Increasing drug resistance in extensively drug-resistant tuberculosis, South Africa.

We expanded second-line tuberculosis (TB) drug susceptibility testing for extensively drug-resistant Mycobacterium tuberculosis isolates from South Africa. Of 19 patients with extensively drug-resistant TB identified during February 2008-April 2009, 13 (68%) had isolates resistant to all 8 drugs tested. This resistance leaves no effective treatment with available drugs in South Africa.

Multidrug-resistant and extensively drug-resistant Mycobacterium tuberculosis: epidemiology and control.

The emergence of multidrug-resistant (MDR)-TB and, more recently, of extensively drug-resistant (XDR)-TB is a real threat to achieve TB control and elimination. Over 400,000 new cases of MDR-TB occur each year and, although their number is currently unknown, XDR cases are recognized in every setting where there has been the capacity to detect them. The long-term vision for the full control of MDR-TB requires the scaling-up of culture and drug-susceptibility testing capacity, which is very limited in disease-endemic countries, and the expanded use of high-technology assays for rapid determinati

Transmission of Extensively Drug-Resistant Tuberculosis in South Africa.

BACKGROUND: Drug-resistant tuberculosis threatens recent gains in the treatment of tuberculosis and human immunodeficiency virus (HIV) infection worldwide. A widespread epidemic of extensively drug-resistant (XDR) tuberculosis is occurring in South Africa, where cases have increased substantially since 2002. The factors driving this rapid increase have not been fully elucidated, but such knowledge is needed to guide public health interventions. METHODS: We conducted a prospective study involving 404 participants in KwaZulu-Natal Province, South Africa, with a diagnosis of XDR tuberculosis bet

Laboratory-based surveillance of extensively drug-resistant tuberculosis, China.

To estimate the prevalence of extensively drug-resistant tuberculosis (XDR TB) in China, we retrospectively analyzed drug-resistance profiles of 989 clinical Mycobacterium tuberculosis isolates. We found 319 (32.3%) isolates resistant to ≥1 first-line drugs; 107 (10.8%) isolates were multidrug resistant, of which 20 (18.7%) were XDR. XDR TB is of major concern in China.

Adjunctive resectional lung surgery for extensively drug-resistant tuberculosis.

Extensively drug-resistant (XDR) tuberculosis (TB) poses significant management challenges as there are limited pharmacological treatment options for cure. Adjunctive resectional lung surgery decreases case-fatality rates for some patients with multidrug-resistant tuberculosis (MDR-TB), but its use has not been well documented for patients with XDR-TB. We describe 17 XDR-TB patients treated with surgery as part of their case management in Latvia during 1999-2005. One patient had no previous TB treatment history, 10 were previously treated for drug-susceptible TB and six were previously treated

[Extensively drug resistant and extremely drug resistant tuberculosis forms after multi-drug resistant tuberculosis: new faces of the old disease].

Drug resistance in tuberculosis is a growing global problem. The emergence of multi-drug resistant tuberculosis cases, particularly in the 1990s, has become an important health problem and threatens tuberculosis control worldwide. Resistance to isoniazid and rifampicin, two of the most potent anti-tuberculosis drugs currently available, in multi-drug resistant tuberculosis cases is clinically quite important. The treatment of multi-drug resistant tuberculosis requires prolonged use of costly second-line drugs with significant toxic potentials under supervision and long-term hospitalization of

Risk factors for extensively drug-resistant tuberculosis: a review.

INTRODUCTION: Extensively drug-resistant tuberculosis (XDR-TB) is emerging as a global public health problem. Its treatment is more expensive and difficult, and the outcomes much severe. The identification of risk factors for XDR-TB is of paramount importance to design effective TB control strategies. OBJECTIVE: To review published articles on risk factors for XDR-TB. METHODS: We identified 249 English articles on PubMed, and 182 were excluded by the abstract. The remaining articles were retrieved for full-text detailed evaluation by authors, and 27 relevant articles were selected for final

Preventing the next generation of extensively drug-resistant tuberculosis.

Multidrug-resistant and extremely drug-resistant tuberculosis strains threaten to become an intractable problem. Misuse of antibiotics and inadequacy of diagnostic tools have fostered drug resistance. Effective diagnostic technology would eliminate this problem, but it remains unavailable in high-burden areas. New drugs with novel targets may help combat drug resistance. However, if added singly to existing combination regimens, resistance will increase. To protect the efficacy of a new drug, it should first be used only as a second-line drug, in cases that have undergone drug susceptibility t

Adjunctive ibuprofen in pre-extensively drug-resistant and extensively drug-resistant tuberculosis: a phase IIA open-label pilot clinical trial.

Drug-resistant tuberculosis remains a major global health challenge, and excessive inflammation may contribute to tissue damage and poor outcomes. We conducted a phase IIA prospective open-label pilot trial (ClinicalTrials.gov: NCT02781909) evaluating adjunctive ibuprofen in adults with pulmonary pre-extensively drug-resistant (pre-XDR) or extensively drug-resistant (XDR) tuberculosis in Georgia. Twenty-eight participants received individualized background anti-tuberculosis regimens alone (n = 14) or with ibuprofen 400 mg daily for 2 months (n = 14) and were followed for 6 months. Can Multidrug-Resistant and Extensively Drug-Resistant Tuberculosis be Treated More Successfully?

BACKGROUND: Signs and symptoms of tuberculosis (TB) are a result of a disturbed balance between Mycobacterium tuberculosis and the patient's tolerance to its pathogenic properties. Pharmacological treatments elicit the evolution of drug resistance, and there is a need to consider alternative treatment methods. We performed a statistical analysis of World Health Organization (WHO) data, combined with an analysis of old medical records, aimed at suggesting a new treatment. METHODS: Data from the WHO for all countries worldwide (N = 215) on the epidemiology of all TB cases, human immunodeficienc

Meropenem-clavulanate is effective against extensively drug-resistant Mycobacterium tuberculosis.

beta-lactam antibiotics are ineffective against Mycobacterium tuberculosis, being rapidly hydrolyzed by the chromosomally encoded blaC gene product. The carbapenem class of beta-lactams are very poor substrates for BlaC, allowing us to determine the three-dimensional structure of the covalent BlaC-meropenem covalent complex at 1.8 angstrom resolution. When meropenem was combined with the beta-lactamase inhibitor clavulanate, potent activity against laboratory strains of M. tuberculosis was observed [minimum inhibitory concentration (MIC(meropenem)) less than 1 microgram per milliliter], and st

Management of multidrug-resistant tuberculosis and extensively drug-resistant tuberculosis: current status and future prospects.

Resistance in Mycobacterium tuberculosis arises from man-made selection of genetic mutants that result from spontaneous chromosomal alterations. Thus, drug-resistant tuberculosis (TB) is generally due to inappropriate treatment regimen, poor drug quality, erratic drug supply and poor patient adherence to treatment, reflecting failure in the implementation of an effective TB control programme. Multidrug-resistant TB (MDR-TB) usually denotes bacillary resistance to at least isoniazid and rifampicin. Proper implementation of the directly observed treatment, short-course (DOTS) strategy should ach

Extensively Drug-Resistant Tuberculosis with Conflicting Resistance Testing Results, Lesotho.

A patient with extensively drug-resistant tuberculosis in Lesotho recovered successfully after failed treatment with bedaquiline, delamanid, linezolid, and clofazimine. Whole-genome sequencing and broth microdilution testing results were not in agreement, illustrating the urgent need for studies that correlate phenotypic and genotypic resistance testing with clinical response.

Extensively drug-resistant tuberculosis: a case report and literature review.

Extensively drug-resistant tuberculosis (XDR-TB) is an emerging health problem that threatens tuberculosis (TB) control worldwide, since suitable treatment for this disease has not yet been found. We report a case of secondary pulmonary XDR-TB in a 54-year-old, HIV-negative male from Goiânia, Brazil. The patient had long-standing pulmonary tuberculosis (nine years) with extensive bilateral lung damage and had been treated with multiple antituberculosis drugs (self-administered) before XDR-TB diagnosis. The strain of Mycobacterium tuberculosis was resistant to R- rifampicin, H-isoniazid, E-etha

Old antibiotics for emerging multidrug-resistant/extensively drug-resistant tuberculosis (MDR/XDR-TB).

Recently, multidrug-resistant tuberculosis (MDR-TB) has become a therapeutic challenge. In addition to drug resistance, drug adverse events, intravenous delivery, cost and availability of some antibiotics in low-income countries have led to a look back to old drugs, especially those efficient against closely related organisms such as Mycobacterium leprae. Here we review the available drugs that respect the conditions above and could be upgraded to first-line therapy for treating MDR-TB and extensively drug-resistant tuberculosis (XDR-TB).
