Environmentalizing Bioethics: Planetary Health in a Perfect Moral Storm.

Many of humanity's most serious problems are global, intergenerational, and ecological, yet current institutions are poorly placed to confront such problems. In part, this institutional challenge reflects difficulties with our basic concepts and theories. Bioethics is a central area where such questions arise. Although some have argued for an environmentalized bioethics since its inception, biomedicine has thus far failed to embrace the challenge, and some accuse most bioethicists of being "asleep at the wheel" (Schenck and Churchill 2021). This paper discusses the basic ethical challenge, off

Disagreement About Withholding Treatment on the Basis of Disability.

The parents of an infant born with arthrogryposis multiplex congenita (AMC) insist that she will not have a life worth living and ask the clinicians not to pursue life-preserving interventions and to discontinue feeding her. The neonatology team disagrees and worries that the parents are not making care choices in the patient's best interests. This challenging case is analyzed from 3 unique perspectives. First, a scholar living with AMC shares her experience navigating this diagnosis from childhood to adulthood alongside her family. A neonatologist-ethicist argues for neonatologists to inform

Professional Socialization and Competency-Based Role Development in Clinical Ethics.

This essay argues that the professional formation of clinical ethicists should be neither reduced to technical competency nor equated with possession of authoritative moral answers. In conditions of reasonable moral pluralism, the distinct professional identity of the clinical ethicist is better grounded in a disciplined practice: rigorous ethical analysis, transparent reason-giving, role clarity, respect for affected persons, attention to power and exclusion, and a willingness to revise judgments. I define professional moral humility as recognition of the fallibility and situated character of

Women and AIDS: the ethics of exaggerated harm.

This article examines the way in which some biomedical ethicists have constructed sexually transmitted AIDS as a significant threat to women's health. We demonstrate that the familiar claim that 'women are the fastest growing group' -- whether of HIV-infected or of AIDS patients -- is misleading because it obscures the distinction between proportional rate of growth and absolute increase. Feminist ethicists have suggested that misogyny of a male dominated health care system has led to underreporting of women AIDS cases in order to support these feminists' claim of AIDS being a real threat to w

Research, education, ethics consultation: evaluating a Bioethics Unit in an Oncological Research Hospital.

BACKGROUND: This study aims to quantitatively and qualitatively evaluate the activities of a Bioethics Unit (BU) 5 years since its implementation (2016-2020). The BU is a research unit providing empirical research on ethical issues related to clinical practice, clinical ethics consultation, and ethical education for health care professionals (HPS). METHODS: We performed an explanatory, sequential, mixed-method, observational study, using the subsequent qualitative data to explain the initial quantitative findings. Quantitative data were collected from an internal database and analyzed by desc

Ethical implications of extracorporeal interval support for organ retrieval (EISOR).

With a growing demand for whole organs for transplantation, non-heart-beating organ donation is being revisited as a method of organ procurement. Extracorporeal interval support for organ retrieval (EISOR) is a new method to improve organ viability in non-heart-beating organ donation, but it potentially introduces ethical consequences. Our methodology included literature searches (Medline, Ovid, and bibliographies), abstract reviews, meeting presentations, and interviews with leaders in the field of organ donation and EISOR. No published articles on EISOR were found. Of the people interviewed (

Consensus, clinical decision making, and unsettled cases.

The model of clinical ethics consultation (CEC) defended in the ASBH Core Competencies report has gained significant traction among scholars and healthcare providers. On this model, the aim of CEC is to facilitate deliberative reflection and thereby resolve conflicts and clarify value uncertainty by invoking and pursuing a process of consensus building. It is central to the model that the facilitated consensus falls within a range of allowable options, defined by societal values: prevailing legal requirements, widely endorsed organizational policies, and professional standards of practice and

Two infants, same prognosis, different parental preferences.

A central principle of justice is that similar cases should be decided in similar ways. In pediatrics, however, there are cases in which 2 infants have similar diagnoses and prognoses, but their parents request different treatments. In this Ethics Rounds, we present such a situation that occurred in a single NICU. Three physician-ethicists analyze the issues.

A psychosocial evaluation process for living liver donors: the University of Rochester model.

OBJECTIVE: The donation of livers by living donors entails complex processes, both surgically and psychosocially, potentially involving risks in both domains. Thorough psychosocial evaluation is necessary to minimize those risks, yet little has been written about the donor assessment process. This article describes one such process, utilized by a transplant program in upstate New York. METHOD: Donor candidates undergo multiple psychosocial interviews early in the overall transplant evaluation process. Evaluators subsequently meet as a group, along with an independent ethicist, to determine ps

Orthotopic cardiac transplantation for the neonate. The dilemma of the anencephalic donor.

From 1981 to 1985, a total of 46 cases of hypoplastic left heart syndrome were diagnosed in Kentucky hospitals. During the same period, there were 64 live births of anencephalic neonates in Kentucky, half of whom were of adequate size for organ donation. Philosophic and pragmatic discussions among physicians, ethicists, and legislators must take place if organ donation by anencephalic subjects is to be used to ease the growing demand.

Psychedelic Medicine Exceptionalism.

Research on psychedelic medicines is experiencing a revival. Some clinicians, scientists, and ethicists believe that psychedelics are so different from other treatments that they warrant special consideration in how they are researched, regulated, commercialized, and administered. Others argue that psychedelic medicines show clinical potential, but they should be treated like other medical interventions. In other words, identical standards should apply. This article analyzes whether psychedelic medicines warrant special consideration from a regulatory and ethical perspective.

The "Ladder of Inference" as a Conflict Management Tool: Working with the "Difficult" Patient or Family in Healthcare Ethics Consultations.

Conflict resolution is a core component of healthcare ethics consultation (HEC) and proficiency in this skill set is recognized by the national bioethics organization and its HEC certification process. Difficult interpersonal interactions between the clinical team and patients or their families are often inexorably connected to the normative disputes that are the catalyst for the consult. Ethics consultants are often required to navigate challenging dynamics that have become entrenched and work with patient-provider or family-provider relationships that have already broken down. The first step

Three Kinds of Humility in Bioethics Certification.

The author's skepticism about certifying bioethicists has a 20-year history. The hazards of certification include doubts about whether an online, multiple-choice exam measures what is important in bioethical deliberation. Other worries include the potential neglect of informal reasoning processes used by patients and families, the creation of a false sense of expertise, and how certification can disenfranchise lay members of ethics committees. This essay does not seek to reverse the growing trend toward certification but urges humility both in the process of certification and in interpreting t

The ASBH Approach to Certify Clinical Ethics Consultants Is Both Premature and Inadequate.

In November 2018 the American Society for Bioethics and Humanities (ASBH) administered the first Healthcare Ethics Consultant Certification examination to 138 candidates, 136 of whom (98.5 percent) passed and were "certified" as "healthcare ethics consultants." I believe this certification process is both premature and inadequate. Certification for ethics consultants is premature because, as Kornfeld and Prager state repeatedly in their article in this issue of The Journal of Clinical Ethics, "The Clinician as Clinical Ethics Consultant: An Empirical Method of Study," there is a lack of "empir

Bioethics and corruption: a personal struggle.

The author attempts to give a general picture of corruption, especially in the area of healthcare. Corruption ranges from fraud, through deceit, bribery and dehumanisation, to immeasurable moral decay. As a bioethicist who has challenged corruption in various ways, the author approaches this worldwide plague mainly on the basis of his personal experience. He does not offer a recipe for successfully combating corruption, but tries to provide some ways and means to fight immorality without self-defeat. Bioethics is not a discipline whose task is to investigate, expose, or punish corrupt people.

Ethical considerations of biomarker use in neurodegenerative diseases--a case study of Alzheimer's disease.

A major issue in current Alzheimer research presents in a growing dysbalance between a swiftly advancing biological marker and diagnostic research field (including advances in genetic research towards genetic profiling), key aspects of which have already found their way into newly proposed diagnostic criteria and international clinical dementia guidelines, and still rather limited and stagnating therapeutic and preventive options for physicians and patients worldwide. While Alzheimer's disease (AD) to date can be diagnosed with high accuracy years ahead of the late stage clinical syndromal dem

Decision-making at the borderline of viability: Who should decide and on what basis?

Parents and medical staff usually agree on the management of preterm labour at borderline viability, when there is a relatively high risk of long-term neurodevelopmental problems in survivors. If delivery is imminent and parents and staff cannot agree on the best management, however, who should decide what will happen when the baby is delivered? Should the baby be resuscitated? Should intensive care be initiated? Three ethicists, one of whom is also a neonatologist, discuss this complex issue.

Informed consent to septoplasty: an anecdote from the field.

This paper tells the story of events that led up to a septoplasty and the consequences that followed it. The patient is a medical ethicist. After scratching the inside of a nostril in 1976, he suffered with occasional bleeding and irritation for almost two decades. He tried topical treatment. As this failed, he sought help from an ENT specialist. The paper relates the conduct of the patient and others (friends in the medical field, the patient's spouse, nurses and anesthesiologists) vis-à-vis informed consent.

Big Mistake: Knowing and Doing Better in Patient Engagement.

Pushing back on policies favored by dying patients is a challenging endeavor, requiring tact, engagement, openness to bidirectional learning, and willingness to offer alternative solutions. It's easy to make missteps, especially in the age of social media. Holly Fernandez Lynch shares her experience learning with and from the amyotrophic lateral sclerosis (ALS) community, first as a caricature of an ivory tower bioethicist and more recently as a trusted advisor, at least for some. Patient-engaged bioethics doesn't mean taking the view that patients are always right, but even when disagreement

Ethics roundtable debate: child with severe brain damage and an underlying brain tumour.

A young person presents with a highly malignant brain tumour with hemiparesis and limited prognosis after resection. She then suffers an iatrogenic cardiac and respiratory arrest that results in profound anoxic encephalopathy. A difference in opinion between the treatment team and the parent is based on a question of futile therapy. Opinions from five intensivists from around the world explore the differences in ethical and legal issues. A Physician-ethicist comments on the various approaches.
