a curse of outpatient management of delayed graft function.

Delayed graft function (DGF) is a common complication occurring most often after deceased donor kidney transplant with several donor characteristics as well as immunologic factors that lead to its development post-transplant. These patients require dialysis and close kidney function monitoring until sufficient allograft function is achieved. This has resulted in limited options for DGF management, either prolonged hospitalization until graft function improves to the point where dialysis is no longer needed or discharge back to their home dialysis unit with periodic follow up in the transplant

Diagnosis and treatment of hyper-delayed graft function after renal transplantation.

BACKGROUND: Renal transplant recipients may experience delayed graft function (DGF), but recovery can take many months, a condition we define as hyper-delayed graft function (HDGF). METHODS: A retrospective review of 50 renal transplant recipients who had HDGF and comparison with patients who had immediate graft function (IGF) and DGF. RESULTS: Acute renal tube necrosis (ATN) during or soon after surgery was the most common cause of HDGF. Following standard treatment, 48 HDGF patients transitioned from oliguria to polyuria in 45 days (± 3) and renal function of the kidney fully recovered in

Factors Associated with Delayed Graft Function and Renal Graft Survival from Donors after Controlled Cardiac Death.

BACKGROUND: Delayed graft function is a common situation that leads to increased long-term rates of graft rejection and loss. It is seen increasingly more often, as the use of kidneys from donors after controlled cardiac death has become more widespread. This study aimed to identify factors contributing to its onset and determine how these factors may influence graft survival. METHODS: This study analysed kidney transplants with grafts from donors after controlled cardiac death performed at our hospital since the start of program in March 2013 until December 2023. Data on delayed graft functi

Mitochondrial subtypes in renal ischemia reperfusion injury guide delayed graft function and Long-Term graft prediction.

Background Ischemia reperfusion injury (IRI) in kidney transplantation (KTx) is closely associated with acute rejection, delayed graft function (DGF), and graft failure. Evidence highlights the significant correlation between mitochondrial dysfunction and IRI. However, there is a scarcity of predictive models for DGF and long-term graft survival specifically focused on mitochondrial-related genes (MGs). Methods RNA-seq and Microarray datasets from the GEO database were utilized. Differential expression analysis identified differentially expressed MGs (DE-MGs), and consensus clustering analysis
