International journal of surgery case reports2018eng
INTRODUCTION: An esophagorespiratory fistula (ERF) can cause severe pneumonia or a lung abscess which progresses to life-threatening sepsis. A case of a patient with esophageal cancer and an esophagopulmonary fistula (EPF) who underwent separation surgery with drainage tube-less (DRESS) esophagostomy and was promptly started on definitive chemoradiotherapy (CRT) is reported.
PRESENTATION OF CASE: A 79-year-old man visited a clinic with a month-long history of dysphagia. Esophageal cancer at the middle thoracic esophagus was detected, and invasion of the left main bronchus and lower lobe of th

An aim of the present study is to clarify the changes of esophageal function after surgical treatments of the esophageal varices or the congenital esophageal stenosis in children. Esophageal manometric studies were performed in sixteen children undergoing the esophageal transection with paraesophageal devascularization or the esophago-esophagostomy with partial esophagectomy before, within 1 month and over 7 months after the operation. The pressure of lower esophageal sphincter (LES), the length of LES, the LES relaxation test and the gastroesophageal reflux (GER) inducing test were measured.

Pathologic aerophagia is sometimes seen in patients with neurologic disorders. It rarely causes massive bowel distention, ileus, and volvulus. Here, we report the use of esophagogastric separation and abdominal esophagostomy via jejunal interposition to prevent bowel distention caused by severe aerophagia in 2 patients with neurologic disorders in whom the usual nonoperative methods of management failed. In both cases, swallowed air was evacuated via the jejunostomy, eliminating bowel distention. This operation may be useful in patients with neurologic disorders associated with severe aerophag

The management of long-gap pure esophageal atresia (LGEA) remains controversial. An 8-month-old girl with LGEA underwent a laparoscopic and thoracoscopic gastric pull-up and esophago-esophagostomy in the right thorax combined with intrathoracic fundoplication. She was positioned supine, and three 5-mm ports were placed in the standard locations for laparoscopic Nissen fundoplication. The gastrostomy was taken down and an additional 5-mm port was inserted at the gastrostomy site. Laparoscopic gastric mobilization was performed; the short gastric and left gastric vessels were divided using harmo

A gelding was diagnosed with dysphagia and left guttural pouch mycosis. Treatments included topical antifungal drugs, systemic voriconazole, and balloon occlusion of the internal carotid artery. Ongoing dysphagia of neurological origin necessitated extra-oral feeding through an esophagostomy tube. Complementary case management included acupuncture. Clinical remission occurred over 10 weeks.

We investigated the effect of sitagliptin, a dipeptidyl peptidase 4 inhibitor, on plasma incretin concentrations after glucose administration through an esophagostomy tube or feeding in healthy cats. Six cats were used for the glucose administration experiment and 5 cats were used for the feeding experiment. Glucose administration through an esophagostomy tube increased plasma glucagon-like peptide 1 (GLP-1) concentrations by 6-fold, whereas plasma glucose-dependent insulinotropic polypeptide (GIP) concentrations did not change. Feeding increased both plasma GLP-1 concentrations by 1.5-fold an

OBJECTIVE: To describe placement of a radiopaque polyurethane esophagostomy tube in a tortoise using MILA's tunneler device.
ANIMALS: A 20-year-old 4.235-kg spayed female yellow-footed tortoise (Chelonoidis denticulatus) was presented for egg coelomitis and feeding tube placement.
METHODS: A polyurethane esophagostomy tube (18 French; MILA International Inc) was advanced orally into the stomach. The small end of a tunneler device (MILA International Inc) was placed into the oral cavity and advanced into the midcervical esophagus. The tube exiting the mouth was attached to the opposite end of

Pancreatic pseudocysts and peripancreatic fluid collections extending into the mediastinum are unusual. Endoscopically, intra-abdominal pseudocysts can be drained transmurally through the stomach or duodenum depending on the location. An endoscopic ultrasound-guided esophageal approach for mediastinal pseudocysts has not been reported commonly in the literature. We report a rare case of a large mediastinal pseudocyst that was drained through the distal esophagus with eventual complete resolution of symptoms and pseudocyst.

INTRODUCTION: Intrathoracic esophagogastric anastomotic leakage is considered the most severe complication. We successfully performed T-drain esophagostomy under thoracoscopy for intrathoracic esophagogastric anastomotic leakage.
PRESENTATION OF CASE: A 44-year-old man visited a local hospital because of vomiting during the night. Upon examination, the patient was diagnosed with c-T2N0M0 stage II adenocarcinoma in Barrett's esophagus. We performed laparoscopic proximal gastrectomy and lower esophagectomy and gastric conduit reconstruction using the posterior mediastinal route with intrathorac

BACKGROUND: The putative role of the gut in amplification of systemic inflammation in acute pancreatitis is gaining credence, and intraluminal nutrition has been shown to decrease inflammation in experimental models of pancreatitis. Prepyloric feeding often is used in people with acute pancreatitis, but has not been evaluated in dogs.
HYPOTHESIS: Early intervention with enteral nutrition (EN) delivered proximal to the pylorus will be well tolerated in dogs with acute pancreatitis and provide justification for further larger trials.
ANIMALS: Ten dogs with severe acute pancreatitis in an open-

The work analyses the results of one-stage resection and plastics of the esophagus with a tube formed from the greater curvature of the stomach and creation of an ++extra-cavitary anastomosis on the neck in combined and surgical treatment of carcinoma of the thoracic esophagus in 279 patients. Various complications occurred in 181 (64.8%) patients. The mortality was 19.3% (16.8% among patients who underwent radical operation and 25.6% among those treated by a palliative operation). The number of complications in the groups of surgical and combined treatment was approximately equal. The results

A 10-year-old neutered male Labrador Retriever dog was diagnosed with idiopathic megaesophagus. Despite receiving conventional treatments including elevated feeding, the dog showed repeated regurgitation and aspiration pneumonia, consequently developing weight loss and severe malnutrition. For the purpose of controlling regurgitation, an esophagostomy tube was placed for draining the esophageal fluid. Additionally, an esophagogastric tube was placed for nutritional support. After tube placement, the average frequency of regurgitation was reduced from 2.4 times a day to 0.1 times a day. The nut

METHODS: The distal esophagus was exteriorized on to the left upper abdominal wall (abdominal esophagostomy) in 15 babies who had esophageal atresia with or without tracheo-esophageal fistula. The indications for this procedure were long gap atresia with or without tracheoesophageal fistula in which primary anastomosis was not possible and a major anastomotic dehiscence requiring cervical esophagostomy and gastrostomy. In all these patients a decision to replace the esophagus had been made, and a cervical esophagostomy was constructed. The distal esophagus was mobilized either from the thorax

Many infants with a repaired esophageal atresia (EA) undergo fundoplication, aortopexy, or glossopexy because the mechanisms most responsible for airway obstruction and/or apparent life-threatening event (AO/ALTE) syndrome are considered to be gastroesophageal reflux (GER), tracheal compression (TC), or obstructive apnea, respectively. In the present study, we investigated whether these mechanisms are independent or interrelated. We developed a database of 120 consecutive patients with EA treated by the senior author between 1967-2002. We studied the clinical manifestations of patients with a

A 4.5-month-old Standardbred colt presented for neck swelling and fever. Endoscopy and contrast radiography identified a full thickness esophageal perforation. Surgical intervention with fasciotomies and placement of an esophageal feeding tube was elected. Intensive postoperative treatment was carried out with broad-spectrum antibiotics, gastroprotectants and probiotics, calculated feeding plan, esophagostomy and fasciotomy site lavages, care, and cleaning. Complications included recurrent bilateral pneumothorax and development of an incomplete fistula at the esophagostomy site following remov

Reconstruction of extended post-irradiation pharyngo-esophagostomy is still a formidable challenge in head and neck surgery. Early rehabilitation of speech, swallowing and chewing and prevention of severe hemorrhage of the carotid artery by soft tissue coverage is mandatory. If previous attempts with local lipocutaneous and myocutaneous flaps have failed, a two stage reconstruction by microsurgical transfer of a jejunal patch in combination with a myocutaneous latissimus dorsi flap will lead to sufficient lining and a permanent closure of the defect. The indication and the technical procedures

A review of the experience with esophageal atresia and tracheoesophageal fistula over a 25-year period appears to lead to the advisability of the following procedures in surgical management:* Emergency gastrostomy under local anesthesia in all patients.* Extrapleural interruption of tracheo-esophageal fistula and end-to-end esophago-esophagostomy in patients who have the common type of upper esophageal atresia with distal tracheo-esophageal fistula.* Upper esophageal stretching and eventual esophago-esophagostomy in patients with proximal and distal esophageal atresia with or without proximal

A mare was admitted for progressive swelling of the neck, lethargy and anorexia. Radiography revealed perilaryngeal, peritracheal, and periesophageal emphysema. Endoscopy revealed pharyngeal and tracheal roof collapse, and a small laceration on the trachea. Treatment with antimicrobials, fluids, and flunixin was initiated. To prevent exacerbation of the emphysema, temporary tracheostomy was performed. Clipping for the tracheostomy revealed a hoofprint mark on the ventral neck. Subsequent endoscopies revealed laceration of the esophagus and guttural pouch septum communicating with the visceral

We present a 50 years old man who was admitted for severe pain in the chest appeared after vomiting, dyspnoea, cardiovascular collapse. The diagnosis of spontaneous perforation of the esophagus (Boerhaave's syndrome) was confirmed by a radiopaque swallow. We performed a laparotomy, mediastinal drainage, cervical esophagostomy and jejunostomy. Postoperatively, the general state was severe--with high fever, important pleurezia, necessitating pleural drainage, mechanical ventilation for 8 days, parenteral and jejunostomy nutrition. It was possible to close the esophagostomy in the 43rd postoperat
