Disability evaluation, social security, and the international classification of functioning, disability and health: the time is now.

OBJECTIVE: To present the International Classification of Functioning, Disability and Health (ICF) as a standard in disability evaluation and to discuss the usefulness and challenges of the ICF when applied in disability evaluation. CONCLUSIONS: Disability evaluation can be described and measured using the ICF and ICF-related tools such as the Generic Set, ICF Core Sets specific to health conditions or settings, and measurement instruments that have been linked to the ICF. Wide implementation of the ICF in disability evaluation, specifically in work disability and social security, is needed a

Evaluation of executive functioning in people with intellectual disabilities.

BACKGROUND: Executive functioning (EF) is an important concept in cognitive psychology that has rarely been studied in people with intellectual disabilities (IDs). The aim of this study was to examine the validity of two test batteries and the structure of EF in this client group. METHODS: We administered the children's version of the Behavioural Assessment of the Dysexecutive Syndrome (BADS-C) and the Cambridge Executive Functioning Assessment (CEFA) for people with ID, to 40 participants who attended day centres for people with mild to moderate learning disabilities [mean full-scale intelli

Impairment and disability evaluation: the role of the family physician.

Physicians are frequently involved in the assessment of impairment and disability as the treating physician, in consultation, or as an independent medical examiner. The key elements of this assessment include a comprehensive clinical evaluation and appropriate standardized testing to establish the diagnosis, characterize the severity of impairment, and communicate the patient's abilities, restrictions, and need for accommodation. In some cases, a functional capacity evaluation performed by a physical or occupational therapist or a neuropsychological evaluation performed by a neuropsychologist

Evaluating facilitated communications of people with developmental disabilities.

A quasi-experimental message-passing procedure was used to assess the validity of the facilitated communication (FC) by people with autism and mental retardation or with mental retardation. The 23 participants were classified as having intellectual skills within the range of severe to profound mental retardation. Message-passing consisted of showing and verbally labeling a picture of a familiar object with the facilitator absent, and subsequent facilitation to generate a label or description of the object. Three-trial blocks were conducted with each participant on two different days. Blocks we

Behavioral Testing Design for Evaluation of Cognitive Disabilities.

Intellectual disabilities (ID) constitute a class of human neurodevelopmental diseases and are a major medical and socioeconomic problem owing to their high incidence and enormous burden to the families of those affected. In the past three decades, mutant mouse technologies have provided powerful tools for elucidating the pathophysiological mechanisms underlying behavioral and developmental alterations related to IDs and for addressing new therapeutic strategies, and major progress has been made revealing previously unidentified genes involved in ID. However, the pathological hallmarks of IDs

The evaluation of the health care needs of women with disabilities.

There are 27 million disabled women in the United States. These women face many unique challenges in accessing medical care and frequently report an insensitivity to their specific health care needs. Disabled women also have a wide array of health-related issues, including multiple concurrent medical disorders, psychosocial issues, and socioeconomic concerns. Their overall experience is that of a fragmented health care delivery system that often is inadequate in many important areas such as reproductive health, post-reproductive health, sexuality, etc.In order to address these issues, the depa

Prospective evaluation of pain, swelling, and disability from copperhead envenomation.

CONTEXT: In light of the existing controversy regarding antivenin treatment for copperhead envenomation, a more detailed analysis of the disability from this species is needed. OBJECTIVE: Our objective was to prospectively determine the duration of pain, swelling, and functional disability, i.e., residual venom effects, in patients with copperhead envenomation. METHODS: Patients with venomous snakebite reported to the North Texas Poison Center between April 2009 and November 2011 were assessed. Patients with confirmed envenomations were contacted by a specialist in poison information. Day ze

Hemophilic arthropathy: evaluation of clinical and radiological characteristics and disability.

Hemophilia is an inherited bleeding disorder which produces its greatest morbidity in the musculoskeletal system. Musculoskeletal complications of hemophilia include acute hemarthrosis, chronic arthritis and hemophilic arthropathy. We studied the clinical and radiological characteristics of joint involvement in hemophiliacs. Functional loss was also demonstrated. There were 25 patients with a mean age of 17; the mean coagulant factor level was 4.2 percent. All the patients had hemarthrotic attacks. Knees were the most commonly affected joints followed by elbows, ankles and hips. The mean numbe

An Educational Evaluation of Medical Students' Attitudes Towards Intellectual Disability.

BACKGROUND: There are 1.5 million people with intellectual disability living in the United Kingdom. It is recognised that people with intellectual disability are a marginalised group whose health experiences and outcomes are lower than those without intellectual disabilities. A contributing factor is a lack of knowledge and skills in the medical workforce. APPROACH: One medical school in England sought to address this challenge by developing an Intellectual Disability Training session for fourth-year medical students. EVALUATION: We sought to assess the impact of the new Intellectual Disabil

Evaluation of responsiveness of Oswestry low back pain disability index.

AIM: To assess the response of Oswestry disability index (ODI) among patients undergoing caudal epidural steroid injections for lumbosacral radicular pain. METHODS: A total number of 107 consecutive patients were analysed. ODI was obtained at preassessment and then at 6 weeks post-injection. At follow-up, patients were asked to rate their condition on an ordinal scale as compared to pre-injection based on their response to five questions (much better, better, same, worse, much worse). As a measure of responsiveness, the effect size and standardized response mean (SRM) was calculated. RESULTS

Describing functional low back pain disability in the American population: a crowd-sourced evaluation of the Oswestry disability index.

BACKGROUND CONTEXT: The Oswestry disability index (ODI) is widely utilized as a patient reported outcome (PRO) tool to assess patients presenting with low back pain (LBP) and following thoracolumbar spine surgery. No primary study has calculated the baseline range of ODI values in the diverse American population. Establishing age-adjusted normative values for ODI in the American population is crucial for assessing the utility of treatment strategies. PURPOSE: The purpose of this study is to describe the baseline range of functional low back disability as measured by the ODI in an American pop

A Codeveloped Web-Based Disability Disclosure Toolkit for Youth With Disabilities: Mixed Methods Pilot Evaluation.

BACKGROUND: Youth and young adults with disabilities experience many barriers in securing employment such as discrimination, inaccessible environments, and lack of support. Youth often need to decide whether and how they should disclose their need for accommodations to employers, which can help them to do their best at work. However, few evidence-based toolkits focusing on disability disclosure exist for youth with various types of disabilities. Supporting youth to develop self-advocacy skills is salient because they are an underrepresented and marginalized group in the labor market. OBJECTIV

Development of a Modified Disability Questionnaire for Evaluating Disability Caused by Backache in India and Other Developing Countries.

STUDY DESIGN: Prospective observational study. PURPOSE: To evaluate the disability domains relevant to Indian patients with low backache and propose a modified disability questionnaire for such patients. OVERVIEW OF LITERATURE: The Oswestry Disability Index (ODI) is a self-reported measurement tool that measures both pain and functional status and is used for evaluating disability caused by lower backache. Although ODI remains a good tool for disability assessment, from the Indian perspective questions related to weight lifting and sexual activity of ODI are questioned in some of the earlier

Riding the rapids: living with autism or disability--an evaluation of a parenting support intervention for parents of disabled children.

Evidence on the effectiveness of interventions to support parents of disabled children to manage their child's behaviour problems is limited. The aim of this study was to evaluate a group-delivered intervention (Riding the Rapids) which was specifically developed for parents of a child with a disability or autistic spectrum condition. This programme has been routinely delivered by a community-based mental health team across an urban, multi-ethnic locality for a number of years. A non-randomised controlled study design comprising an intervention group (n=48) and comparator (no intervention) gro

Preparticipation and Sideline Evaluation of Athletes with Intellectual and Developmental Disability.

Individuals with intellectual and developmental disability face several barriers to participation in sports and physical activity. Fortunately, there are opportunities for athletes with intellectual and developmental disability to participate in sport. Special Olympics continues to be the most prominent opportunity. Unfortunately, many physicians and other health care providers may feel underprepared to evaluate and treat athletes with intellectual and developmental disability. While national and international events often have a large number of resources available, access to medical care for
