Improving the accuracy of automated gout flare ascertainment using natural language processing of electronic health records and linked Medicare claims data.

BACKGROUND: We aimed to determine whether integrating concepts from the notes from the electronic health record (EHR) data using natural language processing (NLP) could improve the identification of gout flares. METHODS: Using Medicare claims linked with EHR, we selected gout patients who initiated the urate-lowering therapy (ULT). Patients' 12-month baseline period and on-treatment follow-up were segmented into 1-month units. We retrieved EHR notes for months with gout diagnosis codes and processed notes for NLP concepts. We selected a random sample of 500 patients and reviewed each of their

"The Computer as a Sophisticated Disguise of the Devil" : Supposed Technophobia in German-Speaking Protestant Churches towards Electronic Data Processing (EDP).

Christian churches are sometimes perceived as being hostile towards technology due to the supposed incompatibility of the cold, rational world of technology and the warm, humane organization of the church. Even when electronic data processing (EDP) was introduced in German-speaking Protestant churches in Western Europe, a lack of acceptance of technology was repeatedly problematized by church digitization pioneers as a central hurdle in church digitization processes. According to them, the "synchronization" (David Gugerli) of church and computer was therefore to prove particularly prone to con

Beyond electronic health record data: leveraging natural language processing and machine learning to uncover cognitive insights from patient-nurse verbal communications.

BACKGROUND: Mild cognitive impairment and early-stage dementia significantly impact healthcare utilization and costs, yet more than half of affected patients remain underdiagnosed. This study leverages audio-recorded patient-nurse verbal communication in home healthcare settings to develop an artificial intelligence-based screening tool for early detection of cognitive decline. OBJECTIVE: To develop a speech processing algorithm using routine patient-nurse verbal communication and evaluate its performance when combined with electronic health record (EHR) data in detecting early signs of cogni

A Systematic Process for Assessing Fitness-for-Purpose of Health Outcomes for Computable Phenotyping With Electronic Health Record Data.

PURPOSE: Information from electronic health records (EHRs) may be incorporated into computable phenotype algorithms in efforts to overcome inaccuracies of algorithms based on administrative claims data alone. However, such efforts can be resource-intensive and unsuccessful. Assessing the feasibility of computable phenotyping for a health outcome of interest (HOI) before proceeding is therefore recommended. METHODS: We developed a systematic fitness-for-purpose (FFP) assessment process to implement concepts outlined in a previously described general framework for computable phenotyping incorpo

Advanced processing of differential phase contrast data: Distinction between different causes of electron phase shifts.

Differential phase contrast, in its high resolution modification also known as first moment microscopy or momentum resolved STEM [1-7] , basically measures the lateral momentum transfer to the electron probe due to the beam interaction with either electrostatic and/or magnetic fields, when the probe transmits the specimen. In other words, the result of the measurement is a vector field p→(x,y) which describes the lateral momentum transfer to the probe electrons. In the case of electric fields, this momentum transfer is easily converted to the electric field E→(x,y) causing the deflection, and

Using Natural Language Processing of Clinical Notes to Supplement Structured Electronic Health Record Data for Phenotyping Smoking and Obesity in a Healthcare System.

PURPOSE: Studies based on electronic health records (EHR) often rely on structured data, which may incompletely capture important clinical phenotypes in EHR notes. The purpose of this study was to assess two natural language processing (NLP) tools to extract phenotypes from unstructured EHR notes, and to evaluate the added value of integrating NLP-derived phenotypes with structured EHR data at a health system scale. METHODS: This retrospective study is based on inpatient and outpatient EHR data from the Mass General Brigham healthcare system between January 1, 2019 and December 31, 2020. Two

Development and Validation of a Natural Language Processing Algorithm Using Electronic Health Record Data to Identify Breast Cancer Patients With Low Social Support.

PURPOSE: Social support is important in the management of breast cancer. We developed a series of structured variables or modules from electronic health record (EHR) data to form the basis for the development of EHRsupport, an EHR-based measure of social support. METHODS: We built a natural language processing (NLP) algorithm from clinical notes in 7,989 women diagnosed from January 2006 to September 2021 with invasive breast cancer. Our team reviewed charts, developed a lexicon, manually crafted rules, applied the NLP algorithm to 565,258 EHR notes and 68,760 patient messages -1 to +3 months

Bias and fairness assessment of a natural language processing opioid misuse classifier: detection and mitigation of electronic health record data disadvantages across racial subgroups.

OBJECTIVES: To assess fairness and bias of a previously validated machine learning opioid misuse classifier. MATERIALS & METHODS: Two experiments were conducted with the classifier's original (n = 1000) and external validation (n = 53 974) datasets from 2 health systems. Bias was assessed via testing for differences in type II error rates across racial/ethnic subgroups (Black, Hispanic/Latinx, White, Other) using bootstrapped 95% confidence intervals. A local surrogate model was estimated to interpret the classifier's predictions by race and averaged globally from the datasets. Subgroup analy

Implementing a Structured Head and Neck Cancer Care Pathway in an Electronic Health Record: Iterative Process and Effects on Data Quality.

The digital availability of health data not only improves processes in primary care, but it also facilitates the evaluation of healthcare delivery. Nevertheless, the preprocessing of data for secondary use is still time-consuming and expensive, particularly in head and neck cancer (HNC), where patients undergo complex multidisciplinary treatment trajectories. Therefore, we have looked further into the effects on data quantity and quality following the implementation of structured care pathways. Leveraging data extracted from these care pathways, we assessed the potential of real-time quality-o

[Calculation of the true angle in malposition of the skeletal system using electronic data processing. Retrospective study of 53 axis deviations in healed femoral fractures].

Under standard conditions deformations in the skeletal system are diagnosed by two X-ray photographs whose planes of projection form a right angle. The degree of deviation from the axis is measured directly on the X-ray photograph with a protractor. The analysis of the position of the axis of a tabular bone is carried out by determining the varus/valgus or ante/recurvation deformities resp. in the a.-p. and lateral rays. The degree of deformation is expressed through the supplementary angle. We asked ourselves whether a diagnostic procedure using X-rays taken in two planes is suitable for subs

A mid-level data fusion approach for evaluating the internal and external changes determined by FT-NIR, electronic nose and colorimeter in Curcumae Rhizoma processing.

The aim of this study was to establish a robust and rapid method for identification of the changes of Curcumae Rhizoma (CR) in its physical properties and the components after processing. Color and odor used as the indexes for physical property evaluation in this study were quantified by non-targeted bionic detectors colorimeter and e-nose according to Chinese pharmacopoeia, and all chemical changes of the samples were observed by NIR. The technique of mid-level data fusion was adopted to simulate the recognition mode of human, and the prediction accuracy was 100%. A modified NIR band extracti

Developing and testing a framework for coding general practitioners' free-text diagnoses in electronic medical records - a reliability study for generating training data in natural language processing.

BACKGROUND: Diagnoses entered by general practitioners into electronic medical records have great potential for research and practice, but unfortunately, diagnoses are often in uncoded format, making them of little use. Natural language processing (NLP) could assist in coding free-text diagnoses, but NLP models require local training data to unlock their potential. The aim of this study was to develop a framework of research-relevant diagnostic codes, to test the framework using free-text diagnoses from a Swiss primary care database and to generate training data for NLP modelling. METHODS: Th

Quantitative mapping of geological samples by electron microprobe using mean atomic number (MAN) background corrections: method application, data processing and visualization.

Electron probe microanalysis (EPMA) and wavelength dispersive spectroscopy (WDS) are ubiquitous tools for chemical analysis of geological samples. Analysis type is often confined to quantitative but spatially-limited (∼0.5-2+ µm diameter) spot analysis, or qualitative but spatially-resolved mapping. This contribution outlines a method that combines the quantitative aspect of spot analyses with the spatial resolution of EPMA mapping and associated data processing and visualization workflows. The acquisition and processing of spatially-resolved, quantitative chemical maps of geological samples a
