BACKGROUND: A total of 14 million Medicare beneficiaries receive the Low-Income Subsidy (LIS), which reduces cost sharing in Medicare Part D. Losing the LIS may impede medication access and affect mortality.
METHODS: Using 2015-2023 Medicare data, we identified dual-eligible Medicare-Medicaid beneficiaries, who automatically receive the LIS, and calculated annual rates of Medicaid and LIS loss. To examine the relationship between LIS loss and mortality, we leveraged a natural experiment arising from the relationship between the timing of Medicaid disenrollment and subsequent LIS loss. We comp

BACKGROUND: Transgender and gender diverse (TGD) adults experience disability at twice the rate of cisgender (non-TGD) adults in the US. TGD people of color and low-income TGD people experience intersecting discrimination that may compound chronic conditions and disability. To our knowledge, no research has focused on chronic conditions among TGD Medicare beneficiaries with disabilities.
OBJECTIVE: We compared the probability of chronic conditions among TGD adults with disability-based eligibility for Medicare across race, ethnicity, and dual Medicaid-Medicare enrollment.
METHODS: We used cr

OBJECTIVE: To examine the association between state Home- and Community-Based Services (HCBS) caregiver payment policies and timely follow-up visits (in-person and telehealth) within 14 days of hospital discharge among Medicare-Medicaid dual-eligible older adults with dementia.
STUDY SETTING AND DESIGN: We categorized state HCBS caregiver payment policies into three groups: no caregiver payment, payment eligible for other friends/family, and payment eligible for two caregiver types (legally responsible relatives or other friends/family). The primary outcome was the mode of follow-up visit wit

IMPORTANCE: Dementia and urinary incontinence (UI) have been associated in cross-sectional studies. The temporal relationship between these 2 conditions is not well understood.
OBJECTIVE: The aim of the study was to investigate the risk of incident dementia in female adults with and without UI using the Medicare 5% Limited Data Set.
STUDY DESIGN: This retrospective cohort study matched females with UI to continent controls by index year, age, and dual Medicare/Medicaid eligibility. A 2-year look back period was used to exclude prevalent dementia and neurologic disorders. The 5-year cumulativ

OBJECTIVE: To assess how key risk factors contribute to differences in low patient activation between dual-eligible and Medicare-only beneficiaries and to examine whether these contributions vary by disability status or age-informing more equitable engagement strategies.
STUDY SETTING AND DESIGN: This observational study used nationally representative data from the 2019-2022 Medicare Current Beneficiary Survey. Survey-weighted descriptive statistics assessed variation in risk factors between dual-eligible and Medicare-only beneficiaries. Survey-weighted logistic regression models with state,

BACKGROUND: Hospital admissions for cardiogenic shock have increased in the United States. Temporary mechanical circulatory support (tMCS) can be used to acutely stabilize patients. We sought to evaluate the presence of racial, ethnic, and socioeconomic inequities in access to MCS in the United States among patients with cardiogenic shock.
METHODS: Medicare data were used to identify patients with cardiogenic shock admitted to hospitals with advanced tMCS (microaxial left ventricular assist device [mLVAD] or extracorporeal membranous oxygenation [ECMO]) capabilities within the 25 largest core

BACKGROUND: Individuals dually-enrolled in Medicare and Medicaid (dual eligibles) are disproportionately sicker, have higher health care costs, and are hospitalized more often for ambulatory care sensitive conditions (ACSCs) than other Medicare beneficiaries. Primary care may reduce ACSC hospitalizations, but this has not been well studied among dual eligibles. We examined the relationship between primary care and ACSC hospitalization among dual eligibles age 65 and older.
METHODS: In this observational study, we used 100% Medicare claims data for dual eligibles ages 65 and over from 2012 to

BACKGROUND: The decision to screen for breast cancer among older adults with dementia is complex and must often be individualized, as these individuals have an elevated risk of harm from over-screening. Medicare beneficiaries with dementia are increasingly enrolling in Medicare Advantage plans, which typically promote receipt of preventive cancer screening among their enrollees. This study examined the utilization of breast cancer screening among Medicare enrollees with dementia, in Medicare Advantage and in fee-for-service Medicare.
METHODS: We conducted a pooled cross-sectional study of wom

INTRODUCTION: Frailty may influence treatment tolerance, supportive care needs, and quality of life among older women with metastatic breast cancer (MBC). However, racial and socioeconomic status (SES) disparities in frailty burden, and whether these disparities vary by frailty severity, have not been examined.
MATERIALS AND METHODS: Using Surveillance, Epidemiology, and End Results linked to Medicare administrative claims (SEER-Medicare) from 2010 to 2020, we identified women aged ≥66 diagnosed with de novo MBC. Frailty burden 6 months post-diagnosis was measured with the claims-based frailt

OBJECTIVE: To determine whether mandatory participation by hospitals in bundled payments for lower extremity joint replacement (LEJR) was associated with changes in outcome disparities for patients dually eligible for Medicare and Medicaid.
DATA SOURCES AND STUDY SETTING: We used Medicare claims data for beneficiaries undergoing LEJR in the United States between 2011 and 2017.
STUDY DESIGN: We conducted a retrospective observational study using a differences-in-differences method to compare changes in outcome disparities between dual-eligible and non-dual eligible beneficiaries after hospita

BACKGROUND: Antipsychotic medications are extensively used in nursing homes for management of behavioral and psychiatric disorders in the elderly. Prior research suggests that pneumonia is one of the common causes of anti psychotic-related mortality in this population. None of the studies compared typical and atypical antipsychotics with respect to pneumonia.
OBJECTIVE: To examine the risk of pneumonia with use of typical versus atypical antipsychotics in dual eligible elderly nursing home residents.
METHODS: The study involved a retrospective cohort design matched on propensity score using

PURPOSE: Structural brain imaging is guideline-recommended for cases with newly diagnosed cognitive impairment. Adherence to this recommendation and differences by Medicare coverage type are not known. We determined the structural imaging rate overall and by Medicare coverage type: traditional Medicare, Medicare Advantage, and dual eligibility for Medicare and Medicaid ("duals") and diagnosis of mild cognitive impairment (MCI) and dementia.
METHODS: In this cross-sectional study, we used the 100% data for traditional Medicare and Medicare Advantage from 2020 through 2022. A total of 1,095,947

Peripheral artery disease (PAD) has been associated with poorer outcomes based on particular social determinants of health, including insurance status. A unique population to study treatment outcomes related to PAD is those with dual-eligible status-those who qualify for both Medicare and Medicaid-comprising more than 12 million people. We performed a systematic review of the literature surrounding dual-eligible patients and impact on PAD, with final inclusion of six articles. Dual eligibility has been associated with higher rates of comorbidities; more severe symptoms at initial presentation

BACKGROUND: Liver- and lung-directed local therapies can eradicate metastatic disease and prolong survival for selected patients with oligometastatic colon cancer. Evidence about the association of rurality with access to metastasis-directed therapies and survival outcomes is limited.
METHODS: A retrospective study analyzed fee-for-service Medicare beneficiaries with colon cancer metastatic to liver and/or lung using claims from 2016-2019. The exposure of interest was residential rurality, categorized as metropolitan versus non-metropolitan (i.e., rural). The study evaluated the association o

OBJECTIVE: The aim of the study is to examine whether inpatient rehabilitation facility patients' risk-adjusted functional outcomes varied with the following five social drivers of health: Medicare-Medicaid dual eligibility status, race and ethnicity, rural residence, socioeconomic status, and living alone.
DESIGN: This cohort study examined unadjusted and adjusted mobility and self-care change scores during inpatient rehabilitation facility stays for 428,710 Medicare patients with and without social drivers of health. Regression models isolated the mean marginal effect of each of the five so

OBJECTIVE: Long-acting injectable antipsychotics (LAIAPs) enable safe and effective long-term management of chronic psychotic disorders but are underused in clinical practice. The authors examined whether Medicare formulary restrictions (prior authorization [PA] or step therapy) impose barriers to LAIAP uptake.
METHODS: The authors analyzed formulary restrictions and patient cost-sharing for several LAIAPs and estimated the percentage of plans (N=2,494 were available per year) applying formulary restrictions. Analyses were stratified by Medicare Advantage, dual Medicare-Medicaid eligibility p

BACKGROUND: Patients hospitalized with COVID-19 from socioeconomically vulnerable communities are at risk for in-hospital cardiovascular events. However, the association of socioeconomic vulnerability and outcomes after hospitalization is uncertain.
METHODS AND RESULTS: American Heart Association COVID-19 Cardiovascular Disease Registry hospitalizations between March 1, 2020, and June 30, 2022, linked with Medicare fee-for-service claims, were analyzed. We used Centers for Disease Control and Prevention's Social Vulnerability Index to ascertain county-level and Medicare-Medicaid dual eligibil

Background: In 2014, the Affordable Care Act Medicaid Expansion (ME) increased Medicaid eligibility for adults with an income level up to 138% of the federal poverty level. In this study, we examined the impact of ME on mortality and amputation in patients with peripheral artery disease (PAD). Methods: The 100% MedPAR and Part-B Carrier files from 2011 to 2018 were queried to identify all fee-for-service Medicare beneficiaries with PAD using International Classification of Diseases codes. Our primary exposure was whether a state had adopted the ME on January 1, 2014. Our primary outcomes were

IntroductionDespite improved outcomes with PD-1 inhibitors (PD-1i) in advanced non-small cell lung cancer (NSCLC), the determinants of cancer-, cardiovascular disease (CVD)-, and other-cause mortality remain poorly characterized in older adults. This study applies competing-risks methods to estimate cause-specific mortality and to identify predictors for each cause in this population.MethodsA retrospective cohort study was conducted using the 2014-2019 SEER-Medicare linked database, including patients aged 65 years or older with advanced NSCLC who received PD-1i (nivolumab or pembrolizumab).
