Eptifibatide bolus dose postductal stenting intervention: A single-center experience.

OBJECTIVES: Stent thrombosis, a potential complication of ductal stenting, is associated with high mortality. This is a catastrophic complication, which can occur acutely (within 24 h), subacutely (within 30 days), or late (≥30 days) after stent implantation, with rates between 0.8% and 25%. Oral antiplatelet drugs like aspirin have erratic and inconsistent absorption and antiplatelet effects in critically ill neonates. Intravenous (IV) glycoprotein IIb/IIIa inhibitors (GPIs) are antiplatelet agents with rapid effect (84% inhibition of platelet aggregation 15 min after bolus) that may help pre

Phase I study of eptifibatide in patients with sickle cell anaemia.

The alphaIIbbeta3 antagonist eptifibatide is an effective treatment for patients with acute coronary syndromes (ACS). Platelet reactivity and CD40 ligand (CD40L) may play a role in the pathophysiology of sickle cell anaemia (SCA) similar to that in ACS, suggesting that inhibition of platelet aggregation and CD40L release by eptifibatide may benefit patients with SCA. Following eptifibatide infusion, safety and pharmacodynamic data were obtained from four SCA patients in their non-crisis, steady states. Eptifibatide was well tolerated, with no adverse changes in the haematological, biochemical

Eptifibatide: The evidence for its role in the management of acute coronary syndromes.

INTRODUCTION: Acute coronary syndromes and non-Q-wave myocardial infarction are often initiated by platelet activation. Eptifibatide is a cyclic heptapeptide and is the third inhibitor of glycoprotein (Gp) IIb/IIIa that has found broad acceptance after the specific antibody abciximab and the nonpeptide tirofiban entered the global market. Gp IIb/IIIa inhibitors act by inhibiting the final common pathway of platelet aggregation, and play an important role in the management of acute coronary syndromes. AIMS: This review assesses the evidence for therapeutic value of eptifibatide as a Gp IIb/III

Eptifibatide: a cyclic peptide that selectively inhibits platelet glycoprotein IIb/IIIa.

Platelets play a key role in the pathogenesis of coronary syndromes. Glycoprotein IIb/IIIa (GPIIb/ IIIa), also known as integrin alpha(IIb)beta(3), facilitates platelet aggregation through the binding of fibrinogen. In recent years, GPIIb/IIIa has become the target of new therapeutic strategies aimed at improving the efficacy of current antiplatelet therapy. Eptifibatide is a small cyclic heptapeptide that has shown high specificity and affinity for GPIIb/IIIa and has a short plasma half-life with a rapid onset of antiplatelet action and rapid reversibility of action when treatment is stopped.

Local intra-arterial eptifibatide for intraoperative vessel thrombosis during aneurysm coiling.

We report on our experience with the intra-arterial administration of eptifibatide for thrombolysis during aneurysm-embolization procedures. In 4 cases (3 stent-assisted coiling procedures and 1 with posthemorrhagic vasospasm), we noted the formation of thrombus occluding a vessel. We administered eptifibatide (10-15 mg) through a microcatheter proximal to the thrombus. The thrombus rapidly dissolved, resulting in the recanalization of the occluded vessels with no rethrombosis or hemorrhagic complications.

Eptifibatide: a potent inhibitor of the platelet receptor integrin glycoprotein IIb/IIIa.

The platelet glycoprotein (GP) IIb/IIIa integrin plays a key role in mediating platelet aggregation. Blockade of the platelet GP IIb/IIIa receptor prevents arterial thrombosis in animal models much better than does aspirin. Among the most specific inhibitors in this class of drugs is eptifibatide (Integrilin(TM), Millennium Pharmaceuticals, Inc.), a cyclic heptapeptide based on a peptide recognition sequence found in snake venom. Peptide inhibitors, such as eptifibatide, bind competitively to GP IIb/IIIa and have a short half-life, allowing the effect to be rapidly reversible and providing a f

Eptifibatide: a potent inhibitor of the platelet receptor integrin, glycoprotein IIb/IIIa.

Platelet aggregation is intimately involved in the pathophysiology of acute coronary syndromes. Blockade of the platelet glycoprotein IIb/IIIa receptor, the mediator of platelet aggregation induced by all physiologic agonists, prevents arterial thrombosis in animal models far more effectively than aspirin. Eptifibatide (Integrilintrade mark) is a rapidly reversible competitive inhibitor of glycoprotein IIb/IIIa studied in a broad range of ischaemic coronary conditions, including percutaneous coronary intervention, ST-segment and non-ST-segment acute myocardial infarction and unstable angina. Impact of Using Blended Polymers on the Structural and Functional Attributes of Eptifibatide-Loaded Microspheres.

The saturation of generics has shifted the focus on developing technology-based delivery systems, not only for small molecules but also for proteins and peptides. Proteins and peptides require novel delivery systems as conventional delivery systems are of not much use. Biodegradable polymer-based microspheres have received considerable interest for the controlled delivery of many therapeutic proteins and peptides. Despite being known for many years, developing biodegradable polymer-based microspheres has remained a challenge. Among many challenges, selecting an appropriate grade of polymer is

[Eptifibatide in patients with percutaneous coronary intervention in clinical practice. Results of a prospective registry].

BACKGROUND AND PURPOSE: The use of potent platelet inhibitors has been shown to reduce the rate of ischemic complications in patients with percutaneous coronary intervention (PCI) in randomized clinical trials. Eptifibatide is a small-molecule glycoprotein (GP) IIb/IIIa receptor inhibitor. Eptifibatide given with a double bolus of 180 microg/kg 10 min apart followed by an infusion of 2.0 microg/kg/h achieves a high grade of platelet inhibition, which was associated with a highly significant clinical benefit in the randomized ESPRIT Study. The aim of the present prospective registry was to eval

Acute Severe Thrombocytopenia Occurring After Administration of Eptifibatide Postpones Emergent Coronary Artery Surgery.

INTRODUCTION: Eptifibatide is a platelet glycoprotein IIb/IIIa (GP IIb/IIIa) receptor antagonist that inhibits fibrinogen binding to the activated GP IIb/IIIa site and prevents platelet-platelet interaction and clot formation. GP IIb/IIIa inhibitors improve outcome in patients undergoing percutaneous coronary intervention for acute coronary syndrome. Thrombocytopenia is a complication of GP IIb/IIIa inhibitors, but severe thrombocytopenia is unusual. Most reported cases of severe thrombocytopenia after eptifibatide occurred in patients with acute coronary syndrome. The authors describe a patie

Long-term results following switch from abciximab to eptifibatide during percutaneous coronary intervention.

BACKGROUND: The usage of platelet glycoprotein (GP) IIb/IIIa receptor inhibitors improves the outcome during high-risk percutaneous coronary interventions (PCI). The aim of this study was to evaluate the long-term effects after a planned switch from abciximab to eptifibatide during PCI. HYPOTHESIS: A switch from the general use of abciximab to eptifibatide as a GP IIb/IIIa in connection with PCI would not have any negative effects on long-term clinical outcomes. METHODS: To reduce costs, a general switch from abciximab to eptifibatide was instituted in 2004 in 2 university hospitals in Swede

Use of eptifibatide as a bridge antiplatelet agent for intrathecal drug delivery system placement.

Use of antiplatelet agents is becoming increasingly common, and their management may require new strategies if neuroaxial techniques are to be employed in patients who will not tolerate discontinuation of antiplatelet therapy. The patient was a 46-year-old man with a past medical history significant for coronary artery disease and who had undergone 14 stents. He developed stent thrombosis (ST) while on clopidogrel. Following the ST, he was subsequently placed on prasugrel. While on prasugrel, the patient presented for an intrathecal drug delivery system (IDDS) trial and placement due to severe
