Identification of prognostic indicators for patient outcomes in extensor tendinopathy at the elbow.

Extensor tendinopathy is a degenerative overuse condition involving the common extensor tendon at the lateral humeral epicondyle. The diagnosis and management of this condition are clinically based. Our aims were to determine if through patient history and clinical examination we could ascertain prognostic factors affecting time to symptom resolution in extensor tendinopathy. Symptomatic patients (N=28) had a thorough history and examination, and rehabilitation monitored regularly for 6 months. Patient outcomes at 6 months were then correlated with specific factors on initial presentation. Fac

The effects of the addition of Mulligan mobilization with movement to exercise on elbow pain and function associated with lateral elbow tendinopathy.

BACKGROUND: Mulligan mobilization with movement (MWM) and eccentric exercise (EE) have been shown to be effective for the treatment of chronic lateral elbow tendinopathy (LET). However, the addition of MWM to EE effects on LET remained unknown. The objective of the study was to determine the effects of adding of Mulligan mobilization with movement to eccentric exercise for the treatment of LET. METHODS: In this quasi-experimental study, thirty participants with unilateral LET were assigned into two groups: 15 in MWM (received MWM + EE) (7 M and 8 F, mean age 36.8 ± 8.3 years) and 15 in EE (9

Extracorporeal Shock Wave Therapy in Chronic Lateral Elbow Tendinopathy: A Systematic Review.

BACKGROUND: Lateral elbow tendinopathy (LET) is a common musculoskeletal disorder caused by overuse, resulting in microtrauma to the extensor tendons. Because of its complex pathophysiology and high recurrence rate, LET poses significant treatment challenges. The effectiveness of extracorporeal shock wave therapy (ESWT) in treating LET remains controversial. PURPOSE: To evaluate the therapeutic benefits of ESWT by comparing its effects to those of both placebo ESWT and conventional physical therapy. STUDY DESIGN: Systematic review; Level of evidence, 1. METHODS: A systematic review of rando

What Is the Most Effective Eccentric Stretching Position in Lateral Elbow Tendinopathy?

BACKGROUND: A variety of treatment options suggest that the optimal treatment strategy for lateral elbow tendinopathy (LET) is not known, and further research is needed to discover the most effective treatment for LET. The purpose of the present study was to verify the most effective position of eccentric stretching for the extensor carpi radialis brevis (ECRB) in vivo using ultrasonic shear wave elastography. METHODS: A total of 20 healthy males participated in this study. Resting position was defined as 90° elbow flexion and neutral position of the forearm and wrist. Elongation of the ECRB

Lateral elbow tendinopathy: correlation of ultrasound findings with pain and functional disability.

BACKGROUND: Lateral elbow tendinopathy is a common condition often diagnosed by ultrasound. Debate exists regarding which ultrasound findings correlate with disease severity and prognosis. HYPOTHESIS: Sonographic predictors for tendon healing in patients with lateral elbow tendinopathy can be found by correlating initial ultrasound findings with subsequent changes in pain and functional disability scores after a period of nonoperative management. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. METHODS: Sixty-two elbows (34 right, 28 left) in 62 patients (30 male, 32 female) wi

Common tendinopathies around the elbow; what does current evidence say?

Tendinopathies are common causes of pain around the elbow resulting in significant functional impairment in athletes or the working-age population. Patients complain of a gradual onset pain with or without any specific trauma. Tissue histology shows chronic fibroblast and vascular proliferation, with a disorganized collagen pattern and absence of inflammatory mediators. Currently, numerous treatment options are described, but many of these are only supported by a heterogenous evidence base. Thus, management guidelines are difficult to define. Surgery is mostly indicated in selected cases that

Clinical, Functional, and Ultrasonographic Assessment of the Shoulder-Elbow Complex in Padel Players with Lateral Elbow Tendinopathy: A Pilot Observational Study.

INTRODUCTION: Lateral elbow pain (LEP) is traditionally considered a local tendinopathy; however, emerging evidence suggests that proximal shoulder function and kinetic chain adaptations may contribute to its development in overhead and racquet sport athletes such as padel players. This study compared shoulder strength, neuromuscular activation, elbow tendon morphology, and grip strength between padel players with lateral elbow pain and healthy controls, and between pain and non-pain sides within the symptomatic group. METHODS: A pilot study was conducted in padel players with unilateral LEP

Can extracorporeal shock-wave therapy be used for the management of lateral elbow tendinopathy?

Lateral elbow tendinopathy (LET) is one of the two most common tendinopathies of the upper limb. The most effective treatment in the management of LET is the exercise program. Clinicians combine exercise program with other physiotherapy, electrotherapeutic and no, techniques. Extracorporeal shock wave therapy (ESWT) is one of the most common recommended electrotherapeutic modalities for the management of LET. Further research is needed to find out the optimal treatment protocol of ESWT in the management of LET.

Upper limb position affects pain-free grip strength in individuals with lateral elbow tendinopathy.

BACKGROUND AND PURPOSE: Pain-free grip (PFG) force is commonly used to monitor treatment outcomes in lateral elbow tendinopathy (LET); however, it is unclear whether changes in forearm and elbow position affect PFG force values. This study aims to examine the effect of elbow/shoulder and forearm position on non-normalised and normalised PFG force in individuals with unilateral LET. METHODS: A cohort study including 21 subjects with clinically diagnosed unilateral LET (13 females, mean [SD] age 50 [8] years) performed PFG force (symptomatic arm) and maximal grip (asymptomatic arm) tasks using

Grip strength is more sensitive to changes in elbow position than isolated wrist extension strength in patients with lateral elbow tendinopathy.

STUDY DESIGN: This is an observational study. INTRODUCTION: Because isometric wrist extension minimizes the effects of other muscles, the sensitivity of wrist extension strength testing on patients with lateral elbow tendinopathy (LET) should be evaluated. PURPOSE OF THE STUDY: The purpose of the study was to compare the effects of the elbow position on wrist extension and grip strength in patients with LET. METHODS: Patients were screened for at least 2 of 5 clinical tests for LET. Between-day intraclass correlation coefficients (ICC3, 1) for healthy individuals were examined for both grip
