Structural basis of the cofactor- and substrate-assisted activation of human coagulation factor IXa.

Human coagulation factor IX serves both to maintain and to control blood coagulation. The dual function of this hemophilic factor is implemented by a tiered activation mechanism. Processed two-chain factor IXa is catalytically silent; only together with its cofactor VIIIa does factor IXa form the highly potent Xase complex. The detailed mechanism of this secondary activation has remained elusive so far. Here we present the crystal structures of Xase-like factor IXa mutants with several-thousand-fold activity enhancement that mimic the secondary activation by Xase formation. The structures reve

Diffuse Intracerebral Hemorrhage in an Infant With a Novel Homozygous Variant Leading to Severe Protein C Deficiency.

Protein C is a circulating anticoagulant that inhibits factor Va and VIIIa and promotes fibrinolysis. Compound heterozygous or homozygous variants in the Protein C gene (PROC) lead to severe deficiency of protein C and affected neonates typically present shortly after birth with purpura fulminans. We describe an infant who suffered a diffuse intracranial hemorrhage as a neonate and presented with purpura fulminans as an older infant which led to investigations that were consistent with severe protein C deficiency. We demonstrate subacute findings on neuroimaging and suggest this condition shou

Role of tissue factor pathway inhibitor in hormone-induced venous thromboembolism.

Exposure to higher levels of steroid hormones, like that in pregnancy or during combined hormonal contraception, increases the risk of venous thromboembolism. Development of resistance to activated protein C (APC) thought to be the underlying pathomechanism of this prothrombotic state. This coagulation phenomena is largely to be explained by the hormone-induced impairment of the protein S/ tissue factor pathway inhibitor (TFPI) leading to a less efficient inactivation of factor Va and factor VIIIa by APC. APC resistance and decreased protein S/TFPI function were associated with the risk of fir

[Post-tonsillectomy haemorrhage treatment with activated factor VII].

Tonsillectomy is a common procedure in otorhinolaryngology and postoperative haemorrhage is a well-known complication. In this case, a 34 year-old male with obstructive sleep apnoea and no coagulation defects was operated; 30 minutes after the operation he presented with primary post operative haemorrhage and surgical haemostasis was not possible. After one hour of secondary surgery tranexamacid 1 g and phytomenadione 10 mg was given, but the diffuse bleeding continued until recombinant factor VIIa 96 mg/kg was administered.

Recombinant activated Factor VII in major surgery and trauma patients: a response.

The review article by Wilson et al. (Expert Opin. Drug Saf., 2005) on recombinant activated Factor VII (rFVIIa) offers some valuable insights into the ongoing exploration of this agent in the management of uncontrolled bleeding--an area of high unmet clinical need. This editorial highlights that the chosen citation of Riou et al. (Shock, 2004), in reference to two parallel studies conducted in trauma patients with blunt and penetrating injury, was both a report of an early analysis of the data, and was only one of several abstracts on these studies published in 2004. Asymptomatic type B right atrial thrombus in a case with protein S deficiency.

Thirty seven year old asymptomatic male underwent routine medical examination which revealed an abnormal mass in the right atrium. Family history was not suggestive of any cardiac or malignant disease. Detailed investigation detected deficiency of protein S, which is a vitamin K dependent protein and a cofactor for activated protein C mediated cleavage of factor Va and VIIIa. The deficiency of protein S predisposes to venous thrombosis. Further investigation revealed that it was an organized calcified thrombus in right atrium occupying almost whole of the cavity. Various approaches including s

Erythema elevatum diutinum: clinical, histopathologic, and immunohistochemical characteristics of six patients.

Erythema elevatum diutinum (EED) is a chronic cutaneous vasculitis occurring in association with a variety of conditions including autoimmunity, infectious disease, and hematological abnormalities. The role of associated medical problems is controversial, and the exact pathogenesis of EED is unknown. A series of six cases is reported. The typical clinical presentation was that of erythematous papules and plaques involving the extensor surfaces of the extremities. Histologically, a spectrum from leukocytoclastic vasculitis to vessel occlusion and dermal fibrosis was seen. The lesions of EED hav

[Hereditary thrombophilia as a cause of recurrent transplant thromboses].

We report a case of hereditary protein S and protein C deficiency which are a rare defects of the anti-coagulation-system. Protein S is a vitamin K dependent glycoprotein that functions as a cofactor to activated Protein C in the inactivation of coagulation factors V and VIIIa. A deficiency of these proteins caused by a genetic defect increase the risk of recurrent thrombosis at a younger age. Acquired decreases in protein S and C concentration have been reported in connection with age, sex, pregnancy and with oral anticonception. Current perspectives on non-factor therapies for hemophilia.

Hemophilia is an inherited bleeding disorder caused by deficiency of coagulation factor VIII or IX and results in impaired thrombin generation and recurrent bleeding. Prophylactic factor replacement remains the standard treatment; however, its limitations, including the burden of intravenous treatment, inhibitor development, and residual bleeding, necessitate alternative strategies. Non-factor therapies restore hemostatic balance independently of factor replacement. These agents include factor VIIIa-mimetic bispecific antibodies and rebalancing therapies that enhance thrombin generation by inh

Protein C activator from the venom of Agkistrodon blomhoffi ussuriensis retards thrombus formation in the arterio-venous shunt in rats.

Protein C (PC) is an anticoagulant protein which, being activated by thrombin, degrades factors V/Va and VIII/VIIIa and releases a tissue-type plasminogen activator. Some Agkistrodon snake venoms contain PC activators which, in experiments, exert an anticoagulant action. An antithrombotic effect of the PC activator from the venom of A. blomhoffi ussuriensis on the model of thrombus formation in the arterio-venous shunt in rats was under investigation. Administration of the PC activator resulted in a dose-dependent prolongation of the thrombus formation time and a decrease in plasma PC activity

[Soluble thrombomodulin: as a marker of endothelial injury].

Thrombomodulin is an endothelial cell surface glycoprotein that forms a 1:1 complex with thrombin. In this form, thrombin can activate approximately 1,000-fold more protein C than thrombin alone and does not activate coagulation factors, V and VIII, and platelets. Activated protein C inactivates factors Va and VIIIa. Thus thrombomodulin converts thrombin from a procoagulant protease to an anticoagulant. The soluble thrombomodulin present in human urine and plasma appears to represent a truncated form that lacks the transmembrane and cytoplasmic domains of tissue thrombomodulin. The plasma leve

A novel humanized FIX/FX hemophilia A mouse model for preclinical evaluation of FVIIIa-mimetic bispecific antibodies.

The use of nonfactor products such as emicizumab, a bispecific antibody that mimics the cofactor function of activated factor VIII (FVIIIa) by bridging FIXa and FX, has significantly reduced bleeding episodes in people with hemophilia A (HA). However, emicizumab high species specificity in its FVIIIa-mimetic cofactor activity has limited in vivo functional evaluation to an acquired HA nonhuman primate model or congenital HA mouse model with infusion of human FIX (hFIX) and hFX proteins. To overcome the limitations of existing models, we generated hFIX/hFX double-knockin congenital HA model (hu

The Construction of a Molecular Model for the Ternary Protein Complex of Intrinsic Coagulation Pathway Factors Provides Novel Insights for the Pathogenesis of Cross-Reactive Material Positive Coagulation Factor Mutations.

The human coagulation pathway orchestrates a complex series of events vital for maintaining vascular integrity, in which the intrinsic pathway plays a pivotal role in amplifying and propagating the coagulation response. Dysregulation of this pathway can lead to various bleeding disorders and thrombotic complications, posing significant health risks. In this pathway, the activation of Factor (F) X zymogen is catalyzed by the FVIIIa-FIXa binary complex, but knowledge about this is still incomplete. Understanding the structural and functional intricacies of the FVIIIa-FIXa-FX (zymogen) complex is

Combined factor VII/protein C deficiency results in intrauterine coagulopathy in mice.

To determine whether an additional loss of the coagulation factor VII (FVII) gene influenced the coagulopathy observed in protein C gene-deficient (PC(-/-)) embryos and neonates, we crossed mice doubly heterozygous for the factor VII (FVII(+/-)) and protein C (PC(+/-)) genes to produce offspring possessing the 9 predicted genotypic combinations. FVII(-/-)/PC(-/-) embryos, although present at their expected Mendelian frequency, displayed a phenotype that had not been observed in either the FVII or PC singly deficient embryos. At E12.5 days postcoitum (dpc), FVII(-/-)/PC(-/-) embryos demonstrate

Evolutionary Adaptations in Pseudonaja Textilis Venom Factor X Induce Zymogen Activity and Resistance to the Intrinsic Tenase Complex.

The venom of the Australian snake Pseudonaja textilis comprises powerful prothrombin activators consisting of factor X (v-ptFX)- and factor V-like proteins. While all vertebrate liver-expressed factor X (FX) homologs, including that of P. textilis, comprise an activation peptide of approximately 45 to 65 residues, the activation peptide of v-ptFX is significantly shortened to 27 residues. In this study, we demonstrate that exchanging the human FX activation peptide for the snake venom ortholog impedes proteolytic cleavage by the intrinsic factor VIIIa-factor IXa tenase complex. Furthermore, ou

An RNA aptamer exploits exosite-dependent allostery to achieve specific inhibition of coagulation factor IXa.

Hemostasis relies on a reaction network of serine proteases and their cofactors to form a blood clot. Coagulation factor IXa (protease) plays an essential role in hemostasis as evident from the bleeding disease associated with its absence. RNA aptamers specifically targeting individual coagulation factors have potential as anticoagulants and as probes of the relationship between structure and function. Here, we report X-ray structures of human factor IXa without a ligand bound to the active site either in the apo-form or in complex with an inhibitory aptamer specific for factor IXa. The aptame

An update on clinical and basic aspects of the protein C anticoagulant pathway.

The protein C anticoagulant pathway provides a mechanism for regulating the coagulation process through the selective inactivation of factors Va and VIIIa. Recent studies have suggested that factor V may facilitate this process and that a mutation at one of the inactivation sites can contribute to resistance to activated protein C (APC) inactivation of factor Va. This appears to be a common cause of familial thrombophilia. The control mechanisms involved in factor Va inactivation have also begun to become more clear. Membrane surfaces seem to be critical to complete factor Va inactivation, and

Moyamoya syndrome with inherited proteins C and S deficiency: report of one case.

Moyamoya disease (MMD) is a vascular abnormality characterized by progressive narrowing of the internal carotid, middle, anterior and posterior cerebral arteries and the development of basal collaterals forming the classic angiographic appearance-moyamoya. Protein C (PC) and its cofactor, protein S (PS), which are vitamin-K dependent, act as inhibitors of coagulation cascade by inactivating factor Va and factor VIIIa and facilitating thrombolysis. Deficiencies of these proteins leading to a prothrombotic state increase the risk of ischemic cerebrovascular accident. Herein we report a 3-year-ol
