Deaths and cardiovascular injuries due to device-assisted implantable cardioverter-defibrillator and pacemaker lead extraction.

AIMS: An estimated 10,000-15,000 pacemaker and implantable cardioverter-defibrillator (ICD) leads are extracted annually worldwide using specialized tools that disrupt encapsulating fibrous tissue. Additional information is needed regarding the safety of the devices that have been approved for lead extraction. The aim of this study was to determine whether complications due to device-assisted lead extraction might be more hazardous than published data suggest, and whether procedural safety precautions are effective. METHODS AND RESULTS: We searched the US Food and Drug Administration's (FDA)

Sex Differences in Periprocedural Safety Outcomes and Mortality Following Cardiac Implantable Electronic Device Transvenous Lead Extraction.

BACKGROUND: Cardiac implantable electronic devices (CIEDs) play a vital role in modern electrophysiology but are prone to complications, necessitating transvenous lead extraction (TLE) in certain cases. Recent studies suggest sex-based disparities in cardiovascular outcomes, yet little is known about sex differences in TLE complications. OBJECTIVE: This study aimed to evaluate sex-related differences in peri-procedural outcomes following TLE. METHODS: We conducted a retrospective, observational cohort study using the National Inpatient Sample (NIS) from 2016 to 2019. Patients undergoing TLE

Lead extraction for cardiac implantable electronic device infection: comparable complication rates with or without abandoned leads.

AIMS: Abandoned leads are often linked to complications during lead extraction, prompting pre-emptive extraction if leads become non-functional. We examined their influence on complications when extracted for device-related infection. METHODS AND RESULTS: All patients undergoing lead extraction for device-related infection from 2006 to 2017 in our hospital were included. The primary endpoint was major complications. Out of 500 patients, 141 had abandoned leads, of whom 75% had only one abandoned lead. Median cumulative implant times were 24.2 (interquartile range 15.6-38.2) and 11.6 (5.6-17.4

The current practice and perception of cardiac implantable electronic device transvenous lead extraction in the UK.

AIMS: The number of patients requiring an extraction of cardiac implantable electronic devices (CIEDs) is rising. Lead extraction of chronically implanted leads is a highly specialized procedure and the Heart Rhythm Society published a consensus document outlining best practice. We sought to ascertain the current practice and perception of lead extraction in the UK. METHODS AND RESULTS: A prospective e-mail survey consisting of 21 questions was sent to members of Heart Rhythm UK. Key areas for exploration included case volume, settings for extraction procedures, levels of surgical support, pr

Use of AngioVac device in debulking endocarditis vegetations before lead extractions.

BACKGROUND: Systemic infection of cardiac implantable electronic devices (CIEDs) usually manifests as endocarditis, with vegetations involving the lead and cardiac valves. Cardiac implantable electronic device systemic infections involve the transvenous or epicardial portion of the lead in addition to adjacent endocardium. History, blood cultures, and imaging are the cornerstone of diagnosis. The treatment of CIED infections involves intravenous antibiotics, complete removal of the CIED, and reimplantation after clearance of bacteraemia. AngioVac is used for the removal of right atrial masses,

Lead Extraction of Cardiac Rhythm Devices: A Report of a Single-Center Experience.

INTRODUCTION AND OBJECTIVES: The rate of implanted cardiac electronic devices is increasing as is the need to manage long-term complications. Lead removal is becoming an effective approach to treat such complications. We present our experience in lead removal using different approaches, analyzing the predictors of the use of mechanical extractors/surgical removal. METHODS: Retrospective analysis of lead extractions in a series of 76 consecutive patients (mean age 70.4 ± 13.8 years, 73.7% men) between January 2009 and November 2015. RESULTS: One hundred thirty-five leads from permanent pacema

Analysis of extracted cardiac device leads for bacteria type: clinical impact.

The use of cardiac implantable electronic devices (CIED) increased over time, followed by rise of CIED-related complications, mainly infections and malfunctions. A clear diagnosis of CIED infection is of pivotal importance. When infection is confirmed, transvenous lead extraction (TLE) becomes mandatory, with associated risks and mortality. Local lesions at the device pocket often return negative swabs and tissue specimens, but conservative interventions are inconclusive, raising risks of systemic dissemination of infection and difficulties of subsequent TLE any more. When local bacteriologica

X marks the spot: catheter aspiration using the Inari FlowTriever device to debulk defibrillator lead vegetations prior to transvenous lead extraction-a case report.

BACKGROUND: When cardiac implantable electronic device infection occurs, standard therapy is usually total system extraction. Transvenous lead extraction is preferable to open heart surgical extraction, unless contraindicated because of the presence of very large vegetations on the intravenous leads according to the European Society of Cardiology guidelines. Extraction of transvenous leads with vegetations risks distal embolism resulting in obstruction and/or infection in the pulmonary arteries. Catheter aspiration of vegetations or thrombi has been performed prior to transvenous lead extracti

Effect of early diagnosis and treatment with percutaneous lead extraction on survival in patients with cardiac device infections.

Cardiac device infections (CDIs) represent a serious complication after the implantation of pacemakers and defibrillators. In addition to antimicrobials, complete hardware removal, mostly with percutaneous lead extraction (PLE), is necessary to limit recurrences. However, CDI diagnosis is often difficult and is sometimes delayed, and scarce data exist on how the timing of PLE may affect clinical outcomes. In this study, the in-hospital outcomes of 52 consecutive patients with CDIs who underwent PLE were retrospectively analyze. Co-morbidities such as diabetes mellitus, congestive heart failure

Surgical extraction of cardiac implantable electronic device leads based on a heart team approach.

BACKGROUND: As cardiac implantable electronic devices, such as pacemakers, cardioverter defibrillators, and cardiac resynchronization therapies, have become more popular, device extraction has become more frequent. At our institution, individual treatment strategies are discussed at a heart team meeting. Transvenous lead extraction (TVLE) is a first-line treatment; however, surgical lead extraction (SLE) is sometimes selected as a primary choice to provide optimal treatment and maintain the medical safety policy. This study aimed to investigate the validity of this heart team decision-making.
