Stimulation of carotid chemoreceptors and ventilation by doxapram in the cat.

Small doses of doxapram (less than 0.5 mg . kg-1 iv) were used to study its effect on carotid chemoreceptor activity and ventilation in cats anesthesized with alpha-chloralose. The effects were studied at several levels of partial pressures of O2 and CO2 in arterial blood (PaO2 and PaCO2). It was found that doxapram stimulated discharge rate of the carotid chemoreceptor afferents by the same magnitude at all levels of PaO2 and PaCO2 studied. However, the ventilatory effect of doxapram was more than additive to the concomitant stimulation by hypoxia or hypercapnia. This ventilatory effect was e

Interactions between morphine and doxapram in the rabbit and mouse.

Certain actions of doxapram, administered alone and in combination with morphine, have been examined in the rabbit and the mouse. Single doses of doxapram were capable of stimulating respiration in both species. There was an increase in tidal volume in the rabbit and an increase in respiratory rate in the mouse. In both species the duration of action of single doses of doxapram was less than 15 min. In morphine-treated rabbits and mice single doses of doxapram affected neither the time course nor the intensity of the respiratory depression. In the rabbit repeated doses of doxapram did not prod

A controlled trail of doxapram in acute respiratory failure.

The efficacy of a respiratory stimulant (doxapram) in antagonizing carbon dioxide retention associated with controlled oxygen therapy (28 per cent) in acute-on-chronic respiratory failure was assessed by a double-blind crossover trial in eight patients. PaCO2 rose in egery case during the placebo in the initial treatment period (mean increase 10 mm Hg). In those patients receiving doxapram initially PaCO2 fell in three and rose in one (mean decrease 4 mm Hg). All who has shown a rise in PaCO2 during the initial period on placebo showed a fall in the subsequent period on doxapram. In the six pa

Low-dose doxapram therapy for idiopathic apnea of prematurity.

BACKGROUND: Doxapram is contraindicated for newborn infants in Japan because of its serious side effects. However, because of encouraging results of recent studies regarding the efficacy and safety of therapy for apnea of prematurity (AOP) with lower doses of doxapram than those previously proposed, approximately 60% of Japanese neonatologists continue to use doxapram at small doses. Caution is warranted because the sample sizes of the former studies are inadequate to evaluate doxapram for both its beneficial and harmful effects. Therefore, we conducted the present study in order to investigat

QT interval lengthening in premature infants treated with doxapram.

OBJECTIVE: Doxapram, routinely used in premature infants treated for apnea of prematurity unresponsive to methylxanthines, has been related to cardiac conduction disorders. This study was designed to evaluate doxapram cardiac and general tolerance and its relationship to drug plasma concentrations in very premature infants. METHODS: Forty infants (mean +/- SEM, 28.9 +/- 0.3 weeks of gestation) who were given intravenous doxapram, 0.5 to 1 mg/kg per hour, at 15.9 +/- 2.4 days of life were evaluated prospectively. Electrocardiograms were monitored before and during the first 3 days of treatment

Doxapram use for apnoea of prematurity in neonatal intensive care.

Apnoea of prematurity is treated with noninvasive respiratory therapy and methylxanthines. For therapy unresponsive apnoea doxapram is often prescibed in preterm neonates. The duration, dosage and route of administration of doxapram together with its efficacy was evaluated in two Dutch neonatal intensive care. Outcome concerning short-term safety and neonatal morbidity were evaluated. During 5 years, 122 of 1,501 admitted newborns <32 weeks of gestational age received doxapram. 64.8% of patients did not need intubation after doxapram. 25% of treated neonates were <27 weeks of gestation. The effect of doxapram-induced hyperventilation on respiratory mechanics in horses.

To investigate the influence of increased respiratory frequency on respiratory mechanics in the horse, measurements were made in two groups of seven tracheostomized horses before and after the administration of doxapram. The horses in group I had normal base line values for respiratory mechanics, whereas the horses in group II had significantly lower values of dynamic compliance (Cdyn), higher respiratory resistance (R), and a higher total change in pleural pressure (delta P). The administration of 0.3 mg kg-1 doxapram intravenously resulted in a significant increase in respiratory frequency (

Effects of doxapram on carotid chemoreceptor activity in newborn kittens.

Doxapram is a respiratory stimulant which acts on peripheral chemoreceptors and central respiratory neurons in a dose-dependent fashion in the adult cat. In the newborn, the mechanisms of action of doxapram are still unclear. To evaluate the effects of doxapram on the carotid chemosensory discharge and its relationship with dopaminergic mechanisms in the carotid body, two groups of kittens less than 13 days old, anesthetized, artificially ventilated and paralyzed, were prepared for the recording of a single or a few chemosensory afferents of the carotid sinus nerve. The chemosensory activity w

Serum doxapram and respiratory neuromuscular drive in normal man.

To investigate the means by which doxapram affects the control of ventilation, ventilatory function and P0.1 have been related to serum doxapram concentration during a 45-min infusion of doxapram hydrochloride in 7 healthy, conscious subjects under normoxic conditions. Serum doxapram concentrations increased during the infusion: 1.88, 2.48, 3.42, and 3.97 micrograms/ml after 5, 10, 30 and 45 min, respectively. The majority of significant changes in the measurements from the baseline were observed at 30 and 45 min: VE, VT, P0.1, P0.1/end-tidal CO2 tension, VT/Ti and blood pressure were increase

The effect of doxapram on brain imaging in patients with panic disorder.

Administration of doxapram hydrochloride, a respiratory stimulant, is experienced by panic disorder patients to be similar to panic attacks but has reduced emotional effect in normal volunteers, thus providing a laboratory model of panic for functional imaging. Six panic patients and seven normal control subjects underwent positron emission tomography with (18)F-deoxyglucose imaging after a single-blinded administration of either doxapram or a placebo saline solution. Saline and doxapram were administered on separate days in counterbalanced order. Patients showed a greater heart rate increase

Biphasic effect of doxapram on hypnotic activity of pentobarbital in mice.

The effect of doxapram, a respiratory stimulant, on the pentobarbital sleeping time was investigated in mice. The sleeping time induced by the intraperitoneal injection of pentobarbital was prolonged 0--120 min after the administration of doxapram (25-100 mg/kg, i.p.). The pretreatment with doxapram 60 min before had no effect on the anesthetic time induced by ether and on the sleeping time induced by the intracerebroventricular injection of pentobarbital, while increased the lethality of pentobarbital only slightly and the levels of pentobarbital in the plasma and brain significantly. Intravenous diazepam narcosis in the treatment of injuries, with doxapram for recovery.

The effective use of diazepam for narcosis in cases of injury requiring painful manipulations is dependent on adequate dosage, titrated for each patient. Protection against the risk of occasional respiratory depression from this regime is consistently given by the use of intravenous doxapram, which also gives varying degrees of arousal.
