Massive esophageal perforation managed with a covered esophageal stent.

Esophageal stents have become an emergent, effective treatment for esophageal perforation, whether spontaneous or iatrogenic. We report a case of iatrogenic perforation secondary to malposition of a Sengstaken-Blakemore balloon (SBB), which was inserted to control acute variceal bleeding. SBB currently has very limited indications in the management of variceal bleeding given the ongoing advances in endoscopic techniques, and its use is not exempt from complications such as the one reported herein.

Thoracic esophageal perforations.

The authors review their experience with thoracic esophageal perforation at Inova Fairfax Hospital, June 1, 1988, to March 1, 2009. With the exception of 6 patients with occult perforation, all of whom survived with nonoperative therapy, aggressive surgical intervention was the standard approach. Among patients treated aggressively with surgery within 24 hours of perforation, hospital survival was 97 per cent versus 89 per cent for patients treated aggressively surgically after 24 hours. In the absence of phlegmon, implacable obstruction, or delay, primary repair resulted in 100 per cent survi

Esophageal resection and immediate reconstruction in esophageal perforations.

BACKGROUND: Esophageal perforation is a disease with high mortality. Treatment is controversial and should be individualized. Elapsed time, location and perforation all play a role in determining the treatment option: from conservative treatment to esophagectomy. We undertook this study to report on primary esophagectomy and reconstruction in esophageal perforations with expert surgeons and selected patients. It is worth noting the rare complication of perforated peptic ulcer on Barrett's esophagus presented in one of our patients. CLINICAL CASES: We report two patients with esophageal perfor

[Spontaneous esophageal perforation related to fungal esophagitis].

The authors give account of spontaneous esophageal perforation developed on the basis of fungal oesophagitis. The lesion of the lower third of the esophagus classified as grade 4. on Kodsi classification displayed the same picture as an advanced esophageal cancer. They review signs, symptoms and varieties in macroscopic appearance of esophageal candidiasis. They warn that in case of a spontaneous perforation of "malignant" esophageal tumor with no preceding signs the rare condition of fungal esophagitis must certainly be considered.

Esophageal Perforation Successfully Treated With EVT.

Esophageal perforation is rare and carries high morbidity and mortality. A high degree of suspicion is needed for timely diagnosis and treatment. A 54-year-old man presented with fever and confusion. Imaging revealed air in the hepatic inferior vena cava and concern for a fistula between the distal esophagus and the inferior vena cava. An upper endoscopic evaluation revealed a dental floss pick penetrating the distal esophagus. The foreign body was removed, and endoluminal vacuum therapy was used to close the perforation. Endoluminal vacuum therapy is an emerging therapy to treat full-thicknes

Acute Pericarditis Hiding an Esophageal Perforation.

Esophageal perforations due to foreign body ingestion are uncommon; however, they can be associated with extremely high mortality rate. The most dreadful complication of this entity is the infection of adjacent structures, namely, the mediastinum and the pericardium. A prompt diagnosis and a precocious start of treatment are essential to the prognosis. Thus, a high degree of suspicion is required, especially in older patients. Therapeutic options are highly variable, depend on several factors, and should be individualized to every patient and their clinical status. Surgical treatment with esop

[Mechanical damage and spontaneous esophageal perforation].

The article presents the results of treatment of 95 patients with iatrogenic damages and perforations caused by other reasons and spontaneous rupture of the esophagus. A defect of the esophagus was formed in thoracic part in 67.4% cases. Combined suppurative complications such as a neck phlegmon, mediastinitis, pleural empyema, pericarditis, pneumonia and lung abscess occurred in prevalent majority of patients. Surgical interventions were performed through cervical, thoracotomical and laparoscopical accesses. Different plastic methods of covering sutures of the esophagus were used in condition

Pharyngeal perforation masquerading as esophageal atresia.

We present a case of pharyngeal perforation from a nasogastric tube mistakenly diagnosed as esophageal atresia with tracheoesophageal fistula in a full term infant female. The correct diagnosis was identified with rigid bronchoscopy and esophagoscopy immediately prior to the planned thoracotomy which was aborted. After one week of oral restriction and antibiotics, the healed perforation did not demonstrate leakage on a contrasted pharyngoesophagogram and bottle feeds were initiated. The infant was discharged to home on day of life 13 without any further complications.

Esophageal perforation in a sword swallower.

We present the case of a 59-year-old man who sustained an esophageal perforation as a result of sword swallowing. An esophagogram established the diagnosis, and surgical repair was attempted. However, 19 days later, a persistent leak and deterioration of the patient's condition necessitated a transhiatal esophagectomy with a left cervical esophagogastrostomy. The patient recovered and has resumed his daily activities at the circus, with the exception of sword swallowing. This case report presents an unusual mechanism for a potentially lethal injury. ["Pericardiography" in endoscopy-induced esophageal perforation].

Esophageal perforations are rare but most often caused by instrumental lesions during endoscopy. We describe a cases of the iatrogen esophageal perforation accompanied with perforation of the pericardium. Chest radiograph with Gastrografin swallow revealed the perforation. The principles of operative treatment and the postoperative course are demonstrated.

Hypopharyngeal or esophageal perforation in neonates.

In spite of the many manipulations that are done in the upper airways of neonates, esophageal perforations are rare. But they may be life-threatening if not recognized and treated immediately. Along with a case history we present clinical and radiologic symptoms of a right sided pneumothorax and the feeding tube in the posterior phrenic recess. The principles of treatment are conservative with antibiotics and intravenous gavage feeding. Only with complications intensive radiography with contrast agents and surgery become necessary.

Pericarditis with effusion complicating esophageal perforation.

Among 24 patients with esophageal perforation treated during the past 6 years, 3 patients developed pericarditis with effusion as a life-threatening complication. In the first patient pericarditis was found at autopsy, the diagnosis having been suspected but not proved during life. In the other 2 patients pericardial decompression was performed and both survived. Common denominators in the 3 patients were delayed diagnosis and treatment of the perforation, with resultant empyema. Further, the diagnosis of pericarditis with effusion was difficult and delayed, because mediastinitis and associate

Esophageal perforation by a Sengstaken balloon.

Iatrogenic esophageal perforations are rare but associated with a high morbidity and mortality. Secondary to placing a Sengstaken ballon are even more extraordinary because of the infrequency which this measure is used to control upper gastrointestinal bleeding. Therefore, we think it is of interest to communicate this complication, to remember the need to check the correct placement of the Sengstaken ballon before insufflation, since it would help further decrease this complication.

Iatrogenic esophageal perforation--diagnosis and therapy.

The diagnosis and therapy of iatrogenic esophageal perforation in a group of patients treated for this condition. During the period of 1994-2002 twelve patients suffered iatrogenic esophageal perforation with a mortality rate of 33.3%. The therapeutical approach consisted of either primary repair of the perforation, drainage of the mediastinum and pleural cavity or inserting a stent. (Ref. 6.).

[Esophageal perforation--indications for surgical therapy].

From January 1, 1970 to April 1, 1990, we treated 32 patients with esophageal perforations. 19 patients underwent surgical repair by bilateral closing suture (n = 10), mucosal suture, extramyotomy and semifundoplication in the presence of achalasia (n = 6). Three patients had subtotal esophagus resection with esophageal graft. Four of these patients died postoperatively, the site of intervention being unremarkable. We lost 7 out of 13 patients from the group subjected to conservative therapy. These results suggest a low risk in the surgical treatment of esophageal perforations when carried out
