Type II endoleak with an enlarging aortic sac after endovascular aneurysm repair predisposes to the development of a type IA endoleak.

OBJECTIVE: The most common endoleak after endovascular aneurysm repair is type II. Although type II endoleaks (TIIEL) are generally considered benign, there are reports that they can lead to aortic rupture. In this study, we reviewed the effect of TIIEL on sac size change to determine if sac expansion owing to a TIIEL could result in the development of a type IA endoleak (TIAEL). METHODS: After internal review board approval, all aortoiliac endovascular aneurysm repairs performed at a single institution between June 2006 and June 2012 were retrospectively reviewed. Patient demographics, comor

Safety and Efficacy of Coil Embolization for Endoleak Prevention as an Adjunct to Endovascular Repair of Abdominal Aortic Aneurysm or Subsequently for the Repair of Endoleak.

OBJECTIVE: This study assessed the real-world safety and efficacy of coil embolization during endovascular aneurysm repair (EVAR) of abdominal aortic aneurysms (AAA) for prophylactic endoleak prevention or as a reintervention for endoleak repair, using the Cerenovus family of coils (Cerenovus, Irvine, CA, USA). METHODS: This was a multicenter, retrospective cohort study of consecutive patients who underwent embolization of branching arteries during EVAR of an AAA or as a reintervention for endoleak repair, using Cerenovus coils between January 2017 and December 2021 in Japan. The primary outc

Aneurysm Shrinkage Is Compatible With Massive Endoleak in the Presence of an Aortocaval Fistula: Potential Therapeutic Implications for Endoleaks and Spinal Cord Ischemia.

PURPOSE: To present a patient with ruptured abdominal aortic aneurysm (AAA) and aortocaval fistula who was successfully treated with endovascular aneurysm repair in spite of developing a massive endoleak. CASE REPORT: A 70-year-old man with ruptured AAA and aortocaval fistula was treated with endovascular aneurysm repair (EVAR). During 8 years of follow-up, he had massive perfusion of the aneurysm sac by retrograde flow from the inferior mesenteric artery into the caval vein through the aortocaval fistula. The aneurysm diameter decreased continuously in spite of the type II endoleak. This obs

Type II endoleaks: diagnosis and treatment algorithm.

Elective abdominal aortic aneurysm (AAA) repair is recommended for aneurysms greater than 5.5 cm, symptomatic, or rapidly expanding more than 0.5 cm in 6 months. Seventy-five percent of AAAs today are treated with endovascular aneurysm repair (EVAR) rather than open repair. This is fostered by the lower periprocedural mortality, complications, and length of hospital stay associated with EVAR. However, some studies have demonstrated EVAR to result in higher reintervention rates than with open repair, largely due to endoleaks. Type II is the most common, making up 10-25% of all endoleaks. Type I

The diagnostic and treatment challenge of type IIIb endoleaks.

Type IIIb endoleak is a rare complication of endovascular aortic repair caused by endoprosthesis deterioration, leading to aneurysm pressurization and potential rupture. Because of its rarity, few cases are published. We report six cases of type IIIb endoleak in a 15-year period. Appropriate preoperative diagnosis was achieved in five cases; duplex ultrasonography, computed tomography angiography, and contrast-enhanced ultrasonography were useful diagnostic tools in one case, and angiography led to the correct diagnosis in four cases. In the remaining case, only surgical exploration identified

Type 1a Endoleak after Fenestrated Frozen Elephant Trunk Technique.

The patient was a 48-year-old man who had undergone fenestrated frozen elephant trunk (FET) technique for acute type A aortic dissection. Postoperative enhanced computed tomography (CT) imaging revealed a type 1a endoleak from the fenestration. Nevertheless, the stented portion of the FET exhibited circular expansion. However, 2 months after surgery, enhanced CT imaging revealed the flattening of the FET due to the persistent endoleak and we performed an urgent zone 2 thoracic endovascular aortic repair (TEVAR). When type 1a endoleak from the fenestration is observed, the FET can be flattened,

Influence of statin therapy on type 2 endoleak evolution.

BACKGROUND: Endovascular repair (EVAR) for abdominal aortic aneurysm (AAA) is widely adopted; however, the procedure may be jeopardized by type 2 endoleak (T2E). Most T2Es regress over time, but their evolution is unpredictable. There is some evidence about the pleiotropic statin effect on AAA and thrombus stabilization, but there are no data on the influence of statins on T2E. The study's aim is therefore to evaluate a possible effect of statins on T2E evolution. METHODS: A retrospective analysis of patients discharged from 2008 to 2013 with T2E after EVAR was performed. Patients were follow

Persistent type II endoleak: two cases of successful sacotomy.

Endovascular treatment of persistent type II endoleak may not resolve the complication. We report two cases of sacotomy performed to treat this problem: the first case was in an emergency setting for aneurismal sac rupture, and the second occurred in an elective surgery setting after several unsuccessful endovascular procedures. In both cases, sacotomy allowed us identify the bleeding sources without aortic cross-clamping and endograft explantation. By minimizing hemodynamic modifications and reducing operative time, this procedure can be carried out even in patients considered unfit for surge

A Novel Technique and Outcomes for Transcaval Endoleak Embolization.

BACKGROUND: Strategies for embolization of type 2 endoleaks include translumbar, transgraft, transarterial, and transcaval approaches. The transcaval approach is limited by an inconsistent ability to access the aortic sac and the risk of puncturing and damaging the endograft or adjacent structures. We describe a novel technique for caval to aortic aneurysm sac access and report early outcomes. METHODS: A retrospective review of all patients who underwent transcaval embolization (TCE) at a tertiary referral center. From March 2019 to June 2021, 12 patients were identified to have undergone a n

Billowing Of Endologix Powerlink Stent Mimicking Endoleaks.

INTRODUCTION: Endoleaks remains one of the primary concerns of endovascular aortic aneurysm repair (EVAR) and is routinely followed with CT angiography (CTA). However, certain imaging findings can mimic endoleaks. CASE PRESENTATION: A 65-year-old woman who had endovascular aortic repair (EVAR) of an abdominal aortic aneurysm with Endologix Powerlink system developed marked new circumferential cauliflower-like bulging of contrast-filled sacs at mid-stent-graft with enlargement of the excluded aneurysm at 3-year follow-up. CONCLUSIONS: Considering the unique construct of the Powerlink stents,

The significance of endoleaks in thoracic endovascular aneurysm repair.

BACKGROUND: Endoleak is one of the rare complications that occur after thoracic endovascular aneurysm repair (TEVAR). The aim of this study was to assess the incidence of endoleaks and the predictive factors for their occurrence, as well as their effect on secondary interventions after TEVAR. METHODS: Medical and radiological data of all TEVAR procedures performed between 2004 and 2008 were entered prospectively into our database and reviewed retrospectively. Primary endpoints included were the incidence and the type of endoleak, aneurysmal sac expansion, and secondary interventions. RESULTS

Periaortic inflammation after endoleak repair: a risk to watch.

PURPOSE: To evaluate the occurrence of periaortic inflammation, including abscess formation, as a complication following endoleak repair after endovascular aortic repair. MATERIALS AND METHODS: This retrospective single-center study analyzed 88 EVAR revisions for endoleak treatment performed in 59 patients between 2015 and 2025. Patients underwent one to three revision procedures. Endoleaks were classified by type, and treatment was performed via transarterial access or direct sac puncture with a posterior/translumbar access route. Embolization materials for type II endoleaks included n-butyl

Persistent Thrombocytopenia Indicates Endoleak After Complex Endovascular Aneurysm Repair.

OBJECTIVES: High-pressure endoleaks (type I and III) after fenestrated-branched endovascular aneurysm repair (F-BEVAR) pose ongoing rupture risk. Early identification of such endoleaks may facilitate appropriate reintervention. Our objective was to determine if persistent thrombocytopenia after F-BEVAR correlates with high-pressure endoleaks in the early postoperative period. METHODS: Patients in a physician-sponsored investigational device exemption study of physician-modified and Cook-manufactured fenestrated/branched endografts were evaluated. Endoleaks identified on computed tomography wi

Transgluteal approach to hypogastric artery aneurysm type II endoleak.

An enlarging internal iliac artery aneurysm secondary to a type II endoleak after endovascular aortic repair is an uncommon entity. It carries a significant rupture risk and mortality if not addressed. The present patient had had a 6.8-cm, rapidly growing, excluded hypogastric aneurysm. The results included both a failed transarterial approach and successful percutaneous transgluteal internal iliac artery aneurysm embolization using XperCT software (Philips Healthcare, Andover, MA) for guidance. The salient points included that treatment of the "nidus" alone will not be sufficient for complete

Clinical significance of type I endoleak on completion angiography.

PURPOSE: Type I endoleak is known to be associated with sac enlargement and occasional rupture, therefore, the treatment of type I endoleak is recommended at the time of diagnosis. The aim of this study was to identify the significance of early type I endoleak found on completion angiography. METHODS: Between January 2000 and December 2012, a total of 86 patients underwent endovascular abdominal aortic aneurysm repair (EVAR) and 10 patients (11.6%) were diagnosed with type Ia endoleak on completion angiography. Clinical and radiologic data were reviewed retrospectively. RESULTS: Of the 10 pa

External aortic wrap for repair of type 1 endoleak.

INTRODUCTION: Type 1 endoleak is a rare complication after endovascular abdominal aortic aneurysm repair (EVAR) with a reported frequency up to 2.88%. It is a major risk factor for aneurysmal enlargement and rupture. PRESENTATION OF CASE: We present a case of a 68 year old gentleman who was found to have a proximal type 1 endoleak with loss of graft wall apposition on routine surveillance imaging post-EVAR. An initial attempt at endovascular repair was unsuccessful. Given the patient's multiple medical co-morbidities, which precluded the possibility of conventional graft explantation and open

Occult type I or III endoleaks are a common cause of failure of type II endoleak treatment after endovascular aortic repair.

OBJECTIVE: Most type II endoleaks have a benign natural history, but 6% to 8% are associated with sac enlargement and respond poorly to treatment. Our aim was to evaluate whether these enlargements are associated with delayed or occult type I and III endoleaks. METHODS: Patients with interventions for endoleak after endovascular aortic repair from 2000 to 2016 were reviewed retrospectively. Patient demographics, comorbidities, endoleak type, secondary procedures, aortic sac growth (≥5 mm), and mortality were collected. Successful treatment was defined as endoleak resolution with no further ao
