Risk factors for obstetric anal sphincter injuries (OASIS) during vacuum extraction delivery in a university affiliated maternity hospital.

Introduction: Instrumental delivery is a well-known risk factor for obstetric anal sphincter injuries (OASIS). The specific characteristics among patient undergoing vacuum extraction delivery (VE) are less studied. Therefore, we aimed to evaluate risk factors for OASIS among parturient that underwent a VE delivery in a large university affiliated maternity hospital.Material and methods: The study population contained 9116 women who delivered by VE in tertiary medical center from 1988 to 2015. Inclusion criteria included deliveries beyond 24-week gestation. Multiple gestations and pregnancies c

Impact of extractable nuclear antigen, anti-double stranded DNA, antiphospholipid antibody, and anticardiolipin antibody positivity on obstetrical complications and pregnancy outcomes.

BACKGROUND: Extractable nuclear antigen (ENA) and anti-double stranded DNA (anti-dsDNA) positivity and related diseases like systemic lupus erythematosus, Sjögren syndrome, and other autoimmune diseases are known to be associated with obstetrical complications and poor perinatal outcomes. OBJECTIVE: To demonstrate the importance of ENA, anti-dsDNA, antiphospholipid (APL), and anticardiolipin (ACL) antibody positivity on pregnancy outcomes. METHODS: Ninety one pregnant women with known ENA, anti-dsDNA, APL IgG and IgM, and ACL IgG and IgM antibody positivity were retrospectively compared with

Temporal trends in the incidence of and associations between the risk factors for obstetrical anal sphincter injuries.

BACKGROUND: Obstetrical anal sphincter injuries are an important complication of vaginal deliveries that may result in short- and long-term pelvic floor morbidity and a diminished quality of life in young, healthy women. The prevalence of obstetrical anal sphincter injuries ranges from 0.1% to 8.7%. Over recent years, there seems to be a trend of increasing occurrence of obstetrical anal sphincter injuries worldwide. It is unclear why the rates are rising. Previous studies have examined the effect of different risk factors on the prevalence of obstetrical anal sphincter injuries. The change in

[Surgical methods for delivery in modern obstetrics and their influence on maternal and infant health].

The article addresses issues of comparative characterization of deliveries involving surgery and impact thereof on the health of the mother and her child. Risk factors are identified that the mother and her child run in sectio cesarea, in application of obstetrical forceps, and in vacuum-extraction of the fetus. Cesarean section was found out to be the most acceptable mode of delivery in origination of organic and functional nervous system involvement in children but the most ill-chosen and unpropitious one in the mother, especially so in those groups at risk for bleeding, septic complications

[About the choice of extraction instrument for vaginal operative termination in vertex presentation (author's transl)].

The common separation of indications at the First University Clinic for Obstetrics and Gynecology in Vienna concerning the choice of extraction instrument in cases of operative termination of vaginal delivery out of vertex position is being demonstrated. When there is only a slight increase of resistance in the delivery mechanism, when the child is mature, when there is no attitude of deflexion, and where there is no real reason for a speedy termination of delivery, the vacuum extractor comes into use. In cases of child emergency, of considerable increase of delivery mechanism resistance, of i

[Thierry's spatula instrumental extraction: a study of foetal-maternal morbidity].

OBJECTIVE: Assisted delivery is necessary in many obstetrical conditions but is involved in maternal and foetal complications. The legal pressure and the commendable aim consisting in less neonatal morbidity and mortality have called forth a reflection about the type and the way of instrumental foetal extraction. In 1950, Thierry had already felt this problem and he invented spatula to replace obstetrical forceps. Although this instrument appears empirically little deleterious, literature about its evaluation is very poor. We studied this instrument in a retrospective 190 cases series. PATIEN

[Management of second twin delivery].

OBJECTIVE: To determine the optimal mode of delivery of the second twin according to his presentation. METHODS: The PubMed data base has been consulted. RESULTS: Considering longer intertwin intervals are correlated with poorer umbilical artery blood gas results and are associated with greater rates of cesareans for the second twin after vaginal birth of the first twin and increased neonatal morbidity of the second twin, second twin delivery should be actively managed for reducing this interval (EL3). In case of non cephalic presentation of the second twin, immediate total breech extraction,

Evaluation of a new prediction model for the estimation of risk of obstetrical anal sphincter injuries.

BACKGROUND: Obstetrical anal sphincter injuries are complications of vaginal birth that have the potential to cause substantial maternal morbidity. Predicting these injuries might help to improve maternal care as well as antenatal counseling and patient education. Previous attempts to create prediction models have in many cases involved variables only known postpartum, which limits their use in an antenatal setting. Other models include parameters that are not applicable to a Northern European population. OBJECTIVE: This study aimed to develop and validate a clinically useful model for the pr

[Cesarean section for placenta praevia. Perinatal mortality and morbidity].

The severity of placenta praevia is well known and responsible of a non negligible perinatal mortality. This study relates to 65 children born after a cesarean section for placenta praevia and analyzes the fetal complications, their frequency, their prognosis as well as the obstetrical factors connected to this problem. Mortality is 3%, prematurity remains high and so is the anemia, but hypotrophy is rare. The main complications are infection essentially due to an overlapping placenta praevia, its hemorrhagic nature and the increasing number of hemorrhagic episodes and respiratory complication

[Injury hazards in deliveries using the Shute forceps (analysis of 1016 forceps deliveries)].

Report about 1016 deliveries by means of the parallel forceps (W. B. Shute). This group contains pathologic-anatomical results of 30 stillbirthes or died newborns. Accordingly to the classification in subarachnoidal, subependymal or subdural hämorrhage there are only in two cases signs of obstetrical brain damage. The hypoxia is the most important reason of intracraniale haemorrhage. In order to an objektive indication, without progredient signs of fetal hypoxia the extraction by means of parallel forceps is not dangerous. We have used the instrument for extraction as well term as preterm infa

Preliminary report: a clinical appraisal of the McGoldrick rotational vacuum extractor.

The results of a preliminary clinical appraisal of a newly designed vacuum extractor - the McGoldrick rotational vacuum extracter is presented. This instrument is designed to combine the rotation advantages of the various rotating obstetric forceps with the multiple advantages of the conventional vacuum extractor in the management of persistent occipito posterior and occipito transverse positions. The design features include an elliptical shaped cup attached to a series of bars and universal joints which allow both rotational and traction forces to be applied to the foetal scalp.
