Hyposplenic blood picture in systemic amyloidosis. Its absence is not a predictable sign for absence of splenic involvement.

We studied a total of 61 cases of systemic and senile amyloidosis to evaluate the significance of a "hyposplenic blood picture" (the presence of numerous Howell-Jolly bodies) in these patients and to correlate its presence with the pattern and severity of the splenic involvement. To ascertain whether this peripheral blood picture is prevalent with a certain type of amyloidosis, we classified all cases by immunostaining with a panel of antibodies against AL amyloid (kappa and lambda light chains), serum amyloid-associated protein, prealbumin, beta-2 microglobulin, and amyloid p component. Based

Spleen Organoid Units Generate Functional Human and Mouse Tissue-Engineered Spleen in a Murine Model.

Introduction: Splenectomy is common after trauma or hematologic disease, and alters immune protection against pathogens, which may lead to fulminant infection with high mortality. Yet the spleen has demonstrable regenerative capacity and cells might be recovered and reimplanted at the time of injury or excision to avoid these risks. Methods: Tissue-engineered spleen (TESp) was generated from ActinGFP mice (mTESp) or human donor spleen (hTESp) through implantation of spleen organoid units (spleen OU), in NOD/SCID mice with concurrent splenectomy, on a biodegradable scaffold. Explants were evalu

Functional hyposplenism.

In the past 15 years, it has become increasingly evident that diverse conditions may be linked with hyposplenism and that hyposplenic individuals are susceptible to overwhelming infection, particularly by the encapsulated organisms such as pneumococci, meningococci, and Haemophilus influenzae. Identification of Howell-Jolly bodies in the peripheral blood smear is the most important clue to the diagnosis. Associated diseases include gastrointestinal disorders, hemoglobinopathies, autoimmune disorders, amyloidosis, neoplasms, circulatory disturbances, and other diseases. Hyposplenic patients sho

Persistent reticulocytosis in a case of poodle macrocytosis.

A healthy 14-year-old, male neutered, Miniature Poodle was found to have a persistent erythrocyte macrocytosis and reticulocytosis with a normal and stable HCT. The hematologic features of macrocytosis, increased Howell-Jolly bodies, and metarubricytosis, in the absence of anemia or other cytopenias, combined with the cytologic evidence of bone marrow erythroid dysplasia, including megaloblastosis, binuclearity, increased mitotic activity, and nuclear fragmentation, are consistent with previous reports of congenital dyserythropoiesis termed poodle macrocytosis. We speculate that the additional

[Hyposplenism, an underrecognised immune diffeciency: Is sonography helpful?].

The immunodeficiency syndrome of functional hyposplenia/asplenia is frequently unrecognized. The gold standard procedure for the diagnosis of this disease is technetium-99m scintigraphy of heat-denatured erythrocytes and the detection of pitted erythrocytes or Howell-Jolly bodies. Initial work suggests that this clinical presentation is associated with a small spleen. The aim of this review is to describe the value of B-mode and contrast-enhanced ultrasound in the identification of immunodeficiency caused by functional hyposplenism.

Babesia gibsoni infection in three dogs in Victoria.

Small intraerythrocytic parasites were observed in the blood of three related male American Pit Bull Terriers. Two of the dogs, both less than 1-year-old, were anaemic at the time of initial examination and the third, an adult and sire of the two younger dogs, had a normal haemogram and low parasitaemia. The morphological appearance of the erythrocyte inclusions, analysis of a 450-bp region of the 18S rRNA gene and antibody titres provided evidence that this parasite was Babesia gibsoni, a species not previously reported in Australia.

Biomarkers of splenic function in infants with sickle cell anemia: baseline data from the BABY HUG Trial.

We evaluated spleen function in 193 children with sickle cell anemia 8 to 18 months of age by (99m)Tc sulfur-colloid liver-spleen scan and correlated results with clinical and laboratory parameters, including 2 splenic biomarkers: pitted cell counts (PIT) and quantitative Howell-Jolly bodies (HJB) enumerated by flow cytometry. Loss of splenic function began before 12 months of age in 86% of infants in association with lower total or fetal hemoglobin and higher white blood cell or reticulocyte counts, reinforcing the need for early diagnosis and diligent preventive care. PIT and HJB correlated

Rheological properties of erythrocytes in heterozygous and homozygous beta thalassaemia.

The rheological properties of erythrocytes were studied in 12 heterozygous and nine homozygous beta thalassaemia patients, using filtration of erythrocytes through polycarbonate sieves with a pore diameter of 5 micron and viscosity measurements of erythrocyte and ghost suspensions. Erythrocyte deformability of all patients was decreased. In the heterozygous patients moderately diminished flexibility of cells was due to microcytosis of erythrocytes. Rigidity of erythrocytes in homozygous patients was due to altered cell shape, diminished fluidity of haemoglobin, and in splenectomized patients t

Hyposplenism in inflammatory bowel disease.

Splenic function was assessed in 35 patients with ulcerative colitis and 20 patients with Crohn's disease. Hyposplenism was diagnosed if there were Howell-Jolly bodies in the peripheral blood film or if there was prolongation of clearance from the peripheral blood of injected 51-Cr-labelled heat-damaged red blood cells. Thirteen of the patients with ulcerative colitis had hyposplenism as compared with only one patient with Crohn's disease. Conversely, heat-damaged red cell clearance values faster than the normal range were found in six out of the 20 patients with Crohn's disease. Four patients

Assessment of splenic function.

Hyposplenic patients are at risk of overwhelming post-splenectomy infection (OPSI), which carries mortality of up to 70%. Therefore, preventive measures are warranted. However, patients with diminished splenic function are difficult to identify. In this review we discuss immunological, haematological and scintigraphic parameters that can be used to measure splenic function. IgM memory B cells are a potential parameter for assessing splenic function; however, more studies are necessary for its validation. Detection of Howell-Jolly bodies does not reflect splenic function accurately, whereas det

Post-translational modification of lysine residues in erythrocyte α-synuclein.

α-Synuclein is a protein linked to various synuclein-associated diseases ('synucleinopathies'), including Parkinson's disease, dementia with Lewy Bodies and multiple system atrophy, and is highly expressed in the central nervous system and in erythrocytes. Moreover, α-synuclein-containing erythrocyte-derived extracellular vesicles may be involved in the pathogenesis of synucleinopathies and their progression across the blood-brain barrier. Several post-translational modifications of α-synuclein have been reported in brain inclusions, including S129 phosphorylation, but fewer have been found in

Pitting function of the spleen in malaria: ultrastructural observations.

Ultrastructural studies of spleens from monkeys infected with Plasmodium knowlesi suggest that the spleen removes or "pits" malaria parasites from red cells. This function may explain the presence of nonparasitized spherocytic erythrocytes in the peripheral blood and may in part account for the discrepancy between the excessive hemolysis and the number of parasitized erythrocytes in animals with experimentally induced malaria.

Pocked erythrocyte counts in patients with hereditary spherocytosis before and after splenectomy.

The pocked (pitted or vacuolated) erythrocyte count has become increasingly utilized as a simple inexpensive test of splenic reticuloendothelial function. Values are less than 2.0% in normal subjects and 20 to 70% following splenectomy. Because scant and conflicting data are available about pocked erythrocyte measurements in hemolytic anemias other than the hemoglobinopathies, we performed pocked erythrocyte counts in 27 patients with hereditary spherocytosis. Prior to splenectomy patients often had elevated values (mean 4.9%). This unexpected observation suggests that hemolytic anemia may res

Babesiosis: problems in diagnosis using autoanalyzers.

A 76-year-old white man previous diagnosed as having Waldenstrom's macroglobulinemia continued with persistent fevers and sweats for two and a half years. Recently, repeated automated differentials during 11 days of hospitalization failed to note any intracellular inclusions in the RBCs. Blood sent to the Microbiology Laboratory was noted to contain Babesia species. A review of the hematology slides revealed that Babesia species was present on all the slides the analyzer had screened. This failure to note infected RBCs may pose serious diagnostic problems.

Juvenile dermatomyositis: a retrospective review of a 30-year experience.

BACKGROUND: Children with juvenile dermatomyositis (JDMS) have variable initial presentations. OBJECTIVE: Our purpose was to evaluate the epidemiology trends, presenting clinical features, laboratory data, and outcome of patients with JDMS. METHODS: A total of 16 patients were identified at Geisinger Medical Center by a 30-year retrospective chart review. RESULTS: Sex ratio, age at diagnosis, and outcome were similar to data published in previous studies. However, certain trends were noted. The most common initial physical examination findings were an extremity rash (94%) and periungual ery

Functional hyposplenism, a diagnostic clue in amyloidosis. Report of six cases.

Six cases of functional hyposplenism associated with amyloidosis are presented. This association has been infrequently reported. Typical erythrocytic changes were found at initial presentation in five patients, and provided an early clue to the correct diagnosis. Extensive amyloid effacement of the splenic cords was the anatomic basis of the functional hyposplenism. Comments are made on the etiology and infective consequences of adult-acquired functional hyposplenism. A more sensitive method of screening peripheral blood for changes of functional hyposplenism is discussed.
