[Infectious agents in ocular adnexal tumours].

In recent years, infectious agents have been increasingly recognised as an important pathogenetic factor for various malignant tumours of the ocular adnexa. Many of these viruses and bacteria affect the cell cycle and physiological apoptosis. Ocular adnexal lymphoma (OAL), especially extranodal marginal cell lymphoma, is associated with Chlamydophila psittaci and Helicobacter pylori in certain geographic regions. Epstein-Barr virus seems to play a role in the natural killer/T-cell lymphoma subtype of the orbit, as has long been described for Burkitt lymphoma. Bacteria seem to induce reactive l

[Transmission of conventional agents during outpatient care].

Outpatient care bring together in a single location a large number of patients potentially bearing conventional infectious agents (virus, bacteria, fungi, parasites). This report details the germs potentially involved. We emphasize adenovirus epidemic keratoconjunctivitis, which is particularly prevalent in ophthalmology outpatient care. We analyze the modes of contamination and the means to prevent iatrogenic infections.

The ocular immune system and Flaviviruses: A crossroad between defense and disease.

Flaviviruses are emerging arthropod-borne viruses that cause severe endemic infections and epidemics on a global scale. Recent Zika virus outbreaks have highlighted a significant link between the virus and ocular abnormalities, increasing interest in the pathogenesis of Flaviviruses. Flavivirus pathogenesis involves a complex interplay between viral replication and immune responses, leading to a wide spectrum of human diseases. Flavivirus infections can cause ocular complications that are usually self-limiting but can result in irreversible visual impairment and vision loss. These complication

The associations of herpes simplex virus and varicella zoster virus infection with dementia: a nationwide retrospective cohort study.

BACKGROUND: In this study, the risk of dementia in patients with a history of herpes simplex virus (HSV) or varicella zoster virus (VZV) infection was evaluated. METHODS: This nationwide cohort study used data from the Korean National Health Insurance Service collected between 2006 and 2017. A total of 752,205 subjects ≥ 45 years of age not diagnosed with dementia until 2006 were included. A multivariate Cox regression model, adjusted for age, sex, and other comorbidities, was used to assess the hazard ratio (HR) for dementia based on VZV or HSV infection. The interaction effects of both vira

Intraocular delivery of anti-infective drugs-bacterial, viral, fungal and parasitic.

This article reviews the current literature about the use of intraocular drugs for treatment of bacterial, fungal, viral and protozoal intraocular infection. This route of administration has the advantage of delivering antimicrobials directly into the eye achieving a high therapeutic concentration above the MIC(90) of most pathogens and with minimal systemic side effects. It is usually well tolerated but carries a small risk of intraocular complications. Intraocular therapy is now the main therapeutic approach for treatment of bacterial endophthalmitis and has an important adjunctive role in t

A review of technological advances in the management of bacterial, fungal, viral, and Acanthamoeba keratitis.

Microbial keratitis is a major cause of monocular blindness worldwide. Trauma and contact lens wear are major risk factors. Bacteria are a major cause, but fungi, viruses, and Acanthamoeba can also cause the disease. Therapeutic options are dwindling with the rise of antimicrobial resistance in all of these microbes, but especially bacteria. Infection with resistant strains results in worse clinical outcomes. New antimicrobial therapies are being developed but most have yet to be either used to treat ocular infections or become commercially available. For bacterial keratitis, antimicrobial pep

[Microbiological diagnosis of ocular surface infections aside from virology].

The ocular surface is a structure exposed to multiple stresses that can directly or indirectly lead to infectious contamination and compromise visual function. This significant risk justifies multidisciplinary care, often in an emergency context, bringing together ophthalmologists and microbiologists to optimize and guide microbiological testing on small volume samples and ensure the most appropriate treatment. Herein, we present the various steps leading to microbiological diagnosis, from pre-analytical requirements, sampling methods, transport conditions, and then to the analytical steps of

Prevalence of Chlamydia trachomatis in eye specimens of patients suspected of having viral keratitis: a cross-sectional study.

BACKGROUND: Chlamydia trachomatis (C. trachomatis) is a major global health concern, recognized among the leading bacterial causes of sexually transmitted infections and implicated in ocular diseases. Its association with chronic follicular conjunctivitis and severe papillary inflammation underscores the importance of accurate identification in diagnosing trachoma. This study evaluated the prevalence of C. trachomatis in patients suspected to viral keratitis referred to the lab and comparing four different eye specimen types. METHODS: This cross-sectional study (2020-2022) involved 161 suspec

[Viral Conjunctivitis: Findings, Therapy, and Prophylaxis].

Viral conjunctivitis is one of the most common acute eye diseases. The fall and winter months are known to be the main season for viral infections which is also reflected in the ophthalmological outpatient clinics. Viral conjunctivitis is often accompanied by symptoms of the upper and lower respiratory tract, fever, chills, arthralgia or skin lesions. The spectrum of pathogens comprises DNA viruses such as Adeno-, Herpes simplex and Molluscum contagiosum as well as RNA viruses. Symptoms caused by pandemic pathogens such as SARS-CoV-2 and mpox viruses can also cause ocular manifestation. Viral

Natural protection of ocular surface from viral infections - A hypothesis.

A pandemic outbreak of a viral respiratory infection (COVID-19) caused by a coronavirus (SARS-CoV-2) prompted a multitude of research focused on various aspects of this disease. One of the interesting aspects of the clinical manifestation of the infection is an accompanying ocular surface viral infection, viral conjunctivitis. Although occasional reports of viral conjunctivitis caused by this and the related SARS-CoV virus (causing the SARS outbreak in the early 2000s) are available, the prevalence of this complication among infected people appears low (~1%). This is surprising, considering th

Viral ocular manifestations: a broad overview.

The viruses able to affect the eye are taxonomically diverse, ranging from double-stranded DNA viruses, to single stranded RNA viruses, to retroviruses. Any part of the eye may be affected, frequently producing blepharitis, conjunctivitis, keratitis, uveitis, cataract and retinitis. The more common ocular viral infections include the Herpesviruses such as HSV-1, VZV and CMV. The HIV pandemic is placing a serious burden on ophthalmology clinics, particularly in sub-Saharan Africa as the number of viral ocular diseases is increasing. In particular, CMV retinitis is becoming more prevalent where

Practical ophthalmic microbiology.

Infection is commonly encountered in everyday ophthalmic practice. It is of great importance that all ophthalmic personnel are familiar with the basic etiologies of these infections, and the basic techniques which are used in diagnosis. When evaluating a patient with possible infection, it is often of great help to be familiar with the normal microbial flora of the human body. This normal flora has been detailed in the article. The assumption is that knowledge of a patient's flora can help guide decisions about the possible etiologies of an infection. When presented with an obvious ocular infe

Zika Virus Infection in Mice Causes Panuveitis with Shedding of Virus in Tears.

Zika virus (ZIKV) is an emerging flavivirus that causes congenital abnormalities and Guillain-Barré syndrome. ZIKV infection also results in severe eye disease characterized by optic neuritis, chorioretinal atrophy, and blindness in newborns and conjunctivitis and uveitis in adults. We evaluated ZIKV infection of the eye by using recently developed mouse models of pathogenesis. ZIKV-inoculated mice developed conjunctivitis, panuveitis, and infection of the cornea, iris, optic nerve, and ganglion and bipolar cells in the retina. This phenotype was independent of the entry receptors Axl or Mertk

Diagnosis and management of red eye in primary care.

Red eye is the cardinal sign of ocular inflammation. The condition is usually benign and can be managed by primary care physicians. Conjunctivitis is the most common cause of red eye. Other common causes include blepharitis, corneal abrasion, foreign body, subconjunctival hemorrhage, keratitis, iritis, glaucoma, chemical burn, and scleritis. Signs and symptoms of red eye include eye discharge, redness, pain, photophobia, itching, and visual changes. Generally, viral and bacterial conjunctivitis are self-limiting conditions, and serious complications are rare. Because there is no specific diagn

An ocular model of adenovirus type 5 infection in the NZ rabbit.

Ocular adenoviral infections occur worldwide, and currently, there is no ocular animal model for evaluating new antivirals or studying pathogenesis. With a paired-eye design, an ocular model was developed in 32 New Zealand rabbits following topical and intrastromal inoculation with a clinical isolate of adenovirus type 5 (Ad5 McEwen). Clinical signs of infection--conjunctivitis, corneal edema, subepithelial infiltrates, and iritis--and seroconversion were evaluated. Replicating virus on the ocular surface was determined by serial ocular titers. Reproducible acute ocular infection was demonstra

Bilateral Cytomegalovirus Retinitis With Co-infection of Epstein-Barr Virus and Varicella-Zoster Virus: A Rare Case.

Cytomegalovirus retinitis (CMVR) is a form of infectious uveitis, a disease characterized by inflammation of the uvea, the middle layer of the eye. It is caused by the cytomegalovirus (CMV). Although CMVR generally involves a single virus, rare cases have been reported in which multiple viral DNAs have been detected in intraocular fluid using polymerase chain reaction (PCR) testing. The present case report documents a rare case of bilateral infectious uveitis with simultaneous detection of CMV and Epstein-Barr virus (EBV) in the right eye and CMV and varicella-zoster virus (VZV) in the left ey

Herpes simplex eye infections: clinical manifestations, pathogenesis and management.

Herpes infection of the eye may be acquired as the patient's first exposure to the virus (primary infection) or as involvement of a new anatomical site (the eye) in a patient with previous HSV infection. In either case, patients with herpetic eye infection risk recurrent eye disease throughout their lives. The infective lesions of the corneal epithelium (dendritic and geographic ulcers) occasionally develop into noninfective indolent or trophic ulcers, particularly under the influence of cauterizing chemicals or corticosteroids. Inflammation of the corneal stroma may accompany herpetic epithel
