Molecular and neurovascular mechanisms of thermal sensitivity in teeth.

The perception of dental pain and thermal stimuli is governed by the specialized structural and physiological organization of the dentin - pulp complex. Teeth are subjected to rapid thermal changes during normal oral functions, while the dental pulp - an intensely vascularized and innervated tissue - is confined within rigid mineralized structures. Under these anatomical conditions, even mild thermal stimuli - especially cold - can induce rapid and intense pain sensations. The mechanisms underlying thermal sensitivity of dentin have been explained by several theories. The direct neural theory

Topical review. Dental pain and odontoblasts: facts and hypotheses.

Dental pain arises from exposed dentin following bacterial, chemical, or mechanical erosion of enamel and/or recession of gingiva. Thus, dentin tissue and more specifically patent dentinal tubules represent the first structure involved in dentin sensitivity. Interestingly, the architecture of dentin could allow for the transfer of information to the underlying dental pulp via odontoblasts (dentin-forming cells), via their apical extension bathed in the dentinal fluid running in the tubules, or via a dense network of trigeminal sensory axons intimately related to odontoblasts. Therefore, extern

Development and validation of an in vitro model for measurements of cervical root dentine permeability.

OBJECTIVES: The aim of this series of studies was the development and validation of a new model for evaluation of dentinal hypersensitivity (DH) therapies. MATERIALS AND METHODS: Roots from extracted human teeth were sealed with a flowable composite. In the cervical area, a 3-mm-wide circular window was ground through the seal 1 mm deep into dentine. The pulp lumen was connected to a reservoir of artificial dentinal fluid (ADF) containing protein, mineral salts and methylene blue. At increased pulpal pressure, the ADF released through the said window was collected in containers each with 20 m

Odontoblast-mediated regulation of the progression of dentinal caries.

Osteoblasts and odontoblasts are both derived from the same mesenchymal cell line. Our aim was to investigate whether the processes of bone destruction and dentinal caries are biologically similar. The working hypothesis was that after the initiation of caries in the enamel, its rate of progression in the dentine is regulated by cell-mediated factors. Experimental caries was induced in the rat with a high sucrose diet combined with Streptococcus sobrinus infection. Both destruction of dentine and its apposition in the pulp under the carious lesions were measured after vital staining with tetra

Dentinal hypersensitivity: a review.

Dentinal hypersensitivity is generally reported by the patient after experiencing a sharp pain caused by one of several different stimuli. The pain response varies substantially from one person to another. The condition generally involves the facial surfaces of teeth near the cervical aspect and is very common in premolars and canines. The most widely accepted theory of how the pain occurs is Brannstrom's hydrodynamic theory, fluid movement within the dentinal tubules. The dental professional, using a variety of diagnostic techniques, will discern the condition from other conditions that may c

Patterns of fluoro-gold entry into rat molar enamel, dentin, and pulp.

Permeabilities of enamel and dentin are not fully understood despite their importance for caries, restorative materials, and pulp-dentin-enamel interactions. We have found that Fluoro-Gold is useful for examining tooth permeability, and we designed studies to test the effects of aging, injury, neural function, and dentinal repair on its influx into vital rat teeth. We used fluorescence microscopy and immunocytochemistry to show that Fluoro-Gold rapidly penetrates enamel, the dentin-enamel junction, and outer dentinal acellular tubules, and then concentrates in odontoblasts, where it remains fo

Effect of fluoride-releasing filling materials on underlying dentinal lesions in vitro.

Fluoride-releasing materials placed over carious tissue are assumed to enhance remineralisation of the underlying lesion. This remineralisation, however, also depends on the availability of calcium and phosphate, which may be supplied by the pulpal fluid. The aim of this study was to measure the fluoride release of glass ionomer cements (GICs) into underlying dentin and to measure the effect of the released fluoride on the remineralisation of the underlying dentinal lesions using transversal microradiography. Discs of fluoride-releasing GIC were placed on top of dentinal lesions in an in vitro

CPNE7 Induces Biological Dentin Sealing in a Dentin Hypersensitivity Model.

Dentin hypersensitivity commonly occurs due to opened dentinal tubules for many reasons. In our previous study, copine 7 (CPNE7) could induce dentin formation for an indirect pulp-capping model in vivo. This study aims to investigate the formation of tertiary dentin when CPNE7 is applied to intentionally exposed dentin with nothing over it in vivo, whether it affects microleakage of the teeth, and the penetration ability of CPNE7 molecules through dentinal tubules in vitro. Cervical dentin areas of 6 maxillary incisors of 5 beagles were exposed to a class V-like lesion, and 1 side of 3 maxilla

Sealer penetration and apical microleakage in smear-free dentin after a final rinse with either 70% isopropyl alcohol or Peridex.

Tensioactive agents may alter dentinal wettability allowing increased sealer penetration into dentinal tubules. This may improve the apical seal and better entomb remaining bacteria. The purpose of this study was to determine if a final rinse with either 70% isopropyl alcohol or Peridex in instrumented, smear-free canals would affect apical microleakage or Roth's 801 sealer penetration into dentinal tubules. Sixty root segments were instrumented followed by smear layer removal. The segments were randomly divided into three groups receiving a final rinse with 6% sodium hypochlorite (NaOCl), 70%

The systemic theory of dental caries.

There is growing awareness of oral/systemic links, especially with regard to periodontal disease, diabetes, and cardiovascular disease, among others. The process of dental caries has similar links. Bacterial and other acids in the oral environment can erode enamel and potentially initiate an inflammatory response in the dentin. The body's own matrix metalloproteinases, mostly from within the dentin, become activated, resulting in the caries process. A simplified explanation of the oxidative stress causing inflammation is developed from three equations, namely Health, Disease, and Disease. The

Sealing properties of a self-etching primer system to normal caries-affected and caries-infected dentin.

PURPOSE: To compare the ability of an experimental antibacterial self-etching primer adhesive system to seal exposure sites in normal, caries-affected and caries-infected human dentin. METHODS: 30 extracted human third molars were used within 1 month of extraction. 10 intact normal teeth comprised the normal group. 20 teeth with occlusal caries that radiographically extended halfway to the pulp were excavated using caries-detector solution (CDS) and a #4 round carbide bur in a slowspeed handpiece. Half of those teeth were fully excavated free of CDS-stained material without exposing the pulp,

Neurophysiological mechanisms of dentin hypersensitivity.

In hypersensitive teeth pain is typically induced by cold and hot fluids or foods brought to contact with the sensitive dentin or by osmotic and mechanical stimuli. The results of animal experiments indicate that intradental A-type nerve fibres are responsible for the sensitivity of dentin. They are most probably activated by the hydrodynamic mechanism. Thus, their activation is to a great extent dependent on the condition of dentin with either open or blocked dentinal tubules. Blocking of the tubules effectively prevents the nerve activation. Correspondingly, in human experiments, the conditi

The role of toothpaste in the aetiology and treatment of dentine hypersensitivity.

Dentine hypersensitivity (DH) is a common, painful dental condition with a multi-factorial aetiology. The hydrodynamic mechanism theory to explain dentine sensitivity also appears to fit DH: lesions exhibiting large numbers of open dentinal tubules at the surface and patent to the pulp. By definition, DH can only occur when dentine becomes exposed (lesion localisation) and tubules opened (lesion initiation), thus permitting increased fluid flow in tubules on stimulation. Erosion, particularly from dietary acids appears to play a dominant role in both processes. Toothbrushing with most toothpas

Dentin bonding-variables related to the clinical situation and the substrate treatment.

The wetness of dentin surfaces, the presence of pulpal pressure, and the thickness of dentin are extremely important variables during bonding procedures, especially when testing bond strength of adhesive materials in vitro with the intention of simulating in vivo conditions. The ultimate goal of a bonded restoration is to attain an intimate adaptation of the restorative material with the dental substrate. This task is difficult to achieve as the bonding process is different for enamel and for dentin-dentin is more humid and more organic than enamel. While enamel is predominantly mineral, denti

Effects of a supersaturated pulpal fluid on the formation of caries-like lesions on the roots of human teeth.

Root surfaces were exposed to demineralizing buffer in a continuous-flow system, with or without simultaneous perfusion of the pulp chamber with a supersaturated surrogate dentine fluid (SDF). Experimental lesions formed with perfusion of the pulp were significantly less deep than control lesions formed without perfusion, but the application of hydrostatic pressure to the SDF (15 or 30 cm water) did not produce significantly greater reductions. The lesion depth reduction was attributed to reduced undersaturation at the advancing lesion front through interdiffusion of the demineralizing buffer

The neurophysiological basis and the role of inflammatory reactions in dentine hypersensitivity.

Recent studies indicate that intradental A-type nerve fibres are responsible for the sensitivity of dentine and are activated by fluid movements in dentinal tubules (hydrodynamic mechanism). The patency of the tubules affects dentine sensitivity to a great extent. Both A delta- and A beta-type nerve fibres respond to dentinal (hydrodynamic) stimulation in a similar way. Only a few studies have been made on the regional sensitivity of dentine or the receptive areas of intradental nerve fibres. The results indicate that the fibres innervating different parts of coronal dentine are equally sensit
