MR imaging of subchondral osteonecrosis of the vertebral body after percutaneous laser diskectomy.

OBJECTIVE: We describe four cases of subchondral osteonecrosis of the vertebral body that occurred after percutaneous laser diskectomy. Follow-up MR imaging after laser intervention showed abnormal findings in the vertebral body immediately adjacent to the site of diskectomy that are consistent with subchondral osteonecrosis. CONCLUSION: MR imaging features of this complication include a wedge-shaped low signal intensity on T1-weighted images, high and low signal intensities on T2-weighted images, and a contrast-enhanced area corresponding to high signal intensity on T2-weighted images. Percutaneous Endoscopic Diskectomy using an Interlaminar Approach Based on 3D CT/MR Fusion Imaging.

BACKGROUND: With effective preoperative planning, percutaneous endoscopic diskectomy (PED) via an interlaminar approach (PED-IL) can reduce the need for partial laminectomy. Our aim was to assess the clinical outcomes of PED-IL, planned using three-dimensional fusion of computed tomography and magnetic resonance (3D CT/MR) images. MATERIAL AND METHODS: Our retrospective analysis was based on data from 102 patients (66 were men) treated by PED-IL. Preoperative planning was based on the positional relationship between the nerve root and the lumbar disk herniation, visualized on 3D CT/MR fusion

Combination of Transforaminal and Interlaminar Percutaneous Endoscopic Lumbar Diskectomy for Extensive Down-migrated Disk Herniation.

BACKGROUND AND STUDY AIMS: The technique and instrumentation of percutaneous endoscopic lumbar diskectomy (PELD) have significantly improved. Thus its indications have been gradually expanding. Down-migrated disk, in particular, is regarded inaccessible by rigid instrumentation due to poor visualization and limited accessibility. We introduce a combination of transforaminal and interlaminar PELD for extensive down-migrated disk herniation at the L4-L5 level. PATIENTS AND METHODS: In the first case, a 48-year-old man had left L5 radicular pain. Magnetic resonance imaging (MRI) showed that L4-L

Percutaneous Full-Endoscopic Anterior Transcorporeal Diskectomy for Massive Migrated Cervical Disk Herniation Treatment: Case Report and Review of the Literature.

BACKGROUND: With the continuous advancement of the spinal endoscopic technique in recent years, full-endoscopic operations provide a new approach for the treatment of migrated cervical disk herniation. CASE DESCRIPTION: A 42-year-old man presented with nuchal pain and limb numbness for 2 months, with recently developed symptoms of unstable walking. Physical examination revealed a diminished skin sensitivity to pain and temperature, beginning at the C4 dermatome. The muscle strength of both upper limbs decreased to grade 4, and the muscle strength was grade 4 on both sides of the lower limbs.

Comparison of Clinical and Radiographic Outcomes Between Percutaneous Endoscopic Transforaminal Diskectomy and Unilateral Biportal Endoscopic Diskectomy for the Treatment of L4/5-Level MSU Size-3 Lumbar Disk Herniation: A 2-Year Retrospective Cohort Study.

BACKGROUND AND OBJECTIVES: Percutaneous endoscopic transforaminal diskectomy (PETD) and unilateral biportal endoscopic diskectomy (UBED) have demonstrated favorable clinical outcomes in managing lumbar disk herniation (LDH). The surgical treatment of Michigan State University (MSU) Size-3 LDH remains technically challenging. The aim of this study was to compare the clinical and radiographic outcomes of PETD and UBED, specifically in the treatment of L4/5-level MSU Size-3 LDH. METHODS: The study included 138 patients who underwent either PETD (75 cases) or UBED (63 cases). During a follow-up p

Study on the efficacy and safety of the combination of Shi's manual therapy and percutaneous endoscopic lumbar diskectomy for lumbar disc herniation with radiculopathy: study protocol for a multicenter randomized controlled trial.

BACKGROUND: Lumbar disc herniation (LDH) is a common chronic musculoskeletal disorder that seriously affects quality of life. The percutaneous endoscopic lumbar diskectomy (PELD) technique was developed to address spinal nerve root compression through direct visualization of pathological findings while minimizing tissue destruction upon exposure. It is an effective and safe treatment for LDH. However, recurrent LDH is a major concern after lumbar discectomy for primary LDH. A considerable number of clinical studies have reported that patients with LDH with radiculopathy could benefit from manu

Laser-assisted diskectomy performed by an internist resulting in cauda equina syndrome.

An internist performed percutaneous laser-assisted diskectomies (PLADs) on a patient with a sequestrated disc and stenosis. Subspecialists who perform PLADs should be trained in patient selection and lumbar diskectomy techniques. Chymopapain, percutaneous nucleotome-assisted diskectomy, and PLADs are alternatives to microdiskectomy for the management of lumbar disc herniations. PLADs were performed at the L4-5/L5-S1 levels in a 38-year-old woman with magnetic resonance (MR)-documented L4-5 stenosis and disc disease. After PLADs, she developed a subacute cauda equina syndrome. Two months later,

Percutaneous Endoscopic Interlaminar Unilateral Ventral Dural Approach for Symptomatic Bilateral L5-S1 Herniated Nucleus Pulposus: Technical Note.

BACKGROUND: Percutaneous endoscopic lumbar diskectomy is a good treatment modality for lumbar disk herniation. However, when a patient complains of bilateral lower limb radicular pain with severe disk protrusion at the L5-S1 level, the transforaminal approach is often unable to resolve both lesions owing to anatomical limitations. It is also very difficult to resolve both lesions in an ipsilateral direction using the percutaneous interlaminar approach. We report our surgical technique and clinical results using a ventral dural approach of percutaneous endoscopic interlaminar lumbar diskectomy

[Treatment of complete lumbar disk herniation by percutaneous discectomy].

Percutaneous diskectomy is a new treatment used in surgery of the spine for the treatment of herniated lumbar disks. The idea is to carry out an exeresis with a posterolateral percutaneous approach on a part of the herniated intervertebral disk; while always remaining outside the spinal canal. This technique eliminates the risks of fibrosis or arachnoiditis. It is carried out under local anesthesia, reduces the surgical insult and requires only a short stay in hospital. Between 1984 and 1988, 188 operations using this technique were carried out on L3-L4, L4-L5 and L5-S1. Of the first 100 cases

Full Endoscopic Lumbar Diskectomy for Lumbar Disk Herniation in the Presence of a Low-Lying Cord.

BACKGROUND: The distal extent of the spinal cord is most often at the level of the L1 or L2 vertebral body. In rare cases, a low-lying cord extends more distally. In this scenario, pathology that normally causes radiculopathy may cause myelopathy due to compression of the cord rather than nerve roots of the cauda equina. CASE DESCRIPTION: A 40-year-old man presented with progressive leg pain, sensory changes, hyperreflexia, and gait disturbance 1 month after a fall. The patient was myelopathic and had central L1/2 and L2/3 disk herniations. After unsuccessful unilateral laminotomy bilateral d

Technique for CT guided microendoscopic dissection of the spine.

The indications for percutaneous nucleotomy techniques are limited to disk herniation without dislocated free fragments. In cases with free fragments we have developed a new technique for percutaneous sequestrectomy. Under CT and fluoroscopic guidance spinal endoscopy was performed. In the past, percutaneous diskectomies were X-ray-guided with or without endoscopy. In this paper we describe the combination of CT-scanning, fluoroscopy and microendoscopy for guidance of nucleotomes, lasers, microendoscopes and micro-instruments. The technique was performed on 20 patients (17 outpatients) with ch

Lumbar Spine Posttherapeutic Imaging.

Management of patients after lumbar spine surgery or interventional radiology can be complex, and postoperative imaging patterns are often poorly understood by nonspecialized radiologists. This article focuses on postoperative imaging features of the lumbar spine in five clinical settings (with corresponding interventions): vertebral osteoporotic fractures (percutaneous vertebroplasty and vertebral augmentation), lumbar disk herniation (surgical diskectomy and percutaneous interventional radiology), lumbar spinal stenosis (surgical decompression), lumbar spondylolisthesis (surgical decompressi

Posterior Percutaneous Endoscopic Cervical Discectomy for Single-Level Soft, Huge Central Disc Herniation.

BACKGROUND: Posterior percutaneous endoscopic cervical discectomy (PPECD) has been proven safe and effective for foraminal cervical disc herniation (CDH). However, central CDH has long been considered as the contraindication of PPECD, because the path is obstructed by the spinal cord and nerve root. OBJECTIVES: To preliminarily assess the feasibility, safety, and effectiveness of PPECD for single-level soft, huge central CDH. STUDY DESIGN: A retrospective cohort study. SETTING: Zhejiang Provincial People's Hospital (Affiliated People's Hospital, Hangzhou Medical College). METHODS: Between

[Rehabilitation approaches in patients after percutaneous lumbar discectomy].

The lumbar ache syndrome is more and more getting sign of a social disease because of its rate. Beside the conservative treatment very often the operative treatment is used too. Except the classical discectomy, a percutaneous lumbar discectomy had been used since the last year. In the work we gave the basic anatomical characteristics of the spiral column, stages of intervertebral disk dislocations and general characteristics of the acute ache syndrome. According to Aesculaps prospect we described the operating technique of percutaneous lumbar discectomy as well as the rehabilitation treatment

Comparison of Radiation Exposure Among 3 Different Endoscopic Diskectomy Techniques for Lumbar Disk Herniation.

BACKGROUND: Lumbar disk herniation can be successfully treated by lumbar endoscopic spinal procedures. However, one of the most important disadvantages of the endoscopic methods used is radiation exposure. There are multiple endoscopic spinal procedures and this study aims to compare unilateral biportal endoscopic diskectomy (UBED), percutaneous endoscopic lumbar diskectomy (PELD), and microendoscopic diskectomy (MED) methods in terms of radiation exposure. METHODS: A total of 75 people were included in this prospective and multicenter study. The demographic characteristics, operating times (

BACKGROUND: Lumbar disc herniation is one of the commonest conditions affecting the lumbar spine, with disc fragments migrating in 35-72% of patients. It can be treated with either microdiscectomy or percutaneous endoscopic lumbar discectomy (PELD) surgeries. MATERIALS AND METHODS: We searched these databases; Web of Science, PubMed, Cochrane Library, and SCOPUS. Overall, 26 studies, both randomized controlled trials and observational studies, were included. We analyzed different efficacy, safety, and functional outcomes including operation time, estimated blood loss (EBL), length of hospital
