Three patients with unilateral hemispheric symptoms suggestive of extracranial carotid occlusive disease and one patient with bilateral symptoms who were allergic to radiographic contrast media were studied by pulsed Doppler ultrasonic arteriography and real time spectrum analysis. Four high grade (greater than 50% diameter reduction) internal carotid stenoses and one low grade (less than 50% diameter reduction) stenosis were identified. The noninvasive findings were confirmed at endarterectomy in all five arteries. Pulsed Doppler ultrasonic imaging combined with real time spectrum analysis ca

BACKGROUND AND PURPOSE: Increasing life expectancy has resulted in an increasing number of elderly. As the elderly population grows, the incidence of stroke will increase. Many such strokes result from carotid stenosis (CS). In view of the benefits of carotid endarterectomy (CEA) shown in recent clinical trials, it would seem prudent that surgery for CS be considered for prevention of stroke in this population. Traditionally, members of the geriatric population have often been viewed, perhaps arbitrarily, as inappropriate candidates for CEA because of perceived greater operative risks. The pur

Carotid endarterectomy is a commonly performed but controversial procedure. We developed from the literature a list of 864 possible reasons for performing carotid endarterectomy, and asked a panel of nationally known experts to rate the appropriateness of each indication using a modified Delphi technique. On the basis of the panel's ratings, we determined the appropriateness of carotid endarterectomy in a random sample of 1302 Medicare patients in three geographic areas who had had the procedure in 1981. Thirty-five percent of the patients in our sample had carotid endarterectomy for appropria

Spontaneous hemopneumothorax (SHP) is a rare condition characterized by the accumulation of air and blood in the pleural cavity without prior trauma. We present a case of a patient who underwent carotid endarterectomy for carotid artery stenosis. Two days postoperatively, she developed sudden respiratory distress, hypotension, tachycardia, and a decrease in hemoglobin levels. A chest X-ray revealed a massive left-sided hemopneumothorax. A chest tube was inserted, but no active bleeding source was identified on subsequent imaging. Despite initial stabilization, the patient required intubation a

"High-risk" carotid endarterectomy (CEA): fact or fiction? To answer this question we reviewed the available evidence starting from controlled randomized trials, through retrospective population- and large institution-based studies to case-series. CEA can be performed in most "high-risk" patients with low mortality and morbidity. A broad concept of high-risk CEA, based merely on exclusion from previous controlled randomized CEA trials, cannot be justified. The vast majority of evidence suggests that age (> or =80 years) per se should not be considered a high-risk criterion for CEA. However, it

OBJECTIVE: Carotid endarterectomy can be performed under general or locoregional anesthesia. If locoregional anesthesia is chosen, the state of awareness of the patient allows for direct viewing of the effect of vascular clamping of the corresponding neurological territory. We present the results of an anesthetic procedure using only an analgesic in patients who were intubated and ventilated but with a level of consciousness that allowed us to view the effect of carotid clamping on motor functions.
METHOD: Forty-eight patients, ASA II-III, underwent surgical carotid endarterectomy. The anesth

Carotid endarterectomy (CE) is among the most common vascular procedures. Recent studies have examined indications for CE and the usefulness of multiple vascular procedures, and have compared general and locoregional anesthesia. Randomized prospective trials have confirmed that the efficacy of CE exceeds 70% in patients experiencing a transient ischemic attack (TIA) with an ipsilateral stenotic carotid lesion. When both carotid surgery and coronary revascularization are indicated, CE can be performed two weeks before or concurrent with coronary artery bypass. The greatest risk in CE is of neur

Duplex scanning has emerged as an accurate noninvasive method of evaluating patients with extracranial cerebrovascular disease; it has research, clinical and economic implications. The risks associated with cerebral angiography are numerous and well-known. During 1990-1995, 227 carotid endarterectomies (CEA) were performed without preoperative arteriography in 192 patients. 91% were performed under local anesthesia. Indications for surgery without angiography were: renal insufficiency (in 5), allergy to contrast material (11), and need for urgent surgery (25). In 81% CEA was performed due to t

Cerebral infarction, headache, and hypertension are well-known complications of carotid endarectomy (CEA). Seizures are a less frequent, but important complication. We describe eight patients with focal and generalized seizures following CEA. Seizures occurred 6 to 13 days after CEA. All began as focal motor seizures contralateral to the side of the CEA, and six patients developed generalized tonoclonic seizures. Lorazepam and phenytoin sodium controlled the seizures. Five patients without evidence of stroke on computed tomographic scan were normal in follow-up and had no further seizures. The

Carotid eversion endarterectomy appears to be a safe and anatomically acceptable alternative to the standard carotid bifurcation endarterectomy, as evidenced by the absence of permanent neurologic morbidity and mortality in 98 operations. Preliminary clinical, angiographic, and noninvasive laboratory parameters suggest that there is a significant reduction of both residual technical defects and early recurrent stenosis. Specifically, no perioperative thrombosis or early restenosis was encountered in the 98 endarterectomies. Furthermore, only two technical defects have been noted, and one of th

It is estimated that between 1971 and 1987 the number of carotid endarterectomies has increased from 15,000 to over 85,000 per year. Unless the procedure can be performed safely with a combined morbidity and mortality which is below the yearly risk of stroke (5%) for patients with symptomatic carotid artery disease, one should reconsider this operation as a therapeutic option. We review our experience with 891 carotid endarterectomies performed between January 1979 and June 1987. There were 579 (65%) men and 312 (35%) women of ages from 34 to 82 (median 65); risk factors included diabetes mell

Carotid endarterectomy is the preferred treatment for patients with transient ischemic attacks and carotid stenosis. Although clinically these patients do well, the long-term fate of the operated carotid artery has not been well documented. To address this question, repeated noninvasive testing has been employed to follow our carotid endarterectomy patients since 1976. Supraorbital Doppler examination, oculoplethysmography-Kartchner, carotid phonoangiography and, later in this series, spectral analysis of the carotid Doppler velocities were performed after 193 endarterectomies. One hundred fif

This series of 280 consecutive carotid endarterectomies was performed on 222 patients without an indwelling shunt or electroencephalograpic monitoring. Symptomatic patients with lateralized and nonlateralized transient ischemic attacks, ulcerated atherosclerotic plaques. One-third of the patients had severe bilateral stenosis or unilateral stenosis and contralateral occlusion. Adequate cerebral protection was provided during operation by general anesthesia and moderate hypertension. The carotid occlusion time averaged 10 minutes. There was no operative mortality within 30 days. There were no s

During a 10-year period from January 1983 to December 1992, 79 carotid endarterectomies were performed in patients aged 80 years or older. This represented 7.4% of the total patient population undergoing carotid endarterectomy at Emory University Hospital. The indications for surgery in this elderly population were transient ischemic attacks in 24 (30.3%), cerebrovascular accident in 12 (15.2%), amaurosis fugax in seven (8.9%), vascular tinnitus in one (1.3%), and asymptomatic stenosis in 35 (44.3%). The average degree of ipsilateral stenosis was 76.8%. Concomitant risk factors included corona

The records of 146 patients 80 years of age or older who underwent 183 carotid endarterectomy operations from 1964 through 1990 were reviewed to determine surgical risk. The indications for operation were asymptomatic patients with carotid stenosis (n = 36); ipsilateral transient ischemic attacks (n = 46); ipsilateral stroke (n = 28); ipsilateral retinal embolus (n = 15); nonlateralizing symptoms (n = 40); and asymptomatic side in patients with contralateral symptoms (n = 18). Postoperatively, three patients (1.6% of operations) had a stroke with a residual deficit and three (1.6%) died. All d

The results of 72 carotid endarterectomy operations carried out by three consultant surgeons were analysed. Thirty-eight operations (32.8 per cent) were performed in patients with mild or resolving stroke and 33 operations (45.9 per cent) in patients with transient ischaemic attacks and/or amaurosis fugax. Five patients (7 per cent) died in the early postoperative period, 2 as a result of strokes and the others from cardiac causes. Seventy-six per cent of patients had a satisfactory long term result, remaining free from storke, blindness or recurrent transient ischaemic attacks over the follow

Objective: To compare the clinical efficacy of the modified eversion carotid endarterectomy (M-ECEA) by comparing it with the conventional eversion carotid endarterectomy (C-ECEA). Methods: This interim analysis of a prospective randomized controlled study enrolled eligible patients with carotid artery stenosis admitted to the Department of Vascular Surgery at the Second Affiliated Hospital of Nanchang University between September 2022 and September 2024. Patients were allocated in a 1∶1 ratio to either the M-ECEA group or the C-ECEA group using simple randomization. The key modification of th

In this retrospective study, we review the outcome of femoral eversion endarterectomy (EE) and standard endarterectomy with patch angioplasty (SEP) over a 10-year period. EE technique involves transection of common femoral artery (CFA), eversion endarterectomy of the distal CFA, and primary end-to-end reconstruction. One hundred forty-two patients underwent femoral endarterectomy, among which 38 (26.8%) endarterectomies were performed using the eversion technique. The cumulative 2-year primary patency of EE with or without a concurrent procedure was greater than 86% and did not statistically d
