The most frequent conditions of hypergastrinemia in man are the Zollinger-Ellison syndrome with autonomous gastrin hypersecretion by the tumour cell and reactive hypergastrinemia in type A autoimmune chronic atrophic gastritis with achlorhydria causing unrestrained gastrin release from the gastrin-producing antral G-cells. Both entities differ with respect to the pH in the gastric fluid, which is < 2 in patients with Zollinger-Ellison syndrome and neutral in type A gastritis. Other conditions with moderate hypergastrinemia as treatment with proton pump inhibitors, gastric outlet obstruction, p

Headache takes a large place in daily medical practice, and it is important to try and establish the most precise possible diagnosis. An outline of the diagnosis and differential diagnosis of migraine is followed by a discussion of interval drug treatment forms and counselling. Today's recommended treatment for attacks and the commonest sources of error in migraine management are summarized.

The combination of the two typical symptoms - heartburn and regurgitation - is almost proving gastrooesophageal reflux disease (GORD). Further - atypical - symptoms are dysphagia, odynophagia, pharyngitis, reflux-induced pulmonary symptoms or intermittent chest-pain. Endoscopic signs of GORD are patchy reddening of the mucosa, erosions, ulcers and stricture. Barretts oesophagus is characterized by columnar epithelial metaplasia. Consequence: Typical symptoms of GORD may be treated without further diagnostic procedure whereas in the case of atypical symptoms diagnosis of GORD has to be establis

Since the first classification of chronic hepatitis (CHH) more than 25 years have elapsed. Since then conditions for assessment of the etiology, pathogenesis and therapy of the disease have improved. These findings must be applied in the contemporary diagnosis of CHH. The first task is to assess the correct etiology of the disease because it is decisive for treatment, development and prognosis of the disease. The most important cause of CHH is viral hepatitis. Hepatitis B and C are frequently occurring diseases and may develop into CHH and cirrhosis of the liver. New therapeutic procedures, us

Suprasellar masses are neoplastic, vascular, congenital, or infectious/inflammatory in origin. Magnetic resonance imaging (MRI) is the study of choice for evaluating suprasellar masses, although computed tomography (CT) may provide complementary information. Suprasellar masses present with a variety of neurologic or endocrine dysfunctions depending on their site of origin and mass effect on adjacent structures. Imaging characteristics of the most common suprasellar lesions are described with particular emphasis on MR findings.

Lesser (or central) metatarsalgia is defined as pain in the forefoot under or around the lesser metatarsals and their respective metatarsophalangeal joints. Two common causes of central metatarsalgia are Morton's neuroma (MN) and plantar plate (PP) injury. Because both clinical and imaging features overlap, establishing the correct differential diagnosis may be challenging. Imaging has a pivotal role in the detection and characterization of metatarsalgia. Different radiologic modalities are available to assess the common causes of forefoot pain, so the strengths and weakness of these imaging t

Panniculitides are diseases of the subcutaneous tissue with heterogeneous etiology. They may develop consequent to infections, as a reaction to drugs, after thermal injury, as part of autoimmune diseases, in metabolic disorders or due to infectious organisms. The clinical presentation with subcutaneous nodules is often nonspecific. Moreover, the differentiation from vasculitides of medium-sized vessels can be clinically challenging. Microscopic examination of biopsy specimens is of high importance in the differential diagnosis of panniculitides. Histopathologically, panniculitides can be class

Epilepsy may cause convulsion or transient loss of consciousness. Differential diagnosis of convulsion includes acute symptomatic seizure, psychogenic nonepileptic seizure, and epilepsy. Long duration of seizure, fluctuating course, and asynchronous movements favor psychogenic nonepileptic seizures. Occurrence of seizure from EEG-confirmed sleep, postictal confusion, and stertorous breathing favor epileptic seizures. Differential diagnosis of transient loss of consciousness includes syncope, epilepsy, and hypoglycemia. Causes of syncope include vasovagal syncope, arrhythmia, and orthostatic hy

Headache is a frequent symptom in otorhinolaryngology. Such a secondary headache is typically caused by an organic disease in the head and neck area. Much more frequent is primary headache i.e. that an organic cause cannot be found. The most frequent primary headache types are migraine, tension-type headache, and cluster headache. What has to be done when the otorhinolaryngologist cannot find a cause for a headache in the head and neck area? This continuing education article should enable the otorhinolaryngologist to perform a first differential diagnostic exploration. Often a detailed inquiry

Hoarseness can be the leading symptom of dysphonia. In combination with impaired vocal performance and subjective voice-related discomfort, it can represent an individually different handicap for patients and lead to limited participation in social and professional life. Since the reasons for dysphonia may be not only functional but also organic with a potentially poor prognosis, hoarseness must be clarified using differential diagnosis. In addition to the knowledge of possible diseases, pathogenesis, and treatment options for dysphonia, the differential diagnostic approach requires profound k

Almost 50% of nail pathology is caused by onychomycosis. However, the nail has a limited reservoir of clinical expressions and many conditions may mimic onychomycosis. Its differential diagnosis should be known in order to avoid useless or even toxic treatments. When confronted with ungual alterations restricted to one or some nails, it is fair to evoke onychomycosis but one should keep in mind repeated traumata to the toenails, psoriasis and chronic paronychia on the fingernails. Involvement of a large number of nails, especially all twenty nails, should not first suggest a fungal infection b
