[Subanaesthetic doses of ketamine for obstetric delivery (author's transl)].

Twenty five parturients were anaesthetized by administration of 0.3 mg ketamine per kg body weight intravenously before emergence of the head of the newborn infant. 20 patients had complete amnesia for the time of delivery, but most of them remained cooperative, nevertheless. Of the other 5 patients, 3 recounted that they recalled the birth of their child like remembering a dream, but they did not remember pain. The method proved a failure in the case of two patients. The main advantage of this technique lies in keeping most of the parturients cooperative and able to help with pushing, while o

Impact of vaginism on mode of delivery and obstetrical outcomes.

INTRODUCTION: According to ICD 10, vaginism is defined as the presence of spasm of the pelvic floor muscles surrounding the vagina, causing occlusion of the vaginal opening. Intromission is impossible or painful. Vaginism, whose prevalence is estimated at 1%, is often diagnosed during pregnancy follow-up, when clinical examination is difficult or impossible. However, few studies report obstetrical complications and pregnancy outcomes associated with this pathology. OBJECTIVE: Evaluate the impact of vaginism on mode of delivery and obstetrical complications. MATERIALS AND METHODS: Comparative

Is There Still a Place for Forceps Delivery in Modern Obstetrics?

BACKGROUND: Nowadays, we are witnessing a decrease of vaginal instrumental deliveries and continuous increase of caesarean section rate. However, proper identification of possibility of execution, indications for instrumental delivery and their skilful use may improve the broadly understood maternal and neonatal outcomes. The aim of this study is to present prevalence, risk factors, indications and outcomes of forceps deliveries among the patients at Department of Perinatology, Lodz. MATERIAL AND METHODS: A retrospective study was conducted at the Department of Perinatology, Medical Universit

Obstetrical interventions for term first deliveries in the US.

BACKGROUND: Labour induction and caesarean are increasingly done without clinical indication. However, little is known about the prevalence of such interventions, or the characteristics of women who receive them. We used the 2003 revision of the US birth certificates to summarise recorded interventions and to characterise maternal profiles associated with such interventions. METHODS: We carried out a retrospective study of 2.35 million primiparous women delivering singleton live births at 37-44 weeks in the US from 2005 to 2008. We used the 2003 revision of the birth certificate to define del

A system for the delivery of remote obstetrical care by telepresence.

This paper describes the development of a system to provide obstetrical care to remote and/or medically underserved areas. The ROC project is an attempt to develop new telemedicine hardware and software which will enable the provision of high quality obstetric care to individuals who are normally denied access to this care by reasons of distance, handicap, economics, or other causes. As broadband data communications links and increasingly powerful and affordable computing resources become widely available, the barriers of distance and economics will no longer prevent the provision of this care

Elective delivery of the term fetus. An obstetrical hazard.

Of 1,020 consecutive admissions to a regional neonatal center, 38 infants were admitted following elective delivery in which no medical condition of the mother or fetus had necessitated immediate delivery. Twenty infants had problems that were not primarily the result of elective delivery, developed in 18. Fifteen of these 18 infants were delivered by cesarean section; in none of the mothers had any assessment of fetal maturity or size (other than the menstrual history and physical examination) been made. The infants remained in the hospital for an average of 12.7 days at a median cost of $2,6

Operative vaginal delivery: current trends in obstetrics.

After centuries of use in obstetrics, have forceps and vacuum deliveries become a dying art? Contemporary trends in operative vaginal delivery show increasing numbers of vacuum deliveries and decreasing numbers of forceps deliveries worldwide. Primary drivers of such trends include concerns over neonatal and maternal safety as well as fewer clinicians skilled in forcep use. Current literature reports a comparable efficacy rate for the two instruments, as well as a decrease in maternal morbidity compared with cesarean section. It has also been suggested that the neonatal morbidity once associat

[The influence of delivery methods for twins on obstetric results].

The analysis covered 100 twin gestations over the years 1996-1998 in the material of Gynecology and Obstetrics Clinic in Bydgoszcz. The authors estimated the relations between the mode of delivery and presentations of the twins. Abnormal foetal presentation occurred in 68 pregnancies (68%), and was more often in the second twin (61.8%). The highest percentage of cesarean sections (29%) we have observed in the case of an abnormal presentation of the first twin. The condition of the twins in their first minute after the birth was estimated according to the birth weight, the gestational age, and

Obstetric brachial plexus injury in subsequent deliveries.

It is well known that multiparas who had previously been delivered of newborns with obstetric brachial plexus palsy are at risk of having the same complication in subsequent deliveries. However, none of the studies in the literature have actually attempted to estimate how high is that risk. The aims of the current paper were to investigate obstetric brachial plexus injury in subsequent deliveries and to study other factors associated with birth palsy, perinatal morbidity, mortality, and motor recovery of the brachial plexus. We reviewed retrospectively the records of 28 consecutive multiparas

Mode of Subsequent Delivery After Obstetric Anal Sphincter Injuries (OASIs).

INTRODUCTION AND HYPOTHESIS: Obstetric anal sphincter injuries (OASIs) are severe perineal lacerations, which have a significant impact on quality of life. The rate of OASIs has been increasing worldwide due to improved awareness, recognition, and reporting. When these women conceive again, they are faced with a dilemma regarding the most appropriate mode of delivery. Although counselling about mode of delivery guides women's decisions, there is a large variation in clinical practice and advice provided to women worldwide. To date, limited literature is available to guide counselling for an op

Maternal morbidity in postpartum severe preeclampsia by obstetric delivery volume.

BACKGROUND: Pre-eclampsia is a leading cause of maternal morbidity and mortality in the United States. Emerging data suggests that postpartum pre-eclampsia may be associated with a higher incidence of maternal morbidity compared to hypertensive disorders of pregnancy (HDP) diagnosed antenatally. Understanding postpartum maternal risk across facilities with a spectrum of obstetric services is critical with the rising rates of pre-eclampsia in all healthcare settings. OBJECTIVE: We investigated the relationship between facility delivery volume and rates of nontransfusion severe maternal morbidi

Delivery trends and obstetric outcomes in patients with Fontan circulation.

BACKGROUND: With improved therapies, an increasing number of patients with Fontan circulation reach reproductive age. Pregnant patients with Fontan circulation are at high risk of obstetrical complications. Most data for pregnancies complicated by Fontan circulation and associated complications stem from single-center studies, with limited national epidemiologic data available. OBJECTIVE: This study aimed to evaluate temporal trends in deliveries to pregnant individuals with Fontan palliation using nationwide data and to estimate associated obstetrical complications among these deliveries. Association between day of delivery and obstetric outcomes: observational study.

STUDY QUESTION: What is the association between day of delivery and measures of quality and safety of maternity services, particularly comparing weekend with weekday performance? METHODS: This observational study examined outcomes for maternal and neonatal records (1,332,835 deliveries and 1,349,599 births between 1 April 2010 and 31 March 2012) within the nationwide administrative dataset for English National Health Service hospitals by day of the week. Groups were defined by day of admission (for maternal indicators) or delivery (for neonatal indicators) rather than by day of complication.
