A Simplified Epicardial Ablation Strategy for Ventricular Tachycardia Integrating CT-Derived Epicardial Adipose Tissue Into Three-Dimensional Electroanatomic Mapping.

INTRODUCTION: Epicardial adipose tissue (EAT) can attenuate electrograms and limit lesion formation during epicardial ventricular tachycardia (VT) ablation. CASE PRESENTATION: We developed a simplified workflow using predefined CT attenuation values to integrate EAT into the CARTO 3 system. We applied this approach in three patients with dilated cardiomyopathy undergoing epicardial VT ablation, where EAT visualization guided target selection, avoided ineffective ablation, and informed alternative endocardial strategies. DISCUSSION: EAT-integrated mapping improved the interpretation of epicar

Simultaneous endo-epicardial-high density mapping of ventricular tachycardia with the use of multi-electrode mapping catheters.

We herein describe a case of VT in an ischemic cardiomyopathy patient utilizing a novel technique of simultaneous endo-epicardial high-density mapping. Epicardial access, in addition to retrograde aortic endocardial access, was performed. Following the creation of the respective endocardial and epicardial electrical substrate maps, activation mapping of the VT was then performed simultaneously with two overlapping HD Grid catheters. Full diastolic pathway mapping was achieved, with EGMs recorded in all segments of the diastolic interval, on the epicardial surface. This case highlights the feas

Epicardial ablation of ventricular tachycardia using a new high-density mapping system.

We report the case of a 44-year-old woman who was referred for ablation of recurrent ventricular tachycardia (VT) in the setting of dilated cardiomyopathy secondary to myocarditis. The ECG displayed a right bundle branch block morphology and superior axis in the frontal plane, associated with a pseudo delta wave in the precordial leads that suggested an epicardial origin. Cardiac magnetic resonance performed prior to the procedure showed late gadolinium enhancement at the lateral wall of the left ventricle (LV) and excluded subendocardial fibrosis in either ventricle. This information was cruc

Cardiac resynchronization therapy with intraoperative epicardial mapping via minithoracotomy: 10 years' experience.

BACKGROUND: Cardiac resynchronization therapy (CRT) is considered an efficient method to improve the left ventricular (LV) dysfunction with left bundle branch block. However, coronary venous anatomy is not appropriate in about 10% of the cases; thus other alternatives, such as epicardial lead implantation via minithoracotomy are needed. METHODS: During the period 2007-2017, a total of 57 patients were operated at our institute via left anterior minithoracotomy after an unsuccessful transvenous CRT. The best position of the LV epicardial electrode was determined by intraoperative epicardial ma

Simultaneous Endo-Epicardial Mapping of the Human Right Atrium: Unraveling Atrial Excitation.

Background The significance of endo-epicardial asynchrony (EEA) and atrial conduction block (CB), which play an important role in the pathophysiology of atrial fibrillation (AF) during sinus rhythm is poorly understood. The aim of our study was therefore to examine 3-dimensional activation of the human right atrium (RA). Methods and Results Eighty patients (79% men, 39% history of AF) underwent simultaneous endo-epicardial sinus rhythm mapping of the inferior, middle and superior RA. Areas of CB were defined as conduction delays of ≥12 ms, EEA as activation time differences of opposite electro

Importance of Ventricular Tachycardia Induction and Mapping for Patients Referred for Epicardial Ablation.

BACKGROUND: Many nonischemic cardiomyopathy (NICMP) patients referred for catheter ablation of ventricular tachycardia (VT) undergo an initial epicardial approach under general anesthesia (GA). However, GA may suppress inducibility and decrease tolerance of induced VT, leaving substrate modification as the sole ablation method. OBJECTIVES: Determine the utility of a strategy of initial programmed electrical stimulation (PES) under light sedation in patients referred for epicardial ablation of VT. METHODS: Of 68 NICMP patients referred for VT ablation, 25 were referred specifically for epicar

Men and women's hearts don't beat the same: Epicardial mapping of Bachmann's bundle.

BACKGROUND: There is increasing evidence that presentation, progression, and management of atrial arrhythmias, such as atrial fibrillation (AF), differ between women and men. Bachmann's bundle (BB) is the main route for interatrial conduction, and sex-related differences in structural and electrical remodeling of BB may contribute to differences in AF development between women and men. OBJECTIVE: Investigate whether sex differences in the electrophysiological properties of BB assessed by high-resolution and density maps exist in patients with AF. METHODS: Sinus rhythm at BB was recorded for

Robotic Hybrid Ablation with Novel Epicardial Electrophysiological Mapping: Impact on Atrial Fibrillation Therapy.

We report the first use of the EnSite X system for intraoperative electrophysiological mapping during a robotic hybrid ablation (ROK-AF procedure) for long-standing persistent atrial fibrillation. Epicardial ablation targets were identified through epicardial mapping, and post-ablation electrical silencing was validated using the mapping system. Unlike conventional electrophysiological mapping systems, the orientation-independent omnipolar technology in EnSite X provides directional activation vectors, high-resolution electrograms, and peak frequency analysis, thereby enhancing substrate chara

Simultaneous Comparison of Electrocardiographic Imaging and Epicardial Contact Mapping in Structural Heart Disease.

BACKGROUND: The accuracy of ECG imaging (ECGI) in structural heart disease remains uncertain. This study aimed to provide a detailed comparison of ECGI and contact-mapping system (CARTO) electrograms. METHODS: Simultaneous epicardial mapping using CARTO (Biosense-Webster, CA) and ECGI (CardioInsight) in 8 patients was performed to compare electrogram morphology, activation time (AT), and repolarization time (RT). Agreement between AT and RT from CARTO and ECGI was assessed using Pearson correlation coefficient, ρ AT and ρ RT, root mean square error, E AT and E RT, and Bland-Altman plots. RES

Implementation of a novel interpolating method to epicardial potential mapping for atrial fibrillation study.

Epicardial potential mapping is an efficient way to visualize the potential distribution on the epicardial surface. We found in our previous study, that the traditional linear interpolation used for the epicardial mapping may cause errors and distortions in reconstruction of the electric activities on the epicardial surface especially during the atrial fibrillation. In this study, we devoted on the implementation of a 3D interpolating method, and verified it in comparison with another interpolating method as well as studying of the mechanism of vagal atrial fibrillation (AF). In case studying,

Electromechanical and structural phenotypes from cardiac imaging and epicardial mapping in inherited arrhythmia syndromes.

Inherited arrhythmia syndromes-including Brugada syndrome (BrS), long QT syndrome (LQTS), catecholaminergic polymorphic ventricular tachycardia (CPVT), and short QT syndrome (SQTS)-can cause malignant arrhythmias and sudden cardiac death without overt structural heart disease on conventional evaluation. Advances in cardiac magnetic resonance imaging (CMR), speckle-tracking strain echocardiography, and epicardial electroanatomical mapping have revealed previously unrecognized electromechanical and structural abnormalities in these conditions. In BrS, right ventricular outflow tract (RVOT) fibro

Epicardial Fragmentation Mapping in Brugada Syndrome: A Three-Patient Case Series with Literature Review.

BACKGROUND: Brugada syndrome (BrS) carries a high risk of ventricular fibrillation (VF) despite protective therapy with implantable cardioverter-defibrillators (ICDs). Catheter ablation targeting epicardial substrate in the right ventricular outflow tract (RVOT) has emerged as a promising adjunctive strategy; however, optimal procedural endpoints remain unclear. OBJECTIVE: To present a case series of three BrS patients who underwent high-density (HD) epicardial substrate ablation assisted by electrogram fragmentation mapping, and to review contemporary evidence on Brugada substrate ablation.

Gadolinium-Free Cardiac MR Stress T1-Mapping to Distinguish Epicardial From Microvascular Coronary Disease.

Источник пометил публикацию как отозванную

BACKGROUND: Novel cardiac magnetic resonance (CMR) stress T1 mapping can detect ischemia and myocardial blood volume changes without contrast agents and may be a more comprehensive ischemia biomarker than myocardial blood flow. OBJECTIVES: This study describes the performance of the first prospective validation of stress T1 mapping against invasive coronary measurements for detecting obstructive epicardial coronary artery disease (CAD), defined by fractional flow reserve (FFR <0.8), and coronary microvascular dysfunction, defined by FFR ≥0.8 and the index of microcirculatory resistance (IMR ≥

Simultaneous epicardial ablation based on intraoperative electroanatomic mapping during left ventricular assist device implantation.

Intraoperative ventricular tachycardia (VT) ablation targeting the epicardial substrate using three-dimensional electroanatomic mapping (EAM) was performed during left ventricular assist device (LVAD) implantation. We proved that EAM can be safely performed during LVAD implantation and that an ablation strategy based on electrophysiological information may reduce VT recurrence after LVAD implantation.
