Abnormalities of the distal ejaculatory ducts related to infertility have been well-documented. Although there are no specific findings associated with ejaculatory duct obstruction, several clinical findings are highly suggestive. A diagnosis of ejaculatory duct obstruction is suggested in an infertile male with oligospermia or azoospermia with low ejaculate volume, normal secondary sex characteristics, testes, and hormonal profile, and dilated seminal vesicles, midline cyst, or calcifications on TRUS. Other causes of infertility may be concomitantly present and need to be evaluated and treate…

A 44-year-old man presented with bilateral flank pain, dysuria, and incomplete bladder emptying. Evaluation demonstrated a non-functioning atrophic left kidney, a markedly dilated tortuous ureter, and an ectopic ureter inserting into the left ejaculatory duct with an associated ejaculatory duct stone. Diagnosis was established using CT, MRI, DMSA scan, cystoscopy, and retrograde pyelography. Surgical findings confirmed the anomaly, and postoperative semen analysis showed hemato- and oligoasthenospermia. This rare congenital urological abnormality illustrates the importance of multimodal imagin

AIM: Reflux of urine into the ejaculatory ducts of children may result in recurrent orchitis and sterility. Urethro-ejaculatory duct reflux (UER) is an uncommonly reported condition in children. The diagnosis of this condition can be made using a micturating cystourethrogram (MCUG) to demonstrate the reflux of contrast into any of the ejaculatory ducts. The aim of this study was to look at the incidence of UER in the broader pediatric population and review the management options.
METHODS: A retrospective study was made of all MCUGs done in boys up to 13 years of age in all service departments…

Hematospermia is supposed to be derived from pathological conditions in the seminal vesicle, prostate, testis, epididymis, or urethra. A recent advance in diagnostic procedures has demonstrated the seminal vesicle, the prostate, and midline cyst as potential sources of hematospermia. The authors describe a case of hematospermia caused by ejaculatory duct obstruction, in which a transurethral technique was successful. A 51-year-old male was referred to the authors' clinic with a chief complaint of hematospermia. Transurethral ultrasonography showed a cystic lesion surrounded with hyperechoic ar

Ejaculatory duct cysts are uncommon urological entities that can be difficult to diagnose after prostate surgery. We report a 60-year-old man who underwent TURP for a 65 g prostate and developed hemospermia and hypospermia six months later. Pelvic MRI identified a 12 mm cyst in the ejaculatory duct region above the verumontanum, confirmed cystoscopically. Post-ejaculatory urine contained no spermatozoa, while semen analysis showed oligoasthenospermia (8 million/mL, 15 % progressive motility, volume 0.8 mL). In view of the small cyst, partial preservation of ejaculation, patient age, and comple

Ejaculatory duct reflux is rare and few investigations have focused on this entity, which is usually described in children. This study reports a new case of unilateral ejaculatory reflux in a 32-year-old patient, with a history of urethroplasty at the age of 5 for hypospadias, who presented for right chronic scrotal swelling. Urethrocystscopy showed an anterior urethral stricture and a gaping opening of the right ejaculatory duct. CT scan with opacification through the right ejaculatory duct, showed a dilated right seminal vesicle, associated to a reflux in the right deferent vas and epididymi

Ejaculatory duct obstruction (EDO) is one of the obstructive factors for 1-5% of all cases of male infertility and it is, however, surgically correctable. Congenital developmental abnormality is a most common cause of EDO. The clinical manifestations of EDO are varied, typically with the decline of four semen parameters. Transrectal ultrasonography is an important imaging method for the diagnosis of EDO and guidance in its surgery. MRI provides high-resolution images of the reproductive system as evidence. Transurethral resection of the ejaculatory duct (TURED) is a classical operation, the ap

Symptomatic ejaculatory duct (ED) calculi, typically composed of uric acid, carbonate apatite and calcium phosphate, or calcium phosphate in the form of hydroxyapatite, are rare occurrences. We report a case of bilateral, large ED calculi in a patient with spina bifida myelomeningocele and an augmented neurogenic bladder. A 25-year-old Caucasian male, not compliant with his urological management, presented with abdominal pain, difficulty in self-catheterization, nausea and vomiting. Two of eight large struvite calculi, which blocked the urethra, were identified at the right ED. After endoscopi

A 7-year-old boy was brought to our clinic with the chief complaint of terminal pain on urination. Rectal examination revealed a prostate of almost normal size and consistency, but seminal vesicles were palpated to be hard and in a painful mass. Plain x-ray and urethrogram demonstrated several small stones in the posterior urethra and in seminal vesicles. In the first operation, the stones located in the posterior urethra were removed through the transvesical approach, and the seminal vesicular stones were extirpated in the next operation. The stones were found to consist of magnesium ammonium

Ejaculatory ducts draining into accessory urethral channel opening into perineum is rare. This is a case report of a 27-year-old male who had hypospadias surgery at 3 years of age, presenting with discharge of semen through the perineal opening from adolescence. Cystoscopy and dye study showed that it was a short channel communicating with both ejaculatory ducts. Cystoscopy of the native urethra revealed that the vermontanum had not developed. The mucous lined accessory urethra was anastomosed to the bulbar urethra. Urethrogram done after one year showed that the accessory urethra was draining…

OBJECTIVE: To evaluate the efficacy and safety of transrectal ultrasound-guided ethanol sclerotherapy in the treatment of Müllerian duct cyst accompanied with ejaculatory duct obstruction.
METHODS: Under the transrectal ultrasound guidance, 3 cases of prostate Müllerian duct cyst were treated by 20 G needle puncture and ethanol injection, and closely followed up by ultrasonic monitored on follow-up examinations.
RESULTS: All the cysts involuted completely 6 months after the treatment. Symptoms caused by ejaculatory duct obstruction were relieved in all the patients. No severe complications o

Between June 1997 and September 1999, we performed transurethral unroofing (TUUR) in three patients with hematospermia that recurred repeatedly for one year or more. Patient 1 (48 years old) and Patient 2 (59 years old) were diagnosed as having müllerian duct cysts that communicated with the left ejaculatory duct, and Patient 3 (36 years old) as an ejaculatory duct obstruction with the right ejaculatory duct dilation. A mixture of water-soluble contrast medium and indigocarmine blue dye was injected into the cysts and the ejaculatory duct cavity after incision of the vas deferens in Patients 1

OBJECTIVE: To explore the diagnosis and treatment of ectopic seminal duct opening into the urethra.
METHODS: We reviewed the literature and retrospectively analyzed the clinical data on a case of sex development abnormality. The patient was a 16-year-old gender female seeking medical improvement of female signs, admitted to hospital with "clitoris hypertrophy, no menstruation and chromosome karyotype 46XY", treated by bilateral orchiectomy, and simultaneously examined by seminal vesiculography and cystoscopy.
RESULTS: Seminal vesiculography showed the ectopic opening of the right ejaculatory

Chronic pelvic pain syndrome (CPPS) is the most prevalent form of prostatitis. It is characterized by pelvic pain, voiding symptoms, and ejaculatory symptoms in the absence of bacterial infection. This can be a difficult condition to treat. Many etiologies for CPPS have been proposed including immunologic, neurologic, endocrine, and psychological factors. This article examines a potentially correctable condition that may lead to CPPS, ejaculatory duct obstruction (EDO). EDO is easily correctable with minor surgery. In patients with symptoms of CPPS with associated ejaculatory pain, the diagnos

A 7-month-old patient with a verified urinary tract infection showed an unusual mictiocystography finding of reflux via the right ejaculatory duct into the epididymis. Usually such reflux is associated with anomalies of the genitourinary tract. This case demonstrated that reflux may occur in healthy infants and that it may be clinically associated with urinary tract infections.

The incidence of an ectopic ureter in male patients is low. The ectopic ureter in men often ends at the seminal tract in association with renal dysgenesis. Malignant transformation of this closed, nonfunctional urothelial system has been reported only once. To our knowledge, we report the first case of primary transitional cell carcinoma in a single ectopic ureter with a dysplastic kidney that terminated in the ejaculatory duct.
