Identifying markers of sustained remission in rheumatoid arthritis patients on long-term tapered biological disease-modifying antirheumatic drugs.

To identify features associated with long-term persistent remission in rheumatoid arthritis (RA) patients on tapered biological disease-modifying antirheumatic drugs (bDMARD) (tap-bDMARD) therapy. We carried out a 40-month (m) extension follow-up study of 77 RA patients from a previous 12 m tap-bDMARD study. Disease activity was assessed at baseline and every 3 months. Doppler US investigation of 42 joints for the presence and grade (0-3) of B-mode synovial hypertrophy (SH) and synovial power Doppler signal (i.e., Doppler synovitis) was performed before starting the tap-bDMARD strategy by a rh

Entropy ornamented optimization influenced on electroosmotic blood subtleties drugged with graphene oxide (GO) nanoparticles in a stenosed tapered artery.

This study develops a mathematical model to assess the flow of blood with GO nanoparticles passing through the stenosed artery. Peripheral artery disease is characterized by artery constriction which may result in various complications including poor circulation and heart attacks. Combining the tangent-hyperbolic non-Newtonian fluid and the influence of the electroosmotic forces and entropy generation, the study seeks to investigate blood flow characteristics. To understand the effectiveness of graphene oxide nanoparticles for drug delivery and diagnostics, the improvement in its thermal and e

Biological structural study of emerging shaped nanoparticles for the blood flow in diverging tapered stenosed arteries to see their application in drug delivery.

The magnetic force effects and differently shaped nano-particles in diverging tapering arteries having stenoses are being studied in current research via blood flow model. There hasn't been any research done on using metallic nanoparticles of different shapes with water as the base fluid. A radially symmetric but axially non-symmetric stenosis is used to depict the blood flow. Another significant aspect of our research is the study of symmetrical distribution of wall shearing stresses in connection with resistive impedance, as well as the rise of these quantities with the progression of stenos

Recurrent Leflunomide-Induced Drug Reaction With Eosinophilia and Systemic Symptom (DRESS) Syndrome Despite Prolonged Steroid Taper: A Case Report.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a severe adverse drug reaction characterized primarily by nonspecific systemic symptoms such as fever, a classical rash, and eosinophilia. While this is an adverse reaction more often related to medications such as anticonvulsants, many drugs have been reported to be implicated in this event. We report a case of a 35-year-old male who developed DRESS syndrome within one month of beginning leflunomide therapy. Despite treatment with a prolonged steroid taper, he developed a flare-up with transaminitis less than two months after hi

Circulating calprotectin (S100A8/A9) is higher in rheumatoid arthritis patients that relapse within 12 months of tapering anti-rheumatic drugs.

OBJECTIVE: To investigate whether calprotectin (S100A8/A9 or MRP8/14), an inflammatory complex released by monocytes, could indicate residual subclinical inflammation in rheumatoid arthritis (RA) patients who are in stable remission on disease-modifying anti-rheumatic drugs (DMARDs) and serve as a marker for disease flare after DMARD tapering. METHODS: We used data from two trials. Patients from the IMPROVED study had early (< 2 years) RA, and when they achieved disease activity score remission (DAS44 < 1.6), they stopped methotrexate to attempt drug-free remission. Patients from the RETRO st

Antimodified protein antibody response pattern influences the risk for disease relapse in patients with rheumatoid arthritis tapering disease modifying antirheumatic drugs.

OBJECTIVE: To perform a detailed analysis of the autoantibody response against post-translationally modified proteins in patients with rheumatoid arthritis (RA) in sustained remission and to explore whether its composition influences the risk for disease relapse when tapering disease modifying antirheumatic drug (DMARD) therapy. METHODS: Immune responses against 10 citrullinated, homocitrullinated/carbamylated and acetylated peptides, as well as unmodified vimentin (control) and cyclic citrullinated peptide 2 (CCP2) were tested in baseline serum samples from 94 patients of the RETRO study. Longitudinal study of in vitro CD4+ T helper cell function in recently transplanted renal allograft patients undergoing tapering of their immunosuppressive drugs.

Three distinct T helper activation pathways contribute to interleukin-2 production by human peripheral blood mononuclear cells following in vitro stimulation with HLA alloantigens in a mixed lymphocyte reaction. These pathways involve both CD4+ and CD8+ T helper cells, as well as self and allogeneic antigen-presenting cells. The pathways are differentially susceptible to cyclosporine in vitro, with the CD4+ T helper cell and selfAPC (CD4 approximately sAPC) pathway being the most sensitive. Furthermore, these pathways are differentially susceptible to immunosuppressive drugs in renal allograft

Comparative efficacy and safety of disease-modifying antirheumatic drugs tapering strategies in sustained-remission rheumatoid arthritis: Systematic review and network meta-analysis.

BACKGROUND: International guidelines permit tapering disease-modifying antirheumatic drugs (DMARDs) in patients with rheumatoid arthritis (RA) in sustained remission, but the de-escalation strategy with the lowest risk of flare remains unknown. Therefore, this study aimed to compare and rank flare risk across DMARD de-escalation strategies to guide more specific clinical decisions. METHODS: We performed a systematic review and a frequentist random-effects network meta-analysis (NMA) of clinical studies, evaluating DMARD tapering in adults with RA in remission (≥6 months). The primary network

Are Depot Antipsychotics Ever Self-Tapering? Exploring the Discontinuation of Long-Acting Injectable Psychosis Drugs and Those With Delayed Onset Through In Silico Modelling.

BACKGROUND AND HYPOTHESIS: Reducing and stopping antipsychotic medications can have longer-term positive outcomes for some patients. Despite common wisdom, abrupt discontinuation of several long-acting injectable (LAI) antipsychotic drugs results in rapid D2 occupancy reductions. We explored if abrupt discontinuation of LAI antipsychotics with slow clearance remained within specified constraints on rate of D2 occupancy change (RODOC), which might reduce risk of emerging symptoms of psychosis related to withdrawal or relapse. STUDY DESIGN: Pharmacokinetic data were collated for 6 LAI antipsych

Is a full psychotherapy program necessary to reduce benzodiazepine dependence for insomnia? A Randomized Controlled Trial Comparing Drug Tapering with Single-Session CBT vs. Full Acceptance and Commitment Therapy.

OBJECTIVE: Benzodiazepines and Z-drugs (BZs) are known to present a high-risk for chronic dependence when patients do not adhere to duration of use guidelines. Cognitive Behavioral Therapy for Insomnia (CBT-I) has shown effectiveness to reduce dependence, but is not frequently sought by consumers due to being costly and time intensive. A single session of CBT-I may show promise as a compromise. The efficacy of these two approaches is herein compared in the context of a drug tapering program. METHOD: In this randomized controlled trial, the latest generation of CBT-I, Acceptance and Commitment

Tapering of biological antirheumatic drugs in rheumatoid arthritis patients is achievable and cost-effective in daily clinical practice: data from the Brussels UCLouvain RA Cohort.

BACKGROUND/PURPOSE: Studies have demonstrated that rheumatoid arthritis (RA) patients who achieve low disease activity or remission are able to taper biological disease-modifying antirheumatic drugs (bDMARDs). The aim of this study was to evaluate the proportion of patients in whom bDMARDs can be tapered in daily practice and to analyse the characteristics of these patients. Other objectives were to analyse which bDMARDs are more suitable for dose reduction and the cost savings. RESULTS: Data from 332 eligible RA patients from our Brussels UCLouvain cohort were retrospectively analysed; 140 p

Adrenal insufficiency following systemic glucocorticoid therapy: a systematic review and meta-analysis.

BACKGROUND: Adrenal insufficiency (AI) is a common complication following glucocorticoid (GC) therapy. However, reliable predictors of AI and GC regimens that mitigate this risk remain unclear. Previous systematic reviews (SRs) have included many observational studies with imprecise reporting of GC dose and duration. We conducted a systematic review focusing exclusively on studies with well-defined GC exposure. METHODS: We searched Embase, Web of Science, and MEDLINE for original studies published since 2000 that investigated adults receiving protocolised systemic GC regimens and assessed all
