Microbiota Mediate Enhanced Exercise Capacity Induced by Exercise Training.

PURPOSE: We investigated the effects of gut microbes, and the mechanisms mediating the enhanced exercise performance induced by exercise training, i.e., skeletal muscle blood flow, and mitochondrial biogenesis and oxidative function in male mice. METHODS: All mice received a graded exercise test before (PRE) and after exercise training via forced treadmill running at 60% to 70% of maximal running capacity 5 d·wk -1 for 5 wk (POST). To examine the role of the gut microbes, the graded exercise was repeated after 7 d of access to antibiotic (ABX)-treated water, used to eliminate gut microbes. Pe

[Exercise-intolerance and exercise-induced rhabdomyolisis: etiology and diagnosis].

Exercise intolerance, sometimes complicated by rhabdomyolysis is a frequent complaint of patients consulting in neuromuscular centers. It may be the main clinical manifestation of many metabolic myopathies and muscular dystrophies. The first step of diagnosis relies on the performance of in vivo metabolism investigations: forearm or bicycle ergometer exercise tests, phosphorus nuclear magnetic resonance spectroscopy. A few enzymatic defects may be directly assessed on blood samplings, in particular carnitine palmitoyltransferase deficiency; but muscle biopsy is necessary in most cases in order

Metabolic response to light exercise after exercise-induced rhabdomyolysis.

Inherent compromises in substrate metabolism, or impaired perfusion of muscle may contribute to the occurrence of exercise-induced rhabdomyolysis. In this study, the lactate response of the elbow flexor muscles to light exercise was examined in eight subjects (five males, three females) who previously demonstrated rhabdomyolysis with extreme swelling (ES; n = 4) or no swelling (NS; n = 4) of the upper arm after eccentric exercise. Subjects performed identical light exercise bouts (45 s of rapid isotonic biceps curls consisting of both concentric and eccentric actions at 25% of maximum voluntar

Identifying exercise allergies: exercise-induced anaphylaxis and cholinergic urticaria.

Exercise-related allergies vary from the benign rash of cholinergic urticaria to life-threatening exercise-induced anaphylaxis. Rapid diagnosis is essential, but it can be difficult to tell the two conditions apart. The size of the wheals and the patient history provide the best clues. Giving epinephrine and taking steps to protect the patient's airway, breathing, and circulation are standard treatment for exercise-induced anaphylaxis. Effective management for less severe cases involves exercising with a partner, keeping self-injectable epinephrine on hand, and avoiding exercise before and aft

Exercise Above Critical Power Elicits Greater Post-Exercise Hypotension than Heavy Exercise Performed to Task Failure.

BACKGROUND: We have previously shown that critical power (CP) is a key threshold determining the magnitude of post-exercise hypotension (PEH); however, whether this finding depended upon 1) the attainment of task failure or 2) could be augmented by increasing exercise duration above CP remained unclear. PURPOSE: To determine 1) whether a similar magnitude of PEH would occur after exhaustive heavy versus supra-CP exercise, and 2) whether the magnitude of PEH depended upon exercise duration above CP. METHODS: Eighteen healthy participants (14 males and 4 females) performed 3 criterion trials i

Role of Qualified Exercise Professionals in Medical Clearance for Exercise: Alberta Cancer Exercise Hybrid Effectiveness-Implementation Study.

BackgroundCurrent guidelines endorse the integration of exercise into cancer care. The diagnosis of cancer and its treatment, however, may introduce factors that make exercise engagement difficult, especially for individuals with advanced stages of disease. In this paper, we describe the baseline screening and triage process implemented for the Alberta Cancer Exercise (ACE) hybrid effectiveness-implementation study and share findings that highlight the multifaceted complexity of the process and the direct role of the clinical exercise physiologist (CEP).MethodsACE was a hybrid effectiveness-im

Exercise, exerkines and exercise mimetic drugs: Molecular mechanisms and therapeutics.

Chronic diseases linked with sedentary lifestyles and poor dietary habits are increasingly common in modern society. Exercise is widely acknowledged to have a plethora of health benefits, including its role in primary prevention of various chronic conditions like type 2 diabetes mellitus, obesity, cardiovascular disease, and several musculoskeletal as well as degenerative disorders. Regular physical activity induces numerous physiological adaptations that contribute to these positive effects, primarily observed in skeletal muscle but also impacting other tissues. There is a growing interest am

Exercise Testing and Exercise Rehabilitation for Patients With Atrial Fibrillation.

INTRODUCTION AND PURPOSE: Atrial fibrillation (AF) is a common cardiac arrhythmia associated with an increasing prevalence with advancing age. It is associated with dyspnea, exercise intolerance, and increased risk for clinical events, especially stroke and heart failure. This article provides a concise review of exercise testing and rehabilitation in patients with persistent or permanent AF. CLINICAL CONSIDERATIONS: The first goal in the treatment of AF is to reduce symptoms (eg, palpitations) and a fast ventricular rate. The second goal is to reduce the risk of a stroke. Exercise testing an

Frailty and exercise interventions : Evidence and barriers for exercise programs.

BACKGROUND: Due to the worldwide demographic transition healthcare systems are facing new demands and are increasingly confronted with an older population with specific medical needs related to multiple chronic disorders. The majority of older persons have an increased risk of frailty. In addition to pharmaceutical interventions another beneficial approach for counteracting frailty might be exercise or physical activity intervention. OBJECTIVE: The first goal was to narratively investigate the state of the art effective exercise interventions for frail older persons and briefly discuss the im

Case report: exercise-associated hyponatremia with rhabdomyolysis during endurance exercise.

Exercise-associated hyponatremia (EAH) has been well described in the literature, and its pathogenesis has been elucidated. The most common presenting symptoms of EAH are confusion or severe alterations in mental status. Although rhabdomyolysis in association with hyponatremia has been described in other settings, only 1 case of EAH associated with rhabdomyolysis at presentation has been previously described. In this article, we report on the occurrence of EAH with rhabdomyolysis in 4 athletes participating in the West Highland Way Race, a 95-mile endurance foot race. The pathogenesis of this

Impaired exercise capacity and exercise training in maintenance hemodialysis patients.

Patients with advanced chronic renal failure, including those undergoing maintenance hemodialysis (MHD), commonly suffer from sarcopenia and reduced physical exercise capacity. The causes of these disorders include anemia, cardiovascular and pulmonary diseases, other comorbid conditions, altered skeletal muscle metabolism, myopathic changes that are associated with chronic renal failure per se, malnutrition, and physical deconditioning. Exercise training has been shown to increase exercise capacity and, in some studies, to increase muscle mass. In our experience, cardiopulmonary fitness traini

[Cardiopulmonary exercise testing: the exercise stress test of the future?].

Scientific evidence is mounting that cardiopulmonary exercise testing (CPET) offers different potential clinical applications in cardiology. In comparison with traditional ECG stress testing, CPET provides more accurate diagnostic information and prognostic insights as well, in particular in patients with chronic heart failure and primary pulmonary hypertension. The barriers that actually limit the diffusion of CPET among cardiologists are primarily "cultural", because modern ergospirometric tools are easy to use, and the time required for a CPET is not dissimilar from that of a standard ECG s

[Exercise capacity and cardiovascular responses to exercise in hemiplegic patients].

We studied how the severity of hemiparesis was related to cardiovascular and metabolic responses to exercise. Eleven clinically stable patients who had had strokes were studied two to four weeks after they began walking as rehabilitation. After motor function of the lower limbs was assessed, the patients were divided into two groups according to Brunnstrom stage: one group comprised patients at stages III, IV, and V of lower limb function (n = 5), and the other comprised patients at stage VI (n = 6). A control group of 6 patients who had been hospitalized because of benign paroxysmal postural

Exercise capacity correlating to exercise hemodynamics in coronary artery disease.

Forty-seven patients with coronary artery disease characterized by angina pectoris and/or old myocardial infarction underwent two maximal exercise tests, the supine ergometer test and the upright treadmill test, to study the relationship between exercise capacity and exercise hemodynamics. Subjects were divided into 3 groups: Group I (n = 19) achieved 25 or 50 watts, Group II (n = 15) achieved 75 watts and Group III (n = 13) achieved 100 or 125 watts. During ergometer exercise, the mean pulmonary capillary pressure elevated by 25.3 +/- 8.3 mmHg in Group I, 20.8 +/- 8.8 mmHg in Group II and 12.

Exercise modality, but not exercise training, alters the acute effect of exercise on endothelial function in healthy men.

We used a within-subject crossover design to examine the impact of exercise modality, i.e., resistance (RT) and endurance (END), on the acute impact of exercise on endothelial function. Then, we examined whether a 4-wk period of chronic exercise training altered the acute exercise-induced change in endothelial function in healthy individuals. Thirty-four healthy, young men (21 ± 2 yr) reported to our laboratory and completed assessment of endothelial function [using the brachial artery flow-mediated dilation test (FMD)] before and immediately after a single bout of RT (leg-extension) or END (c

Cardiopulmonary Exercise Testing, Rehabilitation, and Exercise Training in Postpulmonary Embolism.

Long-term exercise intolerance and functional limitations are common after an episode of acute pulmonary embolism (PE), despite 3 to 6 months of anticoagulation. These persistent symptoms are reported in more than half of the patients with acute PE and are referred as "post-PE syndrome." Although these functional limitations can occur from persistent pulmonary vascular occlusion or pulmonary vascular remodeling, significant deconditioning can be a major contributing factor. Herein, the authors review the role of exercise testing to elucidate the mechanisms of exercise limitations to guide next

Can Pediatricians Assess Exercise-Induced Bronchoconstriction From Post-exercise Videos?

Objective: Exercise-induced bronchoconstriction (EIB) is a highly prevalent morbidity of childhood asthma and defined by a transient narrowing of the airways during or after physical exercise. An exercise challenge test (ECT) is the reference standard for the diagnosis of EIB. Video evaluation of EIB symptoms could be a practical alternative for the assessment of EIB. We studied the ability of pediatricians to assess EIB from post-exercise videos. Methods: A clinical assessment was performed in 20 asthmatic children (mean age 11.6 years) and EIB was measured with a standardized ECT performed i

Comparison of Supervised Exercise and Home Exercise After Ankle Fracture.

Ankle fractures are common fractures of the lower extremities that have an incidence rate of 101 fractures per 100.000 person-years. It is not clear which rehabilitation intervention should be performed after an ankle fracture. The aims of this study are to compare the effectiveness of a supervised exercise program with that of a home exercise program and to determine and compare the costs of these programs. A supervised exercise program and a home exercise program were performed for 8 weeks. The supervised exercise group consisted of 35 patients (mean age 39.23 years), and the home exercise g
