Medication adherence and costs of medical care among patients with Parkinson's disease: an observational study using electronic medical records.

BACKGROUND: Adherence to antiparkinsonian drugs (APDs) is critical for patients with Parkinson's disease (PD), for which medication is the main therapeutic strategy. Previous studies have focused on specific disorders in a single system when assessing clinical factors affecting adherence to PD treatment, and no international comparative data are available on the medical costs for Chinese patients with PD. The present study aimed to evaluate medication adherence and its associated factors among Chinese patients with PD using a systematic approach and to explore the impact of adequate medication

Discrepancy in medications reported by elderly patients in the dental office and in their electronic medical records: A pilot study.

AIMS: This study's main objective was to analyze the discrepancy between the dental medication record (DMR) and the physician-prescribed active medications recorded in the medical medication record (MMR). METHODS: The study group consisted of 100 adults who attended the University Dental Clinic (Santiago de Compostela, Spain) requesting dental care. A dental history was created for all participants that included the DMR. The MMR were compiled from their electronic medical records. RESULTS: About 80% of the patients consumed at least one drug (94.2% of those >65 years) and 19% took more than

Electronic Medical Record Context Signatures Improve Diagnostic Classification Using Medical Image Computing.

Composite models that combine medical imaging with electronic medical records (EMR) improve predictive power when compared to traditional models that use imaging alone. The digitization of EMR provides potential access to a wealth of medical information, but presents new challenges in algorithm design and inference. Previous studies, such as Phenome Wide Association Study (PheWAS), have shown that EMR data can be used to investigate the relationship between genotypes and clinical conditions. Here, we introduce Phenome-Disease Association Study to extend the statistical capabilities of the PheW

Current Practice for Electronic Medical Device Management in Radiation Therapy.

AIMS: Electronic medical devices (EMDs) must be carefully managed in radiation oncology to prevent radiation-induced malfunction. Only one EMD, cardiovascular implantable electronic devices (CIEDs), currently has a recommended protocol for clinical management. This international survey aims to improve understanding of the current state of EMD management for CIEDs and six other EMDs. Characterizing the current state of EMD management is a crucial step towards the development of additional protocols. MATERIALS AND METHODS: This survey was distributed to more than 2500 physicists via email. The

Characterization and validation of electronic medical record data for pharmacoepidemiologic research.

PURPOSE: Electronic medical records (EMRs) are essential for pharmacoepidemiologic research; however, information on the quality of Canadian hospital EMR data is scarce. We aimed to test data validity in the Epic EMR for key pharmacoepidemiologic themes using QT-prolonging medications and major adverse cardiac events (MACEs) as the research question. METHODS: An entity relationship diagram (ERD) was developed to navigate >20,000 tables. Computational data validation (comparison with Epic SlicerDicer and Canadian Institute for Health Information Discharge Abstract Database (CIHI-DAD) data) and

Clinical large language model centered on electronic medical records.

In the quest to enhance medical consultation, our study introduces AI4Doctor, a sophisticated large-language model (LLM) tailored for the clinical domain. At the heart of AI4Doctor is an innovative integration strategy that synergizes distilled data extracted from electronic medical records (EMR) with empirical insights gathered from practicing physicians during the supervised fine-tuning. Although existing platforms offer informative responses, they fall short of replicating the nuanced decision-making processes of medical professionals, particularly in complex, integrative diagnostic scenari

Electronic Chip-Mimetic Medical Implants for Controllable Sonothermal Treatments.

The sterility and controllable biological functionalities of implantable medical devices determine their lifespan, safety, and therapeutic efficacy. Inspired by the thermal effect of working electronic chips, which consist of various heterostructures, we have developed an electronic chip-mimetic sonothermal platform through constructing metal-semiconductor heterogeneous interface. Through magnetron sputtering on the surface of the sandblasted/acid-etched pretreated titanium (pTi), we find that semiconductor coatings (TiO2, Si, ZnO, and Te) endow pTi with different in situ sonothermal effects (

Drug Reaction with Eosinophilia and Systemic Symptoms in the Electronic Medical Record.

INTRODUCTION: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare severe cutaneous adverse reaction (AR), which can be life threatening [J Am Acad Dermatol. 2013;68(5):693.e1-14]. The reporting of DRESS in Australian electronic medical records (EMRs) has not been studied. METHODS: A dual ascertainment strategy was employed in this multi-centre, retrospective study. Part 1 involved data collection from a 2.5-year cohort of consecutive inpatient admissions. Patients with an AR label of DRESS, eosinophilia or severe rash were identified, along with the causative agents. Part

National Cancer Grid initiative for electronic medical records, India.

PROBLEM: Inefficient workflows, incomplete data and lack of interoperability can hinder the uptake of electronic records systems, challenges particularly relevant in cancer treatment with its complex longitudinal and multidisciplinary nature. Further, products developed in high-income countries are not designed for compatibility with the workflows of low- and middle-income countries, which face additional issues of cost. APPROACH: We evaluated centres with different resources and geographical locations to develop the requirements of our product. We published an invitation to potential vendors

Sequences of Events from the Electronic Medical Record and the Onset of Infection.

We present a novel model of time-series analysis to learn from electronic health record (EHR) data when infection occurred in the intensive care unit (ICU) by translating methods from proteomics and Bayesian statistics. Using 48,536 patients hospitalized in an ICU, we describe each hospital course as an 'alphabet' of 23 physician actions ('events') in temporal order. We analyze these as k-mers of length 3-12 events and apply a Bayesian model of (cumulative) relative risk (RR). The log2-transformed RR (median=0.248, mean=0.226) supported the conclusion that the events selected were individually

Usefulness of the Parental Electronic Diary During Medical Rounds in a NICU.

Parental involvement in the care of their baby in family rooms in neonatal intensive care units (NICUs) can be improved. This could be done with an electronic medical report completed by the parents, which is then linked to the patient record system. The parents selected for this study completed an electronic diary during their stay in the NICU, while the staff answered a questionnaire about their opinion on the usefulness of the parents' diary. The length of stay, length of time the baby spent in Kangaroo care, breastfeeding, time given to breastfeeding, feeling of tiredness, the capability o

Discovering novel disease comorbidities using electronic medical records.

Increasing reliance on electronic medical records at large medical centers provides unique opportunities to perform population level analyses exploring disease progression and etiology. The massive accumulation of diagnostic, procedure, and laboratory codes in one place has enabled the exploration of co-occurring conditions, their risk factors, and potential prognostic factors. While most of the readily identifiable associations in medical records are (now) well known to the scientific community, there is no doubt many more relationships are still to be uncovered in EMR data. In this paper, we

Enhancing pediatric airway safety using the electronic medical record.

OBJECTIVE: Difficult intubations are not uncommon in tertiary care children's hospitals, and effective documentation of the difficult airway is a fundamental element of safe airway management. The primary goal of our quality improvement initiative was to improve access to airway information via an alert and documentation system within the electronic medical record (EMR). METHODS: We created a difficult airway alert within the EMR, linking common airway evaluation templates used by specialists involved in airway management. We assessed the time required for different specialists to answer an a

Learning bundled care opportunities from electronic medical records.

OBJECTIVE: The traditional fee-for-service approach to healthcare can lead to the management of a patient's conditions in a siloed manner, inducing various negative consequences. It has been recognized that a bundled approach to healthcare - one that manages a collection of health conditions together - may enable greater efficacy and cost savings. However, it is not always evident which sets of conditions should be managed in a bundled manner. In this study, we investigate if a data-driven approach can automatically learn potential bundles. METHODS: We designed a framework to infer health con

Identifying Axial Spondyloarthritis in Electronic Medical Records of US Veterans.

OBJECTIVE: Large database research in axial spondyloarthritis (SpA) is limited by a lack of methods for identifying most types of axial SpA. Our objective was to develop methods for identifying axial SpA concepts in the free text of documents from electronic medical records. METHODS: Veterans with documents in the national Veterans Health Administration Corporate Data Warehouse between January 1, 2005 and June 30, 2015 were included. Methods were developed for exploring, selecting, and extracting meaningful terms that were likely to represent axial SpA concepts. With annotation, clinical expe

A C+ + class library foundation for developing an electronic medical record.

The basic features that gave the M (or Mumps, as it originally called) language its initial popularity are still important to many programming applications, especially those involving the health care domain. To date, however, efforts to change the language have been predicated on the axiom that the existing structure must be preserved in order to accommodate legacy applications. To many, it is becoming clear that the 1960s' minicomputer induced interpreter syntax underlying M is inadequate and incapable of further evolution. In this paper, we demonstrate an alternative that preserves those fea

Staff utilisation of an electronic medical record during nursing handover.

BACKGROUND: Electronic medical records (EMRs) are widely implemented to improve information access, patient safety and care coordination. However, less is known about how EMRs are used in routine clinical processes such as nursing handover. Understanding how EMRs influence handover practices is important, given the central role of handover in continuity of care and patient safety. METHOD: An exploratory mixed-methods comparative case study using a cross-sectional survey design was conducted within a large multi-site public healthcare organisation in Victoria, Australia. An organisation-wide s
