System of Psychological Support Based on Positive Suggestions to the Critically Ill Using ICU Doulas.

UNLABELLED: Surviving critical illness often creates a lasting psychological impact, including depression, anxiety, and post-traumatic stress. Memories of frightening and delusional experiences are the largest potentially modifiable risk factor, but currently, there is no proven intervention to improve these inciting factors. Psychological support based on positive suggestion is a psychotherapeutic approach that can be provided even to patients in altered cognitive states and is therefore a viable psychotherapy intervention throughout the ICU stay. Traditional ICU care team members have limite

The Blurred Boundaries of Care: A Qualitative Analysis of Nurse-Doula Role Conflict.

INTRODUCTION: Conflict between nurses and doulas can contribute to miscommunication, delays in treatment, and decreased patient satisfaction, especially among marginalized populations. Nurses and doulas both play essential roles in supporting birthing individuals, yet their interactions are often strained by unclear role boundaries, differing philosophies of care, and institutional policies that fail to integrate doulas into clinical teams. The purpose of this study is to examine the underlying sources of tension between nurses and doulas and identify facilitators that support more effective c

Black Doula Support in US Jails and Prisons: Caring for Those Confined.

INTRODUCTION: As patient advocates, Black community-based doulas aim to address disparities in perinatal health care within both community and carceral settings. Yet, little is known about how they interpret their work with people who are pregnant and incarcerated. The purpose of this study was to examine Black doulas' perspectives on providing care in carceral systems and the institutional constraints of this work. METHODS: This study draws on interviews with 19 Black doulas who provided services in US jails and prisons. Using a directed content analysis and the reproductive justice framewor

Navigating the Perinatal Partnership: Recommendations to Facilitate Integration and Collaboration Between Institutions, Clinicians, and Doulas.

Despite the widely documented benefits of doulas, institutional and clinician opposition persists. This opposition presents a barrier to doula access and integration, limiting birthing women/people's choices and adversely impacting their experiences and health outcomes. This study explored clinicians' experiences working with doulas to identify factors that can impact interprofessional interactions. We analyzed ethnographic field notes from 7 years of participant observation in one community doula program in the Pacific Northwest (United States) and 11 semistructured interviews with clinicians

Adapting the Role of Doulas to Enhance Supports for Perinatal People with Substance Use Disorders.

PURPOPSE: There is a gap in the current literature regarding how doula care may be implemented into existing systems in Montana to best address the needs of individuals with substance use disorders (SUDs) during the perinatal period. Doulas may be particularly well positioned to reach a diverse range of patients, allowing them to feel supported and advocated for within the medical system. This study aims to illustrate the perspectives and experiences of doulas, individuals who have experienced perinatal SUDs, and maternal and mental health providers. MATERIALS AND METHODS: A qualitative descr

Role of Doulas in Improving Maternal Health and Health Equity Among Medicaid Enrollees, 2014‒2023.

Objectives. To assess the relationship between doula utilization and health outcomes of females enrolled in Medicaid-affiliated plans in the United States. Methods. In this retrospective, observational cohort study, we used Medicaid claims data from a national health insurer to compare health outcomes between females who used and who did not use a doula (2014-2023). We conducted propensity score matching using a 1:1 case‒control match, without replacement, and fit logistic regressions to analyze the relative risks for maternal health outcomes. Results. The study population included 722 matched

Continuous labor support by doulas and nurses and its impact on birth outcomes: a systematic review.

BACKGROUND: Continuous labor support provided by doulas, nurses, or midwives is increasingly recognized as an important component of respectful, woman-centered maternity care. Evidence suggests that continuous support during labor may improve maternal and neonatal outcomes by enhancing emotional support, facilitating labor progress, and reducing unnecessary obstetric interventions. However, variations in intervention models and healthcare settings have resulted in inconsistent findings regarding the effectiveness of different support providers. This systematic review aimed to evaluate the impa

The impact of abortion doulas on women and providers' outcomes: An integrative review of the evidence.

The aim of this integrative review is to map the literature on the impact of abortion doulas on women and abortion care providers. This review followed a modified Whittemore and Knafl framework and PRISMA guidelines. Electronic databases (Medline, PubMed, CINAHL, the Cochrane Library, Scopus, Proquest, EMBASE, and PsycINFO) were searched using key terms: abortion and doula. Search was limited by date (2012-2022), type (primary research only), and language (English only). Of the 99 papers found, only 4 met inclusion criteria. The four papers - drawn from 2 studies - were analysed using NICE App

Who plans to give birth with a doula? Demographic factors and perceptions of birth.

PROBLEM: Research has demonstrated the benefits of continuous care during childbirth, particularly with doula support. However, much less is known about the factors underlying pregnant women's plans to have doula support. BACKGROUND: Doulas provide one-on-one continuous care, emotional support, and advocacy, in a culturally sensitive way. AIM AND QUESTIONS: We aimed to investigate the characteristics of pregnant women who consider doula support and whether it is related to their birth experiences, beliefs and concerns, including fear of childbirth and of the staff, beliefs about birth as a n

Use of a doula for labor coaching in a patient with indolent systemic mastocytosis in pregnancy.

BACKGROUND: Few cases of pregnancy in women with systemic mastocytosis have been reported. The effects of this disease on pregnancy have not been well documented, nor have the benefits of doula services for labor been reported for mastocytosis. CASE: A 35-year-old woman with indolent systemic mastocytosis sought preconception counseling regarding the effects of her disease on pregnancy and the effects of drugs and anesthesia on her disease. She then had an uncomplicated pregnancy and delivery, with multidisciplinary coordination and assistance by doulas to reduce the need for medication in la

Doulas as Facilitators to Black Birthing People's Needs in Pennsylvania in the Context of Medicaid Coverage.

INTRODUCTION: Longstanding racial inequities in maternal mortality require tailored approaches to improving outcomes for Black birthing people. Doulas provide continuous emotional, physical, and informational support and have been shown to improve perinatal outcomes, particularly for Black birthing people. In response, some states have implemented Medicaid doula benefits. In January 2025, Pennsylvania-where ∼35% of births are Medicaid-covered-launched such a policy, allowing certified doulas to enroll as Medicaid providers. Doula Services Within a Healthy Start Program: Increasing Access for an Underserved Population.

PURPOSE: Women of color in the United States, particularly in high-poverty neighborhoods, experience high rates of poor birth outcomes, including cesarean section, preterm birth, low birthweight, and infant mortality. Doula care has been linked to improvements in many perinatal outcomes, but women of color and low-income women often face barriers in accessing doula support. DESCRIPTION: To address this issue, the New York City Department of Health and Mental Hygiene's Healthy Start Brooklyn introduced the By My Side Birth Support Program in 2010. The goal was to complement other maternal home

A challenging little balance: How white doulas with mainstream training understand and engage with anti-oppressive practice.

Though the benefits of doula care in reducing birth inequities and mitigating obstetric violence are well established, little is known about how White doulas from mainstream training programs (most doulas in Canada) understand and implement anti-oppressive practice (AOP) in different sociocultural contexts. Guided by interpretative phenomenological analysis, we conducted life history and semi-structured interviews with five White doulas working in Western Canada. The doulas described varying levels of engagement, confidence, and effectiveness across three distinct levels of sociopolitical inte
