[Cimetidine versus ranitidine in the treatment of peptic esophagitis].

A double blind test with cimetidine and ranitidine conducted on 60 patients with acid reflux oesophagitis revealed cimetidine to be the more effective. Its success is felt to lie more in the drug's ability to activate defensive factors than in its inhibition of acid secretion.

[Surgical treatment of reflux esophagitis and peptic stricture of esophagus].

Reflux-esophagitis is wide-spread and frequently diagnosed disease, which is found in persons of young and adult age. The main kind of treatment is conservative one, however sometimes reflux-esophagitis is accompanied by complications, dangerous for life, which require surgical treatment. Experience of surgical treatment of 720 patients is available. Fundoplication procedure in RCS modification combined with SPV is main and optimal operation. This operation is possible in more than 2/3 cases of surgical treatment of reflux-esophagitis and peptic stricture of the esophagus. If it is found to be

[Treatment of complicated peptic esophagitis. Role of total duodenal diversion].

Total duodenal diversion was performed in 60 patients with reflux oesophagitis complicated by stricture, brachyoesophagus, endobrachyoesophagus or previous oesophago-gastric surgery. The standard operation (truncal vagotomy, antrectomy, 70 cm Roux-en Y anastomosis) was carried out in 41 patients; technical adjustments were necessary in 19 patients previously operated. One patient died of post-operative pulmonary embolism. Lasting cure of the oesophagitis was obtained within less than 3 months in 56/59 patients (93 per cent). Three-hour post-prandial pH measurements showed control of the reflux

[Peptic esophageal stenosis and its relation to reflux esophagitis and esophageal cancer].

Carcinoma of the lower oesophagus can be a complication of a peptic oesophageal stricture caused by reflux oesophagitis. In most cases there is also a hiatal hernia. The main symptoms are dysphagia and epigastric retrosternal pain. The most important examinations are x-ray contrast film of the oesophagus and oesophagogastroscopy combined with biopsy.

[Total duodenal diversion in the treatment of complex peptic esophagitis].

Two duodenal diversion was performed in 107 patients with complex peptic oesophagitis (peptic stenosis, Barrett's mucosa, past history of gastro oesophageal surgery). A standard operation included truncular vagotomy, antrectomy and gastro-jejunal anastomosis on a 70 cm Y loop in 68 patients. Technical adaptations were required in 39 patients. Two patient died (pulmonary embolism and duodenal fistula). The operation was successful with stable cure of the oesophagitis at 3 months in 89% of the patients. Post-prandial pHmetry over 3 hours confirmed control of the reflux in 92% of the cases. Anast

Management of acid-peptic esophageal strictures.

Eighty-nine patients with benign acid-peptic strictures of the esophagus were evaluated. Of these, 56 patients were treated by dilatation and medical therapy, whereas 33 underwent operative therapy because medical therapy failed. Three patients, all psychotic and having post-emetic strictures, required resection of the stricture and a colon interposition. Twenty-nine patients underwent a Belsey or Nissen fundoplication combined with esophageal dilatation. Fourteen of 29 had an associated vagotomy and 15 did not. Two patients of this 29 had a poor result, one of whom had a post-emetic stricture

Omeprazole treatment of children with peptic esophagitis refractory to ranitidine therapy.

OBJECTIVE: To evaluate the cause of chronic abdominal pain lasting more than 3 weeks in 153 patients aged 6 to 18 years (mean, 9.9 years) who had undergone endoscopy. DESIGN: Those patients with peptic esophagitis as the cause of their chronic pain were treated with high-dose ranitidine hydrochloride, followed by the proton-pump inhibitor, omeprazole, for those who did not respond to a histamine2-receptor antagonist. RESULTS: Eighty-four percent of patients had peptic esophagitis, 3% had Helicobacater pylori gastritis, and 3% had ulcer disease. Seventy percent of the patients with peptic eso

Healing and relapse of severe peptic esophagitis after treatment with omeprazole.

We have studied the response of erosive or ulcerative esophagitis to treatment with omeprazole and its subsequent relapse on cessation of therapy in 196 patients. In the first phase of the study omeprazole (20 or 40 mg daily) was compared with placebo in 64 patients. After 4 wk there was endoscopic healing in 81% (25 of 31) of omeprazole-treated patients and in only 6% (2 of 32) of placebo-treated patients. Endoscopic healing of esophagitis was accompanied by symptom relief and histologic healing of ulceration. In the second (dose finding) phase a further 132 patients were randomized to omepra

[Secretory gastric function and the development of reflux esophagitis in peptic ulcer].

129 patients with peptic ulcer in the acute stage of the disease were studied clinically, roentgenologically, endoscopically and morphologically. The patients were classified into two groups--with or without dyspeptic syndrome. The gastric acidity was examined by the fractionary titration method with pentagastrin as stimulant, Reflux-esophagitis was found in patients with hyperacidity as well as in patients with normal and hypoacidity. The destructive changes in the esophageal epithelium were more frequent in the patients with duodenal ulcer--12.9% than in the patients with gastric ulcer. The

Peptic esophageal stricture in an adolescent with Barrett's esophagus.

Barrett's esophagus (BE) is characterized by the replacement of distal esophageal stratified squamous epithelium by columnar epithelium. It is rare in children and the risk factors may include mental retardation, cerebral palsy, esophageal atresia, etc. Apart from corrosive ingestion, peptic stricture is the other leading cause of esophageal strictures in children. However, BE has not been well characterized in the pediatric population and in children presenting with esophageal strictures. A 16-year-old Indian boy presented with a history of gradually progressive dysphagia to solids (but not l

Laparoscopic fundoplication for dysphagia and peptic esophageal stricture.

Peptic esophageal stricture with dysphagia is a late manifestation of severe gastroesophageal reflux disease (GERD). Although laparoscopic fundoplication is an effective antireflux operation, its efficacy for persons with peptic esophageal stricture and dysphagia has not been well defined. The aim of this study was to evaluate outcomes after fundoplication in this subgroup of GERD patients. Forty GERD patients with moderate, severe, or incapacitating dysphagia and peptic esophageal stricture were compared to a control group of 121 GERD patients without significant dysphagia or stricture. The role of Helicobacter pylori in esophagitis and peptic ulcer disease in Iraq.

OBJECTIVE: The objectives of this study are to determine the prevalences of Helicobacter pylori (H.pylori) infection in symptomatic, but endoscopically normal patients as well as in patients with endoscopically and histologically proven esophagitis, gastritis, duodenitis, duodenal ulcer, and gastric ulcer. METHODS: The study extended over the period November 1999 through June 2000. Biopsy specimens were harvested from intact areas of gastric antral mucosa, duodenal bulb, gastric body and lower third of esophagus of each one of 200 patients undergoing eseophago-gastro-duodenoscopy in endoscopy

Predictors for frequent esophageal dilations of benign peptic strictures.

Recurrence of esophageal peptic stricture necessitating repeated dilation treatments remains a problem for many patients despite optimal acid suppressive therapy. The factors associated with frequent relapses are poorly understood. We studied retrospectively a population of 58 patients with benign peptic strictures and dysphagia treated by esophageal dilation and followed for 66.5 +/- 6.7 months. Data was collected for age, sex, heartburn, weight loss, esophagitis, Barrett's esophagus, number of dilation treatments during the first year of follow-up, frequency and number of subsequent dilation

Refractory peptic lesions. Therapeutic strategies for ulcers and reflux esophagitis that resist standard regimens.

In some patients, peptic lesions fail to heal after 2 to 3 months of standard histamine2 (H2) receptor antagonist or sucralfate (Carafate) therapy. Noncompliance with prescribed treatment, cigarette smoking, gastric acid hypersecretory states (including Zollinger-Ellison syndrome), Helicobacter pylori infection, the use of nonsteroidal anti-inflammatory drugs, abdominal radiation therapy, and malignant tumors are all causes of refractory disease. Treatment options include high-dose H2 receptor antagonist therapy or switching to a more potent drug or one with a different mechanism of action. Oc

[Cimetidine in the treatment of peptic esophagitis. Clinical findings and results with 171 patients].

After a brief physiopathological introduction, the results obtained with Cimetidine in 171 cases of peptic oesophagitis are described. Given the high percentage of successes even in serious oesophagitis and the minimal incidence of side effects, cimetidine is the therapy of choice in conservative treatment of peptic oesophagitis.
