During the past 35 years, cardiac electrophysiologic examinations have evolved into widely employed standard clinical tools in the diagnostics of different symptoms--like unexplained syncope or palpitations etc., as well as in the management of different cardiac arrhythmias. This review article presents current opinion regarding the indications and interpretations of invasive electrophysiologic examination in the bradyarrhythmias and in supraventricular and ventricular tachyarrhythmias. It is stressed that in the course of these studies, therapeutic catheter ablation procedures are being perfo

Recainam, [N-2,6-dimethylphenyl-N'-3-(1-methylethyl-amino)propylurea] hydrochloride (Wy-42,362), is a new class I antiarrhythmic agent that has been shown to be very effective in suppressing premature ventricular contractions in humans. To clarify the mechanism of antiarrhythmic action, the electrophysiologic effects of recainam were examined in canine cardiac Purkinje fibers using standard microelectrode techniques. Recainam at 3-100 microM (1-30 micrograms/ml) produced concentration-dependent decreases in action potential duration (APD), membrane responsiveness, and maximal upstroke velocity

One of the principle aims in the assessment of the patient with hypertrophic cardiomyopathy is assessment of risk for sudden cardiac death. Conventional risk stratification is reviewed together with other non-invasive techniques such as exercise physiology; the signal averaged electrocardiogram and heart rate variability. The role of arrhythmias in the pathogenesis of sudden cardiac death in hypertrophic cardiomyopathy is reviewed together with the role of programmed ventricular stimulation. A novel electrophysiological technique is described in detail which aims to measure the electrophysiolo

Cardiac arrhythmias are associated with significant morbidity and mortality. Ventricular arrhythmogenesis is a dynamic interplay among an arrhythmogenic substrate, myocardial electrophysiologic properties, modifying factors, and triggering factors. Imbalances in the autonomic nervous system can lead to increased myocardial excitability, which is a major contributor to the pathophysiology of ventricular tachyarrhythmias. Myocardial excitability and ventricular arrhythmogenesis are modulated perioperatively through hemodynamic and physiologic management, anesthetic agents, or regional anesthetic

Cardiac arrhythmias are associated with cardiovascular morbidity and mortality. Cardiac electrophysiology studies (EPS) use intracardiac catheter recording and stimulation for profound evaluation of the heart's electrical properties. The main clinical application is investigation and treatment of rhythm disorders. These techniques have been translated to the murine setting to open opportunities for detailed evaluation of the impact of different characteristics (including genetics) and interventions on cardiac electrophysiology and -pathology. Currently, a detailed description of the technique

The insights into the mechanisms of supraventricular tachycardia provided by the techniques of intracardiac recording and programmed cardiac stimulation have had a major impact on our therapeutic approach to this common problem. These techniques have led to a more systematic analysis and precise interpretation of the standard surface electrocardiogram. An accurate diagnosis of the mechanism of supraventricular tachycardia can be made in the majority of patients using the electrocardiogram aided by special leads (eg, Lewis leads) and bedside maneuvers. Identification of the likely electrophysio

Electrophysiologic testing continues to play an important role in injury stratification and prognostication in patients who are comatose after cardiac arrest. As discussed previously, however, the adage about treating whole patients, not just the numbers, is relevant in this situation. EEG and SSEP can offer high specificity for discerning poor prognosis as long as they are applied to appropriate patient populations. As discussed previously, EEG and SSEP patterns change during the first hours to days after cardiac arrest and negative prognostic information should not be based solely on studies

During the past 14 years there have been major advances in the field of clinical electrophysiology. This progress is a result of a more extensive use of intracardiac electrode catheters with recordings from multiple sites in the right and left cardiac chambers, the introduction of programmed electrical stimulation techniques and the use of antiarrhythmic drugs for diagnostic and therapeutic purposes during acute electrophysiologic testing. This article examines the pioneering studies and the subsequent developments in the field of clinical electrophysiology. The specific topics that are review

His bundle pacing produces electrocardiograms (ECGs) that give the impression that it is physiological like no other form of cardiac pacing requiring ventricular stimulation. Several special features of the implantation technique of a His bundle lead that distinguish it significantly from the implantation of other endocardial leads need to be considered. This overview provides a number of practical "hands-on" tips and tricks, from implantation set-up (12-lead ECG, no electrophysiology unit, no His bundle catheter), venous access, handling of delivery catheters and leads, interpretation of intr

The global population of adults with congenital heart disease (CHD) now exceeds that of children, reflecting decades of surgical and medical advances. As this cohort has aged, arrhythmias have emerged as a predominant cause of morbidity, hospitalization, and late mortality. Arrhythmia mechanisms in adult CHD (ACHD) arise from congenital abnormalities, surgically altered substrates, progressive hemodynamic lesions, multisystem comorbidities, and acquired cardiovascular risk factors, creating a distinct electrophysiological (EP) phenotype that diverges fundamentally from that of populations with

The approach to supraventricular tachycardia (SVT) diagnosis can be complex because it involves synthesizing baseline electrophysiologic features, features of the SVT, and the response(s) to pacing maneuvers. In this two-part review, we will mainly explore the latter while recognizing that neither of the former can be ignored, for they provide the context in which diagnostic pacing maneuvers must be correctly chosen and interpreted. Part 1 involved a detailed consideration of ventricular overdrive pacing, since this pacing maneuver provides the diagnosis in the majority of cases. In Part 2, ot

Congenital left ventricular aneurysms and diverticula (LVA/Ds) are rare cardiac malformations that can be detected using echocardiography or other imaging techniques. Some of these patients present with ventricular arrhythmias. This study investigated clinical characteristics of patients with congenital LVA/D presenting with arrhythmic manifestations. Over the previous 20 years 250 patients were diagnosed to have congenital LVA/D at our institution. Diagnosis was made using echocardiography after exclusion of coronary artery disease, local cardiac inflammatory processes, traumatic causes, or c

BACKGROUND: Diagnostic supraventricular tachycardia (SVT) features and pacing maneuvers tend to be specific but insensitive. Therefore, diagnosis often requires the integration of multiple pieces of information and/or pacing maneuvers, which adds to the complexity of catheter ablation procedures.
OBJECTIVE: The purpose of this study was to determine if a single diagnostic pacing maneuver, namely, ventricular overdrive pacing including a basal pacing site near the earliest atrial activation, provides a definitive SVT diagnosis in nearly all patients.
METHODS: Sixty-seven consecutive patients
