Bacterial endocarditis revealed by infectious discitis.

A study of 30 patients admitted for infectious discitis identified four cases with concomitant bacterial endocarditis. All four patients were male and had concomitant streptococcal septicemia. Two patients had known aortic valve disease. The vertebral infection was located to the thoracic or lumbar spine. Ultrasonography disclosed aortic lesions in all four patients and mitral lesions in two. Management consisted of intravenous antimicrobial therapy for four weeks followed by a three-month course of oral antimicrobial therapy. The outcome of the discitis was favorable within the usual period o

[Discitis in small children (author's transl)].

Discitis was diagnosed in 5 children under 3 years of age, the initial clinical manifestations being difficulty in walking and abdominal pains in one case. Diagnosis was not made before periods varying from 8 days to 3 months, and no etiological basis for the disease was discovered. Pinching of the disc was always present in the first radiographic image, and the vertebral plate was ill-defined in 3 cases. Repeat radiological examinations were carried out in 3 children after 6 months, 2 and 5 years respectively. There was partial restauration of the disc space in 2 cases; the last one presented

Discitis and isthmic spondylolisthesis: a case report.

Spondylolysis and spondylolisthesis are common causes of low back pain in children and adolescents. Disc space infection is less common, but is another cause of severe back pain in this population. The combination of both processes in the same segment is rare. This case report is of a 13-year-old patient with isthmic lumbosacral spondylolisthesis and disc space infection at the same level. A patient who presented with severe low back pain and a radiological picture of isthmic slip with end plate irregularities and anterior bridging osteophyte was diagnosed with disc space infection at the slip

[Discitis in children. Report of two cases].

Two cases of intervertebral disk infection in 2 3-year-old boys are presented. The study reported on the anamnesis, clinical, laboratory and instrumental findings as well as therapy in both cases. Diagnosis was suspected on the basis of pain, refusal to assume any position that flexes the spine and irritability. Low-grade fever, the moderate increment of inflammatory indexes and the characteristic changes seen in 1 case on Magnetic Resonance Imaging (MRI) only and in the other one on both Technetium 99m polyphosphate bone-scanning and MRI verified the clinical diagnosis. Furthermore the author

Pitfalls of computed tomography in diagnosis of discitis.

Sixteen patients with discitis from January 1980 through December 1983 underwent 18 CT scans for initial evaluation. In six scans the study produced a false negative result (sensitivity 63%, 11/16). In three of these six the scan was performed at the wrong disc level, and in three the error was interpretive. During the same time period 6 patients had a CT diagnosis of discitis which proved incorrect (positive predictive value 63%), three of which had fractures, two had normal post discectomy changes, and one had a neuropathic arthropathy. These studies were reviewed in a blinded fashion along

Discitis-osteomyelitis: optimizing results of percutaneous sampling.

Vertebral discitis-osteomyelitis is an infection of the spine that involves the intervertebral disc and the adjacent vertebral body but may also extend into the paraspinal and epidural soft tissues. If blood cultures and other culture data fail to identify a causative microorganism, percutaneous sampling is indicated to help guide targeted antimicrobial therapy. Despite limited supporting evidence, withholding antimicrobial therapy for up to 2 weeks is recommended to maximize microbiological yield, although literature supporting this recommendation is limited. During the procedure, technical f

Cervical discitis and epidural abscess after tonsillectomy.

UNLABELLED: Approximately 6 weeks after an uncomplicated tonsillectomy for chronic tonsillitis, a 37-year-old woman presented to our emergency department with complaints of odynophagia and cervical pain persistent since surgery. Computed tomographic and magnetic resonance imaging revealed cervical spinal osteomyelitis with epidural abscess at C2 to 3. The patient underwent treatment with intravenous antibiotics, operative debridement, and cervical spinal stabilization. She recovered with no neurologic deficit. CONCLUSIONS: Significant infectious complications of tonsillectomy are uncommon, an

Septic discitis resulting from Escherichia coli urosepsis.

Septic discitis refers to a primary suppurative process involving the intervertebral disc space and occurs as a result of hematogenous invasion or contamination by pyogenic organisms. A case of septic discitis is described in a 77-year-old woman following an episode of Escherichia coli urosepsis. Despite bed rest, an orthosis, and appropriate antibiotics, the patient ultimately had to undergo surgical disc removal. The diagnosis of septic discitis is often made in the context of other diseases that share common clinical and laboratory findings. Magnetic resonance imaging appears well suited fo

A Rare Case of Staphylococcus carnosus Bacteremia and Discitis.

Spinal discitis osteomyelitis is commonly caused by hematogenous spread of Staphylococcus aureus to the disc space. Coagulase-negative Staphylococcus species, particularly Staphylococcus carnosus has not been reported as the cause before. We report the case of an 84-year-old male patient with a medical history of hypertension and coronary artery disease who presented with a complaint of lower back pain for two weeks before presentation. MRI of the lumbar spine showed findings of marrow edema and T1 hypointensity of the L3 -4 vertebral bodies, raising suspicion for discitis and osteomyelitis. A

Discitis, Diabetes, and Endocarditis: A Challenging Clinical Triad.

A 58-year-old male farmer from southern India with a history of diabetes mellitus presented with a one-year history of fever and progressive lower back pain. He was initiated on antitubercular therapy (ATT) for suspected tuberculous spondylitis. Despite three months of ATT, his symptoms worsened, with progressive constitutional symptoms and features of heart failure. Further evaluation revealed aortic valve vegetation and persistent lumbar spondylodiscitis. Serological and imaging findings, along with occupational exposure to livestock, led to a diagnosis of brucellosis with infective endocard

An Atypical Case of Lactobacillus jensenii Discitis and Osteomyelitis.

Lactobacillusspecies are generally considered beneficial and are not typically pathogenic in humans. Bloodstream and tissue infections are uncommon and usually occur in immunocompromised patients. We present a case of a 63-year-old female with Lactobacillus jensenii lumbosacral discitis and osteomyelitis, complicated by small bilateral psoas abscesses and myositis. L4-L5 lumbar biopsy confirmed the presence of L. jensenii via next-generation sequencing after conventional tissue and blood cultures were negative. The presumed etiology of this infection was a genitourinary source. The patient rec

Lactobacillus Discitis/Osteomyelitis in an Intravenous Drug Abuser.

Vertebral osteomyelitis is usually secondary to hematogenous seeding from direct inoculation during spinal surgery or from adjacent soft tissue infection; the most common organism being Staphylococcus aureus. We present a case of a 31-year-old male who was found to have vertebral osteomyelitis secondary to Lactobacillus species. The patient with a past medical history significant for hepatitis C, intravenous (IV) drug use, and nicotine dependence presented with severe back pain that started one month ago. His pain was located in the middle and lower back, radiating to his abdomen, and both low

Antibiotic penetration into rabbit nucleus pulposus with discitis.

OBJECTIVE: To assess the penetration into nucleus pulposus (NP) of cephazolin and clindamycin in a discitis model. MATERIALS AND METHODS: Twenty New Zealand white rabbits were inoculated with 103 Staphylococcus aureus at lumbar disc space. The rabbits with discitis confirmed by MRI 10 days after inoculation were divided into two groups. One was given cephazolin by intravenous (IV) at 80 mg/kg/day at 1.5 h interval for 5 half-lives; the other was given clindamycin by IV at 30 mg/kg/day at 2.5-h interval for 5 half-lives. Thirty minutes after completion of the last dose, NP and serum were sent

[Discitis in brucellosis is not responsive to anti-brucella treatment].

A 14-year-old girl with fever and low back pain was diagnosed as having brucellosis and diskitis. Treatment with doxycycline and gentamicin had no effect nor was there any improvement when the treatment was changed to doxycycline and rifampicin. With cessation of this treatment and administration of cloxacillin fever was reduced and the back pain disappeared. After 12 days of i.v. cloxacillin, she was released from hospital with instructions to continue cloxacillin orally for another 4 weeks. We conclude that when a patient is suffering from brucellosis and diskitis and does not respond to the

Bone scintigraphy in discitis and related disorders in children.

Thirty-nine children with suspected pathology involving the vertebral column were investigated haematologically, radiographically, and by bone scintigraphy using technetium 99M pyrophosphate. Fifteen children were shown to have inflammatory disease of the vertebrae. A further six suffered from Scheuermann's disease, two from benign tumours, and the remainder from miscellaneous diseases not specifically involving bony pathology. The nuclide scan was abnormal in all cases of discitis and osteomyelitis, and in the two tumours. All of the other conditions were associated with a normal bone scan. T

Aseptic Discitis with Instability after Posterior Spine Surgery.

INTRODUCTION: Lumbar canal stenosis is a common cause of low back pain occurring in elderly population. It is treated with posterior decompression with or without fusion surgery. However, decompression can lead to iatrogenic instability in some cases, and if suspected, must be assessed intraoperatively and treated with instrumented fusion. If not addressed, it can lead to progressive degeneration and instability. CASE REPORT: A 58-year-old male presented with severe low back pain and functional limitation 2 years after undergoing an L4 wide laminectomy for lumbar canal stenosis. While initial
