Milvexian: evaluating the factor XIa inhibitor for the treatment of acute coronary syndrome.

INTRODUCTION: Balancing the prevention of thrombosis with bleeding risk when combining anticoagulants and platelet antagonists remains a concern among clinicians, particularly in patients with acute coronary syndrome (ACS) who are treated with potent antiplatelet therapy. This may be because the available antiplatelet and anticoagulants are unable to uncouple physiological hemostasis and pathological thrombosis. Therefore, their use is associated with an unavoidable elevated risk of bleeding. AREAS COVERED: Evidence available from studies evaluating FXIa inhibitors and milvexian was collected

Factor XI and XIa Inhibitors: A State-of-the-Art Review Across Clinical Indications.

Balancing thrombotic and bleeding risk remains the central challenge of anticoagulation, and clinically significant bleeding continues to limit even direct oral anticoagulants, especially in high-bleeding-risk patients. Factor XI (FXI) and activated FXI (FXIa) inhibitors are a mechanistically novel class that seeks to uncouple thrombosis from hemostasis; FXI amplifies clot formation through the intrinsic pathway but contributes little to physiologic hemostasis. This state-of-the-art review examines three classes of FXI/FXIa inhibitors (antisense oligonucleotides, monoclonal antibodies, and sma

Discovery of potent, highly selective, and orally bioavailable factor XIa inhibitors for anticoagulant therapy.

Factor XIa (FXIa) has emerged as a promising target for novel anticoagulant development since inhibiting it can reduce thrombosis without significant bleeding risks. Despite a few FXIa inhibitors entering clinical trials, none have been approved for the market yet. Here, we present highly selective and orally bioavailable FXIa inhibitors derived from compound 1, 4-(5-chloro-2-(1H-tetrazol-1-yl)phenyl)-5-methoxypyridin-2(1H)-one. Structure-activity relationship studies led to the discovery of promising 1-(pyridin-2-ylmethyl)pyridin-2(1H)-one-based FXIa inhibitors 37, 39b, 43, and 46b, which exh

What is the Future Position of Factor XIa Inhibitors for Patients with Atrial Fibrillation?

Factor XIa (FXIa) may be involved in thrombus formation, but only to a lesser extent involved in haemostasis. Several novel FXIa inhibitors are under investigation, and Phase II trials demonstrated marked reduction of bleeding compared with standard treatment by factor Xa inhibitors. Asundexian is a small molecule that selectively inhibits FXIa. A large-scale Phase III clinical trial, OCEANIC-AF, has been initiated to compare the efficacy and safety between asundexian and apixaban. However, the OCEANIC-AF study was recently halted due to the inferior efficacy of asundexian versus the apixaban

Asundexian: an oral small molecule factor XIa inhibitor for the treatment of thrombotic disorders.

Oral anticoagulants, including warfarin and direct oral anticoagulants, are the standard of care for thrombosis prevention and treatment; however, concerns of bleeding often dictate treatment decisions. Inhibition of the intrinsic coagulation system via factor XIa may allow for selective inhibition of the coagulation cascade without significantly impacting hemostasis after injury. Asundexian is an oral small molecule factor XIa inhibitor that, via this novel mechanism, may prove to be a safe and effective option compared with available anticoagulants. Early clinical data for asundexian was pro

Factor XIa Inhibitors for Secondary Prevention After Noncardioembolic Stroke: A Systematic Review and Meta-Analysis.

OBJECTIVES: Patients with noncardioembolic ischemic stroke or transient ischemic attack (TIA) remain at substantial risk of recurrent ischemic events, despite guideline-recommended antithrombotic therapy. Factor XIa (FXIa) inhibitors may reduce thrombotic risk while preserving hemostasis. METHODS: We systematically searched MEDLINE and Scopus for randomized-controlled clinical trials (RCTs) comparing FXIa inhibitors with placebo in patients with acute noncardioembolic ischemic stroke or TIA. Primary efficacy outcome was first stroke (ischemic, hemorrhagic, or undefined), and primary safety ou

Anticoagulant drugs targeting factor XI/XIa and coagulation tests: we urgently need reliable pharmacodynamic data.

Antifactor (F)XI/FXIa anticoagulants under development include antisense oligonucleotides, monoclonal antibodies, and small molecules. They do not require routine monitoring, but knowledge of their impact on coagulation tests is essential in view of their expected widespread use. A concentration-dependent prolongation of activated partial thromboplastin time has been shown but varies according to reagents, and the lack of comprehensive data makes interpretation of this test difficult. Measurement of FXI clotting activity is relevant only in case of treatment with antisense oligonucleotides. Me

Venous thromboembolism prevention and treatment with factor XI/XIa inhibitors: current status and future perspectives.

Current treatments for venous thromboembolism include warfarin, various heparins, and direct oral anticoagulants. While effective, there's ongoing research for safer alternatives, especially for high-risk patients (e.g., cancer, post-operative, and those with end-stage renal disease). Factor XI has been identified as crucial in abnormal thrombosis but less so in normal hemostasis, suggesting that inhibiting it could reduce thrombosis while also limiting bleeding risks. Recent phase 2 trials on factor XI inhibitors show promise for preventing venous thromboembolism in patients undergoing total

Milvexian: A Focus on a New Oral Anticoagulant that Targets Factor XIa for Thromboembolism Prophylaxis.

Drugs that target factor XI and/or XIa have been evaluated as alternatives to existing anticoagulants, in light of studies that indicate that a decrease in Factor XI/XIa levels or activity may result in a lower risk of thrombosis without a significant increase in bleeding risk. Milvexian is an investigational small-molecule factor XIa inhibitor that has recently completed phase 2 clinical trials. Preclinical studies were suggestive of its potential to prevent arterial and venous thrombosis. It was well-tolerated in healthy participants, as well as in participants with mild or moderate hepatic

A pharmacokinetic and pharmacodynamic evaluation of asundexian: a novel factor XIa inhibitor for stroke prevention.

INTRODUCTION: Antithrombotic therapy is the mainstay of ischemic stroke prevention. Current drugs (antiplatelets and oral anticoagulants) lead to increased bleeding risks, and the rates of stroke recurrence, despite antithrombotic therapy, are still elevated. There is a need for novel antithrombotic therapies with superior effectiveness but without increased bleeding risk. Factor XIa inhibitors might cover this gap. AREAS COVERED: This manuscript examines the pharmacokinetic and pharmacodynamic properties of asundexian and the current clinical evidence regarding its application in preventing

The Kv1.3 channel-inhibitory toxin BF9 also displays anticoagulant activity via inhibition of factor XIa.

The Kv1.3 channel plays potential roles in immune, inflammation and coagulation system. Many studies showed that Kv1.3 channel inhibitors have immunosuppressive and anti-inflammatory activities, but no Kv1.3 channel inhibitors have been found to have anticoagulation activities. Here, based on our previous work about Kv1.3 channel toxin peptide inhibitors, we first attempt to test anticoagulation activities of four known venom-derived Kv1.3 channel inhibitors with different structural folds: BmKTX with CSα/β structural fold, OmTx3 with CSα/α structural fold, BF9 with Kuntz-type structural fold,

Determination of the Potential Clinical Benefits of Small Molecule Factor XIa Inhibitors in Arterial Thrombosis.

Anticoagulants are the mainstay for the prevention and treatment of thrombosis. However, bleeding complications remain a primary concern. Recent advances in understanding the contribution of activated factor XI (FXIa) in arterial thrombosis with a limited impact on hemostasis have led to the development of several FXIa-targeting modalities. Injectable agents including monoclonal antibodies and antisense oligonucleotides against FXIa have been primarily studied in venous thrombosis. The orally active small molecules that specifically inhibit the active site of FXIa are currently being investiga

Use of factor XI/XIa inhibitors in patients with atrial fibrillation: Current progress and future prospects.

Stroke prevention is one of the pillars of management in elderly patients with atrial fibrillation (AF), hence effective and safe anticoagulation strategies are needed. While available anticoagulants target coagulation factors involved in thrombus formation, their bleeding complications and limitations in aging populations underscore the necessity for novel agents. Factor XI/XIa (FXI/XIa), which selectively targets the intrinsic coagulation pathway, offers a promising approach by attenuating thrombin generation without impairing hemostasis. Various novel inhibitors, including antisense oligonu

Tissue Factor/Factor XIa Dual-activated Thrombin Generation is Able to Reliably Measure Thrombin Generation at All Hemophilia A Disease Severities.

Introduction Predicting bleeding in hemophilia A is difficult due to limitations of existing factor VIII (FVIII) activity assays, which correlate poorly with clinical bleeding tendency. Measurement of thrombin generation (TG) by calibrated automated thrombography (CT) may be an alternative to predict bleeding as it measures an individual's complete plasma derived coagulation potential, rather than just focusing on one part of the coagulation cascade. However, lack of standardization limits its applicability. Objective To evaluate the ability of our standardized tissue factor (TF)/factor XI (FX

Factor XIa inhibition for the prevention of thrombosis: mechanism, clinical trial signals, and indication-specific positioning.

PURPOSE OF REVIEW: Factor XI (FXI) and its activated form, factor XIa (FXIa), have emerged as a leading novel anticoagulant target of the past decade. This review summarizes mechanistic and clinical data from 2023 through 2026 and proposes an indication-specific framework for FXIa inhibitor positioning. RECENT FINDINGS: Three large randomized studies diverged by indication. Abelacimab reduced major and clinically relevant nonmajor bleeding by 62% versus rivaroxaban in atrial fibrillation (AZALEA-TIMI 71). Asundexian was inferior to apixaban for stroke prevention in atrial fibrillation (OCEANI

Laboratory Evaluation of Interferences Associated with Factor XIa Inhibitors Asundexian and Milvexian in Routine Coagulation Assays.

Direct oral anticoagulants (DOACs) are increasingly used for the treatment of thrombosis. While inhibitors of factor IIa and factor Xa have shown effectiveness, the risk of bleeding remains a significant concern. Recently, direct factor XIa inhibitors-including asundexian and milvexian-have emerged as potential anticoagulation therapies, based on clinical observations that patients with factor XIa deficiencies seldom present with spontaneous bleeding tendencies. The interferences associated with DOACs in routine and specialised coagulation assays are well-described; however, the interferences

AlphaFold3 assisted screening strategy identifying novel coagulation factor XIa inhibitory peptides from deep-sea mussel Gigantidas platifrons.

Marine mussels represent a valuable source for discovery of novel biologically active peptides targeting key enzymes in the coagulation system. However, traditional iterative isolation methods are often unsustainable for rare deep-sea mussels. To address this challenge, we introduced an integrated strategy that combines traditional enzymatic hydrolysis and bioassay-guided fractionation of Gigantidas platifrons protein hydrolysate (GPk) with AI-assisted virtual screening. By leveraging AlphaFold3 and stringent metrics, we achieved efficient identification while minimizing the use of scarce biol

Development of selective NBD-based fluorescent probes for in vitro detection of factor XIa and Thrombus imaging.

Factor XIa (FXIa) is a promising target for safer anticoagulants. Herein, a series of NBD-based fluorescent probes (FNC2-FNC6) were developed by linking an FXIa inhibitor scaffold to an NBD fluorophore via alkyl chains of varying lengths. Molecular dynamics simulations revealed that shorter linkers, especially a two‑carbon chain, stabilize NBD binding in the FXIa pocket, enhancing fluorescence. Among these probes, FNC2 displayed potent FXIa inhibition (Ki = 14.2 nM) and selectivity over related proteases. It exhibited linear fluorescence increase upon FXIa binding (R2 = 0.9952; LOD = 1.19 μg/m
