Case study: ED acts quickly after anthrax.

After last year's anthrax attacks, EDs at Capital Health System in Trenton, NJ, had to address decontamination of large numbers of patients. Patients who didn't require decontamination were sent to the hospital's employee health facility so they could bypass the ED. Clinicians, nursing staff, educators, and paramedics were trained in the decontamination process. Direct communication with first responders was established with a portable scanner.

"Stay Calm and Pick Up Signs to Understand the Cause": Registered Nurses Use Multiple Strategies for Handling Difficult Emergency Calls.

BACKGROUND: Telephone triage at emergency medical dispatch centers (EMDCs) is often challenging for registered nurses (RNs), and some calls may be difficult to handle. To enable education and interventions for call-taking RNs, lessons can be learned by studying authentic calls. AIM: The aim of this study was to describe the strategies used by RNs in difficult calls to EMDCs. METHODS: Call data from 2022 to 2023 were extracted for 14 RNs at three EMDCs in Sweden. Difficult calls were identified based on prior research and operationalized primarily using keyword searches in the free text call

Image transmission and video live-streaming in emergency medical communication centre: a narrative review of its feasability, use, and impact.

INTRODUCTION: Emergency Medical Communication Centres (EMCC) are responsible for assessing calls, providing pre-arrival instructions, and dispatching resources. Telephone-based triage is limited by the lack of visual information, which may lead to under-triage or over-triage. Telemedicine, including image transmission and video live-streaming has emerged as a complementary tool, but its actual impact in EMCC remains uncertain. We aimed to summarise the existing literature on the feasibility, use and impact of telemedicine using image transmission or video live-streaming in EMCCs. METHODS: We

[Integration of the hospital coronary care unit with the first aid service for the domiciliary treatment of cardiovascular emergencies].

To evaluate the feasibility and the utility of an integrated service for the pre-hospital diagnosis and care of cardiovascular emergencies a new pre-hospital intensive care system has been developed. Such an emergency medical service relies on the telephone transmission of ECG and the conversation between the first aid service and the cardiologist of the hospital coronary care unit. It also implies early therapeutic intervention performed at home under the responsibility of the chief physician of the referring centre. From March '86 to December '88, 311 telephone ECG transmissions were obtaine

Critical issues in urban emergency medical services for children.

In order to be effective those wishing to improve emergency care of children in an urban environment must be aware of barriers as well as resources. Urban children are at high risk for requiring emergency care as a result of both illness and injury. These children face a dangerous environment resulting from the problems of poverty, homelessness, overcrowded living conditions, drug abuse, and a shrinking tax base. They face this nation's highest rates of violent injury (intentional and unintentional), immunization delays, and preventable infectious diseases such as TB and measles. In addition,

LiVE-STROKE: live video from bystander to the medical dispatcher in potential stroke cases.

BACKGROUND: Identification of stroke during an emergency call remains challenging. We aimed to explore performed stroke examination with prehospital live video transmission during an emergency call from a bystander's smartphone to the medical dispatcher. METHODS: Since 2019, the Emergency Medical Services (EMS) in Copenhagen, consisting of the Emergency Medical Coordination Centre (EMCC) and the Out-of-Hours Health Service (OOHS), have enabled medical dispatchers to add live video to emergency calls. Dispatchers are registered nurses or paramedics. Live video is initiated via text message lin

Which emergency medical dispatch codes predict high prehospital nontransport rates in an urban community?

BACKGROUND: The Medical Priority Dispatch System (MPDS) is a commonly used computer-based emergency medical dispatch (EMD) system that is widely used to prioritize 9-1-1 calls and optimize resource allocation. There are five major priority classes used to dispatch 9-1-1 calls in the San Francisco System; Alpha codes are the lowest priority (lowest expected acuity) and Echo are the highest priority. OBJECTIVE: We sought to determine which MPDS dispatch codes are associated with high prehospital nontransport rates (NTRs). METHODS: All unique MPDS call categories from 2009 in a highly urbanized

[ACN (Automatic Collision Notification)--reducing fatalities in traffic accidents by automated accident reporting].

To improve patient's outcome of seriously injured occupants after vehicle accident rescue should be performed as soon as possible. While the rescue-time-period after EMS alarming is well-defined the rescue-time-period before EMS-alarm is very variable from minutes to several hours. To reduce the rescue-time-period between accident occurrence and EMS-alarm the Automatic Collision Notification (ACN) was developed. The ACN is a new invehicle-equipment which detects a severe vehicle crash and alarms via cellular phone the EMS automatically. Simultaneously the exact accident location is transmitted

[Emergency medical service response to major incidents. A study of 21 cases].

UNLABELLED: Major incidents are high-profile events where many lives are at stake. The response of the health-care-related agencies has to be well-planned and co-ordinated, thus retaining the public's confidence in the emergency services whilst efficiently responding to those in need. The communication between supervising officers such as medical incident officer (MIO) and ambulance incident officer (AIO) with the ambulance personnel is vital for the proper employment of doctors and ambulance teams at the incident scene. In Germany the experience gained at such events has not yet been collecte

Accidents and trauma care--1983.

Except for categoric grants, the federal government has divested itself of financing emergency medical services. Now the leadership for regional planning of emergency medical services must come from the state, usually from the health department. If we are to obtain improved hospital care of trauma patients, it is critical that we separate hospitals that have made a genuine commitment to the care of the multiply injured patient from hospitals that have not in order to avoid the tragedy of patients being delivered to hospitals that have inadequate resources or commitment or organization to meet

Stroke symptoms and the decision to call for an ambulance.

BACKGROUND AND PURPOSE: Few acute stroke patients are treated with alteplase, partly because of significant prehospital delays after symptom onset. The aim of this study was to determine among ambulance-transported stroke patients factors associated with stroke recognition and factors associated with a call for ambulance assistance within 1 hour from symptom onset. METHODS: For 6 months in 2004, all ambulance-transported stroke or transient ischemic attack patients arriving from a geographically defined region in Melbourne (Australia) to 1 of 3 hospital emergency departments were assessed. Legal Barriers to Implementing AI-Enabled Emergency Medical Services in Korea: Focus on the Medical Service Act, Emergency Medical Service Act, and Personal Information Protection Act.

OBJECTIVES: In prehospital and in-hospital emergency care, artificial intelligence (AI)-enabled emergency medical services (EMS) systems are reshaping clinical workflows through real-time triage, automated hospital assignment, telemedicine, and biometric monitoring. However, their implementation in Korea has revealed substantial legal and regulatory barriers. This study analyzed the structural incompatibilities between AI-enabled EMS and Korea's current legal framework, with particular attention to the Medical Service Act, Emergency Medical Service Act, and Personal Information Protection Act.

A satellite infrastructure for health early warning in post-disaster health management.

The risk of epidemics and emerging or re-emerging diseases such as avian flu, tuberculosis, malaria and other vector-borne diseases, is rising. These risks can be contained with prevention, early warning, and prompt management. Despite progress in information technology, communication is still a bottleneck for health early warning and response systems in post-disaster situations. This paper presents Satellites for Epidemiology (SAFE), a component-based interoperable architecture for health early warning that employs satellite, radio, and wireless networks, geographic information systems, integ

Medical dispatchers recognise substantial amount of acute stroke during emergency calls.

BACKGROUND: Immediate recognition of stroke symptoms is crucial to ensure timely access to revascularisation therapy. Medical dispatchers ensure fast admission to stroke facilities by prioritising the appropriate medical response. Data on medical dispatchers' ability to recognise symptoms of acute stroke are therefore critical in organising emergency stroke care. We aimed to describe the sensitivity and positive predictive value of medical dispatchers' ability to recognise acute stroke during emergency calls, and to identify factors associated with recognition. METHODS: This was an observatio

Criteria codes for unrecognized stroke/TIA patients by Emergency Medical Services dispatchers in Copenhagen, Denmark.

BACKGROUND: Early diagnosis is essential in stroke treatment. Medical dispatchers play a key role in ensuring quick assessment, transport, and treatment of stroke patients by recognizing stroke symptoms. We investigated which diseases were suspected by the dispatchers as well as the dispatch priority in the emergency response, for unrecognized strokes/transient ischemic attacks (TIAs) in calls to Emergency Medical Services in the Capital Region of Denmark in the years 2015–2021. METHODS: In this retrospective registry study, we collected data from the Danish Stroke Registry and from the electr

How many emergency dispatches occurred per cardiac arrest?

BACKGROUND: The Medical Priority Dispatch System (MPDS) is an emergency medical dispatch (EMD) system that is widely used to prioritize 9-1-1 calls and optimize resource allocation. Calls are assigned an MPDS determinant, which includes a number (1-32) representing chief complaint and priority (Alpha through Echo) representing acuity. OBJECTIVE: This study evaluates the number of emergency dispatches per cardiac arrest (NOD-CA) in cardiac arrest and non-cardiac arrest MPDS determinants. METHODS: All patients assigned a determinant by MPDS from January 1, 2008 to June 30, 2009 in a large metr

Trauma care systems in Spain.

Trauma care systems in Spain are provided by the Nacional Health Service in a decentralized way by the seventeen autonomous communities whose process of decentralization was completed in January 2002. Its organisation is similar in all of them. Public sector companies of sanitary emergencies look after the health of citizens in relation to medical and trauma emergencies with a wide range of up to date resources both technical and human. In the following piece there is a description of the emergency response teams divided into ground and air that are responsible for the on site care of the pati

Recommended modifications and applications of the Hospital Emergency Incident Command System for hospital emergency management.

The Hospital Emergency Incident Command System (HEICS), now in its third edition, has emerged as a popular incident command system model for hospital emergency response in the United States and other countries. Since the inception of the HEICS in 1991, several events have transformed the requirements of hospital emergency management, including the 1995 Tokyo Subway sarin attack, the 2001 US anthrax letter attacks, and the 2003 Severe Acute Respiratory Syndrome (SARS) outbreaks in eastern Asia and Toronto, Canada. Several modifications of the HEICS are suggested to match the needs of hospital e

Impact on hospital functions following the 2010 Chilean earthquake.

OBJECTIVE: The objective of this study was to assess the impact of the 2010 Chilean earthquake on hospital functions and services. Hospitals functioning in a post-disaster environment must provide emergency medical care related to the event, in addition to providing standard community health services. This study focused on damage to both structural and nonstructural components, as well as to utility services. METHODS: Site visits were made to every hospital in a single province (Bio-Bio). Engineers conducted damage assessments while interviews of hospital administrators were conducted. The su
