[Parkinson disease with dementia--responsible locus for dementia].

Responsible locus for dementia in Parkinson disease (PD) was investigated. Serial 1,395 autopsy cases were studied for the combined pathology of PD and Alzheimer disease (AD). Following the one-year rule by the first Consensus Guidelines, definite AD pathology was quite rare in PD with dementia (PDD) but common in dementia with Lewy bodies (DLB) . Plaque-dominant senile changes apparently enhanced neocortical Lewy-body pathology in both the conditions. About the hypometabolism in the visual cortex of PDD, a 66-year old man presented with fluctuation in hallucination commensurate with fluctuati

Mixed dementia--the most common cause of dementia.

The existence of vascular dementia (VaD) was first identified by Marie, who described the etat lacunaire, and by Binswanger, who identified white matter lesions. The role of cortical lesions, while clear to clinicians, was highlighted only later by the pathological studies of Tomlinson and the clinical demonstrations of Hachinski et al., who have defined multi-infarct dementia. Lately, the emphasis shifted to pathogenic mechanisms with the identification of a multitude of processes, including lipohyalinosis, cardiac dysfunction, and genetic causes, to name only a few. Epidemiologic studies hav

Reversible dementia: subclinical seizure in early-onset dementia.

We report a case of early-onset dementia with subclinical seizures. Aggressive seizure control improved the patient's cognition. Commonly, an EEG is only performed following overt behavioral seizures. Therefore, subclinical seizures tend to be underdiagnosed. Serial or extended EEG should be seriously considered in patients with early-onset dementia.

[S3 guidelines on dementia. Symptomatic therapy of dementia].

Current treatment of Alzheimer's disease comprises pharmacological therapy and psychosocial interventions for patients and caregivers in the context of a symptom and severity dependent management concept. Treatment is targeted towards the core symptoms of dementia (cognitive and functional deficits) and if necessary, towards the behavioral symptoms of dementia. The treatment of Alzheimer's dementia with acetylcholine esterase inhibitors (AChE-I; donepezil, galantamine, rivastigmine) and memantine is evidence-based and recommended. For all drugs, the highest tolerable dose should be given. The

Dementia of frontal type and dementias with subcortical gliosis.

The group of Frontotemporal dementias (FTD) is composed of non-Alzheimer forms of dementia characterized clinically by behavioural and personality change leading to apathy and mutism. The disorder is associated with a progressive atrophy of the frontal, anterior temporal and anterior parietal lobes of the brain with several types of underlying pathology. One type (frontal lobe degeneration) is characterized by a microvacuolar degeneration of the outer cortical laminae along with a mild and mainly subpial gliosis and a loss of nerve cells, mostly from layers II and III. Another type shows trans

Distinctions between the dementia in amyotrophic lateral sclerosis with frontotemporal dementia and the dementia of Alzheimer's disease.

The clinical entity of Amyotrophic Lateral Sclerosis with Frontotemporal Dementia (ALS-FTD) has only recently been recognized as an important neurodegenerative disease. As in isolated FTD, the behavioral and personality changes in ALS-FTD might be more characteristic than its cognitive changes. We aimed to characterize the behavioral and cognitive deficits in ALS-FTD, and contrast this profile with that of the most common form of dementia, AD, to assist ALS clinicians in recognizing the syndrome early in its course. Specifically, we hypothesized that a modified version of the Frontal Behaviora

[Cardiogenic dementia].

Generally, when discussing vascular diseases of the brain, practitioners refer to diseases of the large (cervicocephalic) and small (perforating) arteries, requiring a differentiated approach to diagnosis, treatment, and prevention. The anatomical and functional unity of the cardiovascular system makes this approach quite arbitrary. Unfortunately, it often escapes attention that the most important component of the cardiovascular system (heart) and its diseases play an important, often dominant role in the development of cerebrovascular disorders, including cognitive impairment and dementia. Frontotemporal dementia.

Frontotemporal dementia (FTD) is the second commonest cause of young onset dementia. Our understanding of FTD and its related syndromes has advanced significantly in recent years. Among the most prominent areas of progress is the overlap between FTD, MND, and other neurodegenerative conditions at a clinicopathologic and genetic level. In parallel major advances in neuroimaging techniques, the discovery of new genetic mutations as well as the development of potential biomarkers may serve to further expand knowledge of the biologic processes at play in FTD and may in turn propel research toward

Frontotemporal dementia.

Frontotemporal dementia is an umbrella clinical term that encompasses a group of neurodegenerative diseases characterised by progressive deficits in behaviour, executive function, or language. Frontotemporal dementia is a common type of dementia, particularly in patients younger than 65 years. The disease can mimic many psychiatric disorders because of the prominent behavioural features. Various underlying neuropathological entities lead to the frontotemporal dementia clinical phenotype, all of which are characterised by the selective degeneration of the frontal and temporal cortices. Frontotemporal dementia.

Frontotemporal dementia (FTD) encompasses several clinical syndromes that involve a progressive change in behavior and/or language; it is more common than Alzheimer's disease in early-onset dementia under the age of 60 years. In the behavioral variant of FTD (bvFTD) patients have social and emotional changes with prominent disinhibition, apathy, lack of empathy, changes in diet, and repetitive behaviors. Motor neuron disease or parkinsonism are seen in association with bvFTD. Frontal and/or temporal atrophy are often seen on structural brain imaging. Several pathological entities can cause bvF

[Vascular dementia].

The increasing prevalence of dementia with age, combined with the rapid aging of the population, heralds an oncoming epidemic of dementia in industrialized countries. The current prevalence of dementia over 65 years of age is close to 7% (2/3 Alzheimer's disease and 1/3 vascular dementia). Vascular dementia refers to a severe cognitive decline due to brain lesions of vascular origin. It is in fact a heterogeneous syndrome with multiple etiopathogenic varieties such as subcortical small artery disease, isolated strategic stroke, multiple infarcts, chronic cerebral hypoperfusion, and amyloid ang

[Frontotemporal dementia].

BACKGROUND: Among the primary degenerative dementias, frontotemporal dementia (FTD, Pick's disease) is very important along with Alzheimer's disease. The estimated prevalence is 15:100,000 in the 45- to 64-year-old population; thus, it appears that the FTD as a cause for so-called presenile dementia is nearly as common as Alzheimer's disease. CASE REPORT: The case of a 52-year-old woman is described that presented with slowly progressive lack of concentration and disturbance of memory. Furthermore, the immediate family had noticed a change in her premorbid personality. Due to additional depre

[Vascular dementia].

Vascular dementia is one of the most frequently occurring dementia syndromes. Its prevalence is about 5% among subjects above 85 years of age. Elevated blood pressure and atherosclerosis are the most important risk factors. According to international criteria, vascular dementia usually occurs within three months after having a stroke. However, the diagnosis can be difficult as some strokes are clinically 'silent' and may go unnoticed. Other factors may also contribute to the dementia syndrome, and concomitant depression may mask its clinical picture. In population studies, treatment of vascula

Vascular dementia.

Many cases of age-related cognitive dementia are caused by cerebrovascular lesions, and various vascular syndromes can lead to cognitive impairment and dementia. Repeated cortical infarcts due to embolic disease of the heart or major cerebral vessels can cause progressive deterioration towards dementia and incapacitation. In classic multi-infarct dementia, cognitive deterioration is stepwise rather than smoothly progressive. While diagnostic technologies have vastly improved and added to general knowledge of the pathology of cerebrovascular disease, MRI, PET, and transcranial Doppler scans hav

Frontotemporal dementia.

BACKGROUND: Frontotemporal dementia accounts for up to 20% of cases of dementia in the presenium, yet remains poorly recognised. Diagnostic criteria have been devised to aid clinical diagnosis. AIMS: To provide an overview of clinical and pathological characteristics of frontotemporal dementia and its nosological status. METHODS: The review summarises consensus diagnostic criteria for frontotemporal dementia and draws on the authors' clinical experience of 300 frontotemporal dementia cases, and pathological experience of 50 autopsied cases. RESULTS: Frontotemporal dementia is characterised

[Frontotemporal dementia].

Defined barely one decade ago, Frontotemporal Dementia emerges as a new clinical entity that reestablishes the classical Pick's disease as part of a extensive syndrome with important and unsuspected prevalence. Frontotemporal Dementia includes all primary degenerative processes starting in the anterior portions of the brain. This type of dementia is clinically characterized by behavior and personality disorders, more than cognitive alterations. In the present research we performed a bibliographic review of the subject including clinical characteristics, laboratory tests, and neuropathology. Fu
