HMOs don't have to fail.

One of the bright spots in the sometimes black picture that is painted of our national health care system is HMOs. Health maintenance organizations provide health services to an enrolled group of patients for a fixed, prepaid fee. This arrangement induces HMOs to try to keep patients healthy for a competitive rate. Because of its special nature, however, an HMO cannot be managed like a fee-for-service hospital or group practice. And because few managers fully understand them, more and more HMOs are failing. The authors of this article say, however, that some failures can be avoided. Good manag

How User Fees Influence Contraception in Low and Middle Income Countries: A Systematic Review.

Accessible and quality reproductive health services are critical for low- and middle-income countries (LMICs). After a decade of waning investment in family planning, interest and funding are growing once again. This article assesses whether introducing, removing, or changing user fees for contraception has an effect on contraceptive use. We conducted a search of 14 international databases. We included randomized controlled trials, interrupted-time series analyses, controlled before-and-after study designs, and cohort studies that reported contraception-related variables as an outcome and a ch

Outsourced eye care in the Finnish capital region: Switching from fee-for-service to bundled payment model.

PURPOSE: The demand for specialised eye care has quadrupled over the last two decades, driven by an aging population and the advent of new treatments. This surge has led to increased outsourcing by the Helsinki and Uusimaa Hospital District (HUS) Eye Hospital, initially relying on a fee-for-service model. This model, however, has been marked by excessive bureaucracy and transaction costs. Between 2019 and 2021, HUS Eye Hospital embarked on a comparative analysis of outsourcing models to identify the most cost-effective, efficient and suitable approach. METHODS: After the review of literature

Adult Medicaid Coverage for Periodontal Treatment: A State-to-State Comparison.

Dental care remains an optional benefit for adults with Medicaid, with coverage varying across states. Recent expansions increased periodontal service coverage, but the scope of coverage and the policies that govern that scope remain unknown. This study analyzed coverage policies and fees for four periodontal services across 43 Medicaid programs between January and March 2024. Frequency limitations were the most common coverage policies identified, followed by prior authorization requirements, clinical requirements, and quadrant limitations. Veteran Health Administration fees were roughly thre

Health Services Researchers' Use of the Dartmouth Atlas of Health Care: A Literature Review and Network Analysis 1996-2023.

The Dartmouth Atlas of Health Care published data on regional variations in healthcare for Medicare fee-for-service beneficiaries from 1996 to 2022. Despite its revolutionary impact on health services research and policymaking, to our knowledge, no comprehensive review has been undertaken to systematically study how the Dartmouth Atlas has been used by the research community. We identified all 8507 publications that cited the Dartmouth Atlas between 1996 and 2023. With this corpus, we conducted authorship network analyses and conducted a content analysis of a 5% random sample (n = 403). Networ

HMO enrollment by Medicare beneficiaries in heterogeneous communities.

This study examines the effect of ethnicity, race, and other socioeconomic and utilization variables on belonging to a fee-for-service or capitated plan by Medicare beneficiaries. Native-born beneficiaries cite health status, physical access to health care facilities, and income levels as affecting their choice. Immigrants, on the other hand, mention age, a lifestyle of living alone, and income as influencing their decision. Among foreign-born nationals, race appears to play an important role in choosing health care plans. A key factor affecting enrollment among immigrant populations appears t

Methods for a population-based Comprehensive Eye care Workload Assessment (CEWA) study in Southern India.

Eye care programs, in developing countries, are often planned using the prevalence of blindness and visual impairment, often estimated from Rapid Assessment of Avoidable Blindness (RAAB) surveys. A limitation of this planning approach is that it ignores the annual overall eye care requirements for a given population. Moreover, targets set are arbitrary, often influenced by capacity rather than need. To address this lacunae, we implemented a novel study design to estimate the annual need for comprehensive eye care in a 1.2 million populations. We conducted a population-based longitudinal study

Implementing a Fee-for-Service Cervical Cancer Screening and Treatment Program in Cameroon: Challenges and Opportunities.

BACKGROUND: Cervical cancer screening is one of the most effective cancer prevention strategies, but most women in Africa have never been screened. In 2007, the Cameroon Baptist Convention Health Services, a large faith-based health care system in Cameroon, initiated the Women's Health Program (WHP) to address this disparity. The WHP provides fee-for-service cervical cancer screening using visual inspection with acetic acid enhanced by digital cervicography (VIA-DC), prioritizing care for women living with HIV/AIDS. They also provide clinical breast examination, family planning (FP) services,

Medicaid program; payments for services furnished by certain primary care physicians and charges for vaccine administration under the Vaccines for Children program. Final rule.

This final rule implements Medicaid payment for primary care services furnished by certain physicians in calendar years (CYs) 2013 and 2014 at rates not less than the Medicare rates in effect in those CYs or, if greater, the payment rates that would be applicable in those CYs using the CY 2009 Medicare physician fee schedule conversion factor. This minimum payment level applies to specified primary care services furnished by a physician with a specialty designation of family medicine, general internal medicine, or pediatric medicine, and also applies to services rendered by these provider type

Innovation, value and reimbursement in radiation and complex surgical oncology: Time to rethink.

BACKGROUND AND PURPOSE: Complex surgery and radiotherapy are the central pillars of loco-regional oncology treatment. This paper describes the reimbursement schemes used in radiation and complex surgical oncology, reports on literature and policy reviews. MATERIAL AND METHODS: A systematic review of the literature of the reimbursement models has been carried out separately for radiotherapy and complex cancer surgery based on PRISMA guidelines. Using searches of PubMed and grey literature, we identified articles from scientific journals and reports published since 2000 on provider payment or r

Examining supplier-induced demand under regulated fees: dynamic panel evidence from Iran.

Cataract surgery is among the most frequently performed procedures globally and in Iran, often regarded as highly effective and cost-efficient. However, its rapid growth, particularly under a fee-for-service model, raises concerns about supplier-induced demand, where providers may influence patients to undergo potentially discretionary procedures to maintain income. We analyzed administrative data from the Iranian Armed Forces Insurance Organization, covering active and retired military personnel and their families across all 31 provinces. In total, 108 055 cataract surgeries performed by 644

[Marginal case of a practice fee for psychiatric emergency service].

Since 1 January 2004, the German Act for the Modernisation of Health Insurance (GMG) has regulated collection of a so-called practice fee-a charge to patients for their first outpatient health care in any given quarter. We report the case of a slightly inebriated patient who, against his will, was taken to a psychiatric outpatient department by local authorities, where he had to pay the quarterly charge of 10 EUR. Considering that he was forced and therefore had to pay the fee against his will, the present regulation is being discussed with respect to legal and moral implications in psychiatri

Powering Value-Based Care with Clinician-Led, Patient-Centered Design: The MyHealth Bundles Program.

Value-based care has long been championed for its potential to align clinical outcomes with financial incentives, but many health systems struggle to implement truly transformative models. Fee-for-service constraints, payer fragmentation, and complex claims processing often limit progress to incremental gains rather than transformation to true value-based arrangements. Herein, the authors present an innovative approach to bundled payments that overcomes many of these hurdles by placing physicians and patients at the center of the redesign process while partnering with self-insured employers. Pivoting to the QUAD AIM-Lessons Learned From the Central Texas Market.

INTRODUCTION: Since 2009, the Military Health System (MHS) has represented its mission as that of attaining the Quadruple Aim (QUAD AIM): increased readiness, better health, better care, and low per capita costs. The journey to reach the four goals is challenging and ongoing. Leaders in the MHS's Central Texas Market (CTM) sought to understand and overcome the root-cause obstacles that interfered with achieving the QUAD AIM. This process required a self-critical and thoroughly objective review of the behavioral economics of the system. We hypothesized that two corporate behaviors fed upon each

The effect of different methods of remuneration on the behaviour of primary care dentists.

BACKGROUND: Methods of remuneration have been linked with the professional behaviour of primary care physicians. In dentistry, this can be exacerbated as clinicians operate their practices as businesses and take the full financial risk of the provision of services. The main methods for remunerating primary care dentists include fee-for-service, fixed salary and capitation payments. The aim of this review was to determine the impact that these remuneration mechanisms have upon primary care dentists' behaviour. OBJECTIVES: To evaluate the effects of different methods of remuneration on the leve

Paying in the dark: third-party administrator fees and the limits of employer cost control in self-insured health plans.

INTRODUCTION: Employers are one of the largest purchasers of health care in the United States, covering more than 165 million people, yet they face persistent difficulty controlling costs. Most enrollees are in self-insured plans, which rely on third-party administrators (TPAs) to build provider networks and adjudicate claims. The fees employers pay these administrators are an under-documented source of health care spending. While recent litigation and news reporting allege that administrators overpay providers and charge large fees, the peer-reviewed literature offers limited detail on how th

Malignant mesothelioma among US Medicare beneficiaries: incidence, prevalence and therapy, 2016-2019.

OBJECTIVES: Mesothelioma is a rare, aggressive cancer caused by exposure to asbestos fibres. Mesothelioma patients who receive trimodal therapy (chemotherapy, surgical resection and radiation) survive longer than those who receive two or fewer therapy modalities. This study analyses the 2016-2019 Medicare claims data to estimate the burden of malignant mesothelioma and describe therapy patterns (when available) among continuously enrolled fee-for-service (FFS; Medicare parts A and B) beneficiaries. METHODS: We analysed claims and enrolment information from 42 529 117 FFS Medicare beneficiarie

Funding the final chapter - capitation versus fee-for-service: a single-centre, prospective cohort study comparing funding models in home-based palliative care.

BACKGROUND: With an ageing population, there is an increasing demand for home-based palliative care to improve end-of-life care. Funding models can impact service utilisation and patient outcomes. AIMS: To compare two funding models to assess the effects on service utilisation, hospital admission, home death rates and concordance between preferred and actual place of death. METHODS: A single-centre, prospective cohort study compared the first 12 months of two funding models. Unblocking Barriers of Access to Hepatitis C Treatment in China: Lessons Learned from Tianjin.

BACKGROUND: The high price is a critical barrier of access to new direct-anting-antiviral (DAA) therapies for hepatitis C for both the patients and the society. Many countries continue to face the challenge of financing such expensive medicines. Such examples include both high-income and middle-income countries. Existing evidence about the efforts of China to address this challenge is limited. To our knowledge, this is the first detailed description of a novel financing model and comprehensive analysis of its impact on patient financial burden of hepatitis C treatment in China. A consensus process to determine the relative complexity-severity of frequently performed surgical services.

Current prevailing fees represent initial charges inflated over time, and it is well known that some services, such as surgical treatment of the coronary arteries, are less demanding services today than in the early days of the specialty, when morbidity, mortality and operative stress were extremely high. Resource costs are difficult to measure accurately and time is an inappropriate indicator of complexity and severity. If society is looking for ways in which physicians will support changes in reimbursement, the C-S index resulting from the consensus process which has been described herein co
