Glucocorticoids dosing in obese subjects: A systematic review.

Glucocorticoids (GCs) are amongst the most widely used and effective treatments to control inflammatory and autoimmune diseases. In obese subjects, drug dosing adjusted by body weight is problematic, all the more so as patients are at higher risk of GC metabolic side effects. We propose here to describe the determinants of drug pharmacokinetics (PK) in obese subjects and GC pharmacology, and to identify the existing PK studies that may help discussing the best size descriptor for GC dosing in obese subjects. A clinician and a pharmacist screened PubMed using the MeSH Terms: "glucocorticoids" O

Malaria prophylaxis in patients with renal impairment: a review.

Malaria is an endemic and potentially lethal disease transmitted by the protozoan parasite Plasmodium. It is currently endemic in more than 100 countries, which are visited by 125 million international travellers every year. For dialysis and renal insufficiency patients it becomes increasingly easier to travel to these countries thanks to the recent advances in renal replacement therapy. However, the pharmacokinetics of some prophylactic agents in malaria are altered, which may modify the effectiveness and safety of such treatments and the way they should be prescribed. Clinicians should be aw

Prescription-dispensing mismatch leading to vitamin D intoxication in an infant.

Vitamin D is a fat-soluble vitamin synthesised in the skin through sunlight exposure and is routinely supplemented in infants due to its limited presence in human milk. The recommended dose is 400 IU/day until 1 year of age. However, excessive supplementation can result in hypervitaminosis D, a serious condition in infants. We report a case of a term male infant who presented with poor feeding and an irritable cry, especially during urination, for 3 days. It was found that he had been receiving 6000 IU/day of vitamin D for 45 days, with a cumulative dose of 320 000 IU. Laboratory results revea

Glucocorticoid-Induced Hyperglycaemia in Hospital: The Insulin Dose Matters Most.

Prescription of glucocorticoids in hospital is a common cause of new onset hyperglycaemia, which is associated with increased mortality and morbidity. However, to date studies have not defined optimal treatment of hyperglycaemia associated with glucocorticoids and this is reflected in clinical guidelines. We recently reported the large range in the daily insulin dose requirements in hospitalized patients prescribed glucocorticoids. Failure to rapidly and accurately approximate the required insulin dose is a likely explanation for the suboptimal glycaemic control commonly reported in glucocorti

Antimicrobial use on 74 Japanese pig farms in 2019: A comparison of Japanese and European defined daily doses in the field.

Defined daily doses (DDD) have been established in human medicine to standardize the measurement of treatment in a population. In veterinary medicine, the European Medicine Agency published defined daily dose (DDDvet) values for antimicrobial agents used in food-producing animals in 2016. National defined doses (DDDjp) for antimicrobials used for pigs in Japan have recently been determined. The aim of this study was to compare the results of calculated antimicrobial use in the field using the DDDjp and DDDvet values. Data from 74 pig farms in Japan relative to antimicrobial use in 2019 was col

Subject-Specific Dosage Estimation for Primary Hypothyroidism Using Sparse Data.

Subject-specific dosage estimation for primary hypothyroidism using subject-specific parameters of the thyrotropic regulation system is presented in this work. The data needed for such personalized modeling are usually sparse. This is addressed by utilizing available data along with domain knowledge for estimation of model parameters but with some uncertainty. Optimization-based dosage estimation approaches may not be applicable in the presence of such uncertainty. In this work, the optimal drug dosage range based on estimated parameter ranges for primary hypothyroid condition is estimated usi

Community level antibiotic utilization in India and its comparison vis-à-vis European countries: Evidence from pharmaceutical sales data.

India was the largest consumer of antibiotics in 2010 in the world. Evidence suggests that countries with high per-capita antibiotic consumption have higher rates of antibiotic resistance. To control antibiotic resistance, not only reduction in antibiotic consumption is required, socio-economic factors like access to clean water and sanitation, regulation of private healthcare sector and better governance are equally important. The key objective of this research was to investigate the five year trends in consumption of major antibiotic classes in India and compare them with European Surveillan

Optimal adaptive control of drug dosing using integral reinforcement learning.

In this paper, a reinforcement learning (RL)-based optimal adaptive control approach is proposed for the continuous infusion of a sedative drug to maintain a required level of sedation. To illustrate the proposed method, we use the common anesthetic drug propofol used in intensive care units (ICUs). The proposed online integral reinforcement learning (IRL) algorithm is designed to provide optimal drug dosing for a given performance measure that iteratively updates the control solution with respect to the pharmacology of the patient while guaranteeing convergence to the optimal solution. [Could it be a little less? Let the dose of thiopental in euthanasia depend on the body weight].

The Dutch 'euthanasia and assisted suicide' practice guideline advises using 2000 mg thiopental to induce coma, followed by a muscle relaxant to cause death by respiratory paralysis. However, when a doctor administers such a high dose of thiopental as a bolus injection to a cachectic patient, there is a high likelihood of immediate death and other side effects, which can be distressing for the family. Doctors who administered less than 2000 mg have been reprimanded for not working according to current standards. Arguments are given concerning in which circumstances it is reasonable to use the

Model-Informed Reinforcement Learning for Enabling Precision Dosing Via Adaptive Dosing.

Precision dosing, the tailoring of drug doses to optimize therapeutic benefits and minimize risks in each patient, is essential for drugs with a narrow therapeutic window and severe adverse effects. Adaptive dosing strategies extend the precision dosing concept to time-varying treatments which require sequential dose adjustments based on evolving patient conditions. Reinforcement learning (RL) naturally fits this paradigm: it perfectly mimics the sequential decision-making process where clinicians adapt dose administration based on patient response and evolution monitoring. This paper aims to

A comparison of the reliability of smartphone apps for opioid conversion.

BACKGROUND: Many medical professionals use smartphone applications (apps) on a daily basis to support clinical decision making. Opioid switching (conversion of one opioid to another at equianalgesic dose) is common in clinical practice and often challenging for doctors. Apps providing an opioid conversion tool can therefore be a useful resource. Despite rapid growth in the use of medical apps, the lack of robust regulation and peer review to ensure the accuracy and reliability of app content is currently an area of concern. METHOD: We searched major online app stores for apps providing an opi

Efficacy of rodenticide baits with decreased concentrations of brodifacoum: Validation of the impact of the new EU anticoagulant regulation.

Anticoagulants are the most frequently used rodenticides at the global scale. Because of their persistency, bioaccumulation and potential for secondary intoxication, they have faced increasing legislative regulations. Recently, the European Union Regulation (EU) 2016/1179 resulted in the production and application of rodenticides with nearly half dose (<30 ppm) of anticoagulants. However, published data on the biological efficacy of rodenticides with decreased doses are scarce in the EU. Therefore, this work compared the efficacy of the original high-dose (50 ppm) and new low-dose (25 ppm) bro

[Application of an interpretable neural network framework based on the LASSO-proj algorithm for warfarin dose prediction].

Warfarin, a classic oral anticoagulant, is characterized by a narrow therapeutic window and considerable interindividual variability in dosing requirements. This makes precise dose adjustment challenging in clinical practice and increases the risk of bleeding or thrombosis. To improve dose prediction, this study developed a streamlined multilayer perceptron (MLP) model using real-world data from the International Warfarin Pharmacogenomics Consortium (IWPC) database. The LASSO-proj algorithm was applied for high-precision feature selection prior to model construction. The resulting model demons
