Early Ambulation to Prevent Delirium After Long-Time Head and Neck Cancer Surgery.

OBJECTIVE: Postoperative delirium is known to have various adverse effects on head and neck surgery patients. This study was designed to identify possible risk factors of delirium following long periods of head and neck cancer surgery and to help prevent postoperative delirium. METHODS: We enrolled 197 patients who underwent long-time (>6 h) head and neck surgery at the Asan Medical Center from January 2017 to December 2018 in this study. Clinical covariates that may be associated with delirium were analyzed retrospectively using univariate and multivariate analyses. RESULTS: Delirium occurr

Timing of inpatient medical complications after adult spinal deformity surgery: early ambulation matters.

BACKGROUND: The substantial risk for medical complication after adult spinal deformity (ASD) surgery is well known. However, the timing of medical complications during the inpatient stay have not been previously described. Accurate anticipation of complications and adverse events may improve patient counseling and postoperative management. PURPOSE: (1) Describe the rate of medical complication and adverse events by postoperative day after ASD surgery and (2) determine whether early ambulation is protective for complications. STUDY DESIGN/SETTING: Single institution retrospective cohort study

Impact of Early Ambulation on the Prognosis of Coronary Artery Bypass Grafting Patients.

BACKGROUND: The effect of delayed ambulation on the outcome of coronary artery bypass grafting (CABG) remains to be clarified. METHODS AND RESULTS: The long-term and in-hospital outcomes of 887 patients who underwent isolated CABG (455 off-pump cases, 135 urgent cases) were evaluated, with a focus on the timing of first ambulation. In-hospital mortality cases were excluded. Early ambulation (first ambulation within 3 days after operation) was achieved in 339 (38%) patients. In the multivariable logistic regression analysis, longer operation time and urgent case, EuroSCORE II, re-thoracotomy,

Successful outcome of early ambulation after extensive skin grafting in extramammary Paget's disease.

Extramammary Paget's disease (EMPD) is a rare cutaneous malignancy that mainly affects the senior population. There is a relatively high risk of postoperative complications from surgery in the senior population, such as disuse syndrome, deep vein thrombosis and postoperative delirium. To prevent such postoperative complications, early ambulation is recommended. However, EMPD often requires extensive skin grafting because of the need for a large resection margin. To avoid skin graft failure, many institutions require that patients have several days of postoperative bedrest. For these reasons, t

Early Ambulation Accelerates Time to First Bowel Movement After Separation Surgery for Spinal Tumors.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To identify factors associated with the timing of bowel function recovery after separation surgery for metastatic spinal tumors, while incorporating the temporal dimension of postoperative rehabilitation. SUMMARY OF BACKGROUND DATA: Separation surgery for metastatic spinal tumors is more extensive compared to conventional spinal surgery and carries a higher risk of postoperative ileus (POI). However, the factors influencing bowel recovery following this procedure remain poorly understood. Prior studies on conventional spinal decompression

Bent Knee Temporary Prosthesis to Support Early Ambulation after Transtibial Amputation: A Case Report.

Postoperative immobility can exacerbate the physical and psychologic effects of limb loss. This case report presents a 37-year-old mother of two children who required a transtibial amputation. She received an early ambulation device called the bent knee temporary prosthesis (BKTP), which aids in early postoperative ambulation. Several early ambulation devices have been utilized in the past but have not found widespread favor. Although there is a paucity of research on this topic, available data suggest patient tolerance and benefit of early ambulation. A customizable, temporary below knee pros

[Limits of early ambulation following heart surgery from the viewpoint of the pediatric cardiologist].

Early mobilization after cardiac surgery in children may be limited by various postoperative problems. These are mainly congestive heart failure, respiratory insufficiency, general or local infections, postoperative cardiac arrhythmias, and a number of other postoperative complications. We examined the postoperative course of 297 children operated in the period 1986-1989 for congenital defects of the heart and great vessels. Fifty of these patients were newborns, 92 were infants, 98 young children, and 57 school-age children. In 37% of the patients, no postoperative problems were observed; 31%

Use of a pylon for early ambulation after below-knee amputation: a preliminary report.

The fitting of a prosthesis immediately after amputation offers many advantages. These include early ambulation, more rapid healing of the amputation site and a decrease in the number of complications resulting from prolonged bed rest. The major problems preventing the widespread use of immediate fitting are the specialized prosthetic care required and the high cost of the appliance. The authors describe a new pylon designed for early ambulation after below-knee amputation. Its application does not require special training, it is inexpensive, safe and effective. This pylon was used in the earl

Early ambulation and discharge in 100 patients with burns of the foot treated by grafts.

Traditional treatment after grafting of foot, ankle, and lower leg burns is bedrest, limb elevation, and gradual ambulation only after 5 to 10 days. In 1982 we suggested that aggressive surgical treatment and early ambulation could shorten hospital stay and decrease morbidity. Our treatment of these burns is excision and grafting, application of an Unna (dome paste) boot immediately in the operating room or the next morning, with normal ambulation 4 hours later and discharge of the patient if there are no other reasons for continued hospitalization. This paper reports the continuation of this

Ultra-early postoperative ambulation in spine surgery: a Michigan Spine Surgery Improvement Collaborative study.

OBJECTIVE: Previous studies have demonstrated the benefit of early ambulation in patients who have undergone elective spine surgery. However, there are limited data on how early patients can feasibly move about in the postoperative period and whether there is further benefit in an ultra-early postoperative ambulation time frame. Current Michigan protocols aim for 80% of all patients ambulating within 8 hours of surgery end time. The goal of this retrospective study was to determine whether patients who ambulate within 4 hours of surgery have any greater benefit than those who ambulate 4-8 hour

Early Ambulation in Patients With External Ventricular Drains: Results of a Quality Improvement Project.

INTRODUCTION: Prolonged immobility in patients in the intensive care unit (ICU) can lead to muscle wasting and weakness, longer hospital stays, increased number of days in restraints, and hospital-acquired infections. Increasing evidence demonstrates the safety and feasibility of early mobilization in the ICU. However, there is a lack of evidence in the safety and feasibility of mobilizing patients with external ventricular drains (EVDs). The purpose of this study was to determine the safety and feasibility of early mobility in this patient population. METHODS: We conducted a prospective, obs

Early ambulation after percutaneous femoral access with use of closure devices and hemostatic agents.

BACKGROUND: Patient satisfaction after percutaneous endovascular procedures is significantly influenced by the amount of time to ambulation postprocedure. The purpose of this study was to assess the complication rates of early ambulation after use of closure devices or topical hemostatic agents for femoral access sites for endovascular procedures. METHODS: A retrospective review was performed of all patients who underwent an endovascular procedure from a femoral access site between January 2004 and March 2008. The access site was closed with an Angio-Seal, StarClose, or D-Stat Dry with pressu

Children With Presumed Early-Onset Idiopathic Scoliosis and Treated in Cast May Have Delayed Ambulation.

BACKGROUND: Serial casting is a safe and effective treatment for early-onset idiopathic scoliosis (I-EOS). However, its impact on a child's ability to walk has not been previously described. This study evaluates the influence of serial casting on ambulatory age in I-EOS patients. METHODS: A retrospective multicenter review was conducted on I-EOS patients who received their first casts before 18 months of age. Excluded were patients who were ambulatory at the first visit or had incomplete records. Data collected included sex, age at first visit and cast application, number of casts, precast/po

What are the prognostic factors of early ambulation after spinal arthrodesis surgery? A retrospective study.

BACKGROUND: After spinal surgery, early mobilisation programmes are playing an increasing role in managing pain symptoms, reducing length of hospital stay and preventing complications. The aim of the study is to investigate what are the prognostic factors of early ambulation after spinal arthrodesis surgery. METHODS: A retrospective cohort study was carried. Patients who underwent vertebral arthrodesis surgery with at least one fixed level of instrumental fixation were included consecutively. The primary outcome was represented by the precocity of ambulation. The variables collected were divi

[Early ambulation and rehabilitation following heart surgery from the viewpoint of the cardiologist for adults].

Traditional concepts about the early phase of postoperative remobilization after cardiac surgery favor physical inactivity--as did earlier concepts for rehabilitation after myocardial infarction like the armchair treatment philosophy. For an overwhelming majority of our patients, however, this concept does not hold, according to our experience during the last decade. In contrast, we propose a model of stepwise mobilization and rehabilitation starting the first postoperative days. At the end of the first week most patients are able to climb staircases. In the second week group physical therapy
