State Laws Banning Prior Authorization For Medications For Opioid Use Disorder Increased Substantially, 2015-23.

Prior authorization is a barrier to accessing medications for opioid use disorder (MOUD). Although private insurance covered about one-third of patients with OUD in 2023, the understanding of prior authorization prohibitions in private insurance remains limited. We synthesized state laws prohibiting prior authorization for MOUD in private insurance in the US. The number of states with such prohibitions grew from two in 2015 to twenty-two in 2023, with variations in the scope of these prohibitions. Seven states have fully prohibited prior authorization for all MOUD ("full prohibitions") since t

From discovery through emergency use to the present: Safety evaluation of the COVID-19 mRNA-1273 (Moderna) vaccine.

The COVID-19 pandemic created an urgent need to develop preventive vaccines to blunt the impact of the greatest global health crisis in a century. Two vaccines, both employing mRNA technology, were developed from bench to bedside in under one year - a milestone considered nearly impossible. This paper examines the evolving safety profile of mRNA-1273, from development under Operation Warp Speed through Emergency Use Authorization, and subsequent deployment at nearly unprecedented scale. Vaccine safety was characterized during clinical development and refined through well-established safety mon

Access to atrial flow regulators for implantation in critically ill children via the US Food and Drug Administration's emergency use pathway.

OBJECTIVES: The authors conducted a review of the US Food and Drug Administration's (FDA) emergency use authorization (EUA) pathway as a mechanism to use the Occlutech Atrial Flow Regulator (AFR) (Occlutech International AB) for critically ill pediatric patients with congenital heart disease. METHODS: The authors describe 8 pediatric patients in whom the EUA pathway was activated to permit AFR implantation and provide potentially lifesaving therapy. RESULTS: Emergency authorization was granted for each of the 8 cases with successful implantation of the AFR. There were 2 cases in which the pa

Temporal changes in auditory adverse event reporting in the U.S. vaccine adverse event reporting system associated with the COVID-19 vaccine rollout: an interrupted time series analysis with negative control evaluation.

OBJECTIVES: To assess whether auditory adverse event (AE) reporting changed after the COVID-19 vaccine rollout, and whether any change reflected a true safety signal or a shift in reporting behavior. METHODS: We analyzed U.S. Vaccine Adverse Event Reporting System (VAERS) data from 2018 through 2025 using interrupted time series (ITS) analysis. The COVID-19 vaccine Emergency Use Authorization (EUA), granted in the fourth quarter of 2020, was treated as a time-level ecological intervention. Because VAERS receives reports for all vaccines, we included every report regardless of vaccine type. We

Supratherapeutic Tacrolimus Concentrations With Nirmatrelvir/Ritonavir in Solid Organ Transplant Recipients Requiring Hospitalization: A Case Series Using Rifampin for Reversal.

Nirmatrelvir/ritonavir was recently granted emergency use authorization for mild to moderate coronavirus disease 2019. Drug-drug interactions between ritonavir and tacrolimus are underappreciated by nontransplant providers. We describe 2 solid organ transplant recipients prescribed nirmatrelvir/ritonavir for outpatient use who developed tacrolimus toxicity requiring hospitalization and were managed with rifampin for toxicity reversal.

Nirmatrelvir and ritonavir combination: an antiviral therapy for COVID-19.

INTRODUCTION: The emergence of the Omicron SARS-CoV-2 variant of concern in late November 2021 presaged yet another stage of the COVID-19 pandemic. Paxlovid, a co-packaged dosage form of two antiviral drugs (nirmatrelvir and ritonavir) developed by Pfizer, received its first FDA Emergency Use Authorization (EUA) and conditional marketing by European Medical Agency in patients at high risk of developing severe COVID-19. AREAS COVERED: We reviewed the timeline of the drug nirmatrelvir from its discovery to authorization by FDA. After 1 year of its authorization, numerous studies and reports on

Developing New Drugs for the COVID-19 Emergency: Anatomy of the U.S. Response.

CONTEXT: To meet the need for effective treatments during the COVID-19 Public Health Emergency, the U.S. government sought to accelerate the discovery and development of new antiviral treatments-a process that normally took 4-12 years. The government changed many features of the established system, selecting the investigational drugs, sponsoring or conducting the clinical testing, and purchasing and managing the distribution of the successful products. METHODS: We focused on novel therapeutic agents for COVID-19 that were funded, clinically tested, and/or received Emergency Use Authorization

Risk of depressive disorders associated with medical cannabis authorization: A propensity score matched cohort study.

There is an increase in the medical use of cannabis. However, the safety of medical cannabis, particularly for mental health conditions, has not yet been clearly established. Thus, this study assessed the risk of emergency department (ED) visits and hospitalization for depressive disorders among medical cannabis users. We conducted a retrospective longitudinal cohort study of patients who received medical authorization to use cannabis from 2014 to 2019 in Ontario, matched (1:3 ratio) to population-based controls using propensity scores. Conditional Cox regressions were used to assess the assoc

Managed care and the pediatric emergency department.

The growth of managed care has provided health benefits to millions of children while attempting to control the increase in health care costs. In adhering to these goals, MCOs are often at odds with emergency departments, and the emergency department physicians providing emergency care. The appropriateness or inappropriateness of emergency department visits can be disputed, but no criteria have been established. Even the definition of emergency is debated, although many states are adopting a prudent layperson standard. Emergency medicine physicians, primary care providers, and MCOs must cooper

Organoid-based evaluation of SA55 and Pemivibart against evolving SARS-CoV-2 variants.

Pemivibart, a class 1/4 monoclonal antibody (mAb), is currently the only FDA-authorized SARS-CoV-2 mAb under an Emergency Use Authorization (EUA) in clinical use. The emergence of subvariants, including KP.3.1.1 and XFG, raises concerns about antibody efficacy. SA55, a fully human class 1/4 mAb, is currently under clinical trial, including a nasal spray formulation. Using the well-validated organoid-based neutralization assays, we compared the potency and breadth of Pemivibart and SA55. Our results demonstrated a significant decrease in Pemivibart's activity against KP.3.1.1 and XFG, with an a

Target Virus or Target Ourselves for COVID-19 Drugs Discovery?-Lessons learned from anti-influenza virus therapies.

The COVID-19 pandemic, after it was reported in December 2019, is a highly contagious and now spreading to over 190 countries, causing a severe public health burden. Currently, there is no vaccine or specific drug to treat COVID-19, which is caused by a novel coronavirus, SARS-2-CoV. For this emergency, the FDA has approved Remdesivir and Hydroxychloroquine for treatment of COVID-19 as Emergency Use Authorization. However, even after this pandemic, COVID-19 may still have a chance to come back. Therefore, we need to come out with new strategies for drug discovery for combating COVID-19 in the

Federal legal preparedness tools for facilitating medical countermeasure use during public health emergencies.

Preparing for and responding to public health emergencies involving medical countermeasures (MCMs) raise often complex legal challenges and questions among response stakeholders at the local, state, and federal levels. This includes concerns about emergency legal authorities, liability, emergency use of regulated medical products, and regulations that might enhance or hinder public health response goals. In this article, lawyers from the U.S. Department of Health and Human Services' (HHS) Office of the General Counsel (OGC), Centers for Disease Control and Prevention (CDC), and Food and Drug A

Helpful Lessons and Cautionary Tales: How Should COVID-19 Drug Development and Access Inform Approaches to Non-Pandemic Diseases?

After witnessing extraordinary scientific and regulatory efforts to speed development of and access to new COVID-19 interventions, patients facing other serious diseases have begun to ask "where's our Operation Warp Speed?" and "why isn't Emergency Use Authorization an option for our health crises?" Although this pandemic bears a number of unique features, the response to COVID-19 offers translatable lessons, in both its successes and failures, for non-pandemic diseases. These include the importance of collaborating across sectors, supporting the highest-priority research efforts, adopting rig

A scoping review of authorisation pathway for COVID-19 vaccines among selected countries.

BACKGROUND: In response to COVID-19 pandemic, NRAs implemented expedited mechanisms such as EUAs and CMAs, relying on international bodies, to authorize vaccines. Documenting these authorization pathways across countries informs NRAs and strengthens future pandemic responses. Existing studies on COVID-19 vaccine approvals are limited, underscoring the need for a comprehensive review. This study aimed to explore approval pathways, investigate supporting strategies, and review times for COVID-19 vaccines. METHODS: A scoping review was conducted, covering countries with stringent regulatory auth

SARS-CoV-2 Vaccination-Induced Transverse Myelitis.

While mass immunization against coronavirus disease 2019 (COVID-19) rolls out around the globe, safety concerns and adverse events that need prompt evaluation are also emerging. We report a case of transverse myelitis and Bell's palsy after receiving Johnson and Johnson COVID-19 vaccination under the emergency use authorization in a healthy young woman with no past medical history. Other possible etiologies of her symptoms were ruled out, and she was treated successfully with steroids and plasma exchange.

Comparative performance and cycle threshold values of 10 nucleic acid amplification tests for SARS-CoV-2 on clinical samples.

BACKGROUND: A number of nucleic acid amplification tests (NAATs) for SARS-CoV-2 with different reagents have been approved for clinical use in Japan. These include research kits approved under emergency use authorization through simplified process to stabilize the supply of the reagents. Although these research kits have been increasingly used in clinical practice, limited data is available for the diagnostic performance in clinical settings. METHODS: We compared sensitivity, specificity, and cycle threshold (Ct) values obtained by NAATs using 10 kits approved in Japan including eight kits th

Considerations and Challenges in the Remdesivir COVID-19 Pediatric Development Program.

The US Food and Drug Administration is committed to the development of effective antiviral regimens for pediatric patients with coronavirus disease 2019 (COVID-19), including infants and neonates. On April 25, 2022, the approved indication of remdesivir (RDV) was expanded to include pediatric patients 28 days and older and weighing at least 3 kg with positive results of direct severe acute respiratory syndrome coronavirus 2 viral testing, who are: Hospitalized, or Not hospitalized and have mild to moderate COVID-19 and are at high risk for progression to severe COVID-19, including hospitalizat

Outcomes of Bebtelovimab Therapy in Patients With Solid Organ Transplantation With Mild and Moderate COVID-19.

Solid organ transplant recipients (SOTRs) are at greater risk of poorer outcomes from coronavirus disease 2019 (COVID-19) as compared to the general population. Because of significant drug-drug interactions between nirmatrelvir-ritonavir and immunosuppressive agents as well as logistical challenges of outpatient administration of remdesivir, anti-severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) monoclonal antibodies (mAbs) had been the mainstay of outpatient treatment of COVID-19 among SOTRs, with bamlanivimab, casirivimab-imdevimab, and sotrovimab having been previously granted em
