[Is the rigid hypopharyngo-esophagoscopy for suspected foreign body impaction still up to date?].

BACKGROUND: The increasing use of flexible esophagoscopes leads to controversies regarding the current status of rigid hypopharyngo-esophagoscopy in case of suspected foreign body impaction. METHODS: In a retrospective investigation we analyzed 309 open tube hypopharyngo-esophagoscopies performed for suspected foreign body impaction. RESULTS: In 108 patients (35.0 %) a foreign body was detected endoscopically. 69 Patients (22.3 %) had other pathologic findings within the upper digestive tract. 72 % of the foreign bodies and 83 % of the pathologic findings were localized within the hypopharyn

Removal of Coin Cell Lithium Battery Lodged in the Pediatric Pharyngoesophageal Junction by Rigid Esophagoscopy; a Case Report.

A coin cell lithium battery is a common foreign body that can become lodged in the pediatric pharyngoesophageal junction. Because the voltage of such batteries is relatively high, their rapid removal is necessary to avoid mucosal necrosis. Despite being the initial choice for removal, flexible endoscopy cannot remove such foreign bodies from the esophagus. Various removal methods, including rigid esophagoscopy, should be considered for removing lithium coin cell batteries. The transcervical approach is feasible for removing esophageal foreign bodies, but it carries the risk of complications su

[Reconstruction of the digestive tract after total gastrectomy. Comments and results in 27 patients operated on by a personal procedure].

The author presents the results obtained from his procedure of reconstruction of the gastro-intestinal tract after total gastrectomy. Only one patient presented a serious oesophagitis. Blood and stool examinations and esophagoscopic and radiological investigations were very satisfactory. The author concludes by outlining the advantages of this procedure, used by him since 1975 and which was proposed in the Journal of Surgery in 1976.

Secondary tracheoesophageal puncture: blind technique with a rigid hysterometer.

OBJECTIVES/HYPOTHESIS: The use of a rigid esophagoscope during tracheoesophageal puncture for speaking valve insertion in laryngectomized patients may be hindered by stenosis of the esophagus, or arthritic deformation of the spine, limiting extension of the neck. To prevent complications we currently perform a blind technique in these patients with the use of a rigid hysterometer instead of the esophagoscope. METHODS: Ten patients underwent secondary tracheoesophageal puncture using a rigid hysterometer. This was inserted through the mouth and gently advanced along the cervical esophagus to t

Swallowing-induced supraventricular tachycardia.

A rare form of paroxysmal supraventricular tachycardia (SVT), associated with swallowing solid food, is described. Repeated triggering of the arrhythmia during esophagoscopy was produced by mechanical stimulation of the anterior esophageal wall, 30 cm from the incisors. Treatment with verapamil abolished this tachycardia. Esophagoscopic examination, the first of its kind in this setting, aided the investigation.

[Foreign bodies in the esophagus].

1496 patients with foreign bodies of esophagus, reported between 1945-1997 to ENT Department of K. Marcinkowski University of Medical Sciences, were analysed. The age ranged from 0.5 to 95 years, but the most numerous group was under 5 years of age. Men outnumber women. Very detailed anamnesis, oropharynx and hypopharynx examination, neck palpation, chest X-ray were crucial for diagnosis. More than 50% of patients reported within first 10 hours after foreign body wedged. The most often met foreign bodies were: bones, coins, denture, fish bones, fruit-stones, buttons. The most dangerous were su

Congenital esophageal stenosis caused by membranous webs occurred in three infants. There were no associated anomalies. First patient applied with respiratory symptoms while the second and third patients reported food impactions. Correct diagnosis was performed during intraoperative (first patient) and esophagoscopic examination (second and third patients). Resection and anastomosis were performed in the first patient and hydrostatic dilatations in the second and third patients. In three patients, symptoms were relieved following treatment. In this report, etiology, diagnostic aids and managem

Variceal bleeding and portal hypertension: new lights on old horizon.

New clinical, endoscopic, and imaging modalities for diagnosing varices and predicting bleeding are being investigated. Transnasal endoscopy and ultrathin battery-powered esophagoscopes are being used to improve patient comfort and compliance. Patients who respond to portal pressure-reducing drugs not only have a reduced risk of bleeding, but also a reduced risk of developing other complications, with improved survival. Nitrates have been shown to have no definite role in primary prophylaxis against variceal bleeding. The hemodynamic response to treatment has an independent prognostic value fo

Esophageal tear after removal of pin ended earrings from hypopharynx of a child: Case report and review of the literature.

This report describes an interesting case of pin-ended earring, a foreign body, in a child, which could not be manipulated by rigid esophagoscope. The surgeon was able to extract it, on the suggestion of the anesthesiologist, by Magill forceps. Examination of the site of impaction of FB showed a small tear attributed to penetration of pin end of the earring. The pediatric surgeon, on post-procedure consultation, advised to follow-up the patient in pediatric intensive care unit with antibiotic prophylaxis. Chest radiograph on the following days showed opacity in the upper right chest region, wh

[Conservative treatment of cicatricial stenosis of the esophagus].

One hundred and eight five patients with an esophageal burn were treated clinically, 53 of them showed already developed strictures. In 31 cases the narrowing could be passed by a bougie 12-14. In 28 of them the esophagus was dilated by means of a routine dilation; in 3 there was perforation of the esophagus necessitating jugular mediastinotomy in the first, transdiaphragmatic mediastinotomy in the second, and resection of the thoracic esophagus in the third, the results being favourable in all of them. In 5 cases dilation was accomplished via an esophagoscope, but in 9 gastrostomized patients

Gastric Mucosal Viability Assessment Using 99mTcO4 in a Case of Pure Esophageal Atresia After Isoperistaltic Gastric Tube Replacement.

Gastric surface mucosal cells are responsible for the uptake and secretion of 99mTcO4, a feature that has been used in imaging heterotopic gastric mucosa. We used the same principle to look for gastric mucosal viability in this case of pure esophageal atresia admitted for cervical stomal closure after a previous isoperistaltic gastric tube (IGT) replacement procedure. 99mTcO4 scintigraphy was done after encountering a failure to maneuver the esophagoscope through the IGT. The study was helpful in assessing the loss of gastric mucosal viability in an intervening segment of the IGT.

Zollinger-Ellison syndrome with pneumopericardium.

A 55-year-old white male was found to have the Zollinger-Ellison syndrome in 1971. Supposed total gastrectomy was performed at that time. When an esophageal ulcer was found, six years later, esophagoscopic biopsy revealed residual gastric mucosa. The patient was given cimetidine 300 mg qid because it was felt he could not tolerate further surgery. After eight months of cimetidine therapy, the patient was admitted to the hospital because of retrosternal pain. Pneumopericardium was discovered, and at autopsy a large penetrating gastrojejunal ulcer was demonstrated.

Midesophageal web in adults.

Stenotic webs in the midesophagus of adults are rare. Including the present patient, only 6 have been described in the literature in English. Clinical presentation is characterized by dysphagia and insidious weight loss in a patient usually over 40 years old. The cause of the lesion is unknown. Based on knowledge of the embryological development of the esophagus and the absence of microscopical inflammatory changes in true webs, a congenital rather than inflammatory basis seems likely. Treatment of midesophageal webs can be accomplished by dilation, divulsion through an esophagoscope, or trans

Fiberoptic endoscopic diagnosis and treatment of a congenital esophageal diaphragm.

A 9-month-old infant suffering from dysphagia to solids presented with esophageal bolus obstruction. Using an adult fiberoptic esophagoscope, a congenital web was found at the junction of the middle and distal thirds of the esophagus. Previous direct rigid esophagoscopy and roentgenologic examination had not visualized the web. The web was successfully ruptured and dilated by inserting a guide wire under fiberscopic control and then inserting metal olives, followed later by blind mercury bougienage. This report illustrates the use of adult fiberoptic instruments for diagnosis and treatment in

Cricopharyngeal myotomy: indications and technique.

Available diagnostic tests evaluating cricopharyngeal dysmotility are expensive, uncomfortable, and unreliable for predicting the results of cricopharyngeal myotomy. Cricopharyngeal myotomy should be performed as a diagnostic test when a patient has "block" dysphagia (in which the food bolus stops rather than the swallow's being painful) localized to the cricoid level, and when no cancer is seen on esophagram. An effective surgical technique relies on the muscular distention provided by the inflated balloon cuff of a large endotracheal tube, and requires cutting the muscle fibers of the upper

Acute dysphagia after carp fish meal: a case report.

An 80-year-old man was admitted to the emergency department with acute dysphagia symptoms of 8 hours' duration. The problem began after a carp fish meal. Prominent symptoms included inability to swallow, mild dyspnea, and cough. Laboratory evaluation and fiber-optic examination of the upper aerodigestive tract was normal except for accumulation of saliva in the hypopharynx. Examination with a flexible esophagoscope revealed a total blockage of the lumen of the lower esophagus due to impaction of carp fish roe (column-like formation). With careful maneuvers and water pressure we were able to re
