This review discusses the safety concerns associated with diving while using psychotropic medication and the limited literature available on the topic. Despite the risks, some divers continue to dive while taking these medications, and their reasons for doing so are unclear. The exact mechanisms of action of these drugs in hyperbaric environments are poorly understood. While current standards and advice for fitness-to-dive assessments are based on limited evidence and expert opinion, developing evidence-based strategies could improve patient care and optimise diving safety. This review apprais

RATIONALE: Decompression sickness (DCS) is a clinical syndrome caused by inert gas bubble formation following a rapid reduction in ambient pressure, with an increased risk in high-altitude diving due to lower barometric pressure. Although neurological symptoms are well-documented, respiratory involvement is less frequently reported.
PATIENT PRESENTATION: A 54-year-old male recreational diver performed his first-ever dive in Lake Van, descending to approximately 30 meters and remaining at depth for several minutes before ascending rapidly to the surface without a decompression stop. Immediatel

An experienced recreational diver developed a severe multisystem life-threatening decompression sickness following a series of eight dives over four days, the profiles of which were all within acceptable limits. The diver had marked haemoconcentration and required large volume intravenous fluid replacement during initial treatment. Similar presentations have previously only been described following deep dives, fast ascents or where mandatory decompression had been missed. The management of this case, including the involvement of a hyperbaric unit co-located to an intensive care department, and

In December, we published an article titled "Dive Medicine Capability at Rothera Research Station (British Antarctic Survey), Adelaide Island, Antarctica" by Wood FNR, Bowen K, Hartley R, et al. The corresponding author would like to include an additional author, as their contribution was significant but was inadvertently omitted in the initial online publication. While this correction has been made in several versions circulated by the journal, not all have been updated. As a result, we are issuing an errata. The title and abstract are as follows: (Wood FNR, Bowen K, Hartley R, Stevenson J, W

There is a dearth of studies in undersea and hyperbaric medicine (UHM), likely due to the limited number of clinicians currently specializing in UHM. Due to the high-consequence nature of diving and the effects of pressure on the human body, medical clearance is of the utmost importance. Despite all efforts to mitigate the possibilities of dive maladies, divers occasionally succumb to the effects of prolonged submersion. This article provides an in-depth look at three individuals who suffered from separate dive-related medical events. In each example, UHM was applied and successfully mitigated

Hyperbaric oxygen therapy (HBOT) involves inhaling 100% oxygen at pressures exceeding one atmosphere within a chamber. It is used across several specialties and has been proposed as an adjunct in burn management to enhance healing and improve outcomes. Despite increasing interest, evidence supporting its efficacy in burn care remains inconsistent, with no clear consensus in practice. A literature search was conducted in September 2025 across PubMed, Cochrane Library, and Embase without date restrictions to identify studies evaluating HBOT for burn treatment. Inclusion criteria were human studi

Hyperbaric oxygen therapy delivers 100% oxygen at supra-atmospheric pressures, elevating plasma oxygen content and tissue oxygenation above normoxic levels. Once limited to decompression illness, hyperbaric oxygen therapy is gaining recognition as a perioperative adjunct with potential to mitigate complications through supra-physiologic tissue oxygenation. This review summarizes current evidence on the biological mechanisms, clinical applications, and considerations of perioperative hyperbaric oxygen therapy. Mechanistically, hyperbaric oxygen therapy has been shown to promote angiogenesis thr

Hyperbaric oxygen is considered an adjunctive treatment to medical and surgical care. We present a unique case in which a male patient with decompression illness affecting inner ear and spinal cord presented a worsened unilateral hydrocele synchronously with the neurological pathology. At the Diving and Hyperbaric Medicine Department, the patient was initially recompressed using a modified United States Navy Treatment Table 6A; on the following days he was treated for decompression illness using hyperbaric oxygen. Hyperbaric oxygen treatment has not been used for the treatment of hydrocele, bu

Closed-circuit rebreathers have opened new frontiers in technical diving, but rebreathers are not invulnerable and therefore a 'bailout' gas supply is required. For extreme dives, open-circuit bailout is logistically impossible. This has led teams to adopt dual rebreather configurations, where a second rebreather serves as bailout. This article presents operational experiences from the Wetmules and COBRA Divers, who independently developed dual rebreather practices for extended range diving. The teams evolved contrasting approaches through extensive field testing. The Wetmules initially adopte

INTRODUCTION: The 1987 UHMS Workshop on the Validation of Decompression Tables provided standards for the design, testing, and acceptance of decompression tables. Forty years later, research funding has become scarce, and diving companies no longer receive support from universities and Navy research centers. Companies build on their experience and safety culture to improve existing decompression procedures. We list principles and propose guidelines for these internal developments.
PRINCIPLES: Editing decompression tables is a technical activity that aims to provide a practical solution to an

INTRODUCTION: Venous gas emboli (VGE) detected via ultrasound can be used as a surrogate marker for decompression stress. While Doppler ultrasound is the historical gold standard, two-dimensional (2D) ultrasonography offers advantages for on-site monitoring, including a wider field of view and reduced dependence on noise-free environments. This systematic review evaluates 2D ultrasonography protocols used in decompression research since the 2015 International Meeting on Ultrasound for Diving Research, identifying methodological similarities, differences, and adherence to consensus recommendati

The evaluation and care of an injured scuba diver requires an understanding of the different types of underwater activities that may be deemed scuba diving. Such activities may range from the complex (eg, commercial or technical diving) all the way up to basic recreational scuba or snorkeling. A thorough physical examination should be completed as early as possible with a focus on specific areas at risk for injury and etiology, such as a detailed cardiopulmonary, skin, and neurologic examination. Serial reassessments and supportive care are as equally important as consultation with a dive medi

BACKGROUND: Hyperbaric oxygen therapy (HBOT) has gained increasing interest in regenerative medicine for its capacity to enhance tissue oxygenation, reduce inflammation, and accelerate healing. While its use in musculoskeletal medicine is expanding, particularly among athletes and in sports orthopaedics, the clinical evidence base remains fragmented across indications and has not been systematically evaluated.
PURPOSE: The aim of this systematic review was to synthesize the available clinical evidence for HBOT in human musculoskeletal pathologies, with focus on its application in orthopaedic

PURPOSE OF THE REVIEW: To summarize existing literature on the outcomes of Hyperbaric Oxygen (HBO2) therapy for RC and focus on recent updates since the original 2018 scoping review.
RECENT FINDINGS: The literature continues to support HBO2's effectiveness at improving gross hematuria with recent studies showing at least a partial response in 66-100% of patients. Recent data further show an improvement in both urinary symptoms and overall quality of life measures. Data further shows that HBO2 is safe and adverse events are usually associated with barotrauma, particularly otic barotrauma. Rece

INTRODUCTION: This study reviews heliox saturation procedures used in offshore commercial diving and focuses on bell excursion dives. It excludes initial compression and final decompression. Our first objective was to trace the history and the reasons behind the successive changes that led to the current practice. Our second objective was to review the current practice and identify problem areas and perspectives.
METHODS: We first present the background of excursion diving and reference key procedures from the US Navy, Comex, and international standards. We then review the procedures of 13 an

INTRODUCTION: We present a case of severe CNS decompression sickness (DCS) that included Broca's aphasia in a recreational diver, offering insights into the application of advanced non-invasive monitoring during hyperbaric oxygen treatment (HBO₂).
CASE REPORT: This 64-year-old male diver experienced the onset of expressive aphasia, vertigo, and right-sided hemiparesis about 30-120 minutes after surfacing following a rapid ascent after a deep dive.
DISCUSSION: Initial monitoring with bispectral index (BIS) and density spectral array (DSA) revealed significant asymmetry between cerebral hemisp

As access and exploration of polar regions has expanded in the past two decades due to greater interest and technology, more recreational scuba divers are exploring these more remote cold-water locations. Polar, or extreme cold-water, diving is a more physically demanding form of scuba diving, and divers need to better understand the effect of near-freezing water temperatures on human physiology. Changes in both physiological and behavioral functions may occur quickly in these types of environments and present the scuba diver with a potentially more challenging and dangerous environment. Diver

INTRODUCTION: Tasmania is a small island state off the southern edge of Australia where a comparatively high proportion of the 558,000 population partake in recreational or occupational diving. While diving is a relatively safe sport and occupation, Tasmania has a significantly higher diving death rate per head of population than other States in Australia (four times the national diving mortality rate).
METHODS: Three compressed gas diving deaths occurred in seven months between 2021-2022 prompting a review of the statewide approach for the immediate response of personnel to diving-related de

Knee osteoarthritis is a common chronic joint disease. Although there are many treatment options available for knee osteoarthritis, most focus on alleviating symptoms rather than halting disease progression, repairing cartilage, reversing joint damage, or restoring the structure and function of the knee. Hyperbaric oxygen therapy works by increasing tissue oxygen concentrations by increasing higher atmospheric pressure. This therapy can directly improve the hypoxic microenvironment in knee osteoarthritis. Hyperbaric oxygen therapy can also improve oxygenation, reduce oxidative stress and regul
