Evidence-based practice in canine artificial insemination.

A number of methods are currently used to predict the optimal date of insemination of the breeding bitch, particularly with the use of frozen-thawed canine semen which has a far shorter lifespan than fresh semen. Aside from confirming cytological oestrus, vaginal cytology is of no assistance in predicting the most fertile day(s) in a bitch; however, a neglected avenue of research suggests that vaginal cytology may be of great importance in confirming the days of optimal fertility retrospectively. Similarly, vaginoscopy provides clues as to the stage of a bitch's cycle but is inadequate as a so

Sebaceous carcinoma: evidence-based clinical practice guidelines.

Sebaceous carcinoma usually occurs in adults older than 60 years, on the eyelid, head and neck, and trunk. In this Review, we present clinical care recommendations for sebaceous carcinoma, which were developed as a result of an expert panel evaluation of the findings of a systematic review. Key conclusions were drawn and recommendations made for diagnosis, first-line treatment, radiotherapy, and post-treatment care. For diagnosis, we concluded that deep biopsy is often required; furthermore, differential diagnoses that mimic the condition can be excluded with special histological stains. For t

Percutaneous Cholecystostomy: Evidence-Based Current Clinical Practice.

The role of percutaneous cholecystostomy (PC) in the management of acute cholecystitis and cholangitis is outlined in the revised 2013 Tokyo Guidelines. These two emergencies constitute the vast majority of PC performed today for therapeutic purposes, and research has repeatedly shown the utility of PC in these conditions. PC is typically employed in the management of critically ill patients who are not surgical candidates. Indications and contraindications to PC are reviewed. Additional innovative applications of PC have been developed since it was first described in 1980. These include bilia

Beyond evidence-based nursing: tools for practice.

AIM: This commentary shares my views of evidence-based nursing as a framework for practice, pointing out its limitations and identifying a wider base of appraisal tools required for making good clinical decisions. BACKGROUND: As the principles of evidence-based nursing take an increasingly greater hold on nursing education, policy and management, it is important to consider the range of other decision-making tools which are subordinated by this approach. EVALUATION: This article summarizes nursing's simultaneous reliance on and critique of evidence-based practice (EBP) in a context of inadeq

Implementing evidence-based practice for urinary catheterization.

Nursing development units (NDUs) are ideal centres for the critical examination of clinical nursing practice. In addition, staff are enabled to clarify appropriate and effective methods of care and interventions based on evidence to date (Department of Health (DoH), 1993, 1998; English National Board (ENB), 1996, 1997; Centre for Policy in Nursing Research, 1997). In July 1996, the staff within the cardiology NDU at Addenbrooke's NHS Trust looked at the potential for various projects that were central to practice. One concerned the management of urinary catheters. A pilot study was initiated,

The Centre for Evidence-Based Orthopaedics Tool for the Implementation of Evidence-Based Practice.

Introduction Updating practices can be challenging in evidence-based practice when integrating the evidence, clinical expertise, and patient values and preferences. Implementing evidence-based practice requires individual, collective, and organizational behavioral changes. Effective behavior change interventions are necessary to facilitate the implementation of new evidence. We propose a tool to facilitate the implementation of evidence in clinical decision-making by changing healthcare providers' behaviors. Methods The Centre for Evidence-Based Orthopaedics (CEBO) tool was developed to bridge

Indwelling catheter management: from habit-based to evidence-based practice.

Indwelling urinary catheters are used in the care of more than five million patients per year. Prevalence rates range from 4% in home care to 25% in acute care. Catheter-associated urinary tract infections account for more than 40% of all nosocomial infections and can be associated with significant complications. Clinical practices in catheter management vary widely and frequently are not evidence-based. Effective nursing measures include: identifying patients who no longer need indwelling catheters, discussing appropriate catheter alternatives, and providing patient and caregiver education. M

The practice-based small group learning approach: making evidence-based practice come alive for learners.

This paper reports the study of a pilot learning programme for the Continuing Professional Development of Practice Nurses (PNs) in Scotland. Two PN groups used the practice-based small group (PBSG) approach to learning, which is designed to encourage learning relevant to patient problems faced in day-to-day practice. Learners kept a log of their experiences of engaging in PBSG learning for three consecutive meetings. Data was also collected through interviews. The findings detail the motivation for joining the PBSGs, group dynamics and the development of cohesion, factors influencing participa

Evidence-based integration of yoga in psychiatric practice.

Yoga has been put to test in clinical medicine to build evidence. There has been a steep rise in yoga research through 2010, threefold in the next decade. Despite challenges, clinicians have explored yoga intervention in several disorders. The available data have been examined using meta-analysis when there are more studies. Psychiatric disorders treated with yoga have attracted more research. Some examples include depression, schizophrenia, anxiety, obsessive-compulsive disorder (OCD), somatoform pain, addiction, mild cognitive impairment, and elderly and childhood disorders. Practice-Based Evidence in Community Guide Systematic Reviews.

OBJECTIVES: To assess the relative contributions and quality of practice-based evidence (PBE) and research-based evidence (RBE) in The Guide to Community Preventive Services (The Community Guide). METHODS: We developed operational definitions for PBE and RBE in which the main distinguishing feature was whether allocation of participants to intervention and comparison conditions was under the control of researchers (RBE) or not (PBE). We conceptualized a continuum between RBE and PBE. We then categorized 3656 studies in 202 reviews completed since The Community Guide began in 1996. RESULTS: F

[Severe pre-eclampsia. An evidence-based practice proposal].

OBJECTIVES: There are several treatment proposals for the obstetric patient with pre-eclampsia, but there is limited evidence on the adequacy of standard treatment. International healthcare organisations recommend that hospitals or anaesthesia departments have written guidelines, protocols or recommendations for dealing with common or severe situations. We propose evidence-based recommendations for the treatment of pre-eclampsia. MATERIAL AND METHODS: A literature review was performed using several sources, bibliography databases, recommendations made by specialist societies, and reviews. Fou

Evidence-based current surgical practice: calculous gallbladder disease.

BACKGROUND: Gallbladder disease is common and, if managed incorrectly, can lead to high rates of morbidity, mortality, and extraneous costs. The most common complications of gallstones include biliary colic, acute cholecystitis, common bile duct stones, and gallstone pancreatitis. Ultrasound is the initial imaging modality of choice. Additional diagnostic and therapeutic studies including computed tomography, magnetic resonance imaging, magnetic resonance cholangiopancreatography, endoscopic ultrasound, and endoscopic retrograde cholangiopancreatography are not routinely required but may play

Using evidence-based practice to select diagnostic tests.

Diagnosis is a primary activity of hand surgeons and therapists. Diagnostic tests can be used to assist in "ruling in" or "ruling out" a condition to best direct prognosis and treatment. However, the assigning of the diagnostic label is not always obvious. Principles of evidence-based practice may help hand care professionals improve their practice in the diagnosis of their patients. Evidence-based practice can be defined in terms of five steps that serve the structure for decision making and ensure optimum use of clinicians' expertise in the diagnosis.

Postinfectious cough: ACCP evidence-based clinical practice guidelines.

BACKGROUND: Patients who complain of a persistent cough lasting >3 weeks after experiencing the acute symptoms of an upper respiratory tract infection may have a postinfectious cough. Such patients are considered to have a subacute cough because the condition lasts for no >8 weeks. The chest radiograph findings are normal, thus ruling out pneumonia, and the cough eventually resolves, usually on its own. The purpose of this review is to present the evidence for the diagnosis and treatment of postinfectious cough, including the most virulent form caused by Bordetella pertussis infection, and mak
