Long-term upper-extremity prosthetic control using regenerative peripheral nerve interfaces and implanted EMG electrodes.

Objective.Extracting signals directly from the motor system poses challenges in obtaining both high amplitude and sustainable signals for upper-limb neuroprosthetic control. To translate neural interfaces into the clinical space, these interfaces must provide consistent signals and prosthetic performance.Approach.Previously, we have demonstrated that the Regenerative Peripheral Nerve Interface (RPNI) is a biologically stable, bioamplifier of efferent motor action potentials. Here, we assessed the signal reliability from electrodes surgically implanted in RPNIs and residual innervated muscles i

Improved control of a prosthetic limb by surgically creating electro-neuromuscular constructs with implanted electrodes.

Remnant muscles in the residual limb after amputation are the most common source of control signals for prosthetic hands, because myoelectric signals can be generated by the user at will. However, for individuals with amputation higher up the arm, such as an above-elbow (transhumeral) amputation, insufficient muscles remain to generate myoelectric signals to enable control of the lost arm and hand joints, thus making intuitive control of wrist and finger prosthetic joints unattainable. We show that severed nerves can be divided along their fascicles and redistributed to concurrently innervate

Conduction studies in peripheral cat nerve using implanted electrodes: I. Methods and findings in controls.

Silicone rubber cuff and patch electrodes with multiple contacts were implanted along the sciatic-tibial-plantar nerves in cat for repeated studies of conduction properties of normal peripheral nerve over periods of time. The contacts were used in various combinations for precise localization of changes in conduction velocities and excitability along the extent of normal nerves. In this paper the particular characteristics and limitations associated with cuff-electrode recordings of neural activity are discussed. The nerve action potential was recorded using a tripolar configuration with a cen

Induced Dipoles and Possible Modulation of Wireless Effects in Implanted Electrodes. Effects of Implanting Insulated Electrodes on an Animal Test to Screen Antidepressant Activity.

There is evidence that Deep Brain Stimulation (DBS) produces health benefits in patients even before initiating stimulation. Furthermore, DBS electrode insertion in rat infralimbic cortex (ILC) provokes antidepressant-like effects before stimulation, due to local inflammation and astrogliosis. Consequently, a significant effect of implanting electrodes is suspected. External fields, similar in magnitude to the brain's endogenous fields, induce electric dipoles in conducting materials, in turn influencing neural cell growth through wireless effects. To elucidate if such dipoles influence depres

Prevention of cochlear implant electrode damage.

PURPOSE OF REVIEW: As the current trend in cochlear implantation is to prescribe cochlear implants for patients with residual hearing and to use electroacoustic stimulation, cochlear implant damage must be prevented. This article summarizes current research endeavors to prevent electrode insertion trauma and resulting hearing loss. RECENT FINDINGS: Alteration in surgical technique is necessary with each new electrode design. Nontraumatic surgical technique also requires minimizing acoustic trauma due to drilling the cochleostomy, mechanical damage from electrode insertion, potential infection

Surgical implantation of electroencephalographic electrodes in the dog.

A simple surgical technique for implanting chronic electroencephalographic electrodes in the skull of the dog was devised to obtain electroencephalograms from nonsedated and nonanesthetized dogs frequently. Orthopedic screws were used as implant electrodes and for securing the recording assembly over the frontal sinus. The electrode assemblies have been tolerated well for as long as 10 months in 17 of 20 dogs and have given reliable and satisfactory electroencephalograms for computer analysis and have been virtually free of all physiologic and motion artifacts in all dogs, with minimal use of

Early peripheral nerve regeneration after crushing, sectioning, and freeze studied by implanted electrodes in the cat.

Regeneration of axons after Wallerian degeneration is influenced by various factors localized to the distal nerve stump. We have examined elongation of axons to assess the relative influence of basal lamina and of Schwann cells in lesions with and without interruption of the Schwann cell tubes. In particular, we wanted to follow the growth of axons throughout the same distal nerve stump. Silicone cuff and patch electrodes with multiple contacts were therefore implanted around hindlimb nerves proximal and distal to the lesion. The tibial nerve was cut and sutured (15 nerves) or crushed (10 nerv

Explorative investigation on effects of multi-day neurofeedback with implanted electrodes in patients with Parkinson's disease.

Neurofeedback, which consists of recording and visualizing neural activity in real-time, is a method currently being investigated as a supplementary treatment for Parkinson's disease (PD). By using implanted deep brain stimulation (DBS) electrodes with interleaved sensing capability, previous studies have demonstrated the efficacy of neurofeedback based on beta oscillations in the basal ganglia. Herein, for the first time, we explored short-term neurofeedback ability over the course of multiple sessions with a fully implanted DBS system. Eight patients with PD participated in the study. Comparative effects of implanted electrodes with differing contact patterns on peripheral nerve regeneration and functional recovery.

Electrical stimulation could enhance nerve regeneration and functional recovery. The objective of this study was to evaluate the regenerative effects of implanted electrodes with different contacts in resected sciatic nerve. Sciatic nerve resection and microsurgical repair models were established and randomly divided into four groups (point contact, 1/4 circle contact; whole-circle contact; no electrodes as control). Electrical stimulation was performed and electrophysiological, morphological and histological exams (of the sciatic nerve and muscle) were conducted at 4 and 10 weeks post-implant

Stimulation of the human lumbar spinal cord with implanted and surface electrodes: a computer simulation study.

Human lumbar spinal cord networks controlling stepping and standing can be activated through posterior root stimulation using implanted electrodes. A new stimulation method utilizing surface electrodes has been shown to excite lumbar posterior root fibers similarly as with implants, an unexpected finding considering the distance to these target neurons. In the present study we apply computer modeling to compare the depolarization of posterior root fibers by both stimulation techniques. We further examine the potential for additional direct activation of motoneurons within the anterior roots. U

Effect of long-term implanted nerve cuff electrodes on the electrophysiological properties of human sensory nerves.

During a long-term implantation (307 days) of a tripolar split cuff electrode around the palmar digital nerve to the radial side of the left index finger, branching off the median nerve in a medullary lesioned C6 patient, the physiological state of the nerve was intensively monitored. The resulting sensory nerve action potential (SNAP) amplitude was recorded, using both near-nerve electrodes and the implanted cuff electrode. The SNAP amplitude declined within 10 days to approximately 50% of the first SNAP cuff amplitude measured on Day 2 after implantation and recovered to the initial amplitud

Grip control and motor coordination with implanted and surface electrodes while grasping with an osseointegrated prosthetic hand.

BACKGROUND: Replacement of a lost limb by an artificial substitute is not yet ideal. Resolution and coordination of motor control approximating that of a biological limb could dramatically improve the functionality of prosthetic devices, and thus reduce the gap towards a suitable limb replacement. METHODS: In this study, we investigated the control resolution and coordination exhibited by subjects with transhumeral amputation who were implanted with epimysial electrodes and an osseointegrated interface that provides bidirectional communication in addition to skeletal attachment (e-OPRA Implan

Lumbar sympathetic chain neuromodulation with implanted electrodes for long-term pain relief in loin pain haematuria syndrome.

Introduction. Loin pain-hematuria syndrome (LPHS) is a rare clinical entity causing unilateral or bilateral intractable flank and loin pain with hematuria. The etiology is poorly understood, and the diagnosis is made by exclusion of urological and nephrological conditions. The management is mainly symptomatic aiming for pain relief with nonopioid and opioids analgesics, and interventions such as capsaicin infusion into the renal pelvis, percutaneous regional nerve blocks, and laparoscopic or open surgical procedures, none of them providing lasting pain relief. Methods. We describe four cases o

Recording and stimulating properties of chronically implanted longitudinal intrafascicular electrodes in peripheral fascicles in an animal model.

The purpose of this experiment was to study the recording and stimulating properties, and biocompatibility of longitudinally implanted intrafascicular electrodes (LIFEs) in a rabbit sciatic nerve model when they were chronically implanted into peripheral fascicles. LIFEs were implanted chronically into sciatic nerve fascicles of rabbits as recording and stimulating electrodes. Motor-evoked potentials (MEPs) and cortical somatosensory-evoked potentials (CSEPs) were recorded by using a transcranial stimulation system (TCS) over 6-month period to observe the change of the signals recorded. New method of deep brain stimulation therapy with two electrodes implanted in parallel and side by side.

Reversibility and adaptability are preferred features of long-term therapeutic deep brain stimulation (DBS). In such therapy, a permanent stimulating electrode with four contact points is placed at the stimulation site and, generally speaking, bipolar stimulation is induced by various pairs of adjacent contact points on one electrode. The stimulation sites are thus all located along the trajectory of the implanted electrode. In a patient with unilateral severe essential tremor, the authors implanted two electrodes side by side and parallel to each other in the unilateral thalamic ventralis int

Regenerative peripheral nerve interfaces (RPNIs) and implanted electrodes improve online control of prostheses for hand and wrist.

Objective.Upper limb amputation severely limits daily activities and independence. Current prosthetic control methods often rely on surface electromyography (sEMG), which suffers from low signal quality and limited functionality. This study investigates whether implanted electrodes in regenerative peripheral nerve interfaces (RPNIs) and residual innervated muscles can provide stable and high-quality control signals to improve dexterous prosthetic hand and wrist function.Approach.Two individuals with upper-limb amputation had RPNIs created by suturing free skeletal muscle grafts to peripheral n

Prospective Evaluation of the Time Course of White Matter Edema Associated with Implanted Deep Brain Stimulation Electrodes.

INTRODUCTION: Deep brain stimulation (DBS) is commonly used in the treatment of medically refractory movement disorders. There have been several reports in the literature of edema developing around the implanted electrode. Most of these studies have been retrospective, suggesting that the time course and incidence of this edema are underestimated. An understanding of the incidence and time course of edema related to DBS leads is important to allow clinicians to better assess the correct course of action when edema following DBS implantation is observed. METHODS: We examined both the time cour
