The paper presents the results of percutaneous lumbar endoscopic diskectomy in 96 patients with discogenic lumbosacral radiculitis. Indications, contraindications, and surgical techniques are discussed. The intervention is shown to be highly effective (91.1%) in young patients with a relatively short history of the disease and hernias measuring as large as 8 mm. The paper analyzes complications and poor results of treatment using percutaneous lumbar endoscopic diskectomy.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2017eng
PURPOSE: To describe the results and functional scores in a group of patients who underwent arthroscopic excision of a symptomatic sternoclavicular joint (SCJ) disk tear with a minimum follow-up period of 2 years.
METHODS: Between April 2010 and December 2014, 14 patients underwent arthroscopic excision of a torn SCJ disk. Patients whose intended surgery was an isolated diskectomy and underwent that surgery only, with no additional procedure, were included. The minimum follow-up period was 24 months. All patients underwent an arthroscopic SCJ diskectomy. Postoperatively, no immobilization was

BACKGROUND: The optimal surgical technique to treat disk herniation radiculopathy is uncertain. Advocates of sequestrectomy cite less perioperative pain and preserved disk architecture, whereas advocates of conventional diskectomy cite less frequent recurrent herniation.
METHODS: Consecutive patients were evaluated retrospectively from 2 independent practices, one in which sequestrectomy was performed and one in which conventional diskectomy was performed. Demographic, radiographic, and outcomes data were analyzed to assess clinical results and incidence of recurrent herniation requiring furt

STUDY DESIGN: Retrospective cohort.
OBJECTIVE: The purpose of this study is to determine the fusion rates of a consecutive series of anterior cervical decompressions and fusions with allograft patella using both static and dynamic plates.
SUMMARY OF BACKGROUND DATA: Anterior cervical diskectomy and fusion (ACDF) has been shown to improve symptoms of radiculopathy and myelopathy. The gold standard for obtaining fusion is using autogenous iliac crest bone graft (ICBG). The complication rate of using ICBG can be as high as 20%. To minimize this morbidity, various forms of allograft are presentl

There have been divergent opinions about the prognosis of temporomandibular joint diskectomy. Therefore we have retrospectively evaluated all diskectomies performed in Sweden during the period from 1965 to 1985. Eighty-seven patients had been operated upon with this technique and 69, with a follow-up time of 6 months or longer (median 20 months), were analyzed. The median of maximal opening increased and 74% of the patients had less frequent and less intensive pain at the follow-up examination than before surgery. The overall results of the operations were good in 51% of the patients (none or

Between January 1996 and December 2005, a total of 1241 patients underwent anterior cervical decompression and fusion for cervical degenerative diseases in the authors' institution. Sixty-three (5.1%) patients underwent revision anterior cervical diskectomy and fusion for adjacent segment disease between October 2003 and August 2010. This series included 35 men and 28 women with a mean±SD age of 52.6±6.7 years. A total of 81 segments underwent revision surgery, and most were distributed at C3-C4 and C6-C7. Patients were evaluated using the visual analog scale and Japanese Orthopaedic Associati

Although diskectomy after hemilaminectomy always produces some fibrosis, only few patients develop clinical symptoms. Fibrosis is one of the causes for failed lumbar disc surgery. Identification of fibrosis as the cause of recurring lumbar or sciatic pain requires the existence of a good clinical history and selective physical examination of the patient. MR is the imaging technique of choice although CT and myelography can be useful in some cases. Surgery can correct recurrent hernias, segmentary instability or spinal stenosis, but good results are less likely in patients with epidural fibrosi

OBJECTIVE: The anterior approach to the cervical spine now serves as the surgical access of choice for cervical spine disease. Vocal fold paralysis (VFP) may follow the procedure as a complication. The authors describe their experience with patients having VFP after anterior cervical diskectomy and fusion (ACDF), with an emphasis on outcome and prognosis.
STUDY DESIGN: Retrospective.
METHODS: Medical records of patients who underwent ACDF between January 1987 and February 1998 were reviewed. Further detailed review of the patients with documented VFP after surgery was then performed.

Percutaneous diskectomy is a new method for reducing lumbar disk herniation. This procedure is simple, safe, and only semiinvasive since it causes no direct damage to the dura or nerve roots. This technique employs a nucleotome (blunt-tipped, suction-cutting probe) with a rotating electric shaver (3 mm in diameter) that was specially developed for this procedure. It was applied to 117 patients with lumbar disk herniation whose sciatica had not been relieved by conservative treatments. The results of the technique were considered effective in 94 patients (80.3%). The improvements were even more

Athletes are at increased risk of lumbar disk herniation due to degenerative and traumatic factors. Prior studies have evaluated the efficacy of traditional open diskectomy in athletes; however, there is limited evidence regarding the utility of endoscopic diskectomy for athletes recovering from lumbar disk herniations. Our objective is to evaluate the efficacy of endoscopic diskectomy in athletes with lumbar disk herniation, focusing on return to play and pain outcomes. Studies on athletes with lumbar disk herniation who underwent endoscopic diskectomy were included. Case reports and case ser

Study Design Case report. Objective The purpose of this report is to discuss the management of a disk herniation during pregnancy and the indication and particularities of surgery in this situation. Methods We describe a case of diskectomy performed in a 35-year-old woman at 18 weeks of gestation. After 4 weeks of trying to manage the disk herniation with rest and medications without success, the muscle strength of the L5 right root decreased to grade 4, and the patient was not responding to intravenous analgesia. After discussion with the patient and family, a decision was made to perform the

Experimental studies have revealed that a pulse surgical laser based on alumo-yttrium garnet with neodymium ensures bloodless diskectomy and evaporation of a pulposus nucleus in the volume sufficient for decompression of the intervertebral disk. Biomechanical studies of spinal resistance following puncture endoscopic laser decompression of the intervertebral disk (PELDID) and microsurgical laser diskectomy (MLD) have demonstrated that these surgical interventions fail to disturb the stability of the spinal portion operated on. A total of 75 patients with radicular syndrome were operated on wit

Indications for spine fusion in combination with removal of a lumbar intervertebral disc are not as well defined or as widely accepted. Extreme opinions have been expressed on both side of this issue, but it seems unreasonable that every segment should be fused after removal of a disc or that none should be. The indication for fusion or for no fusion is often based on the specialist to whom the patient is referred. Orthopedists perform often fusion, neurosurgeons rarely. The problem is not the superiority of combined operation or simple disc excision, but the right indication for one or other

Cervical conjoined nerve root is rare, and medical imaging, such as magnetic resonance imaging and computed tomography, cannot give an accurate preoperative diagnosis.1 Treatment of cervical radiculopathy with root anomaly can be challenging. We report here a case of cervical conjoined nerve root with a 2-dimensional video. A 41-year-old woman without systemic disease presented with a 2-month history of neck and bilateral shoulder pain, upper back tightness, and left upper limb painful numbness, especially of the first to third fingers. The visual analog scale scores of the neck and left upper

STUDY DESIGN: Retrospective patient identification and prospective data collection were performed.
OBJECTIVE: To evaluate the outcome of anterior cervical diskectomy and fusion for discogenic cervical headaches.
SUMMARY OF BACKGROUND DATA: Cervicogenic headaches affect up to 2.5% of the population. One cause is discogenic pain. Because anterior cervical diskectomy and fusion may improve neck pain, the effect of this procedure on discogenic cervical headaches was evaluated.
METHODS: Nine patients with severe refractory cervicogenic headaches who underwent anterior cervical diskectomy and fus

BACKGROUND: Percutaneous diskectomy has been used effectively to treat lumbar disk herniation. The purpose of this review was to determine whether transcutaneous diskectomy is an effective surgical option for treating lumbar internal disk derangement.
METHODS: Comprehensive retrospective review via personal interview included 103 consecutive patients available for long-term follow-up. For determining results as excellent, good, fair, or poor, the criteria included subjective relief of back and/or leg pain, ability to perform preinjury job functions, residual physical restrictions, use of medi
