Motor blockade and EMG recordings in epidural anaesthesia. A comparison between mepivacaine 2%, bupivacaine 0.5% and etidocaine 1.5%.

In a double-blind study young volunteers randomly received 20 ml of mepivacaine 2%, bupivacaine 0.5% or etidocaine 1.5% epidurally, all solutions with adrenaline. The mean cephalad spread of pin-prick analgesia was equal (T10) in the groups, but the duration was longest for bupivacaine and etidocaine. The motor blockade of the rectus abdominis muscles was assessed quantitatively by rectified integrated electromyographic recordings (RIEMG) and as number of turns in EMG recordings [changes in the direction (rise/fall) of the EMG; TURNS] from three different segmental levels, T7, T9 and T11. Absorption of bupivacaine, etidocaine, lignocaine and ropivacaine into n-heptane, rat sciatic nerve, and human extradural and subcutaneous fat.

Absorption of four amide local anaesthetics, including a new experimental agent, ropivacaine, in n-heptane, rat sciatic nerve and human extradural and subcutaneous fat was studied in vitro. The relative n-heptane/buffer (37 degrees C) partitioning of bupivacaine:etidocaine:lignocaine:ropivacaine was 10:39:1:2.9. The absorption of bupivacaine and etidocaine into nerve tissue was identified at steady state, but in extradural and subcutaneous fat etidocaine was taken up significantly more than bupivacaine. The lowest uptake was with lignocaine, and at steady state its mean concentrations were sim

Splanchnic turn-over of some energy metabolites and acid-base balance during intravenous infusion of lidocaine, bupivacaine or etidocaine.

The splanchnic turn-over of energy metabolites in circulating blood has been studied in four groups of young, healthy volunteers, receiving intravenous infusions of physiological saline (n = 5), lidocaine 4 mg/min (n = 7), bupivacaine 2 mg/min (n = 7) and etidocaine 2 mg/min (n = 8). The results showed that the local anaesthetic agents increased the splanchnic blood flow and oxygen consumption, the uptake of glycerol and lactate, and the release of 3-hydroxybutyrate. These effects are adrenaline-like but cannot be explained by one single mechanism involving increased activity of the sympatheti

Evaluation of the toxicity of local anaesthetic agents in man.

Etidocaine given by intravenous infusion has been compared, using a double-blind technique, with bupivacaine and lignocaine in respect of toxic symptoms and signs. The degree of toxicity is affected considerably by the rate of drug infusion. At 10 mg/min subjects could tolerate twice the dose of etidocaine as bupivacaine. Tolerance to etidocaine was less at 20 mg/min but still compared favourably with bupivacaine at 10 mg/min. Considering the difference in potency of the two agents it was considered that the therapeutic ratios would not be substantially different. Lignocaine at 20 mg/min was b

Differential sensitivity of A and C nerve fibres to long-acting amide local anaesthetics.

The differential sensitivities of A beta, A delta and C fibres of rat vagus nerve to bupivacaine, etidocaine and AL-381 were studied in vitro. In A beta fibres, at 35-37 degrees C, 50 mumole litre-1 of the drugs had similar effects on the action potential amplitude, while at greater concentrations (100 and 200 mumole litre-1) the greatest mean depression of amplitude was seen with etidocaine (n.s.). AL-381 had the most marked effect on the A delta potentials, and it appeared that it was about twice as potent as the others in blocking these fibres. Etidocaine 100 mumole litre-1 was more depress

On the relative potency of amino-amide local anaesthetics in vivo.

With the aim of comparing the analgesic effectiveness of lidocaine, prilocaine, bupivacaine and etidocaine in vivo, a study of the relationships between dose and duration of infraorbital nerve block (IONB) of various intensities (IONB degrees 3-10) was performed in the rat. With increasing doses longer durations of action were obtained. Further analyses were performed using multiple regression analysis. The log (dose)-duration lines for bupivacaine and etidocaine were found to be linear, whereas those for lidocaine in degree 3 and degree 10 IONB and for prilocaine in degree 10 IONB were so onl

A clinical trial of long-acting local anesthetics for periodontal surgery.

The efficacy of long-acting local anesthetics for anesthesia during periodontal surgery and for analgesia during the immediate postoperative period was evaluated. The rationale for using long-acting local anesthetics such as etidocaine and bupivacaine is that they can provide surgical anesthesia and, because of their long duration, prevent discomfort that may occur for 4-6 hours postoperatively. Two clinical trials were performed. The first enrolled patients requiring bilateral periodontal surgery. Using a matched pair design and double-blind randomized study conditions, 2% lidocaine 1/100,000

Benzodiazepines protect mice from local anesthetic convulsions and deaths.

Median convulsant (CD50) and median lethal (LD50) doses of intraperitoneal lidocaine, bupivacaine, and etidocaine were determined in 149 mice. Another 496 mice were pretreated with intramuscular diazepam, lorazepam, or midazolam, 1 mg/kg. Fifteen minutes later, lidocaine, bupivacaine, or etidocaine was given intraperitoneally and the incidence of convulsions and deaths recorded. Benzodiazepines significantly reduced the incidence or convulsions. For instance, etidocaine, 54.9 mg/kg, produced 50% convulsions in untreated mice compared to 6.9%, 3.4%, and 3.4% after diazepam, lorazepam, or midazo

Plasma concentrations of local anaesthetics after interscalene brachial plexus block.

Plasma concentrations of local anaesthetic agents have been measured after 40 interscalene brachial plexus blocks in 39 patients, using lignocaine, prilocaine, bupivacaine and etidocaine. Lignocaine produced greater concentrations than prilocaine, and bupivacaine greater concentrations than etidocaine. The addition of adrenaline resulted in much lower concentrations in the case of all four agents.

Influence of intravenous local anaesthetics upon quadriceps muscle function.

This study was undertaken to assess the effects of intravenous administration of mepivacaine and etidocaine on muscle function. Seven male volunteers were given mepivacaine (5 mg/kg) and etidocaine (50 mg) intravenously, on separate occasions. A reference group of 11 male volunteers received 0.9% saline solution intravenously. Muscle function was tested by measurements of isometric muscle force of knee extension and by quantitative electromyographic (EMG) recordings from the quadriceps muscle during knee extension at different degrees of isometric muscle force. At the end of the mepivacaine an

Cardiovascular and subjective central nervous system effects of long-acting local anaesthetics in man.

Intravenous infusions of etiodocaine 50, 75 and 100 mg and bupivacaine 75 mg were carried out over ten minutes in healthy young adult males. Cardiovascular sequelae were generally trivial at all doses. A collection of subjective central nervous system symptoms were described which may be regarded as early warning of impending local anaesthetic toxicity. Plasma concentrations of etidocaine were proportional to dose and ranged from 2 micrograms/ml to 5 micrograms/ml at the termination of the infusion. Plasma concentrations of bupivacaine were similar to those from the same dose of etidocaine but

Quality of spinal extradural anaesthesia: the influence of spinal nerve root diameter.

Profiles on the progress of spinal extradural anaesthesia were made in 246 patients using various concentrations of lignocaine, bupivacaine and etidocaine. The advance of analgesia through the different dermatomes showed a consistent and orderly spread on the upper lumbar and thoracic segments. This spread was irregular through the lower lumbar and upper sacral segments. The first sacral segment was especially difficult to block with an overall failure rate of 17.53% with presently available concentrations of various local anaesthetics. This failure was abolished with the use of etidocaine 1.5

High-performance liquid chromatographic method for an automated determination of local anaesthetics in human plasma.

A method is described that allows the rapid and precise determination of the local anaesthetics bupivacaine and etidocaine from biological fluids. This method uses a fully automated system with solid-phase extraction in combination with a column-switching technique. Both sample extraction on a LiChrocart pre-column and elution onto the analytical LiChrospher column, were performed automatically and concomitantly using conventional HPLC equipment in conjunction with an OSP-2 on-line sample preparator from Merck combined with UV detection. Recoveries were found to be 96.7 and 96.4% for 2 microgr

Pharmacology of local anaesthetic agents.

The most important clinical properties of local anaesthetic agents are potency, onset, duration of action and relative blockade of sensory and motor fibres. These qualities are related primarily to the physicochemical properties of the various compounds. In general, lipid solubility determines the relative intrinsic potency of the various agents, while protein binding influences the duration of anaesthesia and pKa is correlated with the onset of action. In general, the local anaesthetics for infiltration, peripheral nerve blockade, and extradural anaesthesia can be classified into three groups

Physico-chemical modification of lidocaine uptake in rat lung tissue.

The uptake of lidocaine, methyllidocaine, bupivacaine, etidocaine was studied in rat lung slices at different pH-values. The accumulation of the quaternary analogue, methyllidocaine, was not changed in the pH interval 7.0--8.0. The uptake of the three other substances was about 3--4 times lower at pH 7.0 than at pH 8.0. The rank order of distribution at a fixed cation/base ratio was bupivacaine greater than etidocaine greater than lidocaine. Interactions between lidocaine and other substances were studied in lung slices and in isolated perfused lungs. Bupivacaine and nortriptyline counteracted
