Accurate Deep Brain Stimulation Lead Placement Concurrent With Research Electrocorticography.

BACKGROUND: Using electrocorticography for research (R-ECoG) during deep brain stimulation (DBS) surgery has advanced our understanding of human cortical-basal ganglia neurophysiology and mechanisms of therapeutic circuit modulation. The safety of R-ECoG has been established, but potential effects of temporary ECoG strip placement on targeting accuracy have not been reported. OBJECTIVE: To determine whether temporary subdural electrode strip placement during DBS implantation surgery affects lead implantation accuracy. METHODS: Twenty-four consecutive patients enrolled in a prospective databa

Shift in electrocorticography electrode locations after surgical implantation in children.

Interpreting electrocorticography (ECoG) in the context of neuroimaging requires that multimodal information be integrated accurately. However, the implantation of ECoG electrodes can shift the brain impacting the spatial interpretation of electrode locations in the context of pre-implant imaging. We characterized the amount of shift in ECoG electrode locations immediately after implant in a pediatric population. Electrode-shift was quantified as the difference in the electrode locations immediately after surgery (via post-operation CT) compared to the brain surface before the operation (pre-i

Identifying functional networks using endogenous connectivity in gamma band electrocorticography.

Correlations in spontaneous, infra-slow (<0.1 Hz) fluctuations in gamma band (70-100 Hz) signal recorded using electrocorticography (ECoG) reflect the functional organization of the brain, appearing in auditory and visual sensory cortex, motor cortex, and the default mode network (DMN). We have developed a data-driven method using co-modulation in spontaneous, infra-slow, and gamma band power fluctuations in ECoG to characterize the connectivity between cortical areas. A graph spectral clustering algorithm was used to identify networks that appear consistently. These networks were compared wit

Association of electrocorticography and seizure outcomes in resective pediatric epilepsy surgery.

OBJECTIVE: Up to 30% of children with focal epilepsy have drug-resistant epilepsy and may be candidates for epilepsy surgery. Intraoperative electrocorticography (iECoG) is a method to acutely delineate the epileptogenic zone during epilepsy resections, but its effectiveness is debated. The authors assessed the association between iECoG findings and seizure outcomes in pediatric epilepsy patients undergoing resective epilepsy surgery. METHODS: The authors conducted a retrospective cohort analysis of 115 patients at UPMC Children's Hospital of Pittsburgh who underwent resective epilepsy surger

The Potential for a Speech Brain-Computer Interface Using Chronic Electrocorticography.

A brain-computer interface (BCI) is a technology that uses neural features to restore or augment the capabilities of its user. A BCI for speech would enable communication in real time via neural correlates of attempted or imagined speech. Such a technology would potentially restore communication and improve quality of life for locked-in patients and other patients with severe communication disorders. There have been many recent developments in neural decoders, neural feature extraction, and brain recording modalities facilitating BCI for the control of prosthetics and in automatic speech recog

Does intraoperative electrocorticography alter seizure outcomes after temporal lobe epilepsy surgery?

OBJECTIVE: Surgery is the treatment of choice in drug-resistant temporal lobe epilepsy (TLE). The estimated seizure freedom after anterior temporal lobectomy and amygdalo-hippocampectomy (ATL-AH) is 70%-80%. Accurate identification of the epileptogenic zone by prompt presurgical evaluation reduces surgical failures. Our study aims to assess the utility of intraoperative electrocorticography (iECoG) in improving seizure outcomes following ATL-AH. METHODS: We enrolled patients with drug-resistant TLE who underwent ATL-AH from January 2009 to December 2018. They were followed up at 3 months, 12

National trends and complication rates for invasive extraoperative electrocorticography in the USA.

Invasive electrocorticography (ECoG) is used in patients when it is difficult to localize epileptogenic foci for potential surgical resection. As MR neuroimaging has improved over the past decade, we hypothesized the utilization of ECoG diminishing over time. Using the USA Nationwide Inpatient Sample, we collected demographic and complication data on patients receiving ECoG over the years 1988-2008 and compared this to patients with medically refractory epilepsy during the same time period. A total of 695 cases using extraoperative ECoG were identified, corresponding to 3528 cases nationwide a

Role of electrocorticography at surgery for lesion-related frontal lobe epilepsy.

BACKGROUND: The prognostic significance of epileptiform activity (EA) recorded intraoperatively at electrocorticography (ECOG) in patients with lesion-related frontal lobe epilepsy (FLE) is unknown. METHODS: The results of ECOG performed in 22 patients with intractable FLE and a circumscribed frontal lobe structural lesion were compared with postoperative seizure control. Three patients underwent re-operation for a total of 25 cases, 23/25 with post-resection ECOG. Lesions were neoplasms (12), hamartomas (6) and arteriovenous malformations (4). RESULTS: Outcomes were 15/25 Class I, 5/25 Clas

Optimizing therapies for neurobehavioral comorbidities of epilepsy using chronic ambulatory electrocorticography.

There is an unmet need to improve therapy for neuropsychiatric comorbidities that are highly prevalent in persons with epilepsy (PWE). However, diagnosing and monitoring the neurobehavioral symptoms is challenging as their presentation can overlap with seizures. In this retrospective study, we report the advantage of chronic ambulatory electrocorticography (ECoG) from implanted Responsive Neurostimulator System (RNS®) in characterizing these psychosomatic paroxysms as a possible ictal, postictal, or interictal phenomenon and how the diagnosis guided the therapy choices. Five out of 21 patients

Electrocorticography During Deep Brain Stimulation Surgery for Movement Disorders: Single-Center Experience.

Objective: Electrocorticography can serve as an intraoperative research tool during deep brain stimulation procedure, when patients are awake to participate in behavioral tasks or to allow recordings while awake but at rest. This report aims to describe the electrocorticography methods used in awake patients undergoing deep brain stimulation surgery at a single center and to describe the feasibility, safety, and usefulness of high-density electrocorticography for capturing high-resolution neurophysiological data during deep brain stimulation surgery. We hypothesize that the use of high-density

Intraoperative hyperventilation vs remifentanil during electrocorticography for epilepsy surgery - a case report.

BACKGROUND: Traditionally, intraoperative intracranial electroen-cephalography-recordings are limited to the detection of the irritative zone defined by interictal spikes. However, seizure patterns revealing the seizure onset zone are thought to give better localizing information, but are impractical due to the waiting time for spontaneous seizures. Therefore, provocation by seizure precipitants may be used with the precaution that spontaneous and provoked seizures may not be identical. OBJECTIVE: We present evidence that hyperventilation induced and drug induced focal seizures may arise from

Endoscopic transorbital approach with electrocorticography for resection of anterior temporal lobe mass.

The authors present the case illustration of a 32-year-old female with worsening seizures with slight right orbital proptosis of unclear etiology. Imaging revealed a growing right temporal pole mass concerning for a glial tumor. The patient was an ideal candidate for transorbital endoscopic resection given its location and her epilepsy history. The mass was completely removed with the aid of electrocorticography (ECOG) to ensure no additional perilesional epileptic focus. The patient has had a 6-month follow-up with no worsening seizures and favorable cosmetic outcome. The video can be found h

Electrocorticography reveals the dynamics of famous voice responses in human fusiform gyrus.

Voice and face processing occur through convergent neural systems that facilitate speaker recognition. Neuroimaging studies suggest that familiar voice processing engages early visual cortex, including the bilateral fusiform gyrus (FG) on the basal temporal lobe. However, what role the FG plays in voice processing and whether it is driven by bottom-up or top-down mechanisms is unresolved. In this study we directly examined neural responses to famous voices and faces in human FG with direct cortical surface recordings (electrocorticography) in epilepsy surgery patients. We tested the hypothesis

Decoding four hand gestures with a single bipolar pair of electrocorticography electrodes.

Objective.Electrocorticography (ECoG) based brain-computer interfaces (BCIs) can be used to restore communication in individuals with locked-in syndrome. In motor-based BCIs, the number of degrees-of-freedom, and thus the speed of the BCI, directly depends on the number of classes that can be discriminated from the neural activity in the sensorimotor cortex. When considering minimally invasive BCI implants, the size of the subdural ECoG implant must be minimized without compromising the number of degrees-of-freedom.Approach.Here we investigated if four hand gestures could be decoded using a si

Mapping cognitive activity from electrocorticography field potentials in humans performing NBack task.

Objective. Advancements in data science and assistive technologies have made invasive brain-computer interfaces (iBCIs) increasingly viable for enhancing the quality of life in physically disabled individuals. Intracortical microelectrode implants are a common choice for such a communication system due to their fine temporal and spatial resolution. The small size of these implants makes the implantation plan critical for the successful exfiltration of information, particularly when targeting representations of task goals that lack robust anatomical correlates.Approach. Intraoperative Ultrasound-Assisted 3D-Electrocorticography for resection of type II focal cortical dysplasias.

OBJECTIVE: Type II focal cortical dysplasias (FCDII) present a clear-cut anatomo-electro-clinical profile and are associated with optimal surgical outcome when completely resected. Alongside presurgical planning and neuroimaging, intraoperative electrocorticography (ECoG) can aid in delineating FCDII boundaries. We report outcomes from patients undergoing FCDII resection using 3D-ECoG with an intracerebral electrode guided by intraoperative ultrasound (ioUS).
