This article reviews the current literature about the use of intraocular drugs for treatment of bacterial, fungal, viral and protozoal intraocular infection. This route of administration has the advantage of delivering antimicrobials directly into the eye achieving a high therapeutic concentration above the MIC(90) of most pathogens and with minimal systemic side effects. It is usually well tolerated but carries a small risk of intraocular complications. Intraocular therapy is now the main therapeutic approach for treatment of bacterial endophthalmitis and has an important adjunctive role in t

INTRODUCTION: Zoonotic diseases caused by different parasites are recently more frequently diagnosed in humans. Contact with domestic animals, mainly dogs and cats increases a risk for the infection. Toxocarosis and dirofilariasis have worldwide distribution. Parasitic infections in humans are diagnosed incidentally because health care providers has a little knowledge about them. In this manuscript we described a case of a polish farmer with visual problems who was diagnosed multiple parasitic coinfection cased by Dirofilaria, Toxocara sp. and Toxoplasma gondii.

BACKGROUND: This study aimed to report a case who was treated with corticosteroids and anti- parasitic agents for ocular toxoplasmosis, but who progressed to acute retinal necrosis, and finally retinal detachment.
CASE PRESENTATION: A 42-year-old man presented to the ophthalmology clinic with a 1-month history of progressive blurred vision and floaters in his right eye. His best visual acuity (VA) was 20/20 in both eyes. The anterior segment was unremarkable. Funduscopic examination of the right eye revealed active lesions of whitish foci of chorioretinitis with surrounding edema along the su

Many reports have been published on the suspected vertical transmission of Encephalitozoon cuniculi; however, prior to 2003, these reports were based on circumstantial evidence, such as histopathological, immunohistochemical, or serological diagnosis of the infection. In 2003, vertical transmission of the parasite was confirmed by detection of E. cuniculi DNA in fetuses with the nested polymerase chain reaction (PCR) technique. However, the passage of the parasite to eyes of fetus during the intrauterine stage still requires verification. In the current study, natively infected with parasite s

Bacterial, fungal, viral, and parasitic pathogens all cause systemic infection and can spread to the eye. Dissemination of pathogens via the bloodstream can lead to direct involvement of the eye. Visual loss is common in bacterial or fungal endophthalmitis, and toxoplasmosis is a major cause of ocular morbidity and poor vision after congenital or acquired infection. Some infections cause intraocular damage by indirect mechanisms (eg, HIV-mediated immunosuppression), leading to opportunistic infections such as cytomegalovirus infection, periocular nerve involvement due to leprosy, and hypersens

INTRODUCTION: Cornea ulceration and infectious keratitis are leading causes of corneal morbidity and blindness. Infectious causes are among the most frequent and most severe. Management strategies for bacterial corneal ulcers have changed significantly over the last decades, however with a more limited progress in the treatment and management of nonbacterial, infectious ulcers.
AREAS COVERED: This paper provides an overview of the current principles, strategies and treatment choices for infectious corneal ulcers in adults.
EXPERT OPINION: Topical application with a broad-spectrum antimicrobi

Ocular cysticercosis results from development of Taenia solium larvae, Cysticercus cellulosae, in the eye. This condition is observed all over the world but is particularly common in developing countries. The eye, like nervous system and muscle tissue, is a prime location for parasitic development because of its rich vascularization. Intraorbital cysticercosis accounts for 75 to 85% of cases and if untreated leads to blindness. Intravitreal involvement is the most common followed by subretinal involvement. Involvement of other structures such as the camera aqueosa, iris, and crystalline lens i

PURPOSE: Corneal epithelial wound healing (CEWH) is a complex process influenced by epigenetic regulation. Ubiquitin-like with plant homeodomain (PHD) and ring finger domains 1 (UHRF1), it plays a key role in integrating epigenetic signals. However, its precise function in modulating CEWH remains poorly understood. We describe here the functional mechanisms of UHRF1 in modulating CEWH.
METHODS: Quantitative Reverse Transcription PCR (RT-qPCR), immunofluorescence, and gene manipulation were used to investigate UHRF1 expression patterns and functions during CEWH. Integrated multi-omics and targ

The epidemiology of microbial keratitis has been investigated in several studies by analysis of organisms cultured from corneal scrapes. However, a comparison of the frequency of different organisms causing keratitis in different parts of the world is lacking. The authors present a review incorporating an analysis of data from studies worldwide. The data provide a comparison of the frequency of culture-positive organisms found in different parts of the world. Associations between a country's gross national income and types of causative organism are explored. The highest proportion of bacterial

PURPOSE: To assess alterations in allergic ocular responses to nonparasite antigens in an experimental system in which mice were skewed toward a Th2 cytokine profile by helminth infection.
METHODS: Mice were inoculated with Ascaris suum (A. suum) eggs concurrent with ragweed (RW) sensitization (RW/acute) or by repeated inoculation before RW sensitization (RW/chronic). Control subjects were divided into RW, A. suum, and sham-sensitized groups. Animals were RW-challenged in the eye and examined for changes in ocular responses, inflammatory cell infiltrates, and in vitro assessment of cytokines

Ocular toxocariasis is an uncommon worldwide parasitic infection that affects mostly children and is found in both rural and metropolitan areas. In many parts of the world, parasitic infections of the eye are a major cause of blindness. The diagnosis of toxocariasis is essentially clinical, based on the lesion morphology and supportive laboratory data such as serum enzyme-linked immunosorbent assay (ELISA) titers and ELISA Toxocara titers on aqueous humor; other diagnostic methods are imaging studies including optical coherence tomography, fluorescein angiography, computed tomography, and ocul

The role of CD4(+) T cells in the pathogenesis of ocular toxoplasmosis was investigated in murine models utilizing inbred C57BL/6 mice deficient either in CD4(+), CD8(+), or B cells (microMT). Severe necrosis and inflammation with replicating parasites were observed in the eyes of control mice after primary ocular infection, and near-normal histology with few tachyzoites was observed in the eyes of mice immunized intraperitoneally with the avirulent ts-4 strain followed by intraocular challenge with the RH strain of Toxoplasma gondii. In contrast, mild inflammation without evidence of necrosis

The authors describe a case of intraocular cysticercosis discovered in the ophthalmology department ef Edouard Herriot Hospital. With no biological signs, the diagnosis was made after vitrectomy because the visit was late in the course of the disorder, the patient lived in the mountains, and he had failed to mention a trip to the West Indies 3 months before. The symptomatology began with uveitis worsened in 10 days by greater inflammation in spite of successive corticosteroids, antibiotic, and antiviral therapy. Repeated examination and serological and radiological investigations did not help

INTRODUCTION: Parasitic infections of the eye are a major cause of ocular-surface diseases globally. While most infections are treatable, parasites can cause varying levels of damage mostly due to late diagnosis or misdiagnosis as a result of doctors' unfamiliarity with their characteristics of latency and crypsis, as well as lack of awareness by the patients.
CASE REPORTS: In this study, we present three cases of phthiriasis palpebrarum, thelaziasis, and ophthalmomyiasis, respectively. Two of the cases were treated at the clinic and did not recur. One patient refused treatment and was lost t

Ocular cysticercosis is a preventable cause of blindness. Medical therapy has been recommended for the retro-orbital and extraocular muscle form. Surgical management is largely done in cases with conjunctival or lid affliction. Though, the medical management is simple; however, the diagnosis is challenging. The anterior segment findings often mimic the more common pathologies and hence often missed at the early stage. This case series of 3 pediatric patients, tries to highlight certain clinical clues which are suggestive of ocular myocysticercosis in children. Literature often recalls anterior

External ophthalmomyiasis is an acute parasitic infection resulting from the infestation of ocular surface tissues by larvae of the order Diptera. It occurs more frequently in tropical and subtropical regions with inadequate sanitation, particularly among individuals in close contact with livestock. Patients often present with severe ocular irritation, including foreign body sensation, redness, pain, and increased lacrimation; severe cases may develop complications such as keratitis or intraocular involvement. Diagnosis primarily relies on the direct visualization of larvae under slit-lamp mic

BACKGROUND: In patients with diffuse unilateral subacute neuroretinitis (DUSN), the presence and, therefore, clinical visualization of subretinal nematode makes the diagnosis obvious. However when located under the retinal pigment epithelium (RPE), diagnosis is presumptive and challenging. We report a case of presumed DUSN to illustrate this diagnostic dilemma and to highlight the clinical signs which suggest the diagnosis.
METHODS: Case report of a patient with DUSN.
RESULTS: In our patient the nematode was not located subretinally and this made the diagnosis difficult. However, the appeara
